ABSTRACT

Objective@#To evaluate the impact of multiparametric magnetic resonance imaging (mpMRI) before confirmatory prostate biopsy in patients under active surveillance (AS). @*Materials and Methods@#This retrospective study included 170 patients with Gleason grade 6 prostate cancer initially enrolled in an AS program between 2011 and 2019. Prostate mpMRI was performed using a 1.5 tesla (T) magnetic resonance imaging system with a 16-channel phased-array body coil. The protocol included T1-weighted, T2-weighted, diffusion-weighted, and dynamic contrast-enhanced imaging sequences. Uroradiology reports generated by a specialist were based on prostate imagingreporting and data system (PI-RADS) version 2. Univariate and multivariate analyses were performed based on regression models. @*Results@#The reclassification rate at confirmatory biopsy was higher in patients with suspicious lesions on mpMRI (PI-RADS score ≥ 3) (n = 47) than in patients with non-suspicious mpMRIs (n = 61) and who did not undergo mpMRIs (n = 62) (66%, 26.2%, and 24.2%, respectively; p < 0.001). On multivariate analysis, presence of a suspicious mpMRI finding (PI-RADS score ≥ 3) was associated (adjusted odds ratio: 4.72) with the risk of reclassification at confirmatory biopsy after adjusting for the main variables (age, prostate-specific antigen density, number of positive cores, number of previous biopsies, and clinical stage). Presence of a suspicious mpMRI finding (adjusted hazard ratio: 2.62) was also associated with the risk of progression to active treatment during the follow-up. @*Conclusion@#Inclusion of mpMRI before the confirmatory biopsy is useful to stratify the risk of reclassification during the biopsy as well as to evaluate the risk of progression to active treatment during follow-up.

Resumen: Objetivo: Presentar la evidencia científica más reciente referente a la definición, diagnóstico y consecuencias de morbilidad extrema en obstetricia (MEO). Metodología de búsqueda: Búsqueda en PubMed, The Cochrane Library, OVID, Science Direct, Google Académico, Artemisa, LILACS e Imbiomed de artículos publicados en inglés y español entre los años 2005 a 2018 con las siguientes palabras clave: severe maternal morbidity, near miss morbidity, severe acute maternal morbidity, obstetric near miss and maternal near miss. Se excluyeron estudios cualitativos. Resultados: La MEO (o near miss) se refiere a cualquier complicación aguda que puede presentarse en el embarazo, labor o hasta seis semanas después de haber concluido el embarazo, estas complicaciones ponen en riesgo la vida de la madre, pero no resulta en su muerte. Los indicadores de MEO fueron desarrollados con base a la incidencia y presencia de trastornos hemorrágicos, hipertensivos y otras alteraciones sistémicas y condiciones maternas graves que pueden tener un impacto en la salud de la madre. En la actualidad, se sugiere que el estudio de los indicadores de MEO son de más utilidad que los de muerte materna. Conclusiones: El ejercicio de la práctica médica con base en la mejor evidencia científica, el estudio y la mejora de la calidad de la práctica clínica, de la planificación familiar, la asesoría prenatal y los programas hospitalarios son estrategias que permitirán ayudar a disminuir los casos de MEO.


Abstract. Objective: To provide the most recent scientific evidence about definitions, diagnosis and consequences of severe maternal morbidity (SMM). Research methodology: We searched databases in PubMed, The Cochrane Library, OVID, Science Direct, Google Scholar, Artemisa, LILACS and Imbiomed from 2005 to 2018 with the following keywords: severe maternal morbidity, near miss morbidity, severe acute maternal morbidity, obstetric near miss and maternal near miss. The search was restricted to articles written in the English and Spanish language and published from 2005 to 2018. Qualitative studies were excluded. Results: SMM or maternal near miss event refers to any acute obstetric complication that immediately threatens a woman's survival but does not result in her death either by chance or because of hospital care she receives during pregnancy, labor or within six weeks of termination of pregnancy. The indicators for SMM were developed and based on the incidence and presence of hemorrhagic disorders, hypertensive disorders, other systemic disorders and severe maternal conditions which could have an impact on maternal health. Nowadays it is suggested that the study of indicators for SMM is a more useful indicator of obstetric care than mortality. Conclusions: Use of best evidence-based practices, studying and improving the effectiveness and quality of clinical practice, family planning, prenatal check-up, and hospital obstetric care programs are strategies that could help to reduce cases of SMM.

ABSTRACT

Objective To review the technique and outcome of perineal urethrostomy or urethral perineostomy and to identify factors related to the procedure failure. Material and methods We studied 17 patients who underwent perineal urethrostomy between 2009-2013 in a single hospital. Success was defined as no need for additional surgical treatment or urethral dilatation. We reviewed the clinical data related to age, weight, previous urethral surgery, diabetes, hypertension, ischemic cardiopathy, lichen sclerosus and other causes and studied their association with the procedure failure (univariate analysis). We completed the analysis with a multivariate test based on binary regression. Results The average follow-up was 39.41 months. From all the causes, we found Lichen Sclerosus in 35%, idiopathic etiology in 29% and prior hypospadia repair in 18%. Postoperative failure occurred in 3 patients, with a final success of 82.4%. The binary regression model showed as independent risk factors ischemic cardiopathy (OR: 2.34), and the presence of Lichen Sclerosis (OR: 3.21). Conclusions The success rate with the perineal urethrostomy technique shows it to be a valid option above all when we preserve the urethral blood supply and plate. Lichen sclerosus and ischemic vascular problems are risk factors to re-stenosis. .

ABSTRACT

Hypersensitivity drug reactions (HDR) consist of an individual abnormal response with the involvement of the immunological system. In addition to specific immunological mechanisms where specific antibodies or sensitised T cells participate, release of inflammatory mediators by non-specific immunological recognition may also occur. Within this category are one of the most common groups of drugs, the non-steroidal anti-inflammatory drugs. In addition to chemical drugs new emerging ones with an increasing protagonism are biological agents like humanised antibodies and others. For IgE dependent reactions both in vivo and in vitro tests can be used for the immunological evaluation. Sensitivity of these is not optimal and very often a drug provocation test must be considered for knowing the mechanism involved and/or establishing the diagnosis. For non-immediate reactions also both in vivo and in vitro tests can be used. Sensitivity for in vivo tests is generally low and in vitro tests may be needed for the immunological evaluation. Immunohistochemical studies of the affected tissue enable a more precise classification of non-immediate reactions. The monitorization of the acute response of the reactions has given clues for understanding these reactions and has promising results for the future of the immunological evaluation of HDR.

el diagnóstico de la toxoplas-mosis ha evolucionado de manera importante en los últimos años. Así como los criterios que se tenían para una toxoplasmosis de adquisición reciente han cambiado: la IgM puede durar hasta dos años en el adulto y su utilidad verdadera es para descartar una infección reciente durante el embarazo cuando ésta es negativa por técnicas de inmuno-captura. La estrategia de avidez para IgG tiene el mismo valor, algunas personas pueden persistir con índices bajos, así que su utilidad es cuando la avidez es alta: todo índice de avidez alto es indicativo de infección de más de 6 meses. En el recién nacido la IgM es positiva en el caso de infección congénita en 70 por ciento de los casos. La IgA ha aportado una nueva posibilidad para aclarar el diagnóstico en el niño. La combinación de PCR e inoculación en ratón para el estudio de líquido amniótico permite diagnosticar hasta el 80 por ciento de los casos de infección en útero. Para los inmunosuprimidos la PCR es una ayuda inestimable, aunque se debe tener en cuenta si ha existido tratamiento o profilaxis previa, lo cual disminuye de manera importante la sensibilidad de las pruebas
