ABSTRACT

Objective:To investigate the relationship between uncertainty in illness, coping with adaptive ability and psychological stress response among patients in Emergency Observation Room, and to provide reference for reducing the psychological stress level of patients.Methods:Questionnaire survey was conducted on 260 patients in Emergency Observation Room in a third-grade hospital in Dalian using the general data questionnaire, the Chinese version of the Mishel’Uncertainty in Illness Scale, the Chinese version of the Coping Adaptation Processing Scale and the Stress Response Questionnaire.Results:Coping with adaptive ability played a part in mediating the relationship between uncertainty in illness and psychological stress among patients in Emergency Observation Room. The mediating effect accounted for 32.23% of the total effect. Coping with adaptive ability can reduce the positive reinforcement effect of uncertainty in illness on psychological stress response through emotional and behavioral response pathways.Conclusions:Uncertainty in illness can directly or indirectly affect the psychological stress response of by responding to coping with adaptive ability. It can reduce the influence of psychological stress response on patients' psychosomatic function by reducing the uncertainty in illness and improving the ability to cope with adaptation.

ABSTRACT

Objective@#To explore the mediating role of empowerment level in self-management and quality of life in patients with coronary heart disease, and to provide evidence for interventions to improve quality of life in patients with coronary heart disease.@*Methods@#215 patients with coronary atherosclerotic heart disease in cardiovascular department of hospital in Dalian were selected as the subjects. The Chronic Empowerment Scale, Self-management Behavior Scale of Coronary Heart Disease, China Cardiovascular Patient Quality of Life Assessment Questionnaire and self-made general information questionnaire were used for investigation.@*Results@#The total empowerment, self-management and quality of life of patients with coronary heart disease were (158.75±20.39), (74.50±11.55), and (73.31±23.01). The empowerment, self-management and quality of life of patients with coronary heart disease were positively correlated (r=0.512-0.642, P<0.01). Self-management has a direct positive predictive effect on quality of life. The level of empowerment has a partial mediating effect between self-management and quality of life, and the contribution rate of mediating effect was 31.5%.@*Conclusions@#Empowerment, self-management and quality of life of patients with coronary heart disease are closely related. Self-management can influence the quality of life through the empowerment. Nurses should develop active and effective empowerment programs to improve patients' self-management ability and their quality of life.

ABSTRACT

Objective@#To comb the thread and trend of critical care fields in China for nearly 15 years, analyze research frontiers, demonstrate research evolution paths, provide reference for researchers and clinicians to carry out related work.@*Methods@#Using CiteSpace software and CNKI database to retrieve research literature about critical care from 2004 to 2018 in China for visual analysis.@*Results@#A total of 3 049 valid articles were retrieved. The research frontiers were mainly about emergency and critical nursing, nursing quality management, intensive care, clinical teaching quality and ICU nurses. Research trends from critical care, critically ill patients, disease condition change, to quality control, family members, nursing skills, basic nursing, now gradually turned to humanized care, nursing strategies, sustainable human care concepts, evolution map showed that emerging trend was more active.@*Conclusions@#In last 15 years, research fields of critical care in China have evolved actively, exploring research frontiers, focus on important topics such as evidence-based nursing,humanistic care nursing concept, intensive care information system, and cluster management strategy, extending and deepening professional development of critical care.

ABSTRACT

Objective To investigate the early warning effect of glomerular filtration rate on cerebralmicrobleeds (CMBs) in patients with lacunar infarction. Methods A retrospective study was performed. One hundred and sixty-eight patients with lacunar infarction from January 2015 to January 2018 were selected in Tai′an Central Hospital. The CMBs group and non-CMBs group were classified according to susceptibility weighted imaging. The estimated glomerular filtration rate (eGFR) was obtained. According to the MDRD formula, the indicators of renal dysfunction in patients with different degrees of cerebral microbleeds were compared, and the related risk factors of cerebral microbleeds were analyzed by single factor and multivariate analysis. Results The blood high sensitive C-reactive protein (hs-CRP) was significantly higher in CMBs group than that in non-CMBs group: (4.29 ± 3.86) mg/L vs. (2.58 ± 2.74) mg/L, P < 0.01. The proportion of patients with elevated eGFR in CMBs group was significantly higher than that in non-CMBs group: 40.7%(33/81) vs. 11.4%(10/88), P<0.01. There were significant differences in the proportion of hypertension history [83.3% (50/60) vs. 52.4% (11/21), χ2=6.947,P=0.008], serum creatinine(SCr) value[(113.75 ± 71.23) μmol/L vs. (75.38 ± 38.36) μmol/L, t=-2.346,P=0.021] and eGFR value [(91.58 ± 31.87) ml/(min·1.73 m2) vs. (109.95 ± 29.47) ml/(min·1.73 m2),t=2.316,P=0.023] between CMBs groups with different distribution (P < 0.05). Multivariate Logistic regression analysis showed that the history of hypertension ( OR=5.408; 95% CI 1.518-19.267, P=0.008) was the risk factor of CMBs in deep/ subtentorial group.The urea nitrogen (BUN)value increased (χ2=28.150, P < 0.01), SCr value (χ2=47.610, P < 0.01) increased, and eGFR value (χ2=39.067,P<0.01) decreased with the severity of CMBs, and the differences were statistically significant. The proportion of CMBs in patients with eGFR decreased group(76.7% , 33/43) was significantly higher than that in normal eGFR group (38.1%, 48/126), and the difference was statistically significant (χ2=19.188, P < 0.01). Logistic regression analysis of multiple factors affecting cerebral microbleeds showed that history of hypertension ( OR=3.135; 95% CI 1.343-7.321, P=0.008), elevated hs-CRP( OR=1.233; 95% CI 1.098-1.385, P < 0.01), eGFR decreased ( OR=1.025; 95% CI 1.002-1.048, P=0.033) were risk factors for cerebral microbleeds. The area under the ROC curve of eGRF was 0.694 (P=0.042). Conclusions A history of hypertension is a risk factor for deep/infratentorial CMBs. Decreased glomerular filtration rate is an independent risk factor for cerebral microbleeds. Therefore, reminding clinicians to actively diagnose and treat patients with glomerular filtration rate decline has important clinical significance for preventing the occurrence and development of cerebral microbleeds.

ABSTRACT

Pattern selection plays an important role in data mining and pattern recognition, especially for large scale bioinformatic data. There are many problems in this field, such as algorithm complexity and numbers of the best feature subset. In this paper, we propose a new pattern selection algorithm, carrying out pattern selection base on Mutual Information (MI). Pattern subset evaluation index was studied to ensure the best feature subset. To pattern selection, algorithm bases on the correlation of patterns and label, as well as the redundancy of each pattern. Neurofuzzy Pattern Subset Evaluation Index was researched to make sure which is the best subset for our pattern subset evaluation. To verify the effectiveness of our method, several experiments are carried out on the data of gene expression of mouse from Leiden University and UCI datasets. The experimental results indicated that our algorithm achieved better results in the complexity and accuracy.

ABSTRACT

Immunohistocheraistry and RT-PCR were used to examine the expression of toll-like receptor 4 (TLR4) in the kidney of diabetic rats.Compared with normal control group,the expression of TLR4 in diabetes mellitus group was increased ( P < 0.05 ).Compared with diabetes mellitus group,the expression of TLR4 in simvastatin treated group was decreased( P<0.05 ).The results suggest that TLR4 may play a role in the pathogenesis of diabetic nephropathy.Simvastatin seems to delay the progress of diabetic nephropathy via reducing TLR4 expression.

ABSTRACT

According to the frequency overlapping of intrinsic mode function (IMF) based on the temporal and spatial filtering of empirical mode decomposition (EMD), which will lead to the question of useful signals and noises filtered together, we proposed a method that numbers of IMF is determined by energy estimate, temporal and spatial filtering combing wavelet threshold and EMD integrating wavelet local signal characteristics of time and scale domain. This method not only used multi-resolution wavelet transform features, but also combined the EMD and Hilbert decomposition of the adaptive spectral analysis of instantaneous frequency and significance of the relationship between energy, so as to solve the problem of useful signal being weakened. With MIT/BIH ECG database standard data subjects, experimental results showed it was an effective method of data processing for handling this type of physiological signals under strong noise.

ABSTRACT

Objective To observe the effect of Jingujian Granule (Granule for strengthening the bones) No.1,2,and 3 on primary osteoporosis in the TCM syndromes of liver-kidney deficiency,both spleen and kidney deficiency,and spleen and kidney deficiency accompanied with blood stasis,and the influence on the bone metabolism indices.Methods Totally 210 primary osteoporosis cases were differentiated according to the TCM differentiation and administered Jingujian Granule No.1,2,3 or Caltrate D accordingly.The observation was carried out for 3 months on the improvement of clinical symptoms,changes in bone mineral density and bone metabolism indices,and safety indices.Results The total effective rate of Jingujian Granule No.1,2,and 3 was 89.92%,while that of Caltrate D was 33.3% (P
