ABSTRACT

@#Abstract: Objective To evaluate the value and significance of rifampicin-resistant real-time fluorescence quantitative nucleic acid amplification detection technology (GeneXpert MTB/RIF) in the diagnosis of pulmonary tuberculosis.   Methods The clinical data of 228 patients with suspected pulmonary tuberculosis, who admitted to Hebei Chest Hospital from January 2018 to December 2019, were analyzed retrospectively. The sputum was collected for GeneXpert MTB/RIF, sandwich cup liquid-based bacterial acid-fast staining smear microscopy (referred to as “sandwich cup method”) and Loop-Mediated isothermal amplification (referred to as “LAMP method”) and the results were statistically analyzed by SPSS 17.0 software. Results Among the 228 patients with suspected cases, 200 cases were clinically diagnosed as pulmonary tuberculosis and 28 were non-tuberculosis. The positive detection rate of GeneXpert MTB/RIF (81.0%, 162/200) was significantly higher than that of sandwich cup method (62.5%, 125/200) and LAMP method (72.5%,145/200) (χ2=16.885, 4.049, P<0.05). Taking clinical diagnosis as gold standard, the sensitivity of GeneXpert MTB/RIF (80.00%,160/200) was significantly higher than that of sandwich cup method (60.00%, 120/200) and LAMP method (70.50%, 141/200) (χ2=19.048, 4.846, P<0.05). The diagnostic consistency of GeneXpert MTB/RIF (K=0.73) was higher than that of sandwich cup method (K=0.39) and LAMP method (K=0.56). Conclusions The GeneXpert MTB/RIF detection method is rapid and simple, and can diagnose pulmonary tuberculosis rapidly and simultaneously detect rifampicin resistance of Mycobacterium tuberculosis with high sensitivity. It has high clinical value for early diagnosis of pulmonary tuberculosis and guidance of treatment in general and specialized hospitals.

ABSTRACT

Objective: To introduce how to build a data platform of data collection and data management for multi-center birth cohort study by using REDCap. Methods: After the REDCap electronic data capture system was installed and set up, the electronic case report forms (eCRFs) were programmed to collect data in multi-centers. The rules of data quality control were programmed, and different levels of user right for access to data platform were assigned to the data managers and data clerks based on their role in research project. Results: With REDCap system being installed, the example project was created, and a series of eCRFs were established for each stage of preconception, pregnancy and childhood through follow-up. After intensive testing to improve and achieve a stable data platform, standardized trainings were provided to data-related team. The REDCap eCRFs were then put in use online. By assigning different user right of access to data platform, data entry can be from multi- research centers and survey sites. This REDCap data platform supported the cohort project on data collection and data management. Conclusion: The data platform established by using REDCap provides strong support to birth cohorts on data collection and data management. This example project of data platform can be applied to other epidemiological studies.

ABSTRACT

To investigate the effects of Pogostemon cablin(patchouli) on gastrointestinal function of rats with the syndrome of damp retention in middle-jiao, and explore its therapeutic mechanism. In this study, gastrointestinal function of rats with the syndrome of damp retention in middle-jiao was evaluated by multiple assays including gastric remnant rate, small intestine propelling rate, gastric juice quantity, pepsin activity and gastrointestinal tissue morphology. ELISA was used to detect gastrointestinal hormones including MTL, GAS, VIP and cytokines including TNF-α and interleukin 10 in rat serum. Real-time fluorescent quantitative PCR technique was used to detect relative mRNA expression of AQP3, AQP4 and AQP8 in gastric and colonic tissues to explore the mechanism of P. cablin in treatment of gastrointestinal functions. The results showed that middle and high dose of P. cablin (3.24, 6.48 g•kg⁻¹) could obviously decrease the gastric remnant rate, promote gastric emptying, increase the small intestine propelling rate(P<0.05), speed up the propulsive movement of gastrointestinal tract, increase the secretion and acidity of gastric juice, increase the activity of pepsin, and improve the injury of gastrointestinal tissue. All the doses of P. cablin could increase the concentration of MTL and GAS in serum, reduce the concentration of VIP, TNF-α and IL-10 in serum, decrease the mRNA expression of AQP3 in gastric and colonic tissues, and increase the expression levels of AQP4 and AQP8 in colonic tissues. The regulatory effects were better in middle and high dose groups. In conclusion, regulation of the levels of gastrointestinal hormones, inflammatory cytokines and aquaporins may be the paths for P. cablin to maintain normal gastrointestinal function of rats with the syndrome of damp retention in middle-jiao. The results of the study laid a foundation for clarifying the treatment mechanism of aromatic damp-resolving drugs for indications including damp retention in middle-jiao and transformation failure of spleen.

Objective To design a data acquisition and monitoring system to facilitate the maintenance of the ventilator.Methods The system was composed of two modules of data acquisition and monitoring.The data acquisition module used MSP430F149 SCM as the master control chip to realize real-time acquisition and storage of operation data,and the monitoring module adopted STM32 SCM as the master control chip to implement equipment monitoring,server serial port conversion and data communication.Wireless data transmission between the two modules was executed with wireless NRF24L01 module.Results The system realized real-time acquisition and storage of the ventilator log,and transmitted acquired data to the server.Conclusion The system eliminates the deficiencies of manual operation and implements basic risk analysis,and facilitates the engineer to monitor and maintain the ventilator.

@#Objective To explore the effect of electroacupuncture at Baihui (DU20) and Shenting (DU24) acupoints on learning and memory ability in rats with middle cerebral artery occlusion and reperfusion (MCAO/R), and the potential mechanisms. Methods A total of 90 healthy male Sprague-Dawley rats were randomly divided into sham group (n=18) and operation group (n=72). The MCAO/R model was established by suture method in the operation group. Finally, 54 qualified rats of the operation group were randomly divided into ischemia group (n=18), electroacupuncture group (n=18) and non-acupoint group (n=18). The electroacupuncture group received electroacupuncture at Baihui (DU20) and Shenting (DU24) for 14 days. The cerebral infarction volume was measured by magnetic resonance imaging (MRI). The learning and memory ability was tested by Morris water maze. The protein expression of 5-hydroxytryptamine 1A (5-HT1A) receptor was detected by immunohistochemistry. Results There was no significant difference in the cerebral infarction volume among three groups before intervention (F=1.678, P>0.05). Compared with the ischemia group and the non-acupoint group, the cerebral infarction volume signif-icantly reduced (P<0.001);the latency significantly shortened (P<0.001) and the times crossing the platform decreased (P<0.05);the expres-sion of 5-HT1A receptor decreased in the left hippocampus (P<0.05) in the electroacupuncture group after intervention. There was no signifi-cant difference in all the indices between the non-acupoint group and the ischemia group after intervention (P>0.05). Conclusion Electroacu-puncture at Baihui (DU20) and Shenting (DU24) could effectively increase the learning and memory ability of MCAO/R rats, which might relate with inhibiting the expression of 5-HT1A receptor in hippocampus.

ABSTRACT

To expand the clinical application of gamboges, it is necessary to study crude gamboges' toxicity after oral administration and attenuation mechanism during processing. In this study, crude gamboges' toxicity was judged by multiple assays, including inflammatory mediums [such as nitric oxide (NO), tumor necrosis factor alpha (TNF-α), and interleukin 6 (IL-6)] released by macrophage RAW264.7, and pathological manifestations of rat stomach and duodenal tissues after oral administration with crude and processed gamboges. The attenuation mechanism during processing was studied by detecting AQP3, AQP4 protein and mRNA expression in rat gastric and duodenal tissues using immunohistochemical assay and real-time fluorescent quantitative PCR technique. According to the results, crude gamboges group showed promotion in release of NO, TNF-α and IL-6 by macrophage RAW264.7 in a dose-dependent manner; Compared with crude gamboges group, processed gamboges group showed reduction in release of NO and IL-6, with increase in TNF-α. Crude gamboges could cause rat diarrhea, white blood cells increase, lymphocytes decrease, hyperemia and edema in rat gastric mucosa, duodenal mucosal necrosis and inflammatory cells infiltration. All of these results proved that gamboges had the inflammatory toxicity in gastric and duodenal tissues after oral administration in a dose-dependent manner, which however reduced after processing. In addition to the inflammatory toxicity, the mRNA and protein expressions of aquaporin 3 (AQP3), aquaporin 4 (AQP4) in gastric and duodenal tissues of high-dose crude gamboges group were increased significantly (P<0.05), while the protein and mRNA expressions of AQP3, AQP4 were weakened in processed gamboges group. The results showed that AQP3, AQP4 protein and mRNA expressions were positively correlated with the inflammatory toxicity. In conclusion, down-regulation of AQP3, AQP4 protein and mRNA expressions may be one of attenuation mechanisms in processing gamboges.

ABSTRACT

Hydrolytic amino acids were extracted by acid hydrolysis method, then derivatized with phenyl isothiocyanate (PITC). And the samples were analysed by HPLC on an Ultimate Prime C18 (4.6 mm x 250 mm, 5 microm) column with gradient elution of 0.1 mol x L(-1) sodium acetate buffer solution (adjusted to pH 6. 5)-acetonitrile (93:7) (A) and acetonitrile-water (8:2) (B) at a flow rate of 1.0 mL x min(-1). Column temperature was 40 degrees C and the detected wavelength was 254 nm. Amino acids derivative solution remained stable in 36 hours. The response was linear for 16 amino acids with a correlation coefficient r > 0.999 5. The average recoveries were 98.01% -101.8%. The method is reliable with good accuracy and repeatability, which is useful for the determination of amino acids in Galli Gigerii Endothelium Corneum.

ABSTRACT

Advanced technologies are used to clarify the meridian tropism theory of traditional Chinese medicine is an important part of theoretical studies of traditional Chinese medicine. In this article, modern pharmacokinetic method was used to investigate tissue distribution characteristics of psoralen and isopsoralen of Psoraleae Fructus decoction in rats, in order to provide research ideas and experimental basis for the meridian tropism theory. In this study, various tissue samples such as heart, liver, spleen, lung, kidney, brain and spermary were collected at different times after oral administration with FP decoction, in order to determine concentration of psoralen and isopsoralen by HPLC. Pharmacokinetic parameters were calculated by DAS 2.0 software. The study results showed that HPLC indexes of psoralen and isopsoralen in various tissues of rats met the determination requirements of biological samples. Both components were distributed in all of the tissues, with AUC(0-t) order of liver > lung approximately kidney > heart > brain approximately spleen > spermary. There was significant difference between liver, kidney, lung and other tissues (P < 0.05). MRT(0-t) of both psoralen and isopsoralen were about 10 h. Therefore, psoralen and isopsoralen showed stronger targeting selection in liver, kidney and lung.

ABSTRACT

<p><b>OBJECTIVE</b>Through a detection of dust in the coal mines workplace, to understand the status of occupational hazards, and the evaluation of occupational hazards, provide subject to control occupational hazards.</p><p><b>METHOD</b>According to production process and "hazardous substances in workplace air monitoring, sampling norms" and other standards to determine the sampling points and sampling of coal dust.</p><p><b>RESULT</b>Underground mining operations in 21 subjects with time-weighted average concentration of dust types pass rate of 28.6%, of which five types of dust hazard grade II, six types of dust hazard rating of 0, and the remaining types of grade I dust hazard levels. Coal dust test six types of time-weighted average concentration of 83.3% pass rate, only one types of dust hazard grade I, all the rest is 0. Calculated by the detection of dust overrun 18 times operating sites, the pass rate of 72.2% results.</p><p><b>CONCLUSION</b>Purified water spray and air flow curtain of dust control has played a certain role, but the work of underground working conditions and environmental constraints, most of the dust concentration in workplace occupational exposure limits do not meet the requirements, recommended the strengthening of dust or Dust the daily management and maintenance of equipment, strengthen the ventilation, personal protection officers to strengthen operations.</p>

ABSTRACT

Objective To evaluate the safety and efficacy of CT fluoroscopic guidance percutaneous acetic acid injection(PAI)tumor ablation and TACE for the malignant liver tumor.Methods PAI had been performed after TACE on 78 patients with malignant tumor(hepatocellular carcinoma 70,metastatic adenocarcinoma 8)since May 2005 to June 2007 in Southwest Hospital.All procedures were performed under CT fluoroscopic guidance.According to the different size of lesions,50%-60% acetic acid 2-10ml was injected into the lesions.PAI was repeated once or twice in some lesions based on the imaging.Results The needle could reach targets under CT real time fluoroscopy in all patients.No complication occurred,but with only moderate pain in 42 cases.The size of tumors shrank in 32 cases,remained unchanged 38 cases,and increased in 8 cases.The efficiency was 90%.All 78 patients were followed up for 3 months with all survival and 48 patients for 1 year with 40 survival.Conclusions CT fluoroscopic guidance PAI tumor ablation combined with TACE in the treatment of malignant liver tumor is safe and effective.(J Intervent Radiol,2007, 16:831-833)

ABSTRACT

OBJECTIVE@#To screen the inflammatory mediators genes regulated by HSF1, and explore the mechanism of downstream genes regulated by HSF1.@*METHODS@#HSF-/- and HSF1+/+ mice were injected with 15 mg/kg LPS intraperitoneally (ip), respectively, and were treated as previous after HSR. The total RNA of lung tissues were extracted and filtrated by SuperArray gene Microarry. The promoter of candidate genes were analyzed by transcription element search software to search for heat shock element (HSE). Select the suppressor of cytokine signaling 3 (SOCS3) with HSE. Macrophage cells were stimulated with 400 ng/mL LPS, and were treated as previous after HSR, then the total RNA was extracted respectively. RT-PCR and northern blot assay were performed to detect the expression levels of SOCS3 mRNA.@*RESULTS@#Fifteen genes were repressed by HSF1, including 9 genes with complete HSE. Eleven genes were accelerated by HSF1 possibly, including 8 genes with complete HSE. The promoter of SOCS3 gene contained one complete HSE. LPS stimulation obviously increased the levels of SOCS3 mRNA in macrophages of RAW264.7 mice, which was inhibited by HSR and over-expression of HSF1.@*CONCLUSION@#HSR or HSF1 inhibits LPS induced expression of SOCS3 mRNA; HSF1 might inhibit LPS-induced expression of SOCS3 mRNA by binding to HSE in the promoter of SOCS3 gene.

ABSTRACT

A fusion gene CTB-PROIN, in which Proinsulin gene was fused to the 3' end of CTB gene by a hinge peptide 'GPGP', was constructed and cloned into pET-30a(+) to obtain a prokaryotic expression vector pETCPI. Subsequently the recombinant plasmid pETCPI was transformed into E. coli stain BL21 (DE3). After induced by IPTG, the expression product was analyzed by sodium dodecyl sulphate-polyacrylamide gel (15%) electrophoresis (SDS-PAGE), and its result indicated that the recombinant protein CTB-PROIN was expressed and accumulated as inclusion bodies. The recombinant CTB-PROIN protein accumulated to the level of 25% of total bacterial proteins. After inclusion bodies was denaturalized and refolded in vitro, significant assembly of monomers had occurred, and the recombinant protein represented assembled pentamers. The results of western blotting analysis also demonstrated that the fusion protein could be recognized by the anti-CT and anti-insulin antibody, respectively. In addition, the result of the CTB-PROIN-GM1 binding assay, that the protein could bind to monosialoganglioside specifically, showed it possesed biological activity in vitro. These results provided the possibility of developing a cheaper and more efficient oral vaccine for type I diabetes using such constructs.
