ABSTRACT

Se presenta el caso de paciente mujer de 79 años que presenta fiebre, placas eritemato-violáceas y ampollas de contenido seroso, dolorosas en el antebrazo y dorso de la mano izquierda.Los hemogramas seriados revelaron la presencia de anemia, leucocitosis marcada con neutrofilia, trombocitopenia y linfopenia. Se realiza lámina periférica donde se observa blastos (79%) y cuerpos de Auer. La biopsia de la lesión cutánea señala un infiltrado denso perivascular de neutrófilos maduros en la dermis superficial sin vasculitis asociada. Se diagnosticó Síndrome de Sweet asociado a Leucemia mieloide aguda, iniciando terapia de acuerdo a protocolo.El síndrome de Sweet es una afección cutánea poco frecuente de causa desconocida que se caracteriza por aparición brusca de fiebre, afectación del estado general y lesiones tipo placas o nódulos eritematosos muy dolorosos, con o sin pseudovesículas, de predominio en el hemicuerpo superior. Además, es frecuente leucocitosis, neutrofilia y PCR elevado. (AU)


We present the case of a 79-year-old female patient with fever, erythematous-violaceous plaques and blisters of serous content, painful on the forearm and back of the left hand.Serial blood tests revealed the presence of anemia, leukocytosis marked with neutrophilia, thrombocytopenia and lymphopenia. Peripheral lamina is where blastos (79%) and Auer bodies are observed. The biopsy of the skin lesion indicates a dense perivascular infiltrate of mature neutrophils in the superficial dermis without associated vasculitis. Sweet syndrome associated with acute myeloid leukemia was diagnosed, initiating therapy according to protocol.Sweet syndrome is a rare skin condition of unknown cause that is characterized by sudden onset of fever, general condition impairment and very painful plaque or erythematous nodule lesions, with or without pseudovesicles, predominantly in the upper hemibody. Also, leukocytosis, neutrophilia and elevated CRP are frequent. (AU)

ABSTRACT

Objetivo: Describirlas características epidemiológicas y resultados clínicos del tratamiento con esquema uno en pacientes con tuberculosis atendidos en los establecimientos de salud del MINSA de la provincia de Ica durante los años 2009-2010. Material y métodos: Se realizó un estudio observacional de diseño descriptivo retrospectivo. Resultados: Se registraron 678 casos de tuberculosis tratados con esquema uno. 529 (78%) de los casos residía en el Cercado de Ica, Parcona y LaTinguiña, 289 (42,63%) fueron de sexo femenino,82.6% fueron de origen pulmonar, mientras que 17.4% fueron extrapulmonares, 77 (11,4%) correspondieron a tuberculosis infantil, 587 (86,6%) ingresaron como casos nuevos, 71 (10,5%)por recaída y 15 (2,2%) como abandonos recuperados. La baciloscopía de esputo al ingreso resultó positiva en 406 (59,9%)casos y al cuarto mes resultó negativa en 535 (78,9%) casos. 588 (86,7%) casos curaron, 6 (0,9%) fracasaron, 45 (6,6%)abandonaron el tratamiento, 26 (3,8%) fallecieron y 4 (0,6%) fueron transferidos sin confirmación. Asimismo, la condición de alta se asoció significativamente al tipo de ingreso del paciente (p=0,001) y al tipo de tuberculosis (p=0,03).Conclusiones: La mayoría de pacientes se concentra en el cercado de Ica y los distritos de Parcona y la Tinguiña. Existe elevado porcentaje de pacientes con resultados clínicos negativos al alta. (AU)

Material and methods: It was realized an observational retrospective design study. Results: There were 678 cases of tuberculosis treated with one. 529 (78 %) of the cases resided in the district of Ica, Parcona and "La Tinguiña", 289 (42.63 %)were female, 82.6 % were of pulmonary origin, while 17.4 % were extrapulmonary, 77 (11.4 %) corresponded to childhood tuberculosis, 587 (86.6 %) were admitted as new cases, 71 (10.5%) by relapse and 15 (2.2 %) as abandonmentre covered. The sputum smears the income was positive in 406 (59.9 %) cases and the fourth month was negative in 535 (78.9 %) cases.588 (86.7 %) cases were cured, 6 (0.9 %) failed, 45 (6.6 %) abandoned the treatment, 26 (3.8 %) died and 4 (0.6 %) were transferred with out confirmation.Also, the condition of discharge was significantly associated with type of patient admission (p=0.001 ) and the type of tuberculosis (p=0.03 ).Conclusions: The majority of patients is concentrated in the district of Ica and the districts of Parcona and "La Tinguiña". There is a high percentage of patients with negative clinical outcomes to the discharge. (AU)
