ABSTRACT

Aiming at the problems of resource allocation, cost control, homogenization management of medical and nursing quality and so on in a hospital with multi-campuses, People′s Hospital of Ningxia Hui Autonomous Region explored an integrated governance system. It incorporated a series of aspects, such as the establishment of Internet hospital platform using payment and settlement as the link, the construction of operation and management mode for integrating information of multiple branches, the completion of " one hospital, multiple branches" organizational structure and integrated operation system, the innovation of personnel and post compensation system in line with features of medical industry, the implementation of new service mode of diagnosis teams and inpatient wards, the execution of budget management and cost accounting control, and the establishment of integrated online and offline epidemic prevention and control system for the joint prevention and control. These practices achieved good effects that quality and efficiency of medical services were greatly improved, cost of medical services was effectively controlled, and the efficiency of hospital operation and management were enhanced. It provided valuable experience for the integrated management of public hospitals with multi-campuses.

ABSTRACT

Objective To explore the impact of work-family conflict on job satisfaction and turnover intention of anesthesiologists in Heilongjiang Province.Methods Questionnaire survey was used for data collection.Descriptive statistics,Pearson correlation analysis and multivariate linear hierarchy regression analysis were performed to analyze the impact of work-family conflict on job satisfaction and turnover intention of anesthesiologists.Results The average value of work-family conflict among anesthesiologists was (2.99 ± 0.57).The finding indicated that work-family conflict of anesthesiologists had a significant negative effect on job satisfaction (β=-0.248,P<0.01) and a positive effect on turnover intention (β=0.329,P<0.01).Conclusion Anesthesiologists' work-family conflict is above the middle level in Heilongjiang Province.The work-family conflict of anesthesiologists can reduce job satisfaction and increase turnover intention.

ABSTRACT

Objective To examine the current situation of the career plateau among anesthesiologists and analyze the impact of occupational stressors on it. Methods A questionnaire survey was conducted on the anesthesiologists. A total of 300 questionnaires were distributed and 278 questionnaires were effectively collected. Statistical analysis using SPSS 19.0 was performed to assess the status quo of career plateau among anesthesiologists. Pearson correlation analysis and stepwise regression analysis were used to analyze the influence of occupational stressors on career plateau . Results The average value of occupational stressors among anesthesiologists was (3.22±0.55), and the average value of career plateau was (3.90±0.70). Occupational interest in the occupational stressors of anesthesiologists is negatively correlated with the occupational plateau (r=-0.552, P<0.01), and career development is negatively correlated with occupational plateau (r=-0.541, P<0.01) as well. Both occupational interest and career development show a negative predictive effect on the career plateau (β=-0.359, P<0.01 andβ=-0.334, P<0.01, respectively). Conclusion Career plateau among anesthesiologists is at a medium-to-high level. Occupational interest and occupational development in occupational stressors have a negative predictive effect on occupational plateaus, so hospital managers should pay attention to them.

ABSTRACT

Liver failure is one of the most serious complications after liver cancer resection.Hemodynamic changes,bile salt imbalance,and impaired immune function can cause liver failure after liver cancer resection,and related risk factors include patient,liver,and surgical conditions.This article summarizes the causes and influencing factors for liver failure after liver cancer resection and points out that prophylactic measures before surgery,expansion of remnant liver volume,adjustment of hemodynamic imbalance of the liver,and optimization of perioperative treatment help to reduce the incidence rate of liver failure after liver cancer resection and improve the prognosis and long-term survival of such patients.

ABSTRACT

OBJECTIVE@#To explore the effects of novaferon on dextran sulfate sodium (DSS)-induced colitis and expression of TNF-α in mice and to evaluate the efficacy of novaferon on ulcerative colitis and the possible mechanisms.@*METHODS@#A total of 70 BALB/C mice [weight (20.0±2.0) g, 8-week years old, female, pathogen free] were randomly divided into 7 groups: a normal group, a model group, a mesalazine treatment group, a prednisone treatment group, a low-dose novaferon group, a middle-dose novaferon group and a high-dose novaferon group (10 mice per group). The normal group-mice were given distilled water. The ulcerative colitis model was established by treated the mice with 4% DSS for 7 continuous days. At the 8th day, the mice in the all of drug treatment groups were injected corresponding drugs (i.p.). During the experiment, the general situation, daily weight, stool trait and occult blood were recorded, and the mice were killed on the 14th day. The disease activity index (DAI), colon length, histological scores were assessed. Immunohistochemistry was used to measure the expression of TNF-α in colonic mucosa.@*RESULTS@#1) The mice treated with DSS solution showed diarrhea, mucous stool and bloody stool, and the DAI score increased gradually. The mesalazine, predinison and nofaferon could ameliorate the general situation of the mice, reduce the DAI and histological scores, and reverse the decrease in the colon length. 2) Compared with the model group, the DAI scores were significantly decreased in the novaferon groups (at low, middle or high dose), the mesalazine group or the prednisone group (all P0.05). The efficacy of novaferon in the middle-dose group and the high-dose group are better than that in the mesalazine group, the prednisone group and the low-dose novaferon group (all P0.05). 4) The TNF-α expression was significantly down-regulated in the novaferon groups (at low-dose, middle-dose or high-dose), the mesalazine group and the prednisone group compared with model group (all P0.05); the decrease of TNF-α expression by novaferon displayed a dose-dependent manner. Compared with the mesalazine group or the prednisone group, the TNF-α expression in novaferon groups at all dosages was dramatically reduced (all P<0.01).@*CONCLUSION@#Novaferon can improve the DAI scores and colonic tissue injury in ulcerative colitis induced by DSS in mice, and down-regulate the TNF-α expression in dose-dependent manner.

ABSTRACT

Objective The method of administration is one of the important factors influencing steroid-induced femoral head necrosis ( FHN) .From the perspective of administration compliance, time and persons needed, and pharmacodynamics, this study compared different administration methods for steroid-induced FHN in rabbits aiming to provide some experimental evidence for selec-ting correct methods of administration. Methods The steroid-induced femoral head necrosis rabbits ( New Zealand, male) were ran-domly divided into five groups according to the different administration methods of intervention:deep oral administration group (n=10), in-tragastric gavage administration group ( n =10 ) , free-drinking drug group (n=10) and model control group (n=5), blank control group (n=5), the administration compliance, administration time, mortal-ity, pharmacodynamic index of lipid content and empty lacunae rate were compared among deep oral administration group, intragastric ga-vage administration group, free-drinking drug group. Results Compliance effect size of deep oral administration group ( 1.78 ± 0.64) lower than intragastric gavage administration group (4.04 ±0.87) and free-drinking drug group (8.94 ±1.05) (P<0.01). Administration time among deep oral administration group ([0.94 ±0.02]min), intragastric gavage administration group ([9.47 ± 0.31]min) and free-drinking drug group ([889.50 ±235.38]min) overall comparison gradually increased (P=0.000), the differ-ence between the two groups was statistically significant (P<0.05).At the 2 and 4 weeks, cholesterol, triglycerides and low-density lipoprotein of deep oral administration group, intragastric gavage administration group, free-drinking drug group compared with model control group and blank control group the difference was statistically significant ( P<0.05) , and deep oral administration group com-pared with free-drinking drug group, the difference was also statistically significant ( P<0.05) .At the 2 weeks, empty lacunae rate of deep oral administration group ([15.44 ±2.68]%), intragastric gavage administration group ([15.02 ±3.34])%), free-drinking drug group ([16.72 ±4.06]%) compared with model control group ([18.59 ±3.12]%) and blank control group ([10.82 ± 2.76]%, the difference was statistically significant (P<0.05).At the 4 weeks, empty lacunae rate of deep oral administration group ([18.53 ±3.26]%), intragastric gavage administration group ([18.85 ±3.17]%), free-drinking drug group ([20.41 ±4.18]%) compared with model control group ([24.66 ±3.74]%) and blank control group ([11.37 ±2.23]%), the difference was also sta-tistically significant ( P<0.05 ) . Conclusion Compared with traditional methods of administration, deep oral administration has better compliance, shorter administration time, and similar to intragastric gavage administration in pharmacodynamics, but more effec-tive than free-drinking drug administration, and it is a new and effective method of administration.

ABSTRACT

Objective To evaluate the prognestic effect of thrombus aspiration combined tirofiban in patients of acute myocardial infarction. Methods 372 patients with acute ST segment elevation myocardial infarction were randomly divided into the observation group (n=183) and the control group (n=189). Patients in the observation group received thrombus aspiration combined tirofiban additional to conventional treatment, and cases in the control group received routine treatment. The curative effect, postoperative complications and adverse reactions were compared between the 2 groups. Results The postoperative TIMI classⅢperfusion was achieved in 152 cases (83.1%) in the observation group and 140 cases (74.1%) in the control group (P 0.05). MACE reorded within 30 d post operation was 10 cases(5.5%) in the observation group and 22 cases (11.6%) in the control group (P < 0.05). No thrombocytopenia recorded in both groups. Conclusions Thrombus aspiration combined tirofiban for patients with acute myocardial infarction during PCI can improve the postoperative coronary artery perfusion, left ventricular ejection function and reduce the incidence of MACE within 30 days.

OBJECTIVE@#To study the role of Novaferon on TNF-α production and expression of NF-κB mRNA in monocytes isolated from normal human peripheral blood and to provide theoretical basis for treatment of immunological diseases with Novaferon.@*METHODS@#Monocytes were isolated from the peripheral blood in 30 healthy volunteers and divided into 5 groups: group A was blank control, group B was stimulated by LPS without Novaferon intervention, group C by LPS together with Novaferon intervention, group D by LPS before Novaferon intervention, which group E by LPS after Novaferon intervention. We detected the concentration of TNF-α after LPS stimulation and Novaferon intervention in the supernatant by ELISA and expression of NF-κB mRNA by RT-PCR.@*RESULTS@#Novaferon inhibited TNF-α production by monocytes isolated from healthy volunteers induced by LPS in vitro in group D compared with group B [(1446.76±72.36) pg/mL vs (946.46±46.12) pg/mL, P0.05; (1446.76±72.36) pg/mL vs (1383.62±86.96) pg/mL, P>0.05]. There was significant change in NF-κB mRNA expression in group D compared with group B (0.2829±0.0365 vs 0.4994±0.0604, P0.05; 0.4767±0.0600 vs 0.4994±0.0604, P>0.05).@*CONCLUSION@#Novaferon can suppress TNF-α secretion by monocytes induced by LPS in vitro, and it can affect the immunity function of monocytes, which may be associated with the downregulation of NF-κB mRNA expression in monocytes.

ABSTRACT

Objective To determine the effect of prokinetic agents such as domperidone, mo-sapride, clarithromycin, and itopride on the electrical activity of the stomach and duodenum in SD rats, and also to explore the mechanism. Methods The organism functional experiment system BL-420E was used to record the myoelectrical activity in the stomach and duodenum of SD rats in all groups using domperidone, mosapride, itopride, clarithromycin, and physiological saline on the inter-digestive phase. The effect of the prokinetic agents on the amplitude and freqency of gastric and duo-denal electromyologram in the SD rats was compared. The antagonists such as atropine, phento-lamine, and propranolol were added to investigate the mechanism of action with all prokinetic agents. Results All prokinetic agents increased the amplitude and frequency of gastric and duodenal fast waves in the SD rats (P<0.05). The effect of itopride was the most obvious among the 3 groups (P<0.05), and clarithromycin had the weakest effect (P<0.05). The amplitude and frequency of gastric and duodenal fast waves in the SD rats in the groups of clarithromycin, domperidone, mosa-pride, itopride, and physiological saline were inhibited by atropine (P<0.05) , but not by phento-lamine and propranolol. Conclusion Itopride, mosapride, domperidone, and clarithromycin can in-crease the amplitude and frequency of gastric and duodenal fast waves in the SD rats. The mechanism may be related to cholinergic receptors, but not adrenergic receptors.

ABSTRACT

OBJECTIVE@#To investigate the pathogenesis of nasal vestibular cyst.@*METHOD@#Forty two case of nasal vestibular cyst were retrospectively analyzed, including clinical manifestation, pathological findings, surgical therapy efficacy.@*RESULT@#Of these 42 cases of the type of lining epithelia, pseudostratified columnar epithelium was identified in 24 cases, stratified squamous epithelium in five cases, simple cuboidal epithelium in five cases, and two kinds of above mentioned epithelia in eight cases. Goblet cells were present in 23 cases. Surgical excision was performed via the labiogingival groove approach for all patients. The duration of follow-up ranged from 6 months to 10 years. No postoperative recurrences were found.@*CONCLUSION@#The abnormal development of nasolacrimal duct systems may take an important role in the pathogenesis of nasal vestibular cyst. According to this, we considered the total excision via the sublabial approach is more reasonable surgery than endoscopic marsupialization, but it is need further observation.

ABSTRACT

To investigate the reliability and feasibility of human papillomavirus (HPV) DNA test in cervical scraping smears with polymerase chain reaction (PCR), 131 cases of cervical scraping specimens were collected, and the positive rates and accuracy of HPV infection were determined in normal subjects and cervical cancer patients. GP5+/GP6+ and E7 primer pairs designed for detecting HPV L1 and HPV type 16 E7 were tested in this study. Our results showed that positive rates of HPV DNA in normal population and cervical cancer patients were 32.99 % and 73.53 % respectively and there was significant difference between them (P<0. 001). In normal subjects, detection rates of HPV DNA with GP5+/GP6+ and E7 primer pairs were 27.84 % and 16.49 % respectively, with statistically significant difference between them (P>0.05). However the detection rates in cervical cancer patients were 38.24 % and 67.65 % for the two markers, with a significant difference found between them (P<0.05). It is concluded that HPV DNA test with PCR for cervical scraping smears was feasible. GP5+/GP6+ primer pairs may be a useful probe to screen HPV infection in normal population, but they are not sensitive enough in cervical cancer patients. It is suggested that high risk type HPV DNA test was very useful in population with high risk of cervical cancer.

ABSTRACT

Full-length coxsackie adenovirus receptor (CAR) eukaryotic expression plasmid was transfected into an ovarian cell line, SKOV3, and its effect on the change of malignant metastasis phenotype was explored. CAR mRNA and protein expression levels among 4 ovarian cancer cell lines (A2780, SKOV3, SW626, CAOV3) and the positive control 293 (a transformed human embryo kidney cell line) was detected by using semi-quantitative RT-RCR and Western blot and compared. CAR-negative SKOV3 was transfected with the eukaryotic expression plasmid containing a full-length CAR cDNA and mock-vector respectively. The positive clones were screened by G418.The biological behavior changes of positive transfected cells were gauged by colony formation in soft agar assay and cell adhesion assay. Among the cell lines, there were obviously different CAR expression levels. CAR could not be detectedin SKOV3. In transfected cell group, CAR expression was enhanced obviously as compared with non-transfected or mock-transfected groups. Cell adhesion in the transfected group was promoted. The number of colony formation was reduced significantly in transfected groups (25.32±8.91) as compared with that in non-transfected group (88.75±13. 98) and mock-transfected group (82. 53 ±19.37). Among the 4 ovarian cancer cell lines,CAR expression level was variable. Exogenous CAR expression had a potential role in inhibiting the malignant metastasis phenotype of ovary cancer cells.

ABSTRACT

To construct eukaryotic expression vector expressing full length anti-sense pituitary tumor transforming gene (PTTG) mRNA and observe its blocking effect on the potential invasion of human ovarian carcinoma cell line SK-OV-3. PCR primers containing designed enzyme cut sites were used for cloning full-length PTTG gene fragment, and the resulting PCR product was inserted into the eukaryotic vector pcDNA3.1 in the antisense direction. The recombinant vector was then transfected into SK-OV-3 by Lipofectamine. The positive cell clone was screened by G418, PTTG and bFGF at protein level expression were detected by Western blot. The biological behavior change of transfection positive cells was observed by colony formation in soft agar assay. Our results showed that SK-OV-3 clones stably expressing full-length recombinant pcDNA3.1-PTTGas were obtained. The expressions of PTTG and bFGF protein in transfected cells were decreased by 61.5% and 52.3%, respectively as compared with non-transfected ones. The number of colony formation was reduced significantly in transfected cells as compared with empty vector transfected and non-transfected cells. It is concluded that the recombinant vector pcDNA3.1-PTTGas is a novel tool and provides an alternative anti-sense gene therapy targeted at PTTG in human carcinoma.

Methods According to the attributes of physiological bend and specialty of windpipe and gular of image, a horniness endoscopic of "C" model was designed with optical interface, CCD image transducer and small crystal display. It could quickly and visually lead tracheal intubatton through nonnasality. Results It was portable, visual, easily-operated. The rate of successful leading of tracheal intubatton is 97.8% . Conclusion It is especially suited to field battle, first aid, abrupt affairs, etc.
