ABSTRACT

Objective@#To study the effect of atorvastatin combined with Zhibitai in the treatment of coronary heart disease with hyperlipidemia and its influence on the level of high-sensitivity C-reactive protein(hsCRP).@*Methods@#A total of 136 patients with coronary heart disease and hyperlipidemia admitted to the Third People's Hospital of Jingdezhen from September 2016 to September 1818 were randomly divided into control group and combination group using stochastic comprehensive balance method, with 68 cases in each group.The control group was treated with atorvastatin and the combination group was treated with lipidine plus atorvastatin. The patient's treatment effect, including the blood lipid compliance rate and the occurrence of adverse reactions during the treatment were observed, as well as the patient's pre-and post-treatment triglyceride(TG), 12h serum total cholesterol(TC), high-density lipoprotein-cholesterol(HDL-C), low density lipoprotein-cholesterol(LDL-C)and high-sensitivity C-reactive protein(hsCRP)and other indicators were measured.@*Results@#(1)The blood lipid compliance rate(89.86%) in the combined group was higher than that in the control group(59.70%), the difference was statistically significant(χ2=14.915, P=0.000). (2)The incidence of adverse reactions in the combined group(1.45%) was lower than that in the control group(17.91%), the difference was statistically significant(χ2=8.836, P=0.003). (3)The results showed that the TC, TG, LDL-C, hsCRP and other indicators of the combined group were lower than those of the control group and before treatment, while the HDL-C index of the combined groupwas higher than that of the control group and before treatment, the differences were statistically significant(t=20.800, 4.903, 11.657, 4.346, 6.262, P=0.000, 0.000, 0.000, 0.000, 0.000).@*Conclusion@#Zhibitai combined with atorvastatin in the treatment of coronary heart disease patients with hyperlipidemia has remarkable effect.It can accurately control TC, TG, LDL-C, HDL-C and hsCRP, improve blood lipid control rate.

ABSTRACT

The clinical evaluation methods of facial paralysis can be divided into functional evaluation scales,neuro-electrophysiologi-cal tests and computer evaluation systems.The commonly used function evaluation scales include House-Brackmann Grading Scale(HB-GS),Burres-Fisch Facial Nerve Scoring System,Nottingham System,Sunnybrook facial grading System(SFGS),Degree of Facial Nerve Paralysis Hierarchical Scale,Facial Disability Index(FDI)and Facial Clinimetric Evaluation(FaCE)Scale,etc.Neuro-electrophysiological tests mainly consist of facial electromyography (EMG), electroneurography (ENoG), blink reflex (BR), and neural excitatory test (NET), etc.The computer evaluation system based on the sensor is mainly divided into the computer evaluation system based on infrared thermal image technology and the computer evaluation system based on biomedicine image recognition.This article briefly summarized the existing methods of facial paralysis evaluation in terms of sensitivity,stability,accuracy,ease of operation and economics.

ABSTRACT

<p><b>BACKGROUND</b>Pediatric emergency rooms (PERs) in Chinese hospitals are perpetually full of sick and injured children because of the lack of sufficiently developed community hospitals and low access to family physicians. The aim of this study was to evaluate the clinical value of a new five-level Chinese pediatric emergency triage system (CPETS), modeled after the Canadian Triage System and Acuity Scale.</p><p><b>METHODS</b>In this study, we compared CPETS outcomes in our PER relative to those of the prior two-level system. Patients who visited our PER before (January 2013-June 2013) and after (January 2014-June 2014) the CPETS was implemented served as the control and experimental group, respectively. Patient flow, triage rates, triage accuracy, wait times (overall and for severe patients), and patient/family satisfaction were compared between the two groups.</p><p><b>RESULTS</b>Relative to the performance of the former system experienced by the control group, the CPETS experienced by the experimental group was associated with a reduced patient flow through the PER (Cox-Stuart test, t = 0, P < 0.05), a higher triage rate (93.40% vs. 90.75%; χ2 = 801.546, P < 0.001), better triage accuracy (96.32% vs. 85.09%; χ2 = 710.904, P < 0.001), shorter overall wait times (37.30 ± 13.80 min vs. 41.60 ± 15.40 min; t = 11.27, P < 0.001), markedly shorter wait times for severe patients (2.07 [0.65, 4.11] min vs. 3.23 [1.90,4.36] min; z = -2.057, P = 0.040), and higher family satisfaction rates (94.23% vs. 92.21%; χ2 = 321.528, P < 0.001).</p><p><b>CONCLUSIONS</b>Implementing the CPETS improved nurses' abilities to triage severe patients and, thus, to deliver the urgent treatments more quickly. The system shunted nonurgent patients to outpatient care effectively, resulting in improved efficiency of PER health-care delivery.</p>

ABSTRACT

Objective To explore the therapeutic effect of low level laser irradiation (LLLI) on cerebral infraction combined with hypertension.Methods Two groups were divided, conventional treatment group and laser irradiation combined with conventional treatment group.LLLI (650 nm, 20 mW, 20 min, twice a day, two weeks therapy) was used by extravascular way in addition to conventional treatment, while control group employed conventional treatment only.Whole blood viscosity, plasma blood pressure, lipid and neurological function were assessed by comparing the index of the two groups.Results Whole blood viscosity, plasma viscosity, whole blood high shear reductive viscosity, hematokit (HTC), erythrocyte deformation index, erythrocyte rigidity index, fibrinogen and blood lipid level of both groups decreased and the decrease of the testing group was more significant than that of control group (P<0.05 or P<0.01).Neurological deficit score an blood pressure of both groups showed significant decrease (P<0.05), and the decrease in blood pressure of testing group was significant than that of the control group (P<0.05 or P<0.01).Conclusions 650 nm extravascular LLLI may be effective in treatment of cerebral infraction combined with hypertension, and has a good application prospect.

ABSTRACT

Currently, pediatric emergency medicine (PEM) as practiced in many developed countries is different from ours in China. Chinese pediatric emergency medicine is just children's internal medicine and does not include general surgery, ear-nose-throat, etc. If children have an emergency condition that require specialized treatments they need to go to different departments. However in Canada, the pediatric emergency physicians will first treat the patients whatever the condition, then, if it is a complicated sub specialty problem, they will consult the specialist or let the patient see the specialist later. In addition, resuscitation is done in the pediatric intensive care unit (PICU) in China, but it is done in the emergency room in Canada. This article compares the differences in the pediatric emergency systems in Canada and China and also introduces the international standard system of pediatric triage.

ABSTRACT

Chemical constituents of the roots and stem of Salacia hainanensis Chun et How were isolated and purified with column chromatography on silica gel, Sephadex LH-20 and preparative HPLC. Their structures were elucidated based on physicochemical and spectral spectroscopic analysis. Depending on the activities of anti-alpha-glucosidase and inhibiting AGEs (advanced glycation end products, AGEs) formation in vitro, nine compounds were identified as 26, 27-dihydroxy-7, 24-tirucalladien-3-one (1), abruslactone A (2), lupeol (3), 21alpha, 30-dihydroxy-D: A-friedooleanan-3-one (4), 15alpha-hydroxyfriedelan-3-one (5), friedelin (6), mangiferin (7), epicatechin (8) and beta-sitosterol (9), separately. Among them, compound 1 is a new compound, and compound 2 was isolated from the Salacia genus for the first time, while, compounds 3, 4, 5, 8 were obtained from this plant for the first time.

ABSTRACT

<p><b>AIM</b>To observe the effects of urea on ECG and sodium currents of ventricular myocyte in mice.</p><p><b>METHODS</b>ECG and patch clamp techniques were used in the experiments, to record ECG of mice and sodium currents of ventricular myocyte in mice.</p><p><b>RESULTS</b>Urea could lead mice heart rate evidently slow down (P < 0.01) with concentration dependent. The heart rate were (556 +/- 29, 469 +/- 37, 378 +/- 48) b minT in low, middle, high groups respectively before using urea and (612 +/- 27, 615 +/- 23, 619 +/- 26) x min(-1) after. The conduction block arrhythmia was happened in middle and high groups. The sodium currents of ventricular myocyte in mice was inhibited by urea(P < 0.05). The sodium currents amplitude value were reduced to (7.32 +/- 0.68, 5.69 +/- 0.64, 4.58 +/- 0.57) nA after using urea in each group and were (8.76 +/- 0.91, 8.87 +/- 1.01, 8.77 +/- 0.96) nA before, submit concentration dependent.</p><p><b>CONCLUSION</b>Urea can inhibit the sodium currents of ventricular myocyte in mice to make it happen conduction block arrhythmia.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the prevention of hepatolithiasis and biliary stricture post choledochojejunostomy using choledochoscopy technique and evaluate feasibility and efficacy of choledochojejunostomy and artificial valve of efferent loop in preventing reflux.</p><p><b>METHODS</b>To analyze the data of 47 patients with hepatolithiasis who had been operated with Roux-en-Y cholangiojejunostomy and artificial valve of efferent loop to prevent bilio-intestinal regurgitation. Of the patients, 19 were marked with silver nip at the jejunum export of bilio-intestinal anastomosis. The regurgitation, recurrence, anastomotic stricture and their managements after the surgery were investigated.</p><p><b>RESULTS</b>The bilio-intestinal regurgitation were found in 32 cases (32/47, 68.1%), it suggested that artificial valve could not prevent bilio-intestinal regurgitation efficiently. Two cases of hepatolithiasis recurred and were cured by sinus tract placement with the aid of silver nip mark under choledochoscope instead of re-operation. Of the 6 cases with anastomotic stricture, 5 cases were treated successfully with stone extraction, biliary stent dilatation under the percutaneous transhepatic cholangioscopy (PTCS) and the other one case died.</p><p><b>CONCLUSIONS</b>Silver nip mark provides safe and simple path for the cholangioscopy, it made the treatment of the recurrent biliary stricture and stone safe and brief, made the cholangioscopy play more important role in the hepatolithiasis, residual stone and biliary stricture. PTCS is mini-invasive, safe, simple and effective.</p>
