Reflexiones éticas sobre una aplicación científico-médica de actualidad: la distracción osteogénica / Osteogenic Distraction: ethical considerations on this current medical application

Un aspecto importante del progreso científico es el experimento. Por lo tanto, las ciencias médicas tienen que apoyarse en este para incorporar nuevas tecnologías, diagnósticas y terapéuticas a la práctica clínica; validando su seguridad y eficacia en animales y seres humanos. La ética de la experimentación con seres humanos no es sólo una de las áreas fundamentales de la bioética, sino también una de las problemáticas que le dieron origen. Se describen en este trabajo algunas implicaciones éticas y aspectos sociales de la experimentación en seres humanos, particularmente en el marco de la incorporación de una tecnología de punta como es la distracción osteogénica en la cirugía maxilofacial, para la reconstrucción de defectos combinados en tejidos duros y blandos; se analizan el origen y evolución de la bioética, los aspectos regulatorios para la realización de ensayos clínicos y su valoración moral, los tipos de experimentación; la realidad mundial y cubana en cuanto a investigación clínica y tecnología y el consentimiento informado. Se concluye que el criterio ético fundamental de las investigaciones en seres humanos es su disposición al servicio del hombre como fin y nunca como medio.

An important aspect of the scientific progress is the experiment. Therefore, the medical sciences have to lean on this to incorporate new technologies, diagnostic and therapeutic to the clinical practice; validating their security and effectiveness in animals and human beings. The ethics of the experimentation with human being is not only one of the fundamental areas of bioethics, but also one of the problems that brought about its origin. Some ethical implications and social aspects of the experimentation in human beings are described in this work, particularly while talking about the incorporation of a high technology in the ostegenic distraction in maxilofacial surgery for the reconstruction of defects combined in hard and soft fabrics; the origin and evolution of bioethics are analysed, the regulatory aspects for the realization of clinical rehearsals and their moral valuation, the experimentation types; the world and Cuban reality as for clinical investigation and technology and the informed consent. As a conclusion it is said that the fundamental ethical approach of the investigations in human beings is its disposition to the man's service as a goal and never as a way.

Las relaciones ciencia- tecnología- sociedad en el diagnostico temprano del cáncer del pulmón / The science-technology-society relationship point of view in the Lung Cancer early diagnosis

El trabajo constituye un análisis al proyecto “Diagnóstico temprano del cáncer de pulmón. Establecimiento de un algoritmo de trabajo”. El objetivo general es proporcionar un estudio social desde la perspectiva del enfoque Ciencia Tecnología Sociedad y fundamentar el proyecto como un proyecto de innovación tecnológica de tipo organizacional. Se revelan conceptos necesarios, como tecnología en su visión tradicional y moderna y el de innovación tecnológica. Se analiza al cáncer de pulmón como objeto de estudio de la ciencia medica y de acción tecnológica y se valoran las condiciones sociales que en Cuba propician el estudio del cáncer pulmón, y cómo se manifiesta la transferencia de tecnología en esta esfera de la salud. Para ello se concibe el razonamiento a través del contexto social, económico, político y científico; se reconocen los problemas y las soluciones en la investigación. Por último, se identifican los impactos de la innovación tecnológica. Se trata en el presente trabajo de hacer válida la tesis que los estudios CTS en Cuba constituyen un campo de reflexión y acción de carácter crítico e interdisciplinario relacionado con las influencias que cada contexto sociocultural ejerce sobre el desarrollo de la ciencia y la tecnología y los impactos sociales de estos.

This work is an analysis of the project “Early diagnosis of lung cancer. Establishment of a work algorithm”. The general objective is to provide a social study from the perspective of the Science Technology Society approach and to support the project as a project of technological innovation of organizational type. Necessary concepts are revealed, as technology in their traditional and modern vision and that of technological innovation. Lung cancer is analyzed as object of study of medical science and of technological activity. The social conditions that propitiate the study of the cancer lung in Cuba are valued, and how technology transfer is manifested in this area of health. To do so reasoning through the social, economic, political and scientific context is conceived; and problems and solutions are identified in the investigation. Finally, the impacts of technological innovation are identified. This work is intended to make valid the thesis that CTS studies in Cuba are a reflection and action area of critical and interdisciplinary character, related to the influences that each sociocultural context has in the development of science and technology as well as their social impact.

El tratamiento de las fracturas de columna: un estudio de caso ciencia- tecnología- sociedad / Treatment of vertebral column fractures: a study on this science-technology-society issue

Es reconocido el principal significado de los estudios sociales: su incidencia en las áreas educativa, en la política científico-tecnológica y también en la investigación, es en esta última que resalta el hecho de permitir la realización de un tipo de reflexividad que retroalimenta a la propia investigación. El presente trabajo coloca como objetivo determinar las condicionantes sociales del proyecto de investigación “Diagnóstico precoz y tratamiento integral de las fracturas de columna” y también sus impactos desde el punto de vista de la interrelación ciencia, tecnología y sociedad. El trabajo tiene la siguiente estructura, en primer lugar se presenta una breve panorámica del marco teórico de los Estudios sociales en los cuáles se moverá el análisis; en segundo lugar, se reflexionará sobre el significado de la fractura de columna como problema de salud y la correspondiente evolución histórica de la cirugía ortopédica para facilitar una mayor comprensión del proyecto de investigación, para finalmente, proporcionar un análisis de las condiciones sociales que a nivel de país y en particular en la provincia de Camaguey han configurado la necesidad social del proyecto.

The meaning of social studies is recognized: their incidence in the educational and investigative areas, as well as in the scientific-technological politics. It is in the investigative area that stands out the fact of allowing the carrying out of a discussion that gives feedback to the own investigation. This work places as objective to determine the social conditions of the investigation project “Early diagnosis and integral treatment of spine fractures” and also its impacts from the point of view of the interrelation science, technology and society. The work has the following structure; in the first place a brief panoramic of the theoretical foundations of social studies that will propitiate the analysis is presented. Second, the meaning of spine fractures as a health problem and the consequent historical evolution of orthopedic surgery to facilitate a bigger understanding of the investigation project will be analyzed. Finally, a deep analysis on the conditions that has determined the social necessity of this project in Cuba and specifically in Camagüey.
