ABSTRACT

Background: NAFLD is a condition defined by excessive fat accumulation in the form of triglycerides (steatosis) in the liver (> 5% of hepatocytes histologically). Non-alcoholic fatty liver disease is increasingly being recognized as a major cause of liver-related morbidity and mortality among 15-40% of the general population. Aim of the study: To evaluate the clinical profile of patients with non-alcoholic fatty liver disease and its association with metabolic syndrome.Methods:The present cross-sectional, retro-spective study was conducted as outdoor patient basis in the Department of Medicine, Jashore medical college hospital & a private diagnostic centre, Jashore.. A total of 74 cases were included for the study. All patients in the study underwent routine investigations including complete blood counts, blood sugar, liver function tests, HBsAg, anti-HCV, lipid profile andUSG of whole abdomen. The data was collected during OPD treatment and was recorded in predesigned and pretested proforma and analyzed.Results:Mean age of the patient was 53.70�22 years. On physical examination findings showed the mean BMI was 27.6�39 kg/m2, mean waist circumference was 74.22�44 cm. Mean diastolic blood pressure (mm Hg) was 92.87�25 and mean systolic blood pressure (mm Hg) 132.0�.17. Maximum 52% patients had triglycerides >150 mg/dl while low serum HDL level was seen in 37% patients and increased waist circumference was found in 32% patients. Altered ALT ?41 IU was observed in 10 (62.50%) of Grade II of patients with NAFLD with metabolic syndrome. Central obesity was observed in 12 (75.00%) of Grade II patients with NAFLD with metabolic syndrome. While 14 (87.50%) Grade II of patients with NAFLD with metabolic syndrome showed impaired fasting glucose (>110 mg/dl). Hypertriglyceridemia (>150 mg/dl) in 12 (70.58%) seen in Grade I of patients with NAFLD without metabolic syndrome.Conclusion:Higher prevalence of all the components of metabolic syndrome in cases of NAFLD was observed. It can be concluded that symptoms and signs of NAFLD are non-specific and occur later in the course of the disease hence the physician should have a high index of suspicion in order to detect NAFLD early in the course of the disease.

ABSTRACT

The study was undertaken to evaluate milk clotting and proteolytic activity of protein fractions of seeds of Lagenaria siceraria. Protein fractions were isolated by the method of differential solubility, and their protein contents were estimated using Bio-Rad protein assay reagent and bovine serum albumin. The effects of pH and temperature on the milk-clotting and proteolytic activity were also evaluated. The isolated protein fractions showed highest milk clotting activity over a broad temperature range of 30-80 ºC and pH range of 3-9. Based upon the observations, milk clotting enzymes present in different fraction of proteins of Lagenaria siceraria are promising candidates for application in industrial scale for production of cheese and might be a potential substitute for commercial animal rennet.

ABSTRACT

A novel, simple and economic reverse phase high performance liquid chromatography (RP-HPLC) method has been developed for the estimation of Simvastatin in bulk and tablet dosage form with greater precision and accuracy. Separation was achieved on Develosil ODS HG-5 RP C18, (150cmx4.6mm i.d. 5m) column in isocratic mode with mobile phase consisting of acetonitrile :phosphate buffer(pH 3.0) (85:15) with a flow rate of 1 mL/min. The detection was carried out at 236 nm. The retention time of Simvastatin was found to be 5.84 min. The method was validated as per ICH guidelines.Linearity was established for Simvastatin in the range 10 – 100 μg / ml with R2 value 0.99. The percentage recovery of Simvastatin was found to be in the range 99.19-99.67 %. The high recovery and low relative standard deviation confirm the suitability of the proposed method for the estimation of the drug in bulk and tablet dosage forms. The LOD and LOQ were found to be 0.341 and 1.023 μg/ml respectively.Validation studies demonstrated that the proposed RP-HPLC method is simple, specific, rapid, reliable and reproducible for the determination of Simvastatin for Quality Control level.

ABSTRACT

Chronic calculous cholecystitis is quite a common disease entity today. However, advances in diagnostic imaging and use of broad-spectrum antibiotics have facilitated its early detection and treatment. Hence complications of untreated calculous cholecystitis have shown a steady decline. Spontaneous cholecystocutaneous fistula is one such rare complication. A 45-year-old female presented with acute onset of pain and swelling in the right hypochondrium. She had leucocytosis. The swelling ruptured spontaneously and 200 ml frank yellow pus along with multiple stones were expelled from the site. Completion drainage was done. A fistulogram confirmed the diagnosis of a cholecystocutaneous fistula. The patient underwent open cholecystectomy and excision of the fistulous tract.

ABSTRACT

Hydrocele of the canal of Nuck is an uncomon and infrequently reported finding in women. A 24-year-old female presented with a painless left groin swelling which was soft, cystic, non-tender with 7.5 cm x 5 cm in measurement. It was more prominent in standing. Ultrasonography revealed an encysted echofree lesion in the left inguinal canal. On surgery, a cyst was found containing clear fluid in the inguinal canal which was dissected out and wound was closed in layers. The patient is doing well till date. Here one such case with diagnosis of hydrocele of the canal of Nuck is reported.
