Resumo A temperatura do ar é um fator climático que afeta a incidência da dengue, com efeitos variando conforme o tempo e o espaço. Investigamos a relação entre a temperatura mínima do ar e a incidência da doença em Minas Gerais, Brasil, e avaliamos a influência de variáveis socioeconômicas e geográficas nessa relação, calculando-se o risco relativo (RR). Este é um estudo de série temporal com análise conduzida em três etapas distintas: modelagem por uso de distributed lag non-linear model (modelos não-lineares distributivos com defasagem), metanálise dos modelos obtidos e metarregressão com dados geográficos e socioeconômicos. A temperatura mínima foi um fator de proteção quando em temperaturas frias extremas (RR = 0,65; IC95%: 0,56-0,76) e moderadas (RR = 0,71; IC95%: 0,64-0,79) e fator de risco em temperaturas de calor moderado (RR = 1,15; IC95%: 1,07-1,24), mas não em extremo (RR = 1,1; IC95%: 0,99-1,22). A heterogeneidade dos modelos foi elevada (I2 = 60%) e essa medida não foi alterada em metarregressão. Temperaturas frias moderadas e extremas causam efeito protetivo, enquanto moderadas quentes aumentam o risco. No entanto, a temperatura mínima do ar não explica nem a variabilidade da região, nem mesmo com as outras variáveis em metarregressão.


Abstract Air temperature is a climatic factor that affects the incidence of dengue, with effects varying according to time and space. We investigated the relationship between minimum air temperature and dengue incidence in Minas Gerais, Brazil, and evaluated the influence of socioeconomic and geographic variables on this relationship. This is a time series study with analysis conducted in three distinct stages: modeling using a distributed lag non-linear model, meta-analysis of models obtained, and meta-regression with geographic and socioeconomic data. Minimum temperature was a protective factor at extreme cold temperatures (RR = 0.65; 95%CI: 0.56-0.76) and moderate cold temperatures (RR = 0.71; 95%CI: 0.64-0.79), and a risk factor at moderate hot temperatures (RR = 1.15; 95%CI: 1.07-1.24), but not at extreme hot temperatures (RR = 1.1; 95%CI: 0.99-1.22). Heterogeneity of the models was high (I2 = 60%), which was also observed in meta-regression. Moderate and extreme cold temperatures have a protective effect, while moderate hot temperatures increase the risk. However, minimum air temperature does not explain the variability in the region, not even with the other variables in meta-regression.


Resumen La temperatura del aire es un factor climático que afecta la incidencia del dengue, con efectos que varían según el tiempo y el territorio. Investigamos la relación entre la temperatura mínima del aire y la incidencia de la enfermedad en Minas Gerais, Brasil, y evaluamos la influencia de variables socioeconómicas y geográficas en esta relación. Se trata de un estudio de serie temporal cuyo análisis se realiza en tres etapas distintas: modelación mediante el uso de distributed lag non-linear model (modelos distributivos no lineales con retraso), metaanálisis de los modelos obtenidos y metarregresión con datos geográficos y socioeconómicos. La temperatura mínima fue un factor de protección ante temperaturas extremadamente frías (RR = 0,65; IC95%: 0,56-0,76) y moderadas (RR = 0,71; IC95%: 0,64-0,79) y factor de riesgo en temperaturas de calor moderado (RR = 1,15; IC95%: 1,07-1,24), pero no en extremo (RR = 1,1; IC95%: 0,99-1,22). La heterogeneidad de los modelos fue alta (I2 = 60%), y esta medida no se modificó en la metarregresión. Las temperaturas frías moderadas y extremas tienen un efecto protector, mientras que las temperaturas moderadamente altas aumentan el riesgo. Sin embargo, la temperatura mínima del aire no explica la variabilidad de la región, ni siquiera con las demás variables en metarregresión.

ABSTRACT Background: The riverine communities of the Amazon comprise different social groups that inhabit the rural areas on the banks of rivers and lakes. Residents usually travel by river to rural and urban areas and are then exposed to urbanized diseases such as those caused by arbovirus infection. In Brazil, emerging diseases such as dengue, Zika, chikungunya, and those caused by infection with Oropouche and Mayaro viruses necessitate epidemiological surveillance. This study was aimed at determining the frequency of positivity for immunoglobulin (Ig)G and IgM antibodies against Zika, chikungunya, and dengue viruses and performing molecular analyses to detect viral RNA for the Zika, chikungunya, dengue virus, Oropouche, and Mayaro viruses, in the same serum samples obtained from riverside populations. Methods: This cross-sectional study was conducted in a riverside population in the Humaitá municipality of the Brazilian Amazon. More than 80% of the local population participated in this study. Entomological samples were collected to identify local mosquito vectors. Results: Analysis of 205 human serological samples revealed IgG antibodies against the dengue virus in 85 individuals. No molecular positivity was observed in human samples. Entomological analyses revealed 3,187 Diptera species, with Mansonia being the most frequent genus. Aedes aegypti and Aedes albopictus were not detected in the two collections. Conclusions: IgG antibodies against the dengue virus were highly prevalent, suggesting previous exposure. The absence of the arbovirus vectors Aedes aegypti and Aedes albopictus in the samples supports the hypothesis that the infections recorded likely occurred outside the riverside communities investigated.

Abstract Objectives: To determine the cut-off point of the cochlear radiation dose as a risk factor for hearing loss in patients with vestibular schwannoma treated with radiosurgery. Methods: A systematic review of the literature was performed without language or publication year restrictions in the MEDLINE/PubMed, EMBASE, Web of Science, LILACS/VHL and Cochrane Library databases. Studies that met the following criteria were included: 1) population: adults of both sexes who underwent radiosurgery for vestibular schwannoma treatment; 2) exposure: cochlear radiation; 3) outcome: hearing loss; 4) type of study: cohort. Two independent reviewers conducted the entire review process. The registration number in PROSPERO was CRD42020206128. Results: From the 333 articles identified in the searches, seven were included after applying the eligibility criteria. There was no standardization as to how to measure exposure or outcome in the included studies, and most studies did not present sufficient data to enable meta-analysis. Conclusion: It was not possible to determine a cut-off point for high cochlear dose that could be considered a risk factor for hearing loss.

Introdução: Dashboards são ferramentas interativas de visualização de dados que simplificam grandes quantidades de informações. No contexto da saúde, eles são utilizados em diversas situações como em dados relativos ao câncer no país, ainda que, normalmente, haja disponibilidade somente para análise de Estados e Capitais. Objetivo: Descrever a construção de um dashboard relacionado ao câncer na Macrorregião de Saúde Sudeste de Minas Gerais (MSSMG). Método: Construção passo a passo de um dashboard com uso de código aberto e sua utilização para análise descritiva de dados relacionados à mortalidade por neoplasias malignas e exames de triagem para o câncer de mama e do colo do útero na MSSMG entre os anos de 2010-2019. Resultados: Foi produzido um dashboard com dados oficiais sobre a mortalidade por câncer, cobertura de exames preventivos e variáveis socioeconômicas, que permite uma análise extensa sobre o tema na Região. Com o uso dessa ferramenta, verificou-se que a mortalidade por câncer entre homens é elevada e o câncer de pulmão foi a principal causa de morte. Ainda foi possível analisar visualmente a distribuição espacial da taxa de mortalidade padronizada e da cobertura média do período dos exames de citologia do colo do útero e da mamografia. Conclusão: Criação de um dashboard abrangente sobre câncer na MSSMG e compartilhamento dos resultados obtidos. O painel demonstrou versatilidade ao simplificar processos estatísticos e visualizações, facilitando a formulação de ideias, hipóteses, e oferecendo informações claras e intuitivas


Introduction: Dashboards are interactive data visualization tools that simplify large amounts of information. In the context of health, they are used in different settings, such as cancer related data in the country, although, usually, only analyzes of States and Capitals are available. Objective: Describe the construction of a cancer-related dashboard in the Southeast Health Macroregion of Minas Gerais (MSSMG). Method:Step-by-step study of the construction of an open-source code dashboard and its utilization for descriptive analysis of data related to mortality by malignant neoplasms and breast and cervical cancer screening in MSSMG between 2010 and 2019. Results: A dashboard was produced with official data on cancer mortality, coverage of preventive exams and socioeconomic assessment, which allows an extensive analysis for the region. Higher mortality by cancer in men was found with the utilization of this tool, and that lung cancer was the main cause of death. It was also possible to visually analyze the spatial distribution of the standardized mortality rate and the mean coverage of cervical cytology and mammography exams in the period. Conclusion: Creation of a comprehensive dashboard on cancer at MSSMG and sharing the results. The panel has been shown to be versatile by simplifying statistical processes and visualization, facilitating the formulation of ideas, possibilities and offering clear and intuitive information


Introducción: Dashboards son herramientas interactivas de visualización de datos que simplifican grandes cantidades de información. En el ámbito de la salud, se utilizan en diferentes contextos, como datos relacionados con el cáncer en el país, aunque, normalmente, solo se dispone de análisis de Estados y Capitales. Objetivo: Exponer la construcción de un panel de control relacionado con el cáncer en la Macrorregión de salud del Sudeste de Minas Gerais (MSSMG). Método: Construcción paso a paso de un dashboard usando código abierto y el uso de esta herramienta para el análisis descriptivo de datos relacionados con la mortalidad por neoplasias malignas y los exámenes de detección de cáncer de mama y cuello uterino en la MSSMG entre los años 2010-2019. Resultados: Se elaboró un dashboard con datos oficiales de mortalidad por cáncer, cobertura de exámenes preventivos y evaluaciones socioeconómicas, que permite un análisis amplio sobre el tema en la región. Con el uso de esta herramienta se percibió que existe una mayor mortalidad por cáncer entre los hombres y que el cáncer de pulmón fue la principal causa de muerte. También fue posible analizar visualmente la distribución espacial de la tasa de mortalidad estandarizada y la cobertura media del período de los exámenes de citología cervical y mamografía. Conclusión: Creación de un dashboard integral sobre cáncer en la MSSMG y intercambio de los resultados obtenidos. El panel demostró versatilidad al simplificar procesos estadísticos y visualizaciones, facilitando la formulación de ideas, posibilidades y ofreciendo información clara e intuitiva.Palabras clave: epidemiología; neoplasias; análisis espacial; salud pública; registros de mortalidad

ABSTRACT Objective: To evaluate the association between the risk of death from COPD and air temperature events in ten major Brazilian microregions. Methods: This was a time series analysis of daily COPD deaths and daily mean air temperatures between 1996 and 2017. Using distributed nonlinear lag models, we estimated the cumulative relative risks of COPD mortality for four temperature percentiles (representing moderate and extreme cold and heat events) in relation to a minimum mortality temperature, with a lag of 21 days, in each microregion. Results: Significant associations were found between extreme air temperature events and the risk of death from COPD in the southern and southeastern microregions in Brazil. There was an association of extreme cold and an increased mortality risk in the following microregions: 36% (95% CI, 1.12-1.65), in Porto Alegre; 27% (95% CI, 1.03-1.58), in Curitiba; and 34% (95% CI, 1.19-1.52), in São Paulo; whereas moderate cold was associated with an increased risk of 20% (95% CI, 1.01-1.41), 33% (95% CI, 1.09-1.62), and 24% (95% CI, 1.12-1.38) in the same microregions, respectively. There was an increased COPD mortality risk in the São Paulo and Rio de Janeiro microregions: 17% (95% CI, 1.05-1.31) and 12% (95% CI, 1,02-1,23), respectively, due to moderate heat, and 23% (95% CI, 1,09-1,38) and 32% (95% CI, 1,15-1,50) due to extreme heat. Conclusions: Non-optimal air temperature events were associated with an increased risk of death from COPD in tropical and subtropical areas of Brazil.


RESUMO Objetivo: Avaliar a associação entre o risco de morte por DPOC e eventos de temperatura do ar em dez grandes microrregiões brasileiras. Métodos: Esta foi uma análise de série temporal de mortes diárias por DPOC e temperaturas médias diárias do ar entre 1996 e 2017. Utilizando modelos de defasagem não linear distribuídos, estimamos os riscos relativos cumulativos de mortalidade por DPOC para quatro percentis de temperatura (representando eventos moderados e extremos de frio e calor) em relação a uma temperatura mínima de mortalidade, com defasagem de 21 dias, em cada microrregião. Resultados: Foram encontradas associações significativas entre eventos extremos de temperatura do ar e o risco de morte por DPOC nas microrregiões Sul e Sudeste do Brasil. Houve associação de frio extremo e aumento do risco de mortalidade nas seguintes microrregiões: 36% (IC 95%, 1,12-1,65), em Porto Alegre; 27% (IC 95%, 1,03-1,58), em Curitiba; e 34% (IC 95%, 1,19-1,52), em São Paulo; enquanto o frio moderado foi associado a um risco aumentado de 20% (IC 95%, 1,01-1,41), 33% (IC 95%, 1,09-1,62) e 24% (IC 95%, 1,12-1,38) nas mesmas microrregiões, respectivamente. Houve aumento do risco de mortalidade por DPOC nas microrregiões de São Paulo e Rio de Janeiro: 17% (IC 95%, 1,05-1,31) e 12% (IC 95%, 1,02-1,23), respectivamente, devido ao calor moderado e 23% (IC 95%, 1,09-1,38) e 32% (IC 95%, 1,15-1,50) devido ao calor extremo. Conclusões: Eventos de temperatura do ar não ideal foram associados a um risco aumentado de morte por DPOC em áreas tropicais e subtropicais do Brasil.

Abstract Objective Pain is the primary limitation to performing hysteroscopy. We aimed to evaluate the predictive factors of low tolerance to office hysteroscopic procedures. Methods Retrospective cohort study of the patients who underwent office hysteroscopy from January 2018 to December 2020 at a tertiary care center. Pain tolerance to office-based hysteroscopy was subjectively assessed by the operator as terrible, poor, moderate, good, or excellent. Categorical variables were compared with the use of the Chi-squared test; an independent-samples t-test was conducted to compare continuous variables. Logistic regression was performed to determine the main factors associated with low procedure tolerance. Results A total of 1,418 office hysteroscopies were performed. The mean age of the patients was 53 ± 13.8 years; 50.8% of women were menopausal, 17.8% were nulliparous, and 68.7% had a previous vaginal delivery. A total of 42.6% of women were submitted to an operative hysteroscopy. Tolerance was categorized as terrible or poor in 14.9% of hysteroscopies and moderate, good, or excellent in 85.1%. A terrible or poor tolerance was more frequently reported in menopausal women (18.1% vs. 11.7% in premenopausal women, p = 0.001) and women with no previous vaginal delivery (18.8% vs. 12.9% in women with at least one vaginal birth, p = 0.007). Low tolerance led more often to scheduling a second hysteroscopic procedure under anesthesia (56.4% vs. 17.5% in reasonable-to-excellent tolerance, p < 0.0005). Conclusion Office hysteroscopy was a well-tolerated procedure in our experience, but menopause and lack of previous vaginal delivery were associated with low tolerance. These patients are more likely to benefit from pain relief measures during office hysteroscopy.

Abstract The purpose of this study was to evaluate the effect of dentin moisture (moist and dry) on the bonding of fiber posts to root dentin with different adhesive strategies (etch-and-rinse, self-etch, and self-adhesive). Seventy-two extracted single-rooted human teeth were endodontically treated and divided into six groups (n = 12) according to the moisture of dentin surface and adhesive systems as follows: a) etch-and-rinse/moist, b) etch-and-rinse/dry, c) self-etch/moist, d) self-etch/dry, e) self-adhesive/moist, and 6) self-adhesive/dry. The specimens were sectioned into six slices for push-out bond strength (BS), nanoleakage (NL) by SEM, and Vickers microhardness (VHN) of the resin cement. A universal testing machine (AG-I, Shimadzu Autograph) was used at a crosshead speed of 0.5 mm/min until post extrusion, with a load cell of 50 kg for evaluation of the push-out strength. Data on BS, NL, and VHN were evaluated by two-way ANOVA and Tukey's test (α = 0.05). Dentin moisture as the main factor was not significantly different for the push-out test. However, higher BS values can be observed for the etch-and-rinse group. A lower percentage of NL was found in the dry dentin groups. The moisture pattern was not significant in the hardness values for the pre-etching groups. Additional moisture did not increase the evaluated properties.

Abstract Objective: Oral Squamous Cell Carcinoma (OSCC) is conventionally treated by surgical resection, and positive surgical margins strongly increase local recurrence and decrease survival. This study aimed to evaluate whether a Three-Dimensional Segmentation (3DS) image of OSCC confers advantage over Multiplanar Reconstruction (MPR) of OSCC using images of computed tomography scan in surgical planning of tumor resection. Methods: Twenty-six patients with locally advanced OSCC had tumor morphology and dimensions evaluated by MPR images, 3DS images, and Surgical Pathology Specimen (SPS) analyses (gold standard). OSCC resection was performed with curative intent using only MPR images. Results: OSCC morphology was more accurately assessed by 3DS than by MPR images. Similar OSCC volumes and dimensions were obtained when MPR images, 3DS images and SPS measurements were considered. Nevertheless, there was a strong correlation between the OSCC longest axis measured by 3DS and SPS analyses (ICC = 0.82; 95% CI 0.59-0.92), whereas only a moderate correlation was observed between the longest axis of OSCC measured by MPR images and SPS analyses (ICC = 0.51; 95% CI 0.09-0.78). Taking only SPS with positive margins into account, MPR images and 3DS images underestimated the tumor's longest axis in eight out of 11 (72.7%) and 5 out of the 11 (45.5%) cases, respectively. Conclusion: Our data present preliminary evidence that 3DS model represents a useful tool for surgical planning of OSCC resection, but confirmation in a larger cohort of patients is required. ABSTRACT Introduction: Among the main challenges faced by coaches of team sports are to create an environment and conditions that will enable players to reach their optimal level of physical performance at the start of the competitive season, and to maintain this level throughout the season. Objective: The purpose of this study was to assess the effects of six weeks of additional training with 3-against-3 (3v3) small-sided games (SSG) on the physical performance of elite nonstarter basketball players. Methods: Eleven professional basketball players (five starters and six nonstarters) performed the same training program throughout the regular season, with the only difference that after each game, the starters (S) performed low intensity shooting drills while the nonstarters (NS) performed half-court 3v3 SSG. At the start and end of the six-week period, the athletes were assessed for muscle power, speed, agility and aerobic power. Two-way analysis of variance and effect sizes were used. Magnitude-based inferential analyses were used to complement the parametric tests. Results: After the six weeks, the S and NS showed improvement (p<0.05) in the squat jump (S=5.8% and NS=7.1%), counter movement jump (S=3.7% and NS=3.6%), 20m-sprint (S=3.4% and NS=2.0%), agility T test (S=3.6% and NS=3.1%) and Yo-yo intermittent recovery (S=5.2% and NS=2.5%). No differences were found between groups and moments. The effect sizes showed moderate effect on agility and little effect on jumping ability, for both groups; moderate, for S, and little, for NS, on speed and aerobic conditioning. Magnitude-based inferences indicated that S were likely to outperform NS in terms of improvement in 20-m sprint ability and possibly also in agility. In regard to the other physical performance variables, the analyses of magnitude-based inferences were inconclusive. Conclusions: The results of this study showed that 3v3 SSG in the short-term on half-court, as an additional training routine for NS, can bring positive results for the physical performance for these players. RESUMEN Introducción: Uno de los principales desafíos de los entrenadores de deportes colectivos es crear ambiente y las condiciones que permitan a los jugadores alcanzar su nivel ideal de desempeño físico al inicio de la temporada competitiva y mantener ese nivel a lo largo de la temporada. Objetivos: El objetivo de este estudio fue evaluar los efectos de seis semanas de entrenamiento adicional con juegos reducidos (JR) 3 contra 3 (3x3) sobre el desempeño físico de jugadores suplentes del baloncesto de élite. Métodos: Once jugadores profesionales (cinco titulares y seis suplentes) realizaron el mismo programa de entrenamiento durante la temporada regular, con la única diferencia de que, después de cada partido, los titulares (T) realizaban ejercicios de lanzamiento de baja intensidad, mientras que los suplentes (S) realizaban JR 3x3 en media cancha. Al inicio y al final de cada período de seis semanas los atletas fueron evaluados en cuanto a la potencia muscular, velocidad, agilidad y potencia aeróbica. Fueron usados análisis de variancia y tamaños de efecto de dos vías. Fueron usados análisis inferenciales basados en magnitud para complementar los tests paramétricos. Resultados: Después de seis semanas, el grupo T y el S mostraron mejora (p <0,05) en el salto con agachamiento (T = 5,8% y S = 7,1%), salto contramovimiento (T = 3,7% y S = 3,6%), sprint de 20 metros (T = 3,4% y S = 2,0%), test T de agilidad (T = 3,6% y S = 3,1%) y el test de recuperación intermitente Yo-yo (T = 5,2% y S = 2,5%). No fueron encontradas diferencias entre grupos y momentos. Los tamaños de efecto mostraron efecto moderado en la agilidad y pequeño efecto en la capacidad de salto para ambos grupos; efecto moderado para el grupo T y pequeño para el S, en la velocidad y acondicionamiento aeróbico. Las inferencias basadas en la magnitud indicaron que el grupo T probablemente superaría al grupo S en términos de mejora de la habilidad de sprint de 20 metros y posiblemente también en agilidad. El análisis de las inferencias basadas en magnitud no fue concluyente con relación a las otras variables. Conclusiones: Los resultados de este estudio mostraron que JR 3x3 en media cancha como entrenamiento adicional para el grupo S puede ser positivo para el desempeño físico de esos jugadores. RESUMO Introdução: Um dos principais desafios dos treinadores de esportes coletivos é criar ambiente e as condições que permitam aos jogadores atingir seu nível ideal de desempenho físico no início da temporada competitiva e manter esse nível ao longo da temporada. Objetivos: O objetivo deste estudo foi avaliar os efeitos de seis semanas de treinamento adicional com jogo reduzido (JR) 3 contra 3 (3x3) sobre o desempenho físico de jogadores reservas do basquetebol de elite. Métodos: Onze jogadores profissionais de basquetebol (cinco titulares e seis reservas) realizaram o mesmo programa de treinamento durante a temporada regular, com a única diferença de que, depois de cada jogo, os titulares (T) realizavam exercícios de arremesso de baixa intensidade, enquanto os reservas (R) realizavam JR 3x3 em meia quadra. No início e no final do período de seis semanas, os atletas foram avaliados quanto à potência muscular, velocidade, agilidade e potência aeróbica. Foram usadas análises de variância e tamanhos de efeito de duas vias. As análises inferenciais baseadas em magnitude foram usadas para complementar os testes paramétricos. Resultados: Depois das seis semanas, o grupo T e o R mostraram melhora (p < 0,05) no salto com agachamento (T = 5,8% e R = 7,1%), salto contramovimento (T = 3,7% e R = 3,6%), sprint de 20 metros (T = 3,4% e R = 2,0%), teste T de agilidade (T = 3,6% e R = 3,1%) e o teste de recuperação intermitente Yo-yo (T = 5,2% e R = 2,5%). Não foram encontradas diferenças entre grupos e momentos. Os tamanhos de efeito mostraram efeito moderado na agilidade e pequeno efeito na capacidade de salto para ambos os grupos; efeito moderado para o grupo T e pequeno para o R na velocidade e condicionamento aeróbico. Inferências baseadas na magnitude indicaram que o grupo T provavelmente superaria o grupo R em termos de melhora da habilidade de sprint de 20 m e possivelmente também em agilidade. A análise das inferências baseadas em magnitude foi inconclusiva em relação às outras variáveis. Conclusões: Os resultados deste estudo mostraram que o JR 3x3 em meia quadra como treinamento adicional para o grupo R pode ser positivo para o desempenho físico desses jogadores. ABSTRACT

A hemofilia é uma doença hemorrágica hereditária ligada ao cromossomo X, decorrente da ausência ou da baixa quantidade no plasma dos fatores de coagulação VIII (hemofilia A) ou IX (hemofilia B). Clinicamente, a hemofilia se caracteriza por episódios recorrentes de sangramentos, que podem ocorrer espontaneamente ou em decorrência de traumas. O sistema músculo-esquelético é freqüentemente afetado pelos eventos hemorrágicos nos indivíduos portadores de hemofilia. Dentro desse sistema, as articulações são alvos de episódios recorrentes de sangramentos, resultando no fenômeno a que chamamos de hemartrose, responsável por estimular a hiperplasia da membrana sinovial articular e caracterizando o quadro das sinovites hemofílicas. O ciclo vicioso típico de hemartrose-sinovite-hemartrose quando estabelecido, tem como consequência a cronificação das alterações agudas articulares. Histologicamente, na sinovite, é observada a proliferação de fibroblastos sinoviais e a presença de um infiltrado de células inflamatórias. Apesar de a articulação temporomandibular (ATM) ser uma articulação do tipo sinovial, são poucos os relatos sobre o seu envolvimento na hemartrose e na sinovite hemofílica. Em razão disso, o objetivo deste estudo foi o de avaliar a ATM por meio de imagens de ressonância magnética (RM) utilizando-se a técnica denominada relaxometria. A relaxometria, também chamada de mapeamento T2, consiste em calcular as variações na intensidade do sinal de radiofrequência (RF) emitido pelos prótons das moléculas de água em função da variação do tempo quando excitados por um pulso externo de RF e submetidos à influência de um campo magnético estático. Esta mensuração é possível de ser realizada por meio da chamada sequência multieco de spin. A literatura científica descreve que a concentração de ferro nos tecidos resulta em encurtamento do tempo T2 e em consequente diminuição da intensidade de sinal das amostras biológicas. Desta forma, a relaxometria realizada com o objetivo de quantificar a concentração de ferro pode servir como uma forma de se estudar as propriedades intrínsecas dos tecidos e o ambiente molecular das estruturas analisadas. Foram selecionados pacientes maiores de 18 anos de idade, comprovadamente portadores de hemofilia A ou B classificadas como grave e que tiveram seguimento regular ambulatorial. O grupo controle foi pareado por idade e sexo com os pacientes, sendo representado por voluntários sem histórico de discrasia sanguínea ou disfunção temporomandibular (DTM). Foram avaliados 39 indivíduos (18 pacientes e 21 controles). Os critérios de exclusão foram: pacientes portadores de próteses metálicas, marca-passo cardíaco, clipes metálicos intracranianos (para aneurisma), claustrofobia severa e não consentimento à participação no estudo. Este estudo observou que a morfologia dos discos articulares se altera com o passar do tempo em indivíduos portadores de hemofilia grave (p-valor = 0,006), entretanto, nenhuma diferença estatística foi observada no que diz respeito à intensidade de sinal calculada em 4 momentos distintos (p-valor > 0,05), sugerindo não haver uma concentração maior de ferro no interior do disco articular na amostra estudada, nem entre os grupos e nem na transição entre os momentos avaliados. Também não foi observada diferença estatística (p-valor > 0,05) no que diz respeito aos sintomas relatados (sintomatologia dolorosa, cefaleia e alteração no movimento de abertura bucal e lateralidade da mandíbula) entre indivíduos portadores de hemofilia e indivíduos do grupo controle, sugerindo que a ATM possui uma forte capacidade de adaptação em comparação com as demais articulações do tipo sinovial diante dos efeitos provocados pela degradação da cartilagem articular.

ABSTRACT

Objectives: Evaluate the agreement between the clinical and histopathological diagnosis of oral lesions in patients submitted to biopsies. Identify the most frequent oral lesions and their correlation with age, gender, and anatomical location. Methods: A retrospective study of 368 pathological examinations collected between 2008 and 2018, corresponding to biopsies performed at the Clínica Universitária Egas Moniz. A detailed analysis of the histopathological reports attached to the patients' files was made and the variables gender, age, anatomical site, clinical diagnosis, and histopathological diagnosis was evaluated. Results: The most affected gender was female (55%); the most common age group was 61-70 years old; The most biopsied anatomical location was the gum (23.9%); the five most common pathological entities were fibroma(26.4%),root cyst(8.7%),oral lichen planus(7.6%), hemangioma (6.3%) and oral leukoplakia (6.0%). On agreement, 74.5% of the cases were concordant and 25.5% discordant. The most concordant lesions were Radicular Cyst (90.6%), Traumatic Injury (87.5%), Hemangioma (82.6%), Fibroma (82.5%) and Mucocele (82.5%). Conclusion: this study proves a significant level of agreement between clinical and histopathological diagnosis in this particular area, consistently obtained in a ten years period of time.


Objetivos: Evaluar el nivel de acuerdo entre el diagnóstico clínico e histopatológico de lesiones orales en pacientes sometidos a biopsias. Identificar las lesiones orales más frecuentes y su correlación con la edad, el sexo y la ubicación anatómica. Métodos: Estudio retrospectivo de 368 exámenes patológicos recogidos entre 2008 y 2018, correspondientes a biopsias realizadas en la Clínica Universitária Egas Moniz. Se realizó un análisis detallado de los informes histopatológicos adjuntos a los archivos de los pacientes y se evaluaron las variables de género, edad, sitio anatómico, diagnóstico clínico y diagnóstico histopatológico. Resultados: El género más afectado fue femenino (55%); el grupo de edad más común fue de 61-70 años; La ubicación anatómica más biopsiada fue la encía (23,9%); las cinco entidades patológicas más comunes fueron fibroma (26,4%), quiste radicular (8,7%), liquen plano oral (7,6%), hemangioma (6,3%) y leucoplasia oral (6,0%). Según el grado de acuerdo, el 74,5% de los casos fueron concordantes y el 25,5% discordantes. Las lesiones más concordantes fueron Quiste Radicular (90,6%), Lesión Traumática (87,5%), Hemangioma (82,6%), Fibroma (82,5%) y Mucocele (82,5%). Conclusión: este estudio demuestra un nivel significativo de acuerdo entre el diagnóstico clínico e histopatológico en esta área en particular, obtenido consistentemente en un período de diez años de tiempo

Abstract The purpose of this study was to analyze the effect of the application of different concentrations of EDTA on the adhesion of fiber posts to root dentin using self-adhesive resin cements. After endodontic treatment, 78 single roots were randomly divided into six groups (n = 13) according to the combination of the following factors: surface dentin treatment - control (distilled water), 17% EDTA and 24% EDTA; and self-adhesive resin cement - RelyX U200 (RX); and Multilink Speed (ML). After fiber post cementation, six slices were obtained for each root. Ten roots of each group were used for bond strength (BS) and three for microhardness (MH) evaluations. Data obtained from BS and MH tests for each resin cement were subjected to two-way ANOVA (surface treatment vs. root region) and to a post-hoc Tukey's test (α = 0.05). The lowest BS value was observed in the 24% EDTA group for RX cement, whereas the highest values for ML cement were observed for the control group in the middle and apical regions. In the MH test, the lowest value for RX was observed for 24% EDTA in the cervical region, whereas and the highest value for the ML cement was observed in the control group. Regarding both self-adhesive resin cements tested, the application of 24% EDTA was not able to improve the adhesion of fiber posts to root canal.

ABSTRACT

Objective: Forensic dentistry has used some methods for the human identification process. However, there is a need to study characteristics that are able to perform identification more specifically, increasing accuracy. Considering the role of dental arch assessment in prediction of gender and ethnicity, the purpose of this study was to assess the volume of pulp chamber as a mean to obtain new forensic evidence. Material and methods: For this task, 1.190 cone beam computed tomography (CBCT) images were retrospectively selected and subdivided according to the population number, gender, age and ethnicity. All DICOM files were imported to the open-source software ITK-SNAP®(http://www.itksnap.org/pmwiki/pmwiki.php). The segmentation process was performed in all pulp chamber aiming to obtain pulp tissue ́s volume. Results: As a result, the pulp chamber volume of mandibular canine teeth showed to be larger in white population than in non-white (P-value = 0.003) and in male individuals in comparison with female (P-value = 0.038). Conclusion: These results, however, must be confirmed by future studies with a larger sample size and by the assessment of other variables, including texture analysis and density of dental tissues. Objetivo: A odontologia forense tem usado alguns métodos para o processo de identificação humana. Porém, há a necessidade de estudar características que consigam de forma mais específica realizar a identificação aumentando a precisão. Considerando o papel da avaliação da arcada dentária na predição de gênero e etnia, o objetivo deste estudo foi avaliar o volume da câmara pulpar mandibular como meio de obtenção de novas evidências periciais. Material e métodos: Foram selecionadas 1.190 imagens de tomografia computadorizada de feixe cônico (TCFC) e subdivididas de acordo com o número da população, sexo, idade e etnia. Todos os arquivos em formato DICOM foram importados para o software de código aberto ITK-SNAP® (http://www.itksnap.org/pmwiki/pmwiki.php) onde o processo de segmentação foi realizado em todas as câmaras pulpares visando a obtenção da volumetria. Resultados: Como resultado, o volume da câmara pulpar dos caninos inferiores mostrou-se maior na população branca do que na não branca (P-valor = 0,003), no sexo masculino em comparação ao feminino (P-valor = 0,038). Conclusão: Esses resultados, entretanto, devem ser confirmados por estudos futuros com maior tamanho amostral e pela avaliação de outras variáveis, incluindo análise de textura e densidade dos tecidos dentários. Abstract Introduction The frontal sinus (FS) is the most complex of the paranasal sinuses due to its location, anatomical variations and multiple clinical presentations. The surgical management of the FS and of the frontal recess (FR) is technically challenging, and a complete understanding of its anatomy, radiology, main diseases and surgical techniques is crucial to achieve therapeutic success. Objectives To review the FS and FR anatomy, radiology, and surgical techniques. Data Synthesis The FS features a variety of anatomical, volumetric and dimensional characteristics. From the endoscopic point of view, the FR is the point of greatest narrowing and, to have access to this region, one must know the anatomical limits and the ethmoid cells that are located around the FR and very often block the sinus drainage. Benign diseases such as chronic rhinosinusitis (CRS), mucocele and osteomas are the main pathologies found in the FS; however, there is a wide variety of malignant tumors that can also affect this region and represent a major technical challenge to the surgeon. With the advances in the endoscopic technique, the vast majority of diseases that affect the FS can be treated according to Wolfgang Draf, who systemized the approaches into four types (I, IIa, IIb, III). Conclusion Both benign and malignant diseases that affect the FS and FR can be successfully managed if one has a thorough understanding of the FS and FR anatomy, an individualized approach of the best surgical technique in each case, and the appropriate tools to operate in this region.

Resumo Objetivo Avaliar a eficácia de uma intervenção na redução das atitudes estigmatizantes e ansiedade perante o doente mental em estudantes de enfermagem. Métodos Estudo quase experimental, realizado com uma amostra intencional de 99 estudantes de enfermagem de um instituto universitário da região centro de Portugal. No grupo de controlo, os estudantes desenvolveram o plano de ensino clínico em Enfermagem de saúde mental e psiquiátrica em serviços de psiquiatria, conforme o plano de estudos (n=50). No grupo de teste, além de cumprirem o plano de estudos, os estudantes participaram de um programa psicoeducativo dirigido ao estigma (n=49). Utilizaram-se as versões portuguesas da Mental Illness: Clinicians' Attitudes Scale (MICA-4) e AQ27 para avaliação do estigma e a Intergroup Anxiety Scale (EAI) para medir a ansiedade intergrupal. Resultados A amostra apresenta homogeneidade de variáveis no momento inicial (p>0,05), com melhorias estatisticamente significativas das atitudes estigmatizantes e redução da ansiedade perante o doente mental em ambos os grupos (p<0,005). O impacto da intervenção não foi significativo, tendo o grupo de teste melhorado as atitudes estigmatizantes de medo (p=0,03) e piorado relativamente à ajuda (p= 0,04). Conclusão Verificou-se um impacto positivo do ensino clínico na redução da ansiedade perante o doente mental, conduzindo a uma redução do estigma em saúde mental. A intervenção dirigida ao estigma não revelou efeitos significativos no estigma global nem na ansiedade intergrupal o que aponta para a necessidade futura da sua reformulação e comparação com amostras de estudantes de outras instituições de ensino de enfermagem.

Abstract Objective To assess the effectiveness of an intervention to reduce stigmatizing attitudes and anxiety towards people with mental illness among nursing students. Methods This was a quasi-experimental study including an intentional sample of 99 nursing students from university of Central Portugal. In the control group, students developed the clinical teaching plan in mental health and psychiatric nursing in psychiatric health services, according to the syllabus (n=50). In the experimental group, in addition to complying with the syllabus, the students participated in a psychoeducational program oriented towards stigma (n=49). To assess stigma, the Portuguese versions of Mental Illness: Clinicians' Attitudes Scale (MICA-4) and AQ27 were used; and to measure intergroup anxiety, the Intergroup Anxiety Scale (IAS). Results The sample presents homogeneous variables in the initial time (p>0.05), with statistically significant improvement of stigmatizing attitudes and reduction of anxiety towards people with mental illness in both studied groups (p<0.005). The impact of the intervention was not significant, the experimental group improved stigmatizing attitudes regarding fear (p=0.03) and the ones regarding help had worsened (p= 0.04). Conclusion We verified a positive impact of clinical teaching in the reduction of anxiety towards people with mental illness, which led to a reduction of stigma associated to mental health. The intervention oriented towards stigma did not reveal significant effects concerning global stigma, nor intergroup anxiety, which showed the future necessity of its reformulation and comparison with samples of students from other nursing universities.

Aim: The roughness and micromorphology of various surface treatments in aged metal-free crowns and the bond strength of these crowns repaired with composite resin (CR) was evaluated in vitro. Methods: A CR core build-up was confectioned in 60 premolars and prepared for metal-free crowns. Prepared teeth were molded with the addition of silicone, and the laboratory ceromer/fiber-reinforced crowns (SR Adoro/Fibrex Lab) were fabricated. Subsequently, the crowns were cemented and artificially aged in a mechanical fatigue device (1.2 X 106 cycles), then divided into 4 groups (n = 15) according to the surface treatment: 1) phosphoric acid etching (PA); 2) PA + silane application; 3) roughening with a diamond bur + PA; and 4) sandblasting with Al2O3 + PA. After the treatments, the crowns (n = 2) were qualitatively analyzed by scanning electron microscope (SEM) and surface roughness (n = 5) was analyzed before and after the surface treatment (Ra parameter). The remaining crowns (n = 8) received standard repair with an adhesive system (Tetric N-Bond) and a nanohybrid CR (Tetric N-Ceram), and the microshear bond strength (SBS) test was performed (0.5 mm/min). Roughness and SBS data were analyzed by one- and two-way ANOVA, respectively, as well as Tukey's post-test (α = 0.05). Results: Sandblasting with Al2O3 + PA resulted in the highest final roughness and SBS values. The lowest results were observed in the PA group, whereas the silane and diamond bur groups showed intermediate values. Conclusion: It may be concluded that indirect ceromer crowns sandblasted with aluminum oxide prior to PA etching promote increased roughness surface and bond strength values

ABSTRACT BACKGROUND: Age-related macular degeneration (AMD) is the third largest cause of blindness worldwide, accounting for 8.7% of all cases. A considerable number of preventive or therapeutic interventions have been used for AMD. OBJECTIVE: This study presents a critical view of the interventions that have been assessed through Cochrane systematic reviews. DESIGN AND SETTING: Review of systematic reviews, conducted in the Discipline of Evidence-Based Medicine, Escola Paulista de Medicina (EPM), Universidade Federal de São Paulo (UNIFESP). METHODS: Review of Cochrane systematic reviews about interventions for AMD. RESULTS: The 18 systematic reviews included assessed the effects of surgical techniques, laser/photo/radiotherapy, intravitreal injections, systemic drugs and phytotherapy/vitamins/supplements. CONCLUSION: The Cochrane systematic reviews found evidence that use of bevacizumab, ranibizumab, pegaptanib, laser photocoagulation, photodynamic therapy and multivitamin compounds may present some benefits for treating AMD. There was insufficient evidence for supporting the use of macular translocation, submacular surgery, steroid implantation, radiotherapy, intravitreal aflibercept, interferon alfa, statins or omega-3 fatty acids for treating AMD; or the use of multivitamin antioxidant vitamins or mineral supplementation for preventing AMD. Future randomized controlled trials are imperative to reduce the uncertainty in several clinical questions regarding AMD.

Abstract Objective The present study aims to understand to what extent obesity is related to adversematernal, obstetrical, and neonatal outcomes in a Portuguese obstetrical population. Methods A retrospective case-control study was conducted at the Department of Obstetrics of a differentiated perinatal care facility. The study compared 1,183 obese pregnant womenwith 5,399 normal or underweight pregnantwomen for the occurrence of gestational diabetes, hypertensive pregnancy disorders, and preterm birth. Mode of delivery, birthweight, and neonatal intensive care unit (ICU) admissionswere also evaluated. Mean blood glucose values were evaluated and compared between groups, in the first and second trimesters of pregnancy. Only singleton pregnancies were considered. Results The prevalence of obesity was 13.6%. Obese pregnant women were significantly more likely to have cesarean sections (adjusted odds ratio [aOR] 2.0, p< 0.001), gestational diabetes (aOR 2.14, p< 0.001), hypertensive pregnancy disorders (aOR 3.43, p< 0.001), and large-for-gestational age ormacrosomic infants (aOR 2.13, p< 0.001), and less likely to have small-for-gestational age newborns (aOR 0.51, p< 0.009). No significant differences were found in terms of pretermbirths, fetal/neonatal deaths, low birthweight newborns, and neonatal ICU admissions among cases and controls. Maternal obesity was significantly associated with higher mean blood glucose levels, in the first and second trimesters of pregnancy. Conclusion Obesity is associated with increased risks of adverse pregnancy and neonatal outcomes. These risks seem to increase progressively with increasing body mass index (BMI) class. Female obesity should be considered a major public health issue and has consequences on maternal-fetal health.


Resumo Objetivo O presente estudo pretende avaliar em que medida a obesidade influencia os desfechos maternos, obstétricos e neonatais em uma população obstétrica portuguesa. Métodos Um estudo caso-controle retrospectivo foi realizado no departamento de obstetrícia de um centro perinatal diferenciado. O estudo comparou 1.183 grávidas obesas com 5.399 grávidas normoponderais ou com baixo peso para a ocorrência de diabetes gestacional, doenças hipertensivas da gravidez e parto pré-termo. Via de parto, peso ao nascimento e admissão na unidade de cuidados neonatais também foram avaliados. Os valores glicêmicos médios foram avaliados e comparados entre os dois grupos, no primeiro e segundo trimestres de gravidez. Apenas as gravidezes unifetais foram avaliadas. Resultados A prevalência da obesidade foi de 13.6%. As grávidas obesas tiveramrisco significativamente superior a ter uma cesariana (odds ratio ajustado [Ora] 2.0, p < 0.001), diabetes gestacional (ORa 2.14, p < 0.001), doenças hipertensivas da gravidez (ORa 3.43, p < 0.001), recém-nascidos grandes para a idade gestacional ou macrossômicos (ORa 2.13, p < 0.001) e menor probabilidade de ter recém-nascidos pequenos para a idade gestacional (ORa 0.51, p < 0.009). Não houve diferença estatisticamente significativa quanto aos partos pré-termo, mortes fetais/neonatais, baixo peso ao nascer e admissão à unidade de cuidados intensivos neonatais. O odds ratio foi ajustado para a idade, número de gestações, paridade, ganho ponderal, doenças hipertensivas da gravidez e diabetes gestacional. A obesidade materna esteve significativamente associada a valores glicêmicos médios superiores, no primeiro e segundo trimestres de gravidez. Conclusão A obesidade está associada a maior risco de desfechos adversos na gravidez e neonatais. Este risco parece aumentar progressivamente com o aumento do índice de massa corporal (IMC). A obesidade feminina deve ser considerada um importante problema de saúde pública e que tem repercussões na saúde maternofetal.

ABSTRACT

O presente estudo, realizado com 123 alunos finalistas do ensino superior, tem como objetivos esclarecer em que medida os níveis de autoeficácia na transição para o trabalho e a empregabilidade percebida predizem as intenções e comportamentos de procura de emprego, e verificar o papel mediador da autoeficácia na transição para o trabalho nas relações entre a empregabilidade percebida e as intenções de procura de emprego e os comportamentos de procura de emprego. Os resultados revelaram que ambas as variáveis predizem as intenções e os comportamentos de procura de emprego e que a autoeficácia na transição para o trabalho atua como variável mediadora na relação que se estabelece entre a empregabilidade percebida e intenções de procura de emprego. Por fim, são discutidas as limitações do estudo, bem como as suas principais implicações para as práticas de aconselhamento de carreira a levar a cabo junto dos finalistas do ensino superior.


The present study, carried out with 123 higher education senior students, aims to clarify to what extent the levels of self-efficacy in school-to-work transition and the perceived employability can predict job-seeking intentions and behaviors, and to verify the mediating role of self-efficacy in the transition to work in the relationship between perceived employability and job search intentions and job search behaviors. The results revealed that both variables predict job-seeking intentions and behaviors, and that self-efficacy in the transition to work acts as a mediating variable in the relationship between perceived employability and job-seeking intentions. Finally, the limitations of the study as well as its main implications for career counseling practices to be pursued with higher education seniors are discussed.


El presente estudio, realizado con 123 estudiantes que están finalizando la enseñanza superior, tiene como objetivos aclarar en qué medida los niveles de autoeficacia y la empleabilidad percibida predicen las intenciones y comportamientos de búsqueda de empleo y, además, verificar el papel mediador de la autoeficacia en la relación entre empleabilidad percibida y las intenciones y comportamientos de búsqueda de empleo. Los resultados han demostrado que ambas variables predicen las intenciones y los comportamientos de búsqueda de empleo y que la autoeficacia actúa como variable mediadora en la relación entre empleabilidad percibida e intenciones de búsqueda de empleo. Por fin, se discuten las limitaciones del estudio, bien como sus principales implicaciones para las prácticas de orientación profesional a estudiantes que finalizan la enseñanza superior.
