ABSTRACT

Diabetes is a common metabolic disorder affecting the world population which accounts for major amount of morbidity and mortality mainly due to its micro vascular and macro vascular complications. This is a comparative study which analyses the correlation between the Platelet indices like Mean platelet volume, platelet distribution width, plateletcrit and total platelet count in diabetic patients with micro vascular complications and without complications. This review of the literature was conducted through an Internet search on a public access website like PubMed Google scholar, Medline databases until 2019. Keywords utilized included Diabetes mellitus, Microvascular complications, platelet indices. The major exclusion criteria was studies which included the patients with macrovascular complications and patients taking drugs which alters the platelet indices. All these articles were analysed. Platelet indices like Mean platelet volume and platelet distribution width was significantly higher in individuals with microvascular complications, predominantly Diabetic Nephropathy and Diabetic Retinopathy when compared to those without microvascular complications. Change in platelet indices were found to be statistically associated with diabetic microvascular complications. Hence these parameters can be used to monitor and to predict the risk of microvascular complications.

ABSTRACT

Background: Peripheral insensate neuropathy is one of the most common and forms of peripheral neuropathy. It is a preeminent cause for disability, foot ulcers, deformity and amputations in individuals who are at risk.Methods: A study was conducted in Mahatma Gandhi Medical College & Research Institute, Pondicherry, India in the year 2017-18, which included examination of 760 people of more than 30 years of age by random sampling method who was attending the outpatient clinic. The Semmes Weinstein monofilament was adopted to ascertain the presence of peripheral insensate neuropathy.Results: In present study, the prevalence of peripheral insensate neuropathy was 3.2%. As the height increased, the prevalence of peripheral insensate neuropathy increased, regardless of other independent risk factors like diabetes, hypertension, etc. The risk of the peripheral insensate neuropathy increases at a height of >171.5cm in males and at a height of >164.5cm in females.Conclusions: The authors confirm that body height is a significant and independent risk factor for peripheral insensate neuropathy, regardless of co-morbidities. Height as a guide, helps the health care professionals in identifying the people who are at risk for peripheral insensate neuropathy.

Background: Tuberculosis being a systemic disease and has a capacity for wide spread dissemination. Present study aims to identify the effects of antituberculous treatment on thyroid profile in new smear positive pulmonary tuberculosis cases.Methods: This study was conducted among 60 new smear positive pulmonary tuberculosis cases attending pulmonary medicine OPD from May 2015 to April 2017. Thyroid function test in the form of free T3, free T4 and TSH was measured before initiating Anti tuberculosis treatment (ATT), at 3 months and at the end of 6 months.Results: Out of 60 patients enrolled in present study, majority were males. Diabetes mellitus was the major co morbidity. Sick euthyroid was found to have decreasing trend during the course of treatment, and hypothyroidism was found to be increasing trend end of 6 months.Conclusions: The common Thyroid Dysfunction seen during the study period was Hypothyroidism and Sick euthyroid. Anti-tuberculous medication preferably Rifampicin probably would explain the cause for these thyroid dysfunctions noticed during the study time. And those patients with significant hypothyroid need to started on thyroid supplements. Among the drugs used for treatment, rifampicin was probably the cause for thyroid dysfunction noticed during the course of treatment. Hence, authors recommend that these patients should be started on thyroid supplements after the diagnosis of significant hypothyroidism.

ABSTRACT

Background: Carotid artery intima-media thickness (CAIMT) measurement in hypothyroidism will help assess the progression of atherosclerosis and timely intervention may prevent vascular complications.Methods: This study included 30 clinical hypothyroid (CH), 30 subclinical hypothyroid (SCH) and 30 euthyroid. As per procedure, informed consent was taken from the patients in prescribed formats before their participation in the study. Patients were divided into 3 groups of CHs, SCH and Controls after obtaining the thyroid function test values. CAIMT on the right side was measured in the three groups for comparison. Other parameters included age, sex, height, weight, body mass index (BMI), total cholesterol and triglycerides. After 4 months of levothyroxine therapy, CAIMT, total cholesterol and triglycerides were reassessed.Results: The CAIMT was increased in CH and SCH group when compared to euthyroid individuals. The mean CAIMT in CH group was 0.60�009cm, in SCH group it was 0.055�010 cm and in controls it was 0.047�006 cm. After 4 months of levothyroxine therapy, there was no change observed in the mean CAIMT values.Conclusions: CAIMT levels were increased in CH and SCH group when compared to euthyroid group. There was no regression of CAIMT after 4 months of levothyroxine therapy
