ABSTRACT

Objective To explore the association of mean platelet volume and neutrophil to lymphocyte ratio with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in elderly patients and its clinical value for the short-term prognosis. Methods A total of 52 patients with AECOPD who had been hospitalized during the period of April 2014 to April 2015 were enrolled in this prospective observational study. Levels of mean platelet volume (MVP),C-reactive protein, complete blood count,D-dimer,fibrinogen,percent-of-predicted FEV1 and percent-of-predicted FEV1/FVC were measured at admission(acute exacerbation)and after 3 months(stable period). Fifty-two age-and sex-matched healthy individuals constituted the control group. Pearson′s correlation was used to analyze the association of MPV with WBC,NLR,CRP,DD,FIB,FEV1%,and FEV1/FVC%. Results In AECOPD,as compared with stable COPD and the control group, levels of WBC,NLR,D-dimer,fibrinogen and CRP elevated;FEV1,FVC,percent-of-predicted FEV1 and percent-of-predicted FEV1/FVC declined;and MPV level decreased obviously. As a cut-off point of MVP level of<8.35fl for predicting AECOPD,it showed a sensitivity of 84.6%and a specificity of 86.5%. A decreased MVP level was significantly correlated with an increased in white blood cell count, neutrophil to lymphocyte ratio,D-dimer,fibrinogen and C-reactive protein level(P=0.01,0.02,0.01,0.02,and 0.01 respectively). Conclusions Mean platelet volume and neutrophil to lymphocyte ratio may be inflammatory markers inAE COPD,and measurement of mean platelet volume level may be useful for identifying elderly patients at increased risks for acute exacerbation.

ABSTRACT

Objective To investigate the feasibility of mono-segment pedicle instrumentation (MSPI)in management of thoracolumbar fracture (AO classification,A1 and A3) by being compared with short-segment(two-segment) pedicle instrumentation(SSPI).Methods Overall 141 patients with tape A1 or A3 thoracolumbar fractures, aged from 20 to 60 years (average,40.5 years),were enrolled in this prospective study.According to a simple randomized method,35 patients with type A1 fracture and 41 patients with type A3fracture were treated with MSPI,while 26 with type A1 fracture and 39 with type A3 fracture were treated with SSPI.Low back outcome score (LBOS) and ASIA2000 were used to evaluate clinical outcome.Eighteenth month postoperatively was assigned as the last follow up period.Wedge index (WI) and sagittal index (SI) of the affected vertebrae on radiography were measured and compared preoperatively, one week postoperatively and at the final follow-up.Results All patients were followed up successfully. The blood loss and duration of operation of MSPI group were significantly less than that of SSPI group, respectively. However, there were no significant differences of clinical outcome between two groups. For type A1 fracture, correction rate and correction loss of WI in MSPI group were better than those in SSPI group. For type A3 fracture, there were no significant differences of correction rate and correction loss of WI and SI between MSPI group and SSPI group; however, the failure rate of MSPI group was significantly higher than that of SSPI group. Conclusion For type A1 and partial type A3 thoracolumbar fractures,MSPI can provide the same or better fixation with less blood loss and operative duration than SSPI.Since MSPI for type A3.2 thoracolumbar fracture has a higher failure rate, the surgical indication should be strictly controlled.

ABSTRACT

Objective To investigate the changes of somatosensory-evoked potential(SEP)during an mild impact spinal cord injury in rats 80 as to evaluate its potential value in prevention of such iatrogenic damage. Methods Twenty-four SD rats weighing(340±28)g were randomly divided into two groups,ie,sham control group(only exposure without impact at C6)and injury group(mild impact spihal cord injury at C6).SEP wss recorded in both groups. The changes of SEP in waveform, amplitude and latency were observed and compared between groups and between operations. The gross dissection and histologic analysis were performed after surgery for comparative study. Results SEP waveforms, amplitude and latency showed no significant change in the sham control group. In contrast, the SEP waveform and amplitude animals showed significant changes in the injury group after impact spinal cord injury and the amplitude was decreased from pre-injury(1.3±0.7)μV to post-injury(0.5±0.4)μV(P<0.05),while the latency showed no significant difference between(11.1±2.1)ms pre-injury and(10.7±1.3)ms post-injury(P>0.05).However,this abnormal change appeared in a temporary period at(5.7±3.2)minutes after impact and lasted for about(7.1±3.3)minutes.Diffused hemorrhagic nidus could be seen in the injured cord, which was not found in the sham control group. Condusions Mild impact spinal cord injury may induce transient abnormalities of SEP in waveform and amplitude, which requires careful monitoring in clinical practice. The sudden change in SEP may be associated with impact and vibration damage to the spinal cord, suggesting timely use of protection measures for spinal cord.

ABSTRACT

BACKGROUND: Homocysteine (HCY) has been verified as an independent risk factor of atherosclerosis and atherothrombosis of cardiovascular disease.OBJECTIVE: To observe the effects of HCY on the secretion and activity of matrix metallopotinase-1 (MMP-1) and tissue inhibitors of metalloproteinase-1 (TIMP-1) in vascular smooth muscle cells (VSMCs).DESIGN : Auto-control observation.SETTING: Pathology Room, Institute of Regeneration Medical Sciences, Jilin University.MATERIALS: In vitro cultured vascular smooth muscles cells (VSMCs) of rats were obtained from male Wistar rats with the body mass of about 150 g from weeks 4-6 supplied by Laboratory of Animals, Norman Bethune Medical Sciences Division, Jilin University.METHODS: The experiment was performed at the Pathology Room, Institute of Regeneration Medical Sciences, Jilin University from May 2001 to May 2003. VSMCs ofin vitro cultured rats were adopted and divided into 5 groups, 0(control group), 0.10, 0.25, 0.50 and 1.00 mmol/L HCY were added, respectively for 48 hours. Effect of HCY on activity of MMP-1 was observed with zymography. The secretions of MMP-1 and TIMP-1 and their mRNA expressions were studied with Western blot and semi-quantitative reverse transcription polymerase chin (RT-PCR).MAIN OUTCOME MEASURES: Activity of MMP-1, secretions of MMP-1 and TIMP-1 and their mRNA expressions.RESULTS: ①Secretions of MMP-1 and TIMP-1: The secretion of MMP-1 in the 0.25, 0.50 and 1.00 mmol/L HCY groups was lower significantly than that in the control group (P < 0.05-0.01). The secretion of TIMP-1 in the 0.10, 0.25, 0.50 and 1.00 mmol/L HCY groups was higher than that in the control group (P< 0.05-0.01). ②The MMP-1 activity decreased with the increase of HCY, but reduced obviously in the 0.50 and 1.00 mmol/L HCY groups (P < 0.01). ③The expression of MMP-1 mRNA in the 4 HCY groups was lower markedly than that in the control group (P < 0.01). The expression of TIMP-1 mRNA in the 0.25 mmol/L HCY group was higher than that in the control group (P < 0.05), and it was higher remarkably in the 0.50 and 1.00 mmol/L HCY groups than that in the control group (P < 0.01).CONCLUSION: HCY can inhibit enzyme activity, decrease collagen degradation and induce collagen accumulation by inhibiting the secretion of MMP-1, which indicates that reduction of collagen degradation induced by HCY is one of the pathogenesies of atherosclerosis.

ABSTRACT

A series of experiments designed in accordance with the biogeochemi-cal hypothesis on the etiology of Keshan disease was performed. It was shown that the pathogenic factors of Keshan disease existed in the grains cultivated in the endemic regions ("endemic regions" mean those localities where Keshan disease has been prevalent). These factors had an ability to elicit a metabolic derangement of calcium in the myocardium and indu- ce some latent damages in myocardial cells. Furthermore, there were definite relations between these factors and the nitrite. With the aid of nitrite, more serious myocardial necroses were produced by these pathogenic factors. It was also proved that the addition of ammonium molybdate to the grains from the endemic regions produced some prophylactic effect in reducing the myocardial injury in expermental animals. After an observation on fertilizing the soil in endemic region with ammonium molybdate for four years, the preliminary result showed that this measure seemed to be effective in preventing Keshan disease.Based on the results cited above, we considered that molybdenum deficiency and nitrite excess might play an important role in the occurrence of Keshan disease, but the exact pathogenic factors in the grains remained to be studied further.
