Aneurisma da artéria coronária esquerda / Left coronary artery aneurism

The coranary artery aneurysm reports are searce in medical literature and habitually related with the atherosclerotic or degeneratives etiology in function of the primary or secundary arteritis. Among the cases described by the literature, the aneurisms it's more in reght coronary artery. The authors report a case of patient with premature precordialgy and miocardic isquemic symptomas. The coronary angiography demonstrated anterior descending artery aneurysm with diagonal branch. Didn't present arteriopaty signs in orthers coraonary organics systems and neither symptomas of the arteritis or sepsis history related with micotic etiology. Electtivally accomplished with cardiopulmonary bypass the aneurysm resection and posteriorly anterior descending artery revascularization with left internal mammary artery graft and first diagonal branch with saphenous vein grafts. The operative elapsed not problems and pacient stayed in hospital for seven days. Actually, it is on tenth month of the attendance with good evolution

Avaliaçäo da disfunçäo diafragmática no pós-operatório de cirurgia cardíaca / Evaluation of diaphragmatic disfunction in postoperativa of cardiac surgery

A primeira descriçäo a respeito do envolvimento do nervo frênico nas complicaçöes pulmonares após as cirurgias cardíacas foi feita por Scannell, em 1963. Os autores avaliam neste estudo 25 pacientes, submetidos à cirurgias cardíacas eletivas em normotermia e proteçäo miocárdica por pinçamento aórtico intermitente, a partir da avaliaçäo da amplitude do movimento hemidiafragmático em radiografias simples de tórax na incidência póstero-anterior, após manobras de inspiraçäo e expiraçäo forçadas. Dentre estes pacientes, 4(16 porcento) apresentam movimentaçäo inferior a 2 cm em um ou ambos hemitórax, sendo estes considerados portadores de disfunçäo diafragmática. Apesar da concordância com a incidência observada na literatura geral, estes pacientes näo foram relacionados expressivamente aos fatores etiopatogênicos mais comumente relacionados na literatura, como a dissecaçäo da artéria torácica como causa de disfunçäo hemidiafragmática esquerda e a hipotermia tópica ou sistêmica
