ABSTRACT

Objective: To observe the clinical efficacy of warm needling moxibustion plus isokinetic muscle strength training for knee osteoarthritis (KOA).Methods: A total of 135 patients with KOA due to Yang deficiency and coagulated cold were randomized into a warm needling moxibustion group, an isokinetic muscle strength training group, and a combined group by the random number table method, with 45 cases in each group. The warm needling moxibustion group was treated with warm needling moxibustion. The isokinetic muscle strength training group was treated with isokinetic muscle strength training. The combined group was treated with warm needling moxibustion plus isokinetic muscle strength training. The Western Ontario and McMaster Universities osteoarthritis index (WOMAC) and visual analog scale (VAS) were scored before and after treatment, and isokinetic indicators of peak torque (PT), total work (TW) and average power (AP) were evaluated. Results: The total effective rate of the combined group was 92.5%, which was significantly higher than 83.3% in the warm needling moxibustion group (P<0.05) and 72.5% in the isokinetic muscle strength training group (P<0.05). After treatment, the scores of WOMAC (total, pain, stiffness, and function) and VAS, and isokinetic indicators (PT, TW, and AP) were all improved compared with those before treatment (P<0.05) in all three groups. The differences among the three groups were statistically significant (P<0.05). The WOMAC total score and score of stiffness in the combined group were lower than those in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), and the scores in the warm needling moxibustion group were lower than those in the isokinetic muscle strength training group (P<0.05). The WOMAC score of pain and VAS score in the warm needling moxibustion group and the combined group were lower than those in the isokinetic muscle strength training group (P<0.05). The differences between the warm needling moxibustion group and the combined group were not statistically significant (P>0.05). The WOMAC function score in the combined group was lower than that in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), while there was no statistical difference between the warm needling moxibustion group and the isokinetic muscle strength training group (P>0.05). PT, TW, and AP in the combined group were higher than those in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), and they were higher in the isokinetic muscle strength training group than in the warm needling moxibustion group (P<0.05). Conclusion: Warm needling moxibustion plus isokinetic muscle strength training has a better effect in the treatment of KOA due to Yang deficiency and coagulated cold than either warm needling moxibustion or isokinetic muscle strength training alone.

ABSTRACT

Objective@#To investigate the interaction between mental elasticityand occupational stress and depressionin female nurses and the mediating effect of mental elasticity, as well as the functioning way of mental elasticity in occupational stress-depression.@*Methods@#From August to October, 2015, cluster sampling was used to select 122 female nurses in a county-level medical institution as study subjects. The Connor-Davidson Resilience Scale (CD-RISC) , Occupational Stress Inventory-Revised Edition (OSI-R) , and Self-Rating Depression Scale (SDS) were used to collect the data on mental elasticity, occupational stress, and depression and analyze their correlation and mediating effect.@*Results@#The 122 female nurses had a mean mental elasticity score of 62.4±15.1, which was significantly lower than the Chinese norm (65.4±13.9) (P<0.05) ; the mean depression score was 41.0±7.7, which was significantly higher than the Chinese norm (33.5±8.6) (P<0.01) , and the incidence rate of depression of 52.5%. Mental elasticity was negatively correlated with occupational stress and depression (r=-0.559 and -0.559, both P<0.01) . Occupational stress and the two subscales mental stress reaction and physical stress reaction were positively correlated with depression (r=0.774, 0.734, and 0.725, all P<0.01) . After adjustment for confounding factors, occupational stress had a positive predictive effect on depression (β=0.744, P<0.01) , and mental elasticity had a negative predictive effect on depression (β=-0.221, P<0.01) . The analysis of mediating effect showed a significant direct effect of occupational stress on depression and a significant mediating effect of mental elasticity (a=-0.527, b=-0.227, c=0.744, c’=0.627; all P<0.01) , and the mediating effect of mental elasticity accounted for 16.08% of the total effect.@*Conclusion@#As a partial mediating variable, mental elasticity has an indirect effect on the relationship between occupational stress and depression and can alleviate the adverse effect of occupational stress and reduce the development of depression.

ABSTRACT

AIM: To investigate the protective effect and mechanisms of di-ao-xin-xue-kang (DK) on myocardial ischemia-reperfusion. METHODS: Models of myocardial ischemia-reperfusion were constructed with sixteen mongrel dogs. The left ventricular pressure (LVSP), the marximal/minimum rate of LVSP (?dp/dt max) and the serum aspartate aminotransferase (AST), creatine kinase (CK), lactate dehydrogenase (LDH), malondialdehyde (MDA) were determined before and 90 minutes after occlusion, 120 and 240 minutes after reperfusion, and the MDA content in myocardial cell membrane prepared at 240 minutes after reperfusion was determined. The oxygen free radical was assayed with electron spin resonace spectroscopy (ESR) technique. RESULTS: ① LVSP and ?dp/dt max in the normal saline control (NS) group decreased with the time progress of occlusion-reperfusion, and it was the same in DK group, but the levels were significantly higher than that in NS group (P
