ABSTRACT

Objective:To explore the application research of isolated esophageal ventilation device in painless gastroscopy.Methods:A total of 200 patients who underwent painless gastroscopy requiring esophageal isolation were selected,and they were randomly divided into an observation group(isolated esophageal ventilation device + gastroscopy)and a control group(ordinary mask ventilation + gastroscopy),with 100 cases in each group.The heart rates,blood pressures,the numbers of breaths and blood oxygen saturation(SpO2)of 5 time points of two groups were recorded at entering time(T0),pre installing ventilation device(T1),post installing ventilation device(T2),during gastroscopy(T3)and post gastroscopy(T4).And then,the incidence of complications,occurrence of adverse events,total dosages of Propofol,operation times of gastroscopy,wake-up times and observation times of two groups were compared.Patients'satisfactions for the examination were evaluated by self-designed satisfaction survey and evaluation scale.Results:The differences of SpO2 between the two groups of patients at T1-T4 time points were statistically significant(t=23.150,t=14.720,t=17.900,t=18.520,P<0.05),respectively,and the difference of that between two groups at T0 time point was not statistically significant.There were no significant differences in blood pressure,heart rate and respiratory frequency between the two groups of patients at different time points.The total incidences of complications in the control group and the observation group were respectively 68.00% and 13.00%.The total incidence of complications in the control group was significantly higher than that in the observation group,and the difference was statistically significant(x2=62.766,P<0.05).The total dosage of Propofol,time of gastroscopy examination and observation time of the observation group were significantly lower than those of the control group,and the differences were statistically significant(t=4.977,t=20.040,t=6.486,P<0.05),respectively.There was no statistically significant difference in the wake-up time between the two groups of patients.The satisfaction rates of the control group and observation group were respectively 79.00% and 95.00%for painless gastroscopy,and the satisfaction rate of the control group was significantly lower than that of the observation group,and the difference was statistically significant(x2=11.317,P<0.05).Conclusion:In the application of the isolated esophageal ventilation device in painless gastroscopy,the SpO2 decrease of preventive operation can improve the oxygenation state of patients,and decrease the incidence rate of chocking cough and other adverse reactions,and reduce the occurrence of gastric reflux,and shorten the time of gastroscopy examination,and enhance the satisfaction of patients for examination,which has better safety.

ABSTRACT

Objective To systematically revise the efficacy and safety of tamsulosin in treatment of proximal ureteral calculi. Methods Electronic search was conducted with PubMed, Cochrane library, EMBASE, Wanfang database, CNKI and VIP to collect randomized controlled trials(RCTs) involving the application of tamsulosin in treatment of upper ureteral calculi. The quality of the included trials was assessed; the data were extracted and analyzed with RevMan 5. 0 software. Results Five RCTs involving 480 patients were identified. The results of meta-analysis showed that, compared with the control group, patients in the tamsulosin group had (1) a remarkably higher stone clearance rate (RR = 1. 25, 95% CI: 1. 12-1. 40, P< 0. 000 1), (2) shorter mean time of calculi expulsion (shortened by 2. 99 days, 95%CI: -3. 61- -2. 37, P<0. 000 01), and (3) decreased incidence of renal colic episodes and mean visual analogue scale pain score (RR = 0. 39,95%CI:0. 26-0. 57, P< 0. 000 01; WMD = -10. 94,95%CI; -21. 30--0. 58, P = 0. 04). Conclusion Our results show that tamsulosin can improve the stone clearance, shorten the mean time for calculi expulsion, decrease the incidence of renal colic episodes and the mean visual analogue scale pain score in patients with proximal ureteral calculi.

ABSTRACT

Objective To study the effect of the EGF-L functional region containing human TN-R protein in prokaryocytes and anti-EGF-L serum on the cortical neuronsin vitro in rats. Methods The anti-EGF-L serum was obtained from the rabbits immunized with the protein of EGF-L, then combine with TN-R coat petri dish to prepare various culture substrate.The effect on adhere,migrate and neurite outgrowth of neuron by differ culture substrate was observed. Results High concentration of antiserum against protein of EGF-Lwas successfully obtained from the rabbits,and it has no effect on the neuron when coated on petri dish as culture substrate with PLL in vitro,but can add the adhesive to the neuron,partly neutralize the inhibitory on the neuron neurite of TN-R.The antiserum can also allow the neuron with its neurite to migrate to the area coated with TN-R. Conclusion The anti-EGF-L serum can weaken the inhibitory on the neuron of TN-R in vitro,the function in vivo need further research,

ABSTRACT

Objective To investigate the effect of collagen scaffold loaded with collagen-binding domain neurotrophin-3 (CBD-NT3) on the extension of cellular processes of dorsal root ganglions (DRGs), and explore the significance of this kind of combinatorial strategies in the spinal cord injury repair. Methods The tail tendons of SD neonatal rats were performed the removal of cellular components to prepare the collagen scaffold; HE staining was employed to evaluate whether the cells were completely removed from the collagen scaffold. The collagen scaffold was loaded with CBD-NT3,and then, they were co-cultured with primary DRGs for 1, 3 and 5 d, respectively. NT3 and PBS were also co-cultured with primary DRGs for 1, 3 and 5 d, respectively, as controls. Cells on the scaffold were stained by fluorescein diacetate (FDA) for morphology observation and the lengths and angles of the processes in each group were also quantitatively analyzed. Scanning electron microcopy (SEM) was employed to observe the topography of scaffold and the ultrastructure of DRGs 3 d after the co-culture.Results HE staining indicated that the cellular components in the scaffold were removed completely.The length of processes elongation in CBD-NT3 treatment group was significantly longer than that in the controls 3 d after the co-culture (P<0.05). The 95% confidence interval of the angle between the line which the process emerged from the cell soma to the growing tip of the process and the long axis of fiber was 18.8-20.7 degrees. The results of SEM showed that cells could rely on the topography of the scaffold to anchor and grow. Conclusion The combinatorial strategies of collagen scaffold with CBD-NT3 can play a double function for oriented guiding and inducing extension of cellular processes effectively,which may provide a better therapeutic approach for spinal cord injury repair.
