ABSTRACT
Objective To evaluate the testing capabilities of laboratories,analyze existing issues,and improve testing quality,through carrying out the external quality assessment(EQA)of clinical laboratories for human papillomavirus(HPV)type 6 and 11 nucleic acid detection.Methods EQA plan was carried out twice a year.Each panel contains 4 positive samples,including one strong positive sample and one weak positive sample of HPV6 and HPV11,made from cervical secretions from patients with clinical manifestations of condyloma acuminata(CA)and positive for HPV6 or HPV11(from Shanghai First Maternity and Infant Hospital).One negative sample was cultured from the C-33A cell line(from Chinese Academy of Sciences).Samples were sent to participating laboratories by cold chain,and laboratories were required to detect test samples and upload their results within the specified time.Shanghai Center for Clinical Laboratory(SCCL)calculated the scores of each laboratory based on the return results.Results A total of 163 sample panels were sent out in the 6 rounds of EQA plan and 140 valid reports were received.The laboratory qualification rate was 96.43%(135/140)and the sample compliance rate was 97.86%(685/700).There were 13 false negative results and 2 false positive results,with weakly positive samples accounting for 76.92%(10/13)of the false negative results.Conclusion The detection accuracy of HPV6/11 nucleic acid in each laboratory was relatively high,and the detection ability of weak positive samples in individual laboratories may need to be improved.The laboratory could discover problems and improve its quality management by participating in EQA.
ABSTRACT
Objective Both flee triiodothyronine (FT3) level and Glasgow Coma scale (GCS) scores have been separately described as prognostic predictors for mortality in hypertensive intracerebral hemorrhage (HICH).This study is conducted to investigate the relationship and prognostic impact of low-T3 syndrome and GCS in HICH patients.Methods Two hundred and thirty patients with HICH,admitted to our hospital from January 2015 to January 2016,were chosen and performed thyroid hormone levels examination (FT3,FT4 and thyroid stimulating hormone [TSH] 3).According to the thyroid hormone results,these patients were divided into low T3 group I (FT3<3.1 pmol/L,normal TSH level) and normal thyroid function group (normal FT3,FT4 and TSH levels).According to best cut-off levels defined by receiver operating characteristic (ROC) curve,these patients were divided into low GCS scores group (GCS<7.5) and high GCS scores group (GCS>7.5),and low T3 group Ⅱ (FT3<2.85 pmol/L) and high T3 group (FT3>2.85 pmol/L).Telephone follow-up was performed every 6 months,and using death or re-bleeding during follow-up period as end point of the event,prognostic values of FT3 level and GCS scores defined by ROC curve in mortality and re-bleeding rote were recorded;survival rate of these patients were analyzed by Kaplan-Meier curves and compared between each two groups;multivariate Cox regression was used to analyze the relations of FT3 level and GCS scores with mortality and re-bleeding rate.Results As compared with normal thyroid function group,low T3 group Ⅰ had significantly higher re-bleeding rate,percentage of patients with blood loss>30 mL,and rate of breaking into the ventricles,and statistically lower GCS scores at admission and FT3 level (P<0.05);the mean age in patients of low T3 group Ⅰ was significantly elder than that in patients of normal thyroid function group (P<0.05).ROC results indicated that the sensitivity and specificity of GCS scores in predicting mortality and re-bleeding rate were 63% and 73%,and those of FT3 level were 45% and 73%.Kaplan-Meier curves showed that both low GCS group and low T3 group Ⅱ had significantly increased mortality and re-bleeding rate as compared with high GCS group and high T3 group (P<0.05).Unified prediction results indicated that patients from low T3 and low GCS group had significantly higher mortality and re-bleeding rate as compared with patients from low T3 and high GCS group,high T3 and low GCS group,and high T3 and high GCS group (P<0.05).Conclusion Low T3 syndrome is common in patients with HICH;FT3 level and GCS scores appear to be important predictors for mortality and recurrence in patients with HICH.
ABSTRACT
Objective To understand the infection status and the main risk factors of soil-transmitted nematodes in children in the poverty-stricken areas of Guizhou Province and Sichuan Province,so as to provide the evidences for making suitable control strategies in these areas. Methods A total of 95 villages and 6 primary schools in 6 poor counties in the two provinces were select-ed as investigation sites according to the stratified random sampling method. Eleven preschool children aged 3-5 years in each sam-ple village and 11 school children aged 8-10 years in each sample school were chose as investigation objectives,their feces were collected and examined by Kato-Katz technique. In addition,the village doctor and cadres in each village were investigated by a standardized questionnaire to understand the deworming condition of children and the social economy and sanitary status in 2009. Then the correlation between average soil-transmitted nematode infection rate and its influencing factors was analyzed by the Tobit model. Results A total of 1 707 children from 95 villages in the two provinces were examined. In Guizhou Province,the total in-fection rate of soil-transmitted nematodes in children was 46.1%,and the infection rates of Ascaris lumbricoides,Hookworm,and Trichuris trichiura were 31.1%,4.1% and 10.8% respectively,and the corresponding rates in Sichuan Provinces were 9.8%, 3.6%,3.5%,2.7%,respectively. In the dimension of school,a total of 890 school-aged kids from 46 schools in the two provinces were examined,the infection rates of soil-transmitted nematodes,Ascaris lumbricoides,Hookworm,and Trichuris trichiura in Guizhou Province were 53.8%,32.7%,6.6% and 14.4%,respectively,and the corresponding rates in Sichuan Province were 7.3%,2.2%,2.9% and 2.2%,respectively. The results from Tobit analysis indicated that the proportion of children accepted deworming treatment and the paddy field in the cultivated area were statistically correlated to the infections of soil-transmitted nematodes and Ascaris lumbricoides(all P < 0.05). Conclusions The soil-transmitted nematode infection rates of children are still at a high level in poor areas of southwest China. In order to decrease the infection rates,besides the long-term deworming, water supply and sanitary improvement,and the health education should be strengthened,and the treatment of soil-transmitted nematodes should be covered by the New Rural Cooperative Medical System.

OBJECTIVE@#To evaluate the condition of cancer-related fatigue (CRF) in breast cancer patients with chemotherapy and to explore the effect of Yoga on it.@*METHODS@#After the completion of Yoga, 100 breast cancer patients with CRF (CFS>0) were selected and were randomly divided into the Yoga group and the control group (n=50). Patients in the control group only received routine cure and care while patients in the Yoga group received extra Yoga exercise, lasting for 4 months. Cancer fatigue scale (CFS) was evaluated in the 2nd, 4th and 6th round of chemotherapy.@*RESULTS@#At the end, 82 cases qualified for the study, 42 cases for the control group and 40 for the Yoga group. The mean score of body fatigue was 12.67±3.46. There was no significant difference in CRF between the Yago group and the control group before the Yoga intervention (P>0.05). After the 4th round of chemotherapy, the mean scores of CFS and body fatigue in the Yoga group were significantly lower than that in the control group (P<0.05). After the 6th round of chemotherapy, the mean scores of CFS, body fatigue and cognitive fatigue in the Yoga group were lower than that in the control group (P<0.05). Repeated analysis of variance showed that the difference in the overall fatigue, body fatigue and cognitive fatigue between the Yoga group and the control group was significant (P<0.05); the time influence on the overall fatigue, body fatigue and emotional fatigue was significant difference between the 2 groups (P<0.05); there were interactions between the effect of Yago and time on the overall fatigue, body fatigue and cognitive fatigue (P<0.05).@*CONCLUSION@#The body fatigue was more serious in breast cancer patients with chemotherapy. Yoga intervention could significantly reduce body fatigue, cognitive fatigue, thus reduce the overall fatigue in breast cancer patients with chemotherapy.
