ABSTRACT

Objective:To explore the effects of anterior temporal lobectomy on the mini-mental state examination (MMSE) score, memory of patients with intractable temporal lobe epilepsy.Methods:104 patients with refractory temporal lobe epilepsy admitted to our hospital from June 2013 to June 2019 were selected, of which 68 cases were treated surgically as the observation group and 36 cases were treated medically as the control group. MMSE score, memory and social functions were compared between the two groups. Results:The total effective rate of the observation group was 95.59%, which was significantly higher than 83.33% of the control group ( P<0.05). The MMSE score of the observation group was significantly higher than that of the control group at 1 month and 6 months after treatment ( P<0.05); the MMSE score of the control group at 6 months after treatment was significantly higher than that before treatment ( P<0.05). Six months after treatment, performance intelligence quotient (PIQ), verbal intelligence quotient (VIQ) and full intelligence quotient (FIQ) in the observation group were significantly higher than those in the control group ( P<0.05), and there was significant difference between the two groups ( P<0.05). The memory quotient (MQ) of observation group was lower than that of control group 1 month after treatment ( P<0.05), and higher than that of control group six months after treatment ( P<0.05). Six months after treatment, the scores of social function and comprehensive quality of life in the observation group were significantly higher than those in the control group ( P<0.05), and there were significant differences between the two groups ( P<0.05). There were 4 cases of complications in the observation group after treatment, all of them improved after drug treatment, without other serious complications. Conclusions:Anterior temporal lobectomy is effective in the treatment of refractory temporal lobe epilepsy, it can improve cognitive and memory functions and social behavior function of patients, and has less complications after operation, with certain clinical application value.

ABSTRACT

Objective To investigate the expression profile of long non - coding RNAs(lncRNAs) in acute kidney injury (AKI) rats upon astragaloside IV(AS - IV) treatment. Methods Eight male SD rats were selected and randomly divided into two groups. AKI model group(group 1):4 rats were subjected to ischemia - reperfusion(I/ R);AS - Ⅳ pretreatment group (group 2):4 rats were orally administered AS - Ⅳ for 7 days prior to I/ R. Renal tissues were collected after I/ R treatment of 24h,total RNA was extracted from renal tissues and tested for quality. Sequencing experiments were carried out after being qualified. The threshold set for up - and down - regulated genes was a fold change(group1 / group2)≥2 and P≤0. 05. Afterwards,GO analysis and KEGG analysis were used to determine the roles that these differentially expressed mRNAs played in these GO terms or pathways. Results Two hundred and thirty - two lncRNAs were differentially expressed(127 lncRNAs were up - regulated and 105 lncRNAs were down -regulated). Three hundred and forty - one mRNAs were differentially expressed(178 mRNAs were up - regulated and 163 mRNAs were down - regulated). GO analysis indicated that differentially expressed mRNAs were mainly involved in biological processes such as nucleosome assembly, chromatin assembly, nucleosome organization, chromatin assembly or disassembly and DNA conformation change. GO analysis indicated that differentially expressed mRNAs were mainly involved in cellular components such as nucleosome, protein - DNA complex, nuclear chromatin, chromatin and nuclear nucleosome. GO analysis indicated that differentially expressed mRNAs were mainly involved in molecular functions such as protein heterodimerization activity, protein dimerization activity,DNA binding, nucleic acid binding. Signal pathway analysis indicated that lncRNAs and mRNAs were involved in systemic lupus erythematosus, alcoholism and viral carcinogenesis. Conclusion LncRNAs expression differed significantly in renal tissues of AKI model group and AS - Ⅳ preconditioning group. Study of the biological functions and pathways of these lncRNAs indicated that they may be involved in the mechanism of AS - Ⅳ ameliorated ischemic acute renal injury.

ABSTRACT

Objective To assess on the effect of the different catheterization on patients with oral poisoning gastric lavage systematically and objectively. Methods A computerized search of PubMed, the Cochrane Library, EMBASE, Springer Link, Science Direct,China Biology Medicine (CBM),China National Knowledge Internet(CNKI), Wanfang Data and VIP database was performed for relevant randomized controlled trials (RCTs) which investigated the effects of the oral intubation gastric lavage on patients with oral poisoning relative to nasogastric lavage, retrieval to January 2016, and back into the study of references. According to include and exclude standard to screening literature, literature which met inclusion criteria was selected for quality evaluation and data extraction. Meta-analysis and trial sequential analysis were performed by using Rev-Man 5.3 and TSA soft-ware to estimate the required information size (RIS). Results 13 studies incorporated into Meta-analysis including a total o 1 296 patients. Meta-analysis results showed that the oral intubation gastric lavage group was better than nasogastric lavage in shortening the time of intubation [MD=-9.05, 95% CI(-12.86--5.23), P=0.00], improving the rate of intubation for the first time [MD=1.36, 95% CI (1.14-1.62), P =0.00], mucosal bleeding complication [MD=0.11, 95% CI (0.04-0.64), P=0.010], nausea and vomiting [MD=0.60, 95%CI (0.37-0.98), P=0.04], kinking [MD=0.14, 95% CI (0.02-0.80), P=0.03], reflex cardiac arrest [MD=0.24, 95% CI (0.08-0.71), P=0.01] and asphyxia [MD=0.45, 95% CI (0.26-0.80), P=0.007], the difference were statistically significant. But no significant difference existed in gastric tube fall off. Conclusions The oral intubation gastric lavage could shorten intubation time of patients, improve the first time intubation rate and reduce certain complications. It suggests that the oral intubation be extensively applied to care patients with oral poisoning.

ABSTRACT

EZH2 is over-expressed in human colon cancer and is closely associated with tumor proliferation, metastasis and poor prognosis. Targeting and inhibiting EZH2 may be an effective therapeutic strategy for colon cancer. 3-Deazaneplanocin A (DZNep), as an EZH2 inhibitor, can suppress cancer cell growth. However, the anti-cancer role of DZNep in colon cancer cells has been rarely studied. In this study, we demonstrate that DZNep can inhibit the growth and survival of colon cancer HCT116 cells by inducing cellular senescence and apoptosis. The study provides a novel view of anti-cancer mechanisms of DZNep in human colon cancer cells.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of slow anti-epileptic drug (AED) taper protocol and a rescue benzodiazepine protocol in video-electroencephalography (video-EEG) monitoring for presurgical evaluation of epilepsy.</p><p><b>METHODS</b>Sixty-two of 109 patients with refractory focal epilepsy underwent pre-surgical video-EEG monitoring with a slow AEDs taper protocol and a rescue benzodiazepine protocol. Seizures were recorded by video-EEG in 56 patients. The time to the first seizure, duration of monitoring, incidence of 4-h and 24-h seizure clustering, secondarily generalized tonic-clonic seizures (sGTCS), status epilepticus, falls and cardiac asystole were evaluated.</p><p><b>RESULTS</b>A total of 191 seizures were recorded in the 56 cases, and the diagnostic efficiency of video-EEG was 90.3%. The mean time to the first seizure was 2.4 days and the time to conclude video-EEG monitoring averaged 6.8 days. Eight (12.9%) patients had 4-h clusters and 24 (38.7%) had 24-h clusters. Seizure clusters were more frequent in extra temporal epilepsy than in temporal lobe epilepsy. While 19 sGTCS were recorded in 15 patients (26.8%), status epilepticus did not occur and no seizure was complicated by cardiac asystole. Epileptic falls with no significant injuries occurred in 4 patients.</p><p><b>CONCLUSION</b>Seizure clustering is common during presurgical video-EEG monitoring, but serious adverse events are rare with a slow AED tapering and a rescue benzodiazepine protocols. These two protocols are effective and save in presurgical video-EEG monitoring for refractory focal epilepsy.</p>

ABSTRACT

The presence of hyl gene in 364 PFGE clones of Enterococcus faecium was detected by colony hybridization under conditions of high stringency. The isogenic hyl-deficient mutant (* hyl) was constructed with suicide pTX4577 and screened by allelic replacement. Moreover, an in vitro study was made on the effect of hyl gene detection on the growth ability of hylgene detection on the mutant, and an in vivo study was made on the decrease of virulence in the mouse peritonitis model. The results showed, in the clinical isolates, the positive percentage of hyl gene was 32.8%, which was significantly higher than that (5.3%) in the non-clinical isolates. The * hyl was selected by kanamycin and identified by PCR, pulsed field gel electrophoresis (PFGE) and Southern blot. The experimental evidence indicated that the growth ability of * hyl was remarkably reduced in comparison with that of the wild-type strain. The percentage survival of mice in TX2466 groups was 0, while that of * hyl groups was 50% at the same inoculum in mouse peritonitis. The differences were significant. These data suggest that hyl gene in specific E. faecium strains may be enriched in determinants that make them more likely to cause clinical infections. Being important in the pathogenesis, hyl gene is probably a major virulence factor of Enterococcus faecium.

ABSTRACT

OBJECTIVE To study the Pseudomonas aeruginosas drug resistance in intensive care unit and its fluoroquinolone-resistant molecular mechanism, and provide scientific basis for rational employment of antibiotics in clinic. METHODS E test was used to determine the minimal inhibitory concentrations(MIC) of 13 antibiotics against 83 P. aeruginosa strains. Twenty-eight fluoroquinolone-resistant strains were selected with standard sensitive strain-ATCC27853 as control. The quinolone resistance-determining region(QRDR)of the gyrA and parC genes was amplified by PCR and sequenced. RESULTS The positive rate of P. aeruginosa in sputum specimen was the highest from 83 strains (71.08%). gyrA Genes of all resistant strains had an ACC to ATC mutation in codon 83,leading to the amino acid substitution of threonine for a an isoleucine and 11 high level resistant strains also showed a GAC to GGC mutation in codon 87,leading to the substitution of an aspartic acid a glycine for. In addition,14 resistant strains also had an TCG to TTG mutation in codon 87 of parC gene, leading to the amino acid substitution of a serine for a leucine. We didn't find parC gene mutation existing independent in fluoroquinolone-resistant P. aeruginosa. CONCLUSIONS Meropenem remains highly active against P. aeruginosa. But the abuse of imipenem and other fluoroquinolones leads to rise in their drug-resistance rate. Fluoroquinolone-resistance has increased rapidly, the mechanism of resistance is gene mutant. It displayed that gyrA and parC gene mutation is associated with fluoroquinolone resistance.
