ABSTRACT

Objective To study the pros and cons of two methods of infusing itraconazole injectionand prevent the blockage of peripherally inserted central catheter (PICC) and improve patients' satisfaction with nursing technology.Methods 172 patients infusing itraconazole were divided into two groups by random digital table method.86 cases established an independent infusion pathway as the control group,another 86 cases using PICC for itraconazole injection and withdrawing plunger of the syringe about 0.5 ml before and after the infusion and then pulsing-flushing it with 10 ml normal saline as the experimental group.Then compared the blockage rate of PICC and the patients' satisfaction with nursing technology.Results The blockage rate of the two groups had no significant difference (x2 =0.206,P > 0.05) while patients' satisfaction with nursing skills was distinct,and the experimental group's was 96.51% (83/86),much higher than 16.28% (14/86) of the control group.Conclusions Withdrawing taken before and after the infusion of itraconazole injection could effectively prevent catheter blockage and improve patients' satisfaction with nursing technology.

ABSTRACT

BACKGROUND:Reduction quality is essential for the healing and function of limb. Traditional X-ray and CT scans have some limitationsin quantitatively and visualy observing the reduction quality of the remnant fracture in three-dimensional displacements. Mimics have advantages in bone rapid modeling and three-dimensional measurement, which offers the chance in quantifying the reduction quality of the displacement in three-dimensional methods. OBJECTIVE:To measure residual displacement in femoral neck fracture patients with satisfactory reduction by using three-dimensional measurement method, and to evaluate the association between residual displacement and femoral head necrosis. METHODS:We enroled 141 patients with femoral neck fracture, who were treated in the Third Department of Joint, Affiliated Hospital of Logistics University of People’s Armed Police Force from January 2012 to December 2013. Al patients were divided into undisplaced fracture group (n=44; Garden1&2) and displaced fracture group (n=97; Garden3&4). Based on DICOM data of CT files, three-dimensional models were constructed in each patient. Three parameters were measured: displacement of femoral head’s center, fossa capitis femoris and rotational displacement. Femoral head necrosis was folowed up. The correlation between residual displacement and femoral head necrosis was explored. RESULTS AND CONCLUSION:(1) The duration of folow-up in those patients ranged from 25 to 43 months. Femoral head necrosis occurred in 7 cases from the undisplaced fracture group and 21 cases from the displaced fracture group. (2) The three parameters (displacement of femoral head’s center, fossa capitis femoris and rotational displacement) were respectively (7.44±2.84) mm, (11.75±4.12) mm and (17.68±8.99)° in the undisplaced group, and (6.51±3.75) mm, (9.34±5.27) mm and (14.67±7.43)° in the displaced group. Except displacement of femoral head’s center, significant differences in the other two parameters were detected between the two groups (P< 0.05). (3) These findings suggest that poor reduction quality is the main factor causing femoral head necrosis. X ray is insufficient to describe the spatial shift and the angle, and has limitations in assessing the reductionquality of femoral neck fracture. Three-dimensional measurement has certain advantages to assess quality of fracture reduction.

ABSTRACT

Objective To study the effects of standardized-lifestyle intervention on the type 2 diabetic elder patients.Methods The study group enrolled 270 individuals with type 2 diabetes mellitus randomly by using the model of “Zhiji Health Care” and the software of the lifestyle and disease prevention system.Parameters were collected during a six month period for the evaluation,such as fasting plasma glucose(FPG),postprandial glucose(PPG),cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),body mass(BMI),glycosylated hemoglobin(HbAlc),blood pressure,daily intake,total physical activity and effective physical activity and other indicators.Results After six months' standardized lifestyle intervention:(1)in the type 2 diabetic patients,the parameters of weight,BMI,waist circumference,systolic and diastolic blood pressure were significantly decreased(t values were:23.82,25.91,19.56,23.37 and 19.53,respectively,all P < 0.01); FPG,PPG,HbA1c,TC,TG and LDL-C were significant decreased compared with that before intervention(t values were: 12.06,11.82,9.74,6.98,5.79 and 6.10,respectively,all P < 0.0l).Meanwhile,HDL-C was significantly increased(t =2.51,P < 0.05).(2)the total daily intake was significantly reduced compared with that before intervention.The effective exercise burden was significantly increased,compared with that before intervention(P < 0.01).Conclusion Standard lifestyle management helps to control the blood glucose,blood pressurem,body weight and other risk factors,with reduction of complications and medical costs.

ABSTRACT

Objective To evaluate the therapeutic efficacy of neoadjuvant chemotherapy in cervical carcinoma. Methods TAC(DDP, EPI and BLM) was applied in 68 patients with cervical carcinoma. Therapeutic efficacy and side effects were documented. Results After 1~3 cycles of neoadjuvant chemotherapy, clinical symptoms remission rares reached 100% ,response rate were 100% (40/40) in Ⅰ b~Ⅱ a,90.9% (20/22) in Ⅱ b and 50% (2/4)in Ⅲ a, respectively. Radical resection reached 91.2 %, and only 6 patients of advanced disease failed to radical resection and turned to radiotherapy. Conclusion Neoadjuvant chemotherapy is a safe and effective therapeutic option which could alleviate clinical symptoms, decrease tumor size, down-staging and increase the radical resection rate.

ABSTRACT

Background and purpose:It has been confirmed that homozygous deletion of p16/p15 gene and its co-deletion of p16/p15 genes were related to the occurrence, progress and prognosis of epithelial ovarian cancer. However, the mono-deletion and co-deletion of the genes has been detected with tissue but not in serum DNA of the epithelial ovarian cancer. In this article, we studied the relationship between homozygous deletion of p16/p15 gene and its co-deletion of p16/p15 genes in serum DNA of the epithelial ovarian cancer.Methods:Primers were used to amplify exon 2 of p16 and exon 2 of p15 gene by polymerase chain reaction. Homozygous deletions of the p16, p15 and co-deletion of p16/p15 genes were studied in either serum DNA of 165 patients with epithelial ovarian cancer, their counterpart lymphocytes DNA, serum DNA of 25 benign ovarian cyst or of 15 health donors.Results:The homozygous deletion rates of either p15 or p16 gene were 27.9%(46/165)and 27.3%(45/165)serum DNA in the patients with epithelial ovarian cancer respectively, while the co-deletion rate of p16/p15 genes was 24.2% (40/165). However, the deletions of p15/p16 genes and its co-deletion were not found in serum DNA of the counterpart lymphocytes,25 benign ovarian cyst and 15 health donors (The P values were 0.000、0.000 and 0.000 respectively). The deletions of either p15 or p16 gene for the patients with stage Ⅰ~Ⅱ were 14.3%(5/35) and 11.4%(4/35), 33.3%(25/75) and 32.0%(24/75) for the patients with stage Ⅲ, 29.1%(16/55) and 30.9% (17/55) for stage Ⅳ, respectively. Although there was no significant differences among the groups, the deletion of p15 and p16 genes in the patients with advanced stage were higher than that with early stage. The deletion was not found to be associated with histopathology of epithelial ovarian cancer.Conclusions:Homozygous deletions of the p16, p15 genes and its co-deletion of p15/p16 genes were commonly found in the serum DNA of epithelial ovarian cancer and might be associated with clinical stage of the disease. It was suggested that detection with serum DNA may be used as a micro-invasive approach and the deletion of genes might served as biological markers for the development and prognosis of the patients with epithelial ovarian cancer.

ABSTRACT

Purpose:To study the role of ovarian cancer tumor suppressor genes (OCTSGs) and to determine the relationship between 3p loss of heterozygosity (LOH) and the pathogenesis of ovarian cancer. Methods:50 cases of epithelial ovarian tumor, including 40 cases with malignant tumors and 10 cases with benign tumors, were examined by polymerase chain reaction (PCR) with D3S1228 and D3S1038 microsatellite polymorphic markers at 3p. Results:Thirty two of the 40 cases (80%)with ovarian cancer showed either LOH at 3p14 or at 3p25, but only one of 10 cases (10.0%)with bengin ovarian tumor there occured 3p LOH. Among them, 24 cases (60.0%) had LOH at 3p14 and 16 cases (40.0%) at 3p25, as well as 8 cases (20.0%) there was co deletion on both at 3p14 and 3p25. According to FIGO staging, the rate of LOH at 3p in ovarian cancer patients with Ⅲa , Ⅲb was higher than that in patients with stage Ⅰ, Ⅱ ( P 0.05). Conclusions:Since 3p14 and 3p25 LOH are related to the staging of ovarian cancer as well as the common occurrence of 3p LOH in epithelial ovarian cancer, it is suggested that there are some candidate ovarian TSGs harbored at 3p regions and the detection of 3p LOHs may be used as a genetic marker for monitoring the development of ovarian cancer.
