ABSTRACT

Objective@#To evaluate the effectiveness and safety of percutaneous nephrolithotomy(PCNL) with the all-seeing needle system(Microperc)for the treatment of upper urinary calculi in children.@*Methods@#Retrospective analysis of 25 cases of upper urolithiasis treated with Microperc from November 2016 to January 2018 in Guizhou Provincial People′s Hospital was performed, including 18 boys and 7 girls.The age of patients was (6.4±3.9) years(10 months-14 years), and 4 cases were less than 3 years old.Among them, 3 cases had upper ureteral calculi and 22 cases had renal calculi.The mean stone size was 14.5 mm(8-20 mm). Stone fragmentation was performed by using a 200 μm holmium(YAG laser fiber).@*Results@#All patients were successfully treated in stage I. In 6 patients, access was achieved through the 4.8Fr all-seeing needle method.Conversion to mini-PCNL (10-12Fr) was required in 19 patients due to intraoperative bleeding and high stone burden.The duration of operation time was (60.8±15.1) min (40-100 min). The mean postoperative drop in hemoglobin was (7.5±5.4) g/L, and no patient required blood transfusion.The hospital stay was (4.9±1.2) days (3-6 days). There were residual fragments in 3 patients, and the overall stone-free rate at 4 weeks was 88%(22/25 cases). Four cases had a fever(Clavien grade Ⅰ) complications, while no grade Ⅱ and grade Ⅲ complications were observed.@*Conclusions@#Microperc can establish a surgical channel through visual images quickly and safely, and it greatly reduces the risk and difficulty in doing operation.This technology is not only suitable for learner of surgery, but also for patients with mild hydronephrosis and children with calculus.

ABSTRACT

Objective To compare the efficacy and safety of novorapid and novolin R in the treatment of patients with gestational diabetes.Methods From September 2014 to October 2015,the clinical data of 58 patients with gestational diabetes in our hospital were retrospectively analyzed.According to the drug used,the patients were divided into two groups.The observation group was given novorapid,and the control group was given novolin R.Before and after treatment,the total effective rate,fasting blood glucose (FBG),2h postprandial blood glucose (2hPBG),average blood glucose control time,body mass index (BMI),alanine transaminase (ALT),aspartate aminotransferase (AST),serum creatinine (SCr),urea nitrogen (BUN) and the frequency of hypoglycaemia,insulin dosage,the pregnancy outcome were compared between the two groups.Results After treatment,the FBG [(5.14±0.39)mmol/L],2hPBG[(7.06±1.25)mmol/L] of the observation group were significantly lower than those of the control group (t=11.72,5.67,all P<0.05).ALT[(25.27±8.71)U/L],AST[(27.98±8.53)U/L],SCr[(69.92±11.39)μmol/L],BUN[(5.23±5.75)mmol/L] of the observation group had no obvious differences compared with the control group (t=0.14,0.07,0.04,0.30,all P>0.05).The total effective rate (93.10%),average blood glucose control time (3.06±0.43)d,insulin dosage (36.15±4.72)U/d of the observation group were significantly better than those of the control group (χ2=4.35,t=14.30,4.92,all P<0.05).The incidence rates of pregnancy-induced hypertension (3.45%),amniotic fluid (6.90%),cesarean delivery (17.24%),macrosomia(3.45%),high blood bilirubin(3.45%) of the observation group were significantly lower than those of the control group(χ2=5.22,5.50,4.08,9.09,7.73,all P<0.05).Body weight [(70.6±3.09)g],BMI[(23.9±0.28)kg/m2] of the observation group were better than those of the control group(t=2.20,12.19,all P<0.05).The incidence rate of hypoglycemic events in observation group(17.24%) was significantly lower than that of the control group(χ2=5.47,P<0.05).Conclusion Compare with novolin R,the treatment effect of novorapid for gestational diabetes is remarkable and has high security,it is worthy of clinical implementation.

ABSTRACT

The antioxidant effects of the triterpenoid-rich extracts from Euryale ferox shell [ES] have been confirmed in vitro. This study examined whether the triterpenoid-rich extract from ES eases human hyperglycemia and diabetes caused by metabolic disorders. Normal and streptozocin [STZ]-induced diabetic mice were used as controls for the four groups that received the triterpenoid-rich extracts of ES suspended in distilled water orally at doses of 200, 300, 400, 500 +/- 2 mg/L. Body weight, blood glucose and pancreatic tissue morphology were observed after 4 weeks. The expression of protein tyrosine phosphatase-1B [PTP1B] and insulin receptor substrate [IRS-1] proteins, which are related to the regulation of glucose metabolism in vivo, were also investigated. Compared with the model group [LD[50] 900 +/- 2 mg/L], it was found that the triterpenoid-rich extracts of ES could regulate glucose metabolism [P<0.01] and cause body weight to return to normal levels [P<0.05]. Islet morphology recovered well, the expression of the negative regulation protein PTP1B gene was reduced and insulin receptor IRS-1 protein expression was increased. These data prove that the triterpenoid-rich extracts from ES have a therapeutic effect on diabetes by insulin resistance

ABSTRACT

Mannosylerythritol lipids (MELs), mainly produced by Ustilago and Pseudozyma, are surface active compounds that belong to the glycolipid class of biosurfactants. MELs have potential application in food, pharmaceutical and cosmetics industries due to their excellent surface activities and other peculiar bioactivities. In recent years, the research field of MELs has regained much attention abroad. However, MELs are rarely studied in China. In this review, the producing microorganisms and production conditions, diverse structures, biochemical properties, structure-function relationship and biosynthetic pathways of MELs are described. Some research problems and prospects are summarized and discussed as well.

ABSTRACT

Interferon beta (IFN-beta) and TNF-related apoptosis-inducing ligand (TRAIL) are effective anticancer agents. Adeno-associated virus (AAV) is one of the current most promising gene delivery vectors. Previously, we constructed tumor-targeting AAV-hTERT-IFN-beta and AAV-hTERT-TRAIL by inserting IFN-beta or TRAIL gene into AAV controlled by hTERT promoter. The studies showed that either single IFN-beta or TRAIL gene therapy exhibited a certain extent anticancer effect. Here, we report their inhibitory effects on A549 lung cancer cell growth in vitro and in vivo by combined AAV-hTERT-IFN-beta and AAV-hTERT-TRAIL. Expression of secreted IFN-beta in lung cancer A549 cells infected by AAV-hTERT-IFN-beta was detected by enzyme-linked immunosorbent assay (ELISA). The growth-suppressing effect of AAV-hTERT-IFN-beta in combination with AAV-hTERT-TRAIL on several cancer cell lines was assessed by MTT assay. Apoptosis of A549 cancer cells infected by AAV-hTERT-IFN-beta alone, AAV-hTERT-TRAIL alone, and their combination was evaluated by apoptotic cell staining and flow cytometry (FCM), respectively. The antitumor effect of the combination of AAV-hTERT-IFN-beta with AAV-hTERT-TRAIL in vivo was further evaluated through A549 lung cancer xenograft in nude mice. The results showed that the combinational treatment was superior to any alone and presented intensified tumor cytotoxic and apoptotic effect on A549 cancer cells. Most importantly, the combination of AAV-hTERT-IFN-beta with AAV-hTERT-TRAIL exhibited significant antitumor effect and eliminated all tumor masses in nude mice, which lay a foundation for exploring the molecular mechanisms of combined IFN-beta and TRAIL anti-tumor activity.

ABSTRACT

Objective To investigate the effects of sepsis bundles in China.Method An observational study of 43 patients with severe pneumonia and septic shock admitted to the respiratory intertive care unit(1/11/2006-31/12/2007)was carried out.The selection criteria were in accordance with criteria set by International Conference On Sepsis in 2001.Implementation of 6 hours and 24 hours sepsis bundles was divided into 3 continu-ous phases consisting of education,trial,and application phase.A cohort of 43 patients with matched disease his-tory(1/1/2004-31/10/2006)was enrolled as control group.The percentages for categorical variables and mean±SD for continuous variables were reported.Chi-Square test.unpaired Student's t -test.paired-samples t test,univariate and multivariate logistic regression models were used.Statistical significance was defined as P<0.05.Results There were very little significant differences in basic characteristics of patients between the two groups.Compared with control group,the differences in serrum lactate,fluid resuscitation and fluid volume infused within 6 hours and blood glucose control in shock subgroup were significant(P values were 0.024,0.009,0.045,and 0.000,respectively).Compared with control group,the differences in respiratory rate and oxygenation index of bundles group at 72 hours later were significant(P values were 0.033 and 0.041,respectively).Compared with control group,the differences in APACHE Ⅱ score and predicted mortality in shock subgroup of bundles were sig-nificant(P values were 0.017 and 0.040,respectively).Compared with control group,the reduction in absolute mortality was 23.30% in bundles group(P=0.019).Conclusions Implementation of sepsis bundles con-tributes noticeably to the significant reduction in mortality of patients with severe pneumonia and septic shock.

ABSTRACT

Objective To analyze the injury types and radiologic findings of the wounded in the major Wenchuan earthquake of May 12, 2008. Methods Radiologic results of 1013 victims admitted to MianYang Central Hospital from May 13, 2008 to May 19, 2008 were studied retrospectively, including X-ray of 854 cases (about 2900 local exam positions) and CT scan of 259 cases (about 300 local exam positions). Results 733 cases showed radiologic abnormalities, including 527 cases of single region injury (accounting for 52. 0%) and 206 cases of multi-region injury (accounting for 20. 3%), the overall positive rate was 72. 4%. The results were in the following order on the basis of the incidence, 288 cases of lower extremity injuries (28.4%), 151 cases of thoracic injuries(14. 9%), 136 cases of upper extremity injuries(13.4%), 133 cases of spine fracture(13.1%), 114 cases of pelvic fracture and dislocation(11.3%),88 cases of craniocerebral injuries (8. 7%), 25 cases of maxillofacial fracture (2. 5%) and 8 cases of abdominal tranma(0. 1%). Thoracic injuries included: rib fracture in 136 cases, pulmonary injury in 59 cases, and sternal fracture in 3 cases. Spinal fracture included: cervical vertebrae involved in 12 cases,thoracic vertebrae involved in 43 cases, lumbar vertebrae involved in 93 cases, and Sacrococcygeal vertebraeinvolved in 12 cases. Craniocerebral injuries included: skull fracture in 41 cases, endocranial abnormalities in 72 cases. Abdominal trauma included: intestinal obstruction in 4 cases, renal contusion in 3 cases and abdominal wall hematoma in 1 case. Conclusion Most parts of the body can be injured by earthquake.Bone fracture and dislocation of lower extremity are the most common manifestations.

ABSTRACT

Objective To evaluate the information of head injury caused by the major earthquake of Wenehuan and the diagnostic value of muhi-slice spiral CT(MSCT). Methods A total of 1557 earthquake victims were hospitalized, of which head CT scan was performed in 292 eases, and abnormalities were found in 225 of them. The composition, type of injury and MSCT findings of head injury were retrospectively analyzed. Results CT abnormalities were found in 225 victims, including 66 cases of skull fracture,50 eases of cerebral contusion and laceration, 55 eases of intracranial hematoma, 16 cases of subaraehnoid hemorrhage, 16 eases of subdural collection of fluid,20 eases of foreign substance of scalp,and 2 eases of traumatic cerebral infarction. Operation was performed in 22 of them. Conclusion The incidence of head injury in earthquake was high, and most of them were not severe. The examination of MSCT was rapid, MPR and 3D images can clearly show the lesion of craniocerebral trauma. MSCT can provide more valuable information for clinical therapy in craniecerebral trauma caused by earthquake.

ABSTRACT

Objective To evaluate the CT features and the role of CT in the diagnosis of Crohn′s disease. Methods CT manifestations of 18 patients with pathology confirmed Crohn′s disease were retrospectively analyzed. The majority of the patients were scanned with spiral CT or multidetector row CT.Results All 18 patients, segmental thickening of the bowel wall was detected. There were two different appearances in contrast-enhanced CT scans: (1) mural thickening without enhancement or mural stratification, and homogeneous attenuation in the thickened wall were observed in 7 patients; (2) mural thickening and stratification, resulting in the target or double-halo appearance, were detected in 11 patients, and the bowel wall thickening ranged from 5 to 10 mm. Nine patients had perienteric abnormalities including fibrofatty proliferation, phlegmon and mesenteric hypervascularity. According to the CT manifestations, 5 patients were initially diagnosed as other diseases including intestinal obstruction (1 case), tuberculosis (1 case), cancer (2 cases), and abscess (1 case), respectively. Conclusion CT has the unparalleled ability to depict the abnormalities in the bowel wall, mesentery, abdominal and pelvic viscera, and contrast-enhanced CT can distinguish active Crohn′s disease from quiescent Crohn′s disease. These features make CT particularly valuable in the management of Crohn′s disease.

ABSTRACT

Objective To improve recongnition of CT manifestations and value in diagnosing aortic dissection and rupture.Methods The CT plain and enhanced findings in 16 cases with aortic dissection were analyzed retrospectively,these 16 cases were classified in to DeBakey type Ⅰ?Ⅱand Ⅲ in 2,4 and 10,in which 3 cases had subacute and chronic rupture.Results Among these 16 patients with AD,the aortic double lumen and intimal flap,6 cases were showed minor calcifications in the dissection membrane,3 cases were acute rupture or slow leakage complications including periaortic mediastinal,pericardiac pleural or retroperitoneal effusion and retroperitoneal diffuse fibrosis.Conclusion CT is a helpful reliable and rather simple technique for AD lesion and complication.It should be suggested if a diffuse fibrosis occure in retroperitoneal space complicating with chronic permeability of AD.

ABSTRACT

Objective To evaluate the value of percutaneous hyperthermia-chemotherapy (PHC)under CT guided in treating original and secondary hepatic and pulmonary malignant tumor.Methods Percutaneous hyperthermia and chemotherapy under CT guided was performed for 21 patient with original and secondary hepatic and pulmonary malignant tumor.Chemical drugs against tumors were warmed to 55~60℃ and injected into the tumors.Injected volume was according to:V=4/3 ?(r+0.5 cm) 3.Observation depends upon attenuation changes of CT scanning and biochemical index(AFP)The therapeutic effect was classified into Ⅰ~Ⅴgrade.Results The period of observation was 36 monthes,In 20 cases,survival period was 8~22 monthes,average survival period was 16 monthes.A patient had treated with PHC and transcatheter arterial embolization and was alive for 28 monthes.Total effective rate was 95.2%.Conclusion PHC under CT guidence is an effective method in treating hepatic and pulmonary malignant tumors.especially for unresected tumors.Cooperating transcatheter arterial embolization(TAE)can raise curative effect.

ABSTRACT

Objective: The effect of oil of Hippophae rhamnoides (OHR) supplements on sGOT,metabolism of free radical and ultrastructure of myocardium was observed in mice.Methods: 70 male ICR mice were randomly divided into six groups: sedentary control group (SCG), sedentary+OHR group (SHG), immediately after exhaustive exercise in control group (IEC) and in OHR group (IEO), 24 hours recovery after exhaustive exercise in control group (REC) and in OHR group (REO). After 6 w swimming program with gradually increasing intensity, these mice carried out an exhaustive swimming before sacrifice. Serum GOT, SOD and MDA in myocardium were measured and the changes of ultrastructure of myocardium were observed by transmission electron microscope (TEM). Results: 1) Serum GOT was significantly different among these groups except between SCG and SHG, IEC and REC. 2) SOD was significantly different among the above groups except between SCG and SHG, IEO and REO. 3) The concentration of MDA was significantly different among the above groups except between IEC and IEO, REC and REO. 4) The integrity of mitochondrion, rough surfaced endoplasmic reticulum and myofilament in myocardium was better in OHR groups than in control groups and the pathological tendency of these ultrastructures was lighter in OHR groups compared with control groups. Conclusion: OHR supplementation can reduce the damage of myocardium caused by exercise and enhance antioxidative status of myocardium; OHR supplementation can protect the ultrastructure of myocardium.
