ABSTRACT

A 44 years old male patient suffered from flame burn of 20% total body surface area was admitted to our hospital on February 14th, 2018. On admission, his abdominal CT was not obviously abnormal. Eleven hours after burn, the patient had left upper abdominal pain, accompanied by reduction of urine output. Then he suffered from sudden hypotension and hypoglycemia. Acute pancreatitis was diagnosed by abdominal CT reexamination. Low glucose level was ameliorated slowly through positive rescue, and pancreatitis crisis progressed rapidly. The family members gave up rescue care, and patient discharged. The case indicates that physicians should pay attention to glucose levels of severe burn patients, and be cautious of appearance of postburn pancreatitis.

ABSTRACT

OBJECTIVE@#: To study the feasibility and effect of PeriCam PSI system guiding the establishment of ischemia/reperfusion injury model in rats.@*METHODS@#: A total of 70 adult male Sprague-Dawley rats were divided into the control group(=6), PSI monitoring group(=34) and traditional operation group(=30). Ischemia reperfusion model was established with reference to improve Zea-Longa line plug method. After the model established, the blood flow to the brain of control group, PSI monitoring group (ischemic 2 h, 24 h reperfusion) were observed and recorded respectively with PSI. The rats were then executed after 24 h, and the 2,3,5-triphenyltetrazolium chloride (TTC) staining and HE staining were used to observe the brain tissue.@*RESULTS@#: The survival rate and modeling success rate of PSI monitoring group were higher than those of the traditional operation group(all <0.05). The blood perfusion in the brain and the distribution of blood vessels were clearly observed in the control group, and the data were normal. In 2 h ischemic group, the arterial flow was interrupted in the right cerebral artery, and the blood flow in the middle arterial blood supply was significantly decreased than that in the control group(<0.05). After the recovery of 24 h, the artery in the right side of the brain was restored to blood flow, but the blood flow in the partial supply area decreased, unable to recover to normal level. The TTC staining results indicated that there were obvious infarcts in the right brain tissue of PSI monitoring group,and the infarct area was more stable than that of the traditional operation group. The results of HE staining showed that the structure of brain tissue in the control group was normal, and the morphological rules of nerve cells were not change. While in brain tissue from PSI monitoring group, cortex and ischemia half dark stripe, nerve cell degeneration, necrosis and glial fiber disintegration, liquefaction, and light color, screen mesh in ischemic central area were observed.@*CONCLUSIONS@#: PSI system can guide ischemia reperfusion model building and improve the success rate of the model.

ABSTRACT

Objective To compare the characteristics of pulmonary function between stable chronic obstructive pulmonary disease (COPD) and asthma-COPD overlap syndrome (ACO) in the elderly males. Methods Three hundred and eighteen elderly male patients with stable COPD were followed up between January 2014 and December 2017. A11 patients underwent pulmonary function tests and bronchial dilation test (BDT) in the stable condition. According to the occurrence of ACO, the patients were divided into ACO group (54 cases) and non-ACO group (264 cases). The indexes of pulmonary function before and after BDT (T0 and T1) in two groups were statistically compared. Results Among 318 elderly male patients with stable COPD, ACO patients accounted for 17.0%(54/318). Compared with non-ACO group, at T0 ACO group had significantly lower values in forced vital capacity (FVC) pred [(83.5 ± 15.4)%vs. (93.9 ± 5.6)%], in forced expiratory volume in 1 second (FEV1) pred [(61.1 ± 13.6)%vs. (74.5 ± 12.0)%], in forced expiratory volume in 3 seconds (FEV3) pred [(70.5 ± 13.4)% vs. (81.8 ± 12.9)%] and in forced expiratory flow rat (FEF25%-75%) pred [(26.7 ± 8.5)%vs.(36.7 ± 10.2)%](P<0.05), but at T1 there were no significant differences in FVC pred, FEV1 pred, FEV3 pred and FEF25%-75% pred between the two groups(all P>0.05). And ACO group also had significantly higher values in residual volume pred [(138.9 ± 25.7)%vs. (117.5 ± 26.6)%] and in residual volume/total lung capacity [(55.8 ± 9.7)% vs. (45.1 ± 8.9)%] (P<0.05). Conclusions ACO is common in the elderly male patients with stable COPD, and ACO patients have lower time vital capacity as compared with non-ACO patients. But after bronchodilation test, the two groups have similar ventilation function, and the small airway function in ACO patients improves more significantly .

ABSTRACT

Objective To investigate the comprehensive intervention residents health literacy with the thera-peutic efficacy of hypertensive patients on diabetes,and to provide a reference for the implementation of clinical inter-ventions.Methods 1 684 cases of community residents were selected as the research objects.Lectures on health,one on one counseling,health manuals mode on the implementation of comprehensive intervention residents health litera-cy.Comparison of the residents after comprehensive intervention China citizen health literacy questionnairesurvey results,and the awareness rate of diabetes,high blood pressure,diabetes hypertension compared after comprehensive intervention,the awareness rate,treatment rate and control rate of blood pressure,blood glucose were investigated. Results After comprehensive intervention,the basic knowledge and concept healthy residents of the ratio of 81.94%,healthy lifestyle and behavior of the ratio of 82.18%to 81.77%the proportion of health,skills,health liter-acy was 54.98%,which were significantly higher than those of intervention,the difference (χ2 =17.88,90.67, 193.72,413.88,all P<0.05);the number of patients of hypertension in 537 cases,the incidence was 31.89%.The prevalence of hypertension in residents after comprehensive intervention,the awareness rate was 64.06%,the stand-ardized treatment rate was 51.02%and the control rate of hypertension was 35.01%,which were significantly higher than those of intervention(χ2 =67.93,24.85,30.66,all P<0.05);prevalence of diabetes in 269 cases,the inci-dence was 15.97%.The prevalence of diabetes in residents after comprehensive intervention,the awareness rate of diabetes was 78.07%,standardized treatment rate was 60.97%,the blood sugar control rate was 37.17%,which were significantly higher than those of intervention(χ2 =68.78,6.33,17.48,all P<0.05).Conclusion The comprehensive intervention residents health literacy is the effective measures to improve the residents'health literacy and health liter-acy,improve help control blood pressure in patients with hypertension and diabetes,glycemic index,which is of posi-tive significance to promoting the control hypertension,diabetes.

ABSTRACT

BACKGROUND:It remains unclear what kind of changes in imaging and histopathology would appear in intervertebral disc after removal of nucleus pulposus over time. OBJECTIVE:To observe the alteration in imaging and histopathology of rabbit lumbar intervertebral disc after nucleus pulposus aspiration. METHODS:A total of 32 Japanese rabbits were subjected to nucleus pulposus aspiration in posterior lateral L 3/4 intervertebral disc with a 21-gauge hypodermic needle. L 2/3 intervertebral disc served as a normal control. At 2, 4, 8 and 12 weeks after aspiration, eight rabbits according to grouping received radiograph in the lateral position of lumbar vertebra. The height of L 3/4 and L 2/3 intervertebral space was measured and intervertebral disc height index was calculated. Rabbits received median sagittal MRI examination and histopathological examination of intervertebral disc. RESULTS AND CONCLUSION:The intervertebral disc height degraded slowly at 2, 4, 8, and 12 weeks after aspiration. However, the difference between 8 and 12 weeks was reduced. Compared with normal control, intervertebral disc height index significantly reduced at various time points (P<0.05). The nucleus pulposus signal intensity was gradual y reduced at 2, 4, 8, and 12 weeks after aspiration, and reached grade 4 of modified Thompson grading criteria at 8 weeks. After aspiration, gelatinous nucleus pulposus of disc slowly exhibited fissures with time, morphous was slowly disordered and showed significant fibrosis performance at 12 weeks. In nucleus pulposus region, more cartilage-like cells were found at 4 weeks after operation. The cells were active. Nucleus pulposus cells decreased significantly. At 8 and 12 weeks after operation, fibroblasts increased in nucleus pulposus region, and the number of cartilage-like cells reduced. The annulus fibrosus gradual y became distorted, disordered, with processes and layers, and fiber breakage appeared. These results indicated that after puncturing lateral annulus fibrosus and aspirating the nucleus pulposus, radiograph height of the intervertebral disc and MRI T2-weighted signal intensity gradual y reduced. Pathological changes were observed. However, the degeneration al eviated between 8-12 weeks.

ABSTRACT

BACKGROUND:Studies have shown that low-intensity and short-time vibration with a certain frequency can reduce the absorption of bone tissue and increase the quantity and quality of bone through promoting the proliferation and differentiation of osteoblasts. OBJECTIVE:To investigate the effects of different frequencies of vibration strains on cycle, proliferation and differentiation potency of RAW264.7 cel s cultured in vitro. METHODS:Passage 6 RAW264.7 cel s in good conditions were randomly divided into six groups, and each group was induced cultured with Dulbecco’s modified Eagle’s medium containing receptor activator of nuclear factor kappa B ligand. The final concentration of receptor activator of nuclear factor kappa B ligand was adjusted to 50μg/L, and then kept without changes. The non-loading group did not loaded with vibration strain, and the other five groups were loaded with 3-10 Hz, 15-35 Hz, 35-45 Hz, 50-70 Hz and 70-90 Hz vibration strains on the RAW264.7 cel s respectively. The other vibration parameters were consistent;the vibration time was 15 min/time with the vibration intensity of 0.3 g, twice per day. The cel cycle and cel proliferation were detected at 3 and 6 days after loading of vibration strains. RESULTS AND CONCLUSION:After composite vibration loading for 6 days, the cel cycle phase in the vibration groups was changed to some extent when compared with the non-loading group. Compared with non-loading group, the cel number in the G 1 phase of the vibration group was significantly increased (P<0.01);the cel number in the S phase and G 2+M phase of the vibration group was significantly decreased (P<0.01);the vibration parameters in the vibration group were significantly decreased (P<0.01). The results indicate that different frequencies of vibration strains can affect the cel cycle and proliferation, and can inhibit the proliferation and differentiation of osteoblasts.

BACKGROUND: Resection of the nudeus pulposus is the classical treatment for intervertebral disc protrusion, except a higher recurrence rate. OBJECTIVE: To verify the feasibility of establishing an animal model of intervertebral disc degeneration by puncture and aspiration via a posterolateral approach to simulate resection of human nucleus pulposus. DESIGN, TIME AND SE'I'FING: The experiment was conducted in the animal Experimental Center of Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from October 2006 to February 2007. MATERIALS: Twenty Japanese big ear rabbits were selected to establish animal models of intervertebral disc degeneration. METHODS: Some nucleus pulposus tissues were abstracted from the L1-2 and L3-4 segment of 20 rabbits by the puncture and aspiration method using a 21-gaege hypodermic needle. Histological analysis was performed at 2, 4, 8, 12 weeks after surgery, and L2-3 segment was used as control group.MAIN OUTCOME MEASURE: Histological structure of the intervertebral disc was observed by homatoxylin-eosin staining. RESULTS: Hematoxylin-eosin staining revealed a great deal of complete nudeus pulposus tissues, clear boundaries between nucleus pulposus and annulus fibrosus in the control group, and the structure of near normal annulus fibrosus was almost normal, nucleus pulposus tissue had a large number of nucleus pulposus cells. In the experimental group, the nucleus pulposus cell reduced in amount in the fourth week, the nucleus pulposus at the twelfth week were mainly full of flbroblests, while few nucleus pulposus cells were found.CONCLUSION: It is successful to establishing an animal model of intervertebral disc degeneration by puncture and aspiration via a posterolateral approach based on simulating the resection of human nucleus pulposus. This model is available for repairing intervertebrai disc degeneration using tissue engineering techniques.
