ABSTRACT

In the new era, with the accelerated evolution of the information technology revolution and industrial transformation, scientific and technological innovation not only brings dividends and benefits to the world, but also brings serious challenges to human beings. To effectively cope with ethical challenges and avoid ethical risks effectively, systematically carrying out ethics education of science and technology in accordance with the times and circumstances to cultivate medical students’ upward and forward-looking ethics of science and technology for medical colleges and universities is necessary, feasible, scientific, and sustainable. Ethics education of science and technology in medical colleges and universities should take the guidance of respecting and seeking truth, adhere to the development concept of righteousness and innovation, take the red line and bottom line as the core action points, and regard the equal importance of the country and the people as the values adherence.

ABSTRACT

Objective To investigate the clinical significance of NaviCam magnetic-controlled capsule endoscopy (NMCE) system in the examination of upper gastrointestinal tract.Methods A total of 39 healthy volunteers were enrolled in the present study.NMCE system was used to examine upper gastrointestinal tract.The safety,gastric preparation,visualization and comfort of the subjects were evaluated.Results Visualization of the Z-line,gastric cardia,fundus,body,angulus,antrum and pylorus was subjectively assessed as more than 75% mucosa in 19 (48.71%),37 (94.87%),25 (64.10%),30 (76.92%),39 (100.00%),39 (100.00%),and 39 (100.00%),respectively.The observation time was 1.5,3.0,8.0,17.0,3.0,3.0,5.0 min respectively.The capsule was driven into duodenum positively in 25 (64.10%).Seven subjects went into small bowel without control.The one-time visualization efficacy was 97.43% (38/39).Only one subject felt foreign body sensation.All subjects extracted the capsule within 7 days.Conclusion Our study provides a preliminary assessment of the NMCE on its feasibility and safety.It is comfortable with no chance of cross-infection.NMCE system is a useful tool for upper GI examination and will have a good future.

ABSTRACT

Objective To explore the efficacy and safety of infliximab (IFX) treatment in inducing and maintaining deep remission (DR) in patients with moderate to severe Crohn's disease (CD).Methods From February 2012 to April 2014,the clinical data of 26 patients with moderate to severe CD received IFX treatment were retrospectively analyzed.Laboratory indexes (erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),albumin),Crohn's disease activity index (CDAI),Crohn's disease simplified endoscopic score (SES-CD),rate of DR and side effects were observed before treatment,at week 14 and week 30.The t test was performed for normal distribution measurement data comparison between two groups.Wilcoxon signed rank test was performed for non normal distribution measurement data comparison between two groups.Chi square test and Fisher exact probability method were used for rate comparison.Results In 26 patients with CD,at week 14,the CDAI significantly decreased compared with that before treatment (225.0(124.0,265.0) vs 80.0(67.0,124.7),Z=-4.265,P<0.01); ESR and CRP levels also significantly decreased while body mass index (BMI) and albumin levels increased.The rate of clinical remission,mucosal healing under endoscope and DR was 80.0 % (21/26),42.3 % (11/ 26) and 34.6% (9/26),respectively.The rate of clinical remission was higher in patients with the disease course less than one year (92.3% vs 69.2%,P=0.32).At week 30,the CDAI of patients significantly decreased compared with that before treatment (225.0(124.0,265.0) vs 81.5(67.0,111.0),Z=-4.877,P<0.01); the ESR and CRP levels significantly decreased; while the BMI and albumin levels increased.The rate of clinical remission,mucosal healing under endoscope and DR was 88.5 % (23/26),57.7%(15/26) and 53.8% (14/26),respectively.Rate of clinical remission was higher in patients with the disease course less than one year (100.0% vs 76.9%,P=0.22).The differences in the rates of clinical remission,mucosal healing and DR between week 14 and week 30 were not statistically significant.Conclusion IFX could induce and maintain DR in patients with moderate to severe CD.

ABSTRACT

Objective To assess the diagnostic value of human epididymis secretory protein (HE4) versus CA125 for endometrial cancer.Methods Serum HE4 and CA125 levels were measured by enzyme-linked immunosorbent assay (ELISA) and electrochemiluminescence immunoassay (ECLI) in 35 patients with endometrial cancer,48 patients with benign endometrial disorders and 40 healthy controls.The best cutoff value,sensitivity and specificity were calculated with pathological results as golden standard.Receiver operating characteristic curves were plotted,and areaunderthecurve (ROC-AUC) was used to compare diagnostic value of HE4 and CA125.Results The median value in HE4 in endometrial cancer were 51.46 pmol/L,which was significantly higher than in the healthy and benign disorder controls (25.65 pmol/L and 27.92 pmol/L).However,CA125 levels did not show statistically significant difference among the 3 groups.The ROC-AUC of HE4 for discriminating endometrial cancer between healthy controls and benign disorder controls were 0.922 and 0.759,respectively,which showed higher diagnostic value than CA125 (ROC-AUC of 0.590 and 0.457,respectively).HE4 had a specificity/sensitivity of 87.5% /86.1% for distinguishing healthy controls from endometrial cancer,and 95.5% /50% for differential diagnosis with benign disorders.Conclusions Measurement of HE4 can be used as a tumor marker for diagnosis of endometrial cancer.

ABSTRACT

Objective To investigate the correlation between cognitive function and cerebral microbleeds (CMBs) in patients with acute cerebral infarction.Methods The clinical and imaging findings in patients with acute stroke were analyzed retrospective.CMBs were counted according to the findings of MRI.The general conditions,CMB site and leukoaraiosis severity of the patients were recorded in detail.The cognitive function of the patients was assessed with the Montreal cognitive assessment scale (MoCA) the next day in the hospital.MoCA assessment follow-up was conducted at 3,6 and 9 months.The changes in cognitive function of the patients with acute cerebral infarction and its relationship with CBMs were analyzed.Results A total of 82 patients with ischemic stroke were enrolled in the study,33 of them had CBMs and 49 had no CBMs.The systolic blood pressure (155.03 ± 19.68 mm Hg vs.142.20 ± 21.22 mm Hg;t =2.762,P =0.007) and the National Institutes of Health Stroke Scale (NIHSS) score (6.21 ±4.57 vs.4.00 ±3.98; t =2.322,P =0.023) in the CBM group were significantly higher than those in the non-CBM group.Multivariate logistic regression analysis showed that the systolic blood pressure level (odds ratio [OR] 1.032,95% confidence interval [CI] 1.008-1.057; P=0.009) and the NIHSS score (OR 1.163,95% CI 1.013-1.311;P=0.014) were the independent predictors of CBMs in patients with acute cerebral infarction.CMBs were closely associated with MoCA scores,and the longer follow-up time,the correlation would stronger.The executive function (rs =0.318,P =0.004),visual space function (rs =0.403,P =0.000) and calculation function (rs =0.362,P=0.001) in patients with CMBs were significantly impaired.The more serious of CMBs,the lower of the scores in above 3 cognitive domains,and the damage would be more serious.Conclusions CMBs are closely associated with cognitive function impairment in patients with acute cerebral infarction.The more serious of CMBs,the more serious of cognitive function impairment will be,and with the extension of time,cognitive function impairment in patients with CMBs will aggravate.

ABSTRACT

Objective In order to analyze the pollution level of PM10 and PM2.5, as well as their sources in large and moderate scale shopping centres. Methods Using the portable aerosol monitor detected the PM10 and PM2.5 on different locale floors in Zhongyuan Supermarket, Commerce Building and Foodstuff Centre in Pingdingshan City area. Results The average mass concentration of PM10 in Zhongyuan Supermarket,Commerce Building and Foodstuff Centre were 0.212 mg/m3, 0.108 mg/m3, 0.056 mg/m3 and that of PM2.5 were 0.179 mg/m3, 0.092 mg/m3, 0.043 mg/m3 respectively. The pollution level of PM10 of all the shopping center didn't exceed the related standard. The average mass concentration of PM2.5 in Zhongyuan Supermarket,Commerce Building all exceeded the standard ,and the times of exceeding standard were 1.75 and 0.42. The people volume of flow was more masses, and the concentration of PM10 and PM2.5 were more higher.
