ABSTRACT

A neurologic compromise associated with vertebral fractures is generally due to a malignancy causes. Therefore, an osteoporotic vertebral fracture can sometimes cause neurologic complications. Report a case of neurologic compromise associated with osteoporotic vertebral fractures. A-62-year-old man suffered from rheumatoid arthritis since 1985, presented a cervical pain associated with quadriparesia secondary to a C5 osteoporotic vertebral fractures. Osteonecrosis may be the cause of neurologic compromise associated with osteoporotic vertebral fractures

ABSTRACT

The aim of our study is to expose a practical screening and diagnosis strategy of HCC occuring in cirrhosis liver, which is nowadays a health public matter in Tunisia. HCC routine screening in cirrhosis liver has been conducted every 6 months using a combination of abdominal sonography and serum alpha-foetoprotein levels. PCUS, CT scan or MRI have been performed each time a focal liver lesion was discovered in US, or in case of elevated serum alpha-foetoprotein levels. 30 cases of HCC were identified.US has shown a unique nodule in 46% of the cases, multiple nodules in 50% of them. No lesion has been discovered in 4% of the cases. Nodule size ranged from 8 to 140 mm. Heterogenous hyperechoic pattern was the most frequent one [41%]. Hypervascular doppler feature was authentified in 20% of the cases. Serum alpha-foetoprotein levels had respectively a sensitivity and a specificity of 14% and 44% in lesions inferior to 3 cm, 62% and 80% in lesions superior to 3 cm. Typical arterial enhancement of HCC was depicted in 1 case for PCUS, in 76% for CT scan and 60% for MRI. Combination of abdominal sonography and serum alpha-foetoprotein levels practiced every 6 months represent a good compromise between cost and effectiveness. Second intention imaging is based on CT scan in Tunisia because of its availability compared to MRI. ABSTRACT

Tesicular tumors are rarely seen in childhood. Most teratoma reported to occur in cryptorchid males. The purpose of this paper is to report imaging features of intra-abdominal testis with teratoma in a 4 months-old child admitted to explore abdominal mass. Abdominal ultrasound and CT scan exploration showed a solid mass occuping the right flank including calcified, liquid and fat areas. Surgical exploration found intraabdominal teratoma developed from the right testis

ABSTRACT

Our study's aim is to evaluate the reliability of computed tomography in diagnosis, extension and post-surgical follow-up of otosclerosis. A group of 11 patients was examined during a period of 25 months from 2002 to 2004. All of them had clinically and audiometrically suspected otosclerosis with bilateral conductive hear loss in 7 cases [3 had surgical management], unilateral in 2, and mixed earing loss in 2. High resolution CT was performed once in 8 patients and twice in the 3 operated. CT showed otosclerosis foci in all our study's patients, which was bilateral in 9 and unilateral in 2. The extension of the Lesions varied from type 1A to type 4B according to the otosclerosis staging. Computed tomography is, so far, the most efficient imaging modality in otosclerosis, showing otosclerosis foci when the clinical features are doubtful, allowing an accurate extension of the lesions and detecting complications during post-surgical follow-up

ABSTRACT

The incidence of neurosarcoidosis is about 5 to 15% of sarcoidosis in most clinical or post-mortem series. Central nervous system involvement is extremely uncommon and more serious than peripheral nerves. We discribe a case of sarcoidosis with central nervous system involvement in a 61 old woman how developped pulmonary sarcoidosis. After 4 months of treatment with corticosteroids, the patient presented with headaches, episods of altered consiousness, cerebellous syndrome and right hemiparesis. Analysis of cerebrospinal fluid was normal. CT scan and MRI examination of the head revealed mass like lesions in cerebellar region and periventricular white matter. A clinical diagnosis of neurosarcoidosis was made and treatment with Methylprednisone was recommended followed by immunosuppressive drug. 3 months after, symptoms had improved but the MRI showed the same mass lesions and the patient died after 12 months. Involvement of the central nervous system is an uncommon and serious complication of sarcoidosis, we should think about it in patients with neurological manifestations who developed a sarcoidosis

ABSTRACT

Aorto-bronchial fistula is a rare etiology of hemoptysis. We related an observation of aorto-bronchial fistula from a tuberculous origin in a 40 years old man. The patient was treated for tuberculous miliary during four months and was admitted for hemoptysis. A chest radiograph showed a posterior madiastinal mass. Computed tomography of the chest and aortography made the diagnosis of an important thrombosed aneurism of the thoracic aorta Tuberculosis origin was suspected with history of miliary, the vertebral lesions, the paravertebral fuseau and was confirmed by histology

We had studied in one year 110 cases of scano- pelvimetry, the clinical examination had not found a shrink pound in 41% revealed by the scenopelvimetry. This method is more precisely than the calssic pelvimetry, but more expansive. The simultaneous study of echography and pelvimetry had contributed to a better obstetrical decision
