ABSTRACT

A pseudo aneurysm results from leakage of blood from an artery after trauma or dehiscence or separation of a surgical anastomosis. The reported rate of pseudo aneurysm in access sites range from 0.88% to 8%. It has some cause like penetrating trauma, blunt trauma and endovascular procedure. The differential diagnoses of this lesion are hematoma, AV fistula, lymphadenopathy, lymphocele, DVT, compartment syndrome, soft tissue tumor. A 16 years old male was referred to our clinic with progressive swelling in his right leg for the past three month. In primary survey [MRI, CT, Bone Scan] patient was diagnosed with soft tissue tumor, but after biopsy and angiography he was diagnosed with pseudo aneurysm of anterior tibialis artery. Despite easy diagnosis of p aneurysm in most cases, the signs and symptoms are more likely to soft tissue mass in rare cases. So pseudo aneurysm should always be considered as one differential diagnosis for soft tissue tumors

ABSTRACT

To determine the histological response to preoperative chemotherapy of the percentage of tumor necrosis, and to assess the relationship between the histological response and the oncological result. Eighty patients with osteosarcoma were managed with preoperative and postoperative chemotherapy and operative resection at Shafa Yahyaeeyan Hospital, Tehran, Iran between 2003-2005. Sections of each operative specimen were examined, and the histological response to chemotherapy was graded. Grade 1 indicated necrosis of 50% of the tumor or less; grade 2, necrosis of more than 50% yet less than 90 percent; grade 3, necrosis of more than 90 percent. The mean duration of the follow-up of the surviving patients, who were continuously free from disease was 1044 days. The histological response to preoperative chemotherapy [p=0.016] was the most important predictor of event-free survival. The rate of event-free short-term survival for the 80 patients entering this study was 86 percent [69 patients] at 12 months, 50% 24 patients at 24 months, and 21% [5 patients] at 40 months, with 5 patients surviving for a median of 1096 days. The histological response to preoperative chemotherapy is an important clinical predictor of the result of operative treatment of osteosarcoma. This indicator should be used to identify patients who are at high risk for metastasis, as such patients may be candidates for more intensive or novel therapy

ABSTRACT

To compare the effects of four physical feed restriction methods during rearing broiler breeder on growth, metabolic status and blood indices of stress. A 4x2 factorial experiment based on a completely randomized design. On hundred and sixty pullets and 160 roosters of broiler breeder. The pullets and roosters were housed in pens and consumed the same amount of feed in both treatments. The effects of feed restriction methods [every day, six days a week, five days a week and skip-a-day] during 3 to 22 weeks of age on body weight, uniformity [coefficient variation], feed efficiency, serum biochemical parameters [glucose, triglyceride, total protein, cholesterol and uric acid] blood indices of stress [such as heterophil to lymphocyte ratio and basophil, monocyte and eosinophil counts] were determint. Analysis of variance [ANOVA]. While in the skip-a-day method the body weight and the feed efficiency were lower, the plasma cholesterol [P<0/05] and heterophil: lymphocyte ratio were higher in females. Because of higher body weight uniformity in five days a week and six days a week methods, these methods can be recommended

ABSTRACT

We present a 73 year-old severely ill Iranian patient who had a cardiac arrest and a successful cardiopulmonary resuscitation [CPR] one hour after admission in N.I.O.C Hospital. He was a known case of abdominal aortic aneurysm which was accidentally diagnosed by ultrasonography two years ago, so he was emergently operated with probable diagnosis of rupture of aneurysm. Meanwhile, he had a history of painless rectorrhagia unrelated to defecation during the past six months. Tissue diagnosis of rectal adenocarcinoma was defined following a total colonoscopy. Colorectal carcinoma and abdominal aortic aneurysm are common diseases in similar age group. Co-existence of these diseases is rare. Treatment strategy is a big challenge for these patients. In this patient, aneurysmectomy was performed first, because of emergent condition of the patient, and treatment of the colorectal carcinoma was postponed
