ABSTRACT

Uric acid (UA) is the final product of purine metabolism in human body,and its metabolic disorder will induce hyperuricemia (HUA).The occurrence and development of HUA are associated with a variety of pathological mechanisms such as oxidative stress injury,activation of inflammatory cytokines,and activation of renin-angiotensin-aldosterone system.These mechanisms directly or indirectly affect the bioavailability of endogenous nitric oxide (NO).The decrease in NO bioavailability is common in the diseases with high concentration of UA as an independent risk factor.In this review,we summarize the mechanisms by which high concentrations of UA affect the endogenous NO bioavailability,with a focus on the mechanisms of high-concentration UA in decreasing the synthesis and/or increasing the consumption of NO.This review aims to provide references for alleviating the multisystem symptoms and improving the prognosis of HUA,and lay a theoretical foundation for in-depth study of the correlations between HUA and other metabolic diseases.

ABSTRACT

Fermented Chinese medicine has long been used. Amid the advance for preservation of experience, the connotation of fermented Chinese medicine has been enriched and improved. However, fermented Chinese medicine prescriptions generally contain a lot of medicinals. The fermentation process is complicated and the conventional fermentation conditions fail to be strictly controlled. In addition, the judgment of the fermentation end point is highly subjective. As a result, quality of fermented Chinese medicine is of great difference among regions and unstable. At the moment, the quality standards of fermented Chinese medicine are generally outdated and different among regions, with simple quality control methods and lacking objective safe fermentation-specific evaluation indictors. It is difficult to comprehensively evaluate and control the quality of fermented medicine. These problems have aroused concern in the industry and also affected the clinical application of fermented Chinese medicine. This article summarized and analyzed the application, quality standards, and the modernization of fermentation technology and quality control methods of fermented Chinese medicine and proposed suggestions for improving the quality standards of the medicine, with a view to improving the overall quality of it.

ABSTRACT

As arsenic widely exists in nature and has been used in the pharmaceutical preparations, the traditional Chinese medicine(TCM) with arsenic include realgar(As_2S_2 or As_4S_4), orpiment(As_2S_3), and white arsenic(As_2O_3). Among the above representative medicine, the TCM compound formulas with realgar are utilized extensively. Just in Chinese Pharmacopoeia(2020 edition), there are 37 Chinese patent medicines including realgar. The traditional element analysis focuses on the detection of the total amount of elements, which neglects the study on the speciation and valence of elements. The activity, toxicity, bioavailability, and metabolic pathways of arsenic in vivo are closely related to the existence of its form, and different forms of arsenic have different effects on organisms. Therefore, the study on the speciation and valence of arsenic is of great importance for arsenic-containing TCMs and their compound formulas. This paper reviewed four aspects of the speciation and valence of arsenic, including property, absorption and metabolism, toxicity, and analytical assay.

ABSTRACT

This review made a systematic textual research on the historical evolution and changes of the origin, producing areas and quality, harvest time and processing methods, and cultivation technique of Poria in famous classical formulas from the Catalogue of Ancient Famous Classical Formulas (the First Batch) by referring to the literature of ancient materia medica and medical books combining with the modern literature in CNKI, Wanfang Data and other databases, which could provide reference for the development and research of the famous classical formulas containing Poria. According to the research, the origin of Poria in ancient times was the dried sclerotia of Poria cocos. The producing area of Poria is widely distributed in China, mainly produced in Anhui, Hubei, Henan and Yunnan provinces. The quality of Poria from Yunnan is better, and the yield is larger in Anhui and Hubei provinces. The quality evaluation of Poria is basically the same from ancient to modern, which has the characteristics of large block, heavy weight, thin and wrinkled skin, and white and delicate cross section. The harvesting period of Poria is usually in August of the lunar calendar. The initial processing has undergone the evolution from drying in shade in the ancient time to drying in shade after sweating in the modern time. From ancient times to the present, Poria has been processed by cutting to prepare the Fuling lumps and Fuling slices. The cultivation technique has experienced the evolution process of "wild Poria-inoculation of live pine root-basswood cultured". At present, Poria is mainly cultivated by artificial basswood with mature technical methods and abundant yields, which can meet the research and development needs of the famous classical formulas.

ABSTRACT

Objective: To investigate the effect of varying concentrations of polyethylene glycol(PEG)400 in receiving solution on in vitro transdermal test of drugs. Method: 5-Fluorouracil(5-FU) was selected as a model drug,by preparing different concentrations of PEG400-phosphate buffer solution(PBS) as the receiving solution,the receiving chamber did not add drug,the excised rat skins were treated with various additives for 12 h,then replaced by PBS and added the saturated model drug into the donor compartment to determine the transdermal parameters of the drug.Meanwhile,scanning electron microscopy(SEM) was employed to monitor the effect of PEG400 with different concentration on the stratum corneum of rat skin. Result: The 10%,15% and 40% PEG400-PBS groups had no significant effect on in vitro transdermal absorption parameters of the 5-FU.The steady transdermal rate and cumulative penetration rate of the drug in 20% and 30% PEG400-PBS groups were significantly higher than that in the PBS group(PPConclusion: In the rat skin transdermal test,the concentration of PEG400 in receiving solution should be controlled below 20%.

ABSTRACT

In this paper, a new type of preparation for treatment of initial dry eye disease, thermosensitive in situ gel, was prepared using levocarnitine as a model drug. Poloxamer 407 and poloxamer 188 were used as the gel matrix, and sodium hyaluronate and sodium carboxymethylcellulose were used as bioadhesive materials. Gelation temperature was determined by a rotor method and the prescription was optimized by central composite design-response surface methodology. The pH value, viscosity value and gelation temperature of the optimal prescription were measured. The release of the drug in vitro was examined by dialysis membrane permeation, and retention time of the thermosensitive in situ gel preparation on the rabbit's ocular surface was observed by a slit lamp microscope. The results showed that the dosage of the poloxamer 407 and poloxamer 188 were 20.81% and 3.46%, respectively, and sodium hyaluronate was 0.02%, sodium carboxymethyl cellulose was 0.10% of the optimal formulation of levocarnitine thermosensitive in situ gel. The pH value was 6.90 ± 0.06 at room temperature and the viscosity value started to rise sharply at 27 ℃ of the optimal formulation. The gelation temperature of the optimal preparation before and after dilution by simulated tear fluid were (26.37 ± 0.06) ℃ and (33.57 ± 0.21) ℃, respectively. In the first 240 min, in vitro release rate per unit area of levocarnitine thermosensitive in situ gel was lower than that of solution (P<0.05), and after 600 min, the cumulative release rate of levocarnitine thermosensitive in situ gel could reach more than 80%. The retention time of the thermosensitive in situ gel preparation on rabbit's ocular surface reached about 25 min, at least 5 times as much as that of the solution. The animal experiment was conducted following the National Institutes of Health Guidelines for the use of experimental animals, and approved by the Ethics Committee of the Experimental Animal Center of Beijing University of Chinese Medicine. The levocarnitine thermosensitive in situ gel showed good characteristics and sustained release property and significantly improved the retention time of the drug on the rabbit's ocular surface.

ABSTRACT

Usnic acid and its derivatives, a group of organic molecules with great importance, are characteristic to lichens, possessing pharmacological activities such as anti-virus, anti-bacteria, anti-humor, anti-inflammatory, analgesic, and anaesthetic effects. Many of them have been widely used as medicine, but also bring side effects such as dermatitis and liver damages. In the past decades, great efforts by isolation, organic synthesis, and structure modification methods were put on discovery of UA derivatives with higher biological activities or less side effects. This paper describes herein the most progress on natural sources, isolation and structure elucidation, structural characteristics, synthesis and modification results, pharmacological activities and toxicities of UA and its derivatives, hopefully to provide valuable reference for further research.

ABSTRACT

Objective To optimize ambi-extracting and inclusion process of volatile oil from Chuanxiong Rhizoma and Angelicae Sinensis Radix. Methods With yield ratio of volatile oil and ferulic acid content in water extract as evaluation indexes, single factor experiments were used to study the extraction process. With the inclusion rate of volatile oil and yield of inclusion as evaluation indexes, saturated aqueous solution was used to L9(34) orthogonal experiments to reach optimum inclusion process. Results The optimum extraction process of Chuanxiong Rhizoma and Angelicae Sinensis Radix was extracted for 8 hours with 8 folds the amount of water, and without soaking. The validation experiments of extraction of volatile oil and ferulic acid content in water extract were 1.23 mL and 0.387 9 mg/g. The optimum conditions of inclusion process were as follows: volatile oil (mL): β-CD (g) was 1:8;inclusion temperature was 40 ℃; inclusion time was 3 hours. The validation experiments of inclusion rate of volatile oil and yield of inclusion were 74.89% and 72.81%. Conclusion Optimum ambi-extracting and inclusion process of volatile oil from Chuanxiong Rhizoma and Angelicae Sinensis Radix are feasible and stable, witch can provide certain supporting data for preparation production.

ABSTRACT

Objective To investigate the clinical significance of serum CEA , CA19-9, CA72-4, CA242, CA50, PGⅠand PGR ( PGⅠ/PGⅡ) in the Diagnosis of preoperative and postoperative gastric cancer.Methods Retrospective study.The levels of CEA , CA19-9, CA72-4, CA242, CA50, PGⅠand PGⅡin serum of 41 patients with gastric cancer preoperative and postoperative and 60 healthy people were detected by AutoLumo A2000 chemiluminescence immunoassay and compared.Statistical analysis was performed using Rank-sum test by SPSS 17.0.Results The median of CEA, CA19-9, CA72-4, CA242, CA50, PGI, PGII and PGR in postoperative gastric cancer group were 3.79 ng/ml, 17.85 U/ml, 3.50 U/ml, 14.52 U/ml, 17.62 U/ml, 32.81 ng/ml, 11.48 ng/ml, 3.35.The postoperative gastric cancer group were 1.67 ng/ml, 7.76 U ml, 1.73 U/ml, 6.30 U/ml, 7.57 U/ml, 20.56 ng/ml, 5.71 ng/ml, 2.94.The healthy group were 1.53 ng /ml, 7.59 U/ml, 1.47 U/ml, 6.08 U/ml, 5.68 U/ml, 90.86 ng/ml, 14.85 ng/ml, 6.67.There were statistical differences in the serum levels of CEA , CA19-9, CA72-4, CA242, CA50, PGⅠ, PGⅡand PGR among different groups (chi-squared values were 79.108, 20.678, 20.374, 7.252, 56.73, 131.212, 20.38, 86.37, P<0.05).By the Mann-Whitney rank sum test,the serum levels of CEA , CA19-9, CA72-4, CA242 and CA50 in patients with preoperative gastric cancer were significantly higher than those in healthy controls (Z values were -8.598, -4.425, -4.365, -2.000,-7.420, P<0.05).The level of postoperative group was significantly lower than that of preoperative group (Z value were -4.641, -2.383, -2.459, -2.399, -2.903, P<0.05).The serum PGⅠ, PGⅡand PGR levels in patients with preoperative gastric cancer were significantly lower than those in healthy controls (Z values were -10.309, -2.695, 8.637, P<0.05).The PGⅠlevel in the postoperative group was significantly lower than that in the preoperative group (Z value was -2.109, P<0.05).PGⅡ,PGR levels of postoperative group were lower than those of preoperative group , but the difference were not statistically significant.(Z values were -1.506,-0.838, P values were 0.132,0.402).Conclusion The detection of the seven serum markers can help to preoperative diagnosis and postoperative monitoring of gastric cancer .

ABSTRACT

Sixteen compounds were isolated from lichen Usnea longissima using of various chromatographic techniques including silica gel, Sephadex LH-20, ODS, and semi-preparative HPLC. By spectroscopic data analyses, their structures were identified by as useanol(1), lecanorin(2), 3-hydroxy-5-methylphenyl 2-hydroxy-4-methoxy-6-methylbenzoate(3), lecanorin E(4), 3'-methylevernic acid(5), evernic acid(6), barbatinic acid(7), 3,7-dihydroxy-1,9-dimethyldibenzofuran(8), orcinol(9), O-methylorcinol(10), methyl orsellinate(11), methyl everninate(12), 2,5-dimethyl-1,3-benzenediol(13), 2-hydroxy-4-methoxy-3,6-dimethyl benzoic acid(14), ethyl everninate(15), and ethyl 2,4-dihydroxy-6-methylbenzoate(16). Compound 1 was obtained as a natural product for the first time, and 3,4, 8,10,12, and 13 were isolated from Usneaceae family for the first time. Compound 1, 8, and 13 showed significant anti-inflammatory activity against NO production in RAW 267.4 cells with IC₅₀ values of 6.8, 3.9 and 4.8 μmol•L⁻¹, respectively, compared with the positive controls curcumin(IC₅₀ 15.3 μmol•L⁻¹) and indomethacin(IC₅₀ 42.9 μmol•L⁻¹).

OBJECTIVE@#To draw on data about publication patterns and citation indicators of Asian Pacific Journal of Tropical Medicine (APJTM) during 2008 and June 2014 in order to know about the current state of the journal.@*METHODS@#Data used in this study were collected based on publications in APJTM from 2008 to June, 2014. Information including publication issue, type of manuscript, country/region of Corresponding author, funded research paper, and international collaboration were aggregated and analyzed with Excel software. Citation indicators including total cites, average cites of each manuscript, h-index, and impact factors were primarily drawn from Web of Science™ database on June 15, 2014 and changes over the past six and half years were interpreted. The top 10 most cited papers in Web of Science™ database were also analyzed.@*RESULTS@#Number of all submissions has arisen from less than 200 in 2008 to over 1 500 in 2013, manuscript acceptance rate has decreased to be less than 14.00% indicating its improvement in quality over this period of time. Out of the 1 115 publiations, 23.77% were fruits of funded projects or produced by funded co-authors, 87.08% of all publications in APJTM were submited by authors from 10 most contributed countries. During the studied period, each published manuscript in the journal has received an average of 1.05 cites, and at least 10 publications has been cited for more 10 times.@*CONCLUSION@#Detailed analysis shows APJTM has made great progress over the past six and half years, but authors' originating countries are still disproportionate. Efforts should be made to improve its citation indicators.

ABSTRACT

Objective To study the effect of Pingchuanguben Decoction on IL-17 level of bronchial alveolar lavage fluid (BALF) and airway remolding in mice with asthma.Methods Forty healthy female Kunming mice were randomly divided into 4 groups (10 rats in each group):normal control group,asthma group,Pingchuanguben Decoction group and Budesonide group.The asthmatic models were established by ovalbumin (OVA) ; the normal control group was treated with phosphate buffer solution (PBS liquid) by celiac injection,during this period Pingchuanguben Decoction was administered to the mice in Pingchuanguben Decoction group each day.In stimulation stage,the normal control group was atomization inhalated PBS liquid,and the rest of the 3 groups were given aerosolized OVA instead.Budesonide atomization was administered to mice in Budesonide group before stimulation.The levels of IL-17 in BALF were measured with enzyme-linked immunosorbent assay (ELISA),and the levels of matrix metalloproteinase-9 (MMP-9)and the tissue inhibitor of metalloproteinase-1 (TIMP-1) in lung tissues were tested with immunohistochemical analysis.Results 1.The levels of IL-17 in BALF in Pingchuanguben Decoction group and Budesonide group were significantly lower than those in asthma group (all P <0.05),and the IL-17 in BALF was increased compared with the normal control group (P <0.01).2.The levels MMP-9,T IMP-1 and MMP-9/TIMP-1 were decreased in the Pingchuanguben Decoction group and Budesonide group compared with those of asthmatic group (all P < 0.01),and the MMP-9 and MMP-9/TIMP-1 were increased compared with the normal control group(all P <0.01).There was no significant difference between Pingchuanguben Decoction group and Budesonide group (all P > 0.05).Conclusions Pingchuanguben Decoction may restrain airway inflammation and remolding,which can replace Budesonide to a certain extent.

ABSTRACT

Dendritic cells (DCs) derived from bone marrow cells are specialized cells for the uptake, processing, and presentation of foreign and self-antigens. The study indicated that re-transfusion of DCs pulsed with tumor-associated antigen can induce an vigorous specific anti-tumor response in clinic. The present study was aimed to investigate the enhancing effect of DCs derived from human cord blood on T cells in killing tumor cells. Human cord blood mononuclear cells were isolated from human cord blood by density gradient centrifugation using lymphocyte separating medium, and cord blood mononuclear cells were obtained by adherence and cultured in a liquid culture system with GM-CSF and IL-4 for 15 days. Then the cells were analyzed for phenotypes of CD1a by indirect immunofluorescence. The capacity of DCs to initiate T cell-dependent anti-tumor immune responses was assayed by MTT kit. The ratios of DCs to tumor cells in experimental groups were 20:1, 50:1 and 100:1 respectively. The DCs were not added in control group. The results indicated that in the presence of GM-CSF and IL-4, the DCs with typical morphological features at days 15 were observed. At that time, (43.12 +/- 5.83)% CD1a(+) cells were obtained. In addition to these phenotypic properties, the DC of experimental groups could remarkably initiate T cell-dependent anti-tumor immune responses with different ratios compared with control group (P < 0.01), there were no significant difference of killing effects between 100:1 and 50:1 groups (P > 0.05), and killing effect of DC in 20:1 group was higher than that in 100:1 or 50:1 groups (P < 0.05). It is concluded that human cord blood mononuclear cells can serve as a better source of DC, which can promote the capacity to initiate T cell-dependent anti-tumor immune responses.

ABSTRACT

Objective To explore the value of temporary cardiac pacemaker therapy on children with Adam-Stoke syndrome(ASS).Methods Nine children with ASS was implanted the emporary cardiac pacemaker under X-ray or blinding inserted beside bed.Results Seven children was successfully implanted the temporary cardiac pacemaker within 1-2 d of ASS attack,and two children was successfully implanted at 10 d and 20 d after ASS attack, respectively. Five patients was cured, two children was died, and two children was implanted the permanent cardiac pacemaker. During pacing, two patients had the electrode shifted,and one patient had the cardiac murmur,and one patient had the local skin infected.Conclusion Temporary cardiac pacemaker can successfully treat children with ASS attack;the earlier the implantation,the better the prognosis.

ABSTRACT

Objective To study the effects of different glucocorticasteroid(GCS) inhalation regimens for remissive children with asthma. Methods Three hundred and twenty - three patients with moderate asthma were enrolled on a 12 - week randomized parallel group remissive treatment after a 4 - week baseline treatment. During the baseline treatment terbutaline sulfate 250 ?g tid a day and bud esonide 200 ?g twice a day were given, and oral bronchodilators were used if necessary. The remissive treatment were composed of budesomde inhabit ion 100 ?g once a day (group A), 100 ?g twice a day(group B) and 200 ?g once a day(group 0). Patients subsequently returned to the clinic for 3 additional clinic visits (4,8 and 12 weeks) or telephone visits . On every clinic visit, the daytime and nocturnal time seventy score were recorded and spirometry was conducted in patients who were capable of performing the maneuver. Results Ultimately, 323 children were enrolled on the baseline treatment and 281 (87%) children achieved clinical remission. The rate of compliance decreased gradually during the remissive treatment, but in group B(P

ABSTRACT

Objective To explore the effect of Okam on airway inflammation in asthmatic mouse.Methods Thirty-two SPF grade Kunming Strain mice were randomly divided into positive control group,glucocorticoid inhalation group,Okam group and negative control group with 8 mice in each group.The mice were sensitized and repeatedly challenged with ovalbumin(OVA) to establish the models of chronic asthma.The glucocorticoid group were given Budesonide(200 ?g) and saline everyday by inhalation,the Okam group were given 50 mg/kg Okam by gavage,and the positive group had saline at the same time,the negative control group received saline at all stages.The inflammation of the lung tissue were scored underwent HE staining.Bronchoalveolar lavage fluid(BALF) cell count and differential were studied,and interferon-?(IFN-?),interleukin-4(IL-4) in BALF were determined by enzyme-linked immunosorbent assay(ELISA).Results There were no inflammatory cell infiltrate of bronchiole in the negative control group.Inflammatory infiltration of lung tissue were obvious in the positive control group.Inflammatory infiltration of lung tissue lightened obviously in the Budesonide and Okam groups.The total cell number,Eosinophils(EOS) and IL-4 level in BALF,and the score of the lung tissue in Okam group were all markedly lower than those in positive control group(t=5.942,7.089,7.078 Pa0.05),IFN-? lower(t=4.275 P

ABSTRACT

Objective To evaluate the change of cellular immunity and its clinical significance in JRA.Methods 7 lymphocyte swbpopulation was analyzed by immunofluorescein and interleukin 2 (IL-2) produced by peripheral blood mononuclear cells in vitro by MTT colorimetric assay. 29 times of various stage with JRA were examined, including 14 clinically active patients, 8 posttreatment or 7 clinically inactive ones. There are 19 healthy children of similar age in control group.Results In active patients, the number of OKT8, OKT4, the ratio of OKT4/OKT8 and the level of IL-2 decreased significantly compared with normal controls. These changes recovered matkedly in remission patients though they did alter affective treatment for (2~4) weeks.Conclusion Patients with active JRA are characterised by aberration of cellular immunity and the aberration reverses obviously slow in comparision with the clinical manifestetions and the routine laboratory investigation.
