ABSTRACT

Objective:To investigate the relationship between Helicobacter pylori(HP) cytotoxin-associated gene A (HP-CagA), HP isolate vacuole-forming toxin gene A (HP-VacA) and gastric cancer occurrence and clinical pathological factors.Methods:Eighty-eight patients with gastric cancer from January 2018 to January 2020 in Suzhou Hospital Affiliated of Anhui Medical University was selected as the observation group, 80 patients with benign gastric lesions during the same period was selected as the benign control group, and 80 healthy patients was selected as the healthy control group. The clinical data, HP-CagA, HP-VacA positive expression rates of the three groups were compared, the risk factors of gastric cancer were analyzed, and the relationship between HP-CagA, HP-VacA and gastric cancer clinicopathological factors were evaluated. Results:Family history of gastric cancer, high-salt diet, preference for hot food, decreased pepsinogen (PG)Ⅰ/PGⅡ, combined with fatty liver, increased triglyceride, total cholesterol and low density lipoprotein cholesterin, smoking and depression were risk factors of gastric cancer ( P<0.05). The positive rate of HP-CagA, HP-VacA in the observation group were higher than those in the benign control group and the healthy control group: 82.93%(73/88) vs. 62.50%(50/80) and 26.25%(21/80), 30.68%(27/88) vs. 7.50%(6/80) and 0, the differences were statistically significant ( P<0.05). The positive of HP-CagA and HP-VacA had correlation with age, pathological type, and degree of differentiation of gastric cancer ( P<0.05). The 1-year survival rate of HP-CagA and HP-VacA positive patients was lower than that of negative patients by Kaplan-Meier analysis ( P<0.05). Conclusions:The positive of HP-CagA and HP-VacA in HP infections are closely related togastric cancer. Strengthening the treatment of HP infection patients with positive HP-CagA and HP-VacA has important clinical value and social significance for cutting off the early stage of gastric cancer and improving prognosis.

ABSTRACT

Objective:To explore the effect of early mother to child separation on neonatal nervous system and its related mechanism. Methods:Randomly selected during January 2015 to September 2015,was born in 120 cases of newborn mice as the research object, the newborn mice was born after the implemented of mother-to-child separation as observation group (60),not the implemented of separation of mother and baby after birth as the control group (60),in view of the two groups of newborn mice of the nervous system, the change of the nerve cells were compared and researched. Results:The apoptotic rate of neurons in the neonatal mice was significantly higher than that in the control group (P <0.05).The neurons of caspase-3 protein expression was significantly higher than the control group (P <0.05).The expression of Caveo-1 protein in the glial cells of 14 days and 21 days was observed in the neonatal mice. The expression of Caveo-1 protein in the glial cells of the control group For comparison,(P <0.05),which were statistically significant. Conclusion:The early implementation of matemal and infant birth separation of neonatal nervous system will produce great influence, and influence the expression of the nervous system in the development of newborn, which affect the behavior of the neonatal adult dysplasia.

ABSTRACT

Objective: To observe curative effect of thoracic sympathetic block on patients with diabetes mellitus (DM) complicated cardiac chamber enlargement and heart failure (HF). Methods: A total of 76 DM patients with cardiac chamber enlargement and HF were randomly divided into routine treatment group (n=36) and epidural nerve block group (nerve block group, n=40, received thoracic epidural nerve block based on routine treatment), and four weeks were a course of treatment. Changes of left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDd), left atrial diameter (LAD), levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), brain natriuretic peptide (BNP) and glycosylated hemoglobin (HbA1c) were observed in two groups. Results: There were no significant difference in all indexes between two groups before treatment (P>0.05). After treatment, total effective rate of cardiac function improvement in nerve block group was significantly higher than that of routine treatment group (95% vs. 66.7%, P<0.05). Compared with before treatment, there were significant decrease in LVEDd [(71.63±9.26)mm vs. (65.4±8.12)mm] and LAD[(45.62±7.10)mm vs. (37.14 ±6.28)mm] (P<0.05 both), significant increase in LVEF [(31.3±11.46)% vs. (42.12± 10.42) % , P< 0.01], and significant decrease in levels of TNF-α [(73.55±16.36)ng/L vs. (28.35±10.08) ng/L], IL-6 [(17.62±5.47) ng/L vs. (7.88±2.29) ng/L], BNP[(13.49±2.94) ng/L vs. (6.28±2.28) ng/L] and HbA1c [(7.9±2.6) % vs. (6.8±1.9) %] in nerve block group after treatment, P<0.05 or <0.001; and levels of LVEF, TNF-α, IL-6 and BNP in nerve block group after treatment were significantly improved than those of routine treatment group, P<0.05 or <0.01. Conclusion: Thoracic sympathetic block can significantly decrease levels of neuroendocrine and inflammatory factors, improve cardiac function, and elevate quality of life in patients with diabetes mellitus complicated heart failure.
