ABSTRACT

Objective To observe the clinical effect of Yanshu compound radix sophore injection combined with radiotherapy in treatment of patients with bone metastases from lung cancer. Methods A total of 116 patients with bone metastases from lung cancer admitted to the Department of Radiation Oncology of Gansu Provincial Cancer Hospital from January 2014 to December 2018 were selected. The 58 patients were treated by using three-dimensional conformal radiotherapy (3D-CRT), zoledronic acid and oxycodone sustained-release tablets (control group); the others 58 patients were treated radix sphore injection on basis of control group (integrated Chinese and western medicine). The two groups were treated continuously for 10 days as 1 course of treatment, taking a rest for one week in the middle, then they took the second course of treatment; after the end of 2 courses, the therapeutic effects were evaluated in the two groups. The pain grading, Karnofsky functional status (KPS) score and myelosuppression were observed before and after treatment in two groups, respectively. Results The proportions of patients with painless and mild pain after treatment in the integrated Chinese and western medicine group were significantly lower than those in control group [painless: 22.41% (13/58) vs. 39.66% (23/58), mild pain: 10.35% (6/58) vs. 46.55% (27/58)], while the proportions of patients with moderate pain and severe pain in the integrated Chinese and western medicine group were significantly higher than those of the control group [moderate pain: 53.45(31/58) vs. 12.07% (7/58), severe pain: 13.79% (8/58) vs 1.72% (1/58), both P < 0.05]. After treatment, the proportion of patients with improved KPS score in integrated Chinese and western medicine group was significantly higher than that in control group [55.17% (32/58) vs. 36.21% (21/58), P < 0.05], and the proportion of patients with stable and decreased KPS score were less than that of control group [stable KPS score:18.97%(11/58) vs. 27.59%(16/58), decreased KPS score:25.86% (15/58) vs. 36.21% (21/58)]. The proportions of patients with blood cell count (WBC), hemoglobin (Hb) and platelet count (PLT) showing no myelosuppression (at the zero degree) after treatment in integrated Chinese and western medicine group were obviously higher than those in control group [WBC: 77.59% (45/58) vs. 36.21% (21/58), Hb: 77.59% (45/58) vs. 70.69% (41/58), PLT: 58.62% (34/58) vs. 32.76% (19/58), all P < 0.05]. Conclusion Yanshu compound radix sphore injection combined with radiotherapy can reudce bone metastasis pain, reduce bone marrow suppression and improve quality of life in patients with advanced lung cancer and bone metastasis.

ABSTRACT

Objective@#To investigate the salvage strategy and efficacy for patients with locally recurrent esophageal squamous cell carcinoma after definitive radiochemotherapy.@*Methods@#A total of 126 patients who met the inclusion criteria were enrolled in this study and divided into the salvage surgery, salvage radiochemotherapy and best supportive care.@*Results@#Fifty-eight of 126 patients received salvage esophagectomy, 52 underwent salvage radiochemotherapy and the remaining 16 patients received best supportive care. The 1-, 3-, 5-year overall survival rates of patients receiving salvage therapy were 51%, 16% and 4% for the three groups, whereas all patients in the best supportive care group died within 12.0 months (P<0.001). The 1-, 3-, 5-year survival rates in the salvage surgery and salvage radiochemotherapy groups were 48%, 20% and 7%, and 51%, 11% and 3%, respectively (P=0.473). Multivariate analysis by Cox proportional hazard model showed that T staging of recurrent tumors and salvage regimen were the independent prognostic factors in patients with locally recurrent esophageal cancer (both P<0.001). Postoperative infection occurred in 16% of the patients in the salvage surgery group, and the incidence of esophagotracheal fistula and mediastinoesophageal fistula was 10% and 6% in the salvage radiochemotherapy group.@*Conclusions@#A survival benefit can be elicited by salvage surgery or salvage radiochemotherapy in patients with locally recurrent esophageal cancer after definitive radiochemotherapy. Nevertheless, extensive attention should be paid to the management of postoperative complications in clinical practice.

ABSTRACT

Objective Toexploretheclinicaldiagnosisvalueofthehighfrequencycolordopplerultrasound(HFCDU)character-istiesofwristdamageinpatientswithrheumatoidarthritis(RA).Methods 78patientswithRA(RAgroup)werecollectedfrom June 2012 to December 2012 in the Affiliated Hospital of Inner Mongolia Medical University ,sonogram anatomy changes and syno-vial thickness of bilateral wrist were measured using colorul trasound ,meanwhile the respective data obtained from 50 healthy peo-ple undergoing physical examination(healthy control group);using color doppler flow imaging and color doppler energy ,synovial blood flow of artery resistance index (RI) and pulse index(PI) in synovial were measured and detected according to semi-quantitative method of alder ,and explord the correlations between RI and PI .Results (1)Among the 78 cases(156 joints) with RA ,join thy-drops(65 cases ,122 joints) ,inflammatory exudation(34 cases ,70 joints) ,synovium proliferation(78 cases ,153 joints) ,pannus(37 cases ,69 joints) and bony erosion(29 cases ,58 joints) were found.The thickness of synovial membrane of wrist in RA group was (3 .50 ± 1 .37)mm ,that in healthy control group was(1 .46 ± 0 .32)mm ,the different was statistically significant(P<0 .05) .(2) 64 cases among the 78 patients with RA were detected blood flow signals ,RI progressively reduced with blood flow grade the increas-ion which there was a negative correlation (r= -0 .79 ,P<0 .05);there was no significant correlation between PI and blood flow grade .(3) Ultrasound examination was more sensitive than rheumatoid factor (RF) and X-ray in detection rate of RA (P<0 .05) . Conclusion Ultrasound examination could detecte the pathological changes of wrist in patients with RA which is more sensitive than RF and X-ray ,and it could improve the detection rate of RA which contribute to early diagnosis and reasonable treatment for RA .

ABSTRACT

<p><b>OBJECTIVE</b>To assess the performance of high-frequency ultrasound in evaluating articular lesions in a rat of model collagen-induced arthritis model (CIA).</p><p><b>METHODS</b>Sixty Wistar rats randomized into CIA group (n=50) and the control group (n=10), and in the former group, CIA was induced by immunization with collagen type II. On day 45, the ankle joint was assessed based on the arthritic index (AI) and examined with high-frequency ultrasound in all the rats. Histological evaluations of the joint were performed and the results were compared with ultrasound findings.</p><p><b>RESULTS</b>In CIA group, 62 ankles were confirmed pathologically to have synovitis with typical inflammatory cell infiltration and pannus formation. AI score-based diagnosis was confirmed in 42 ankles with a detection rate of 68% (42/62), significantly lower than the rate of 87% (54/62) with high-frequency ultrasound (P<0.05).</p><p><b>CONCLUSION</b>High-frequency ultrasound is an ideal method for observing CIA in rats in vivo. High-frequency ultrasound can show blood flow in rheumatoid arthritis and is useful in studies of RA synovial angiogenesis.</p>
