ABSTRACT

Objective To evaluate clinical efficacy and surgical techniques of customized variable-angle locking acetabular wing plates in treatment of complex acetabular fractures in the elderly through the lateral-rectus approach. Methods Data of 11 elder patients of complex acetabular fractures (mean age 75 y,range:60-93 y;8 males,3 females) admitted to our department from March 2016 to March 2017 were retrospectively analyzed.According to Judet-Letournel classification of acetabular fractures, there were 2 cases of anterior column and posterior hemitransverse fractures,9 cases of both-column fractures. The customized variable-angle locking acetabular wing plates were designed by mimics software and then produced before surgery.During the operation, the customized variable-angle locking acetabular wing plate was applied to reduction and fixation via the anterior lataralrectus approach. The effect of fracture reduction was assessed by Matta's criteria. The clinical effect was assessed by Harris Hip score and the modified Merle d' Aubigné-Postal score at the 6th month of postoperative follow-up.Results The average time of designing and producing customized variable-angle locking acetabular wing plates was 4 d (3-5 d),the average operation time was 95 min (45-150 min),and the average intraoperative bleeding was 600 ml (250-1 400 ml).All patients were followed up,with an average follow-up of 18 m (6-24 m).All fractures were healed, with an average healing time of 8 weeks (6-12 weeks).Reduction quality:excellent in 8 cases, good in 2 cases, poor in 1 case, with an overall excellent and good rate of 90.9%.The result of Harris Hip scoring:excellent in 7 cases, good in 3 cases, and acceptable in 1 case, with a total excellent and good rate of 90.9%;Modified Mere d' Aubigné-Postal soring:excellent in 6 cases, good in 3 cases, and acceptable in 2 cases, with an overall excellent and good rate of 81.8%.There were 2 cases of obturator nerve injury,1 case of fat liquefaction of incision,3 cases of traumatic hip arthritis.During the follow-up,no complications such as ectopic ossification, plate fracture and screw loosening were found. Conclusion The customized variable-angle locking acetabular wing plates in treatment of complex acetabular fractures in the elderly via the lateral-rectus approach can achieve satisfactory reduction and firm fixtation.

ABSTRACT

Objective To detect the expression of STAT3 in breast cancer and adjacent paracancerous tissues ,then analyze the correlationship between its expression level and some other clinical features of breast cancer, and to evaluate its significance in prognosis of patients with breast cancer. Methods 130 breast cancer patients were enrolled in the Jiangmen Central Hospital ,and 61 cases of adjacent normal tissues. The expression of STAT3 was detected by IHC. We compared the expression of STAT3 between the two groups and analyzed the relationship between STAT3 and other clinical features. Results The expression of STAT3 in breast cancer group was much higher than the adjacent paracancerous tissues. STAT3 expression showed obvious correlationship with clinical stage ,tumor size ,lymph node metastasis ,the expression of ER and PR ,but no statistical correla-tion with age ,histological grade and Her-2 expression. Survival analysis showed that patients with high STAT3 expression level had a significantly poor prognosis. The Cox regression analysis showed that PR and STAT3 expres-sion were independent prognostic factors for patients with breast cancer. Conclusion These results may implicate that STAT3 will promote the development of breast cancer and higher expression of STAT3 indicates the poor survival rate.

ABSTRACT

Objective To investigate the role of Rho kinase on collagen(COL)Ⅰ and Ⅲ expression of human renal tubular epithelial cells .Methods Human kidney tubular epithelial (HK‐2) cells were cultured in the RPMI‐1640 culture solution containing 15% fetal bovine serum .After 30 min pretreatment by the ALD receptor antagonist eplerenone(10 μmol/L) and Rho kinase inhibi‐tor Y27632(1 μmol/L) ,100 nmol/L ALD acted the HK‐2 cells for 24 h .The expressions of collagenⅠ and Ⅲ mRNA in each group were detected by real time PCR and the expression levels of COL Ⅰ ,Ⅲ and Rho kinase protein were detected by ELISA .Results ALD could up‐regulate the expressions of COLⅠ ,Ⅲ mRNA in HK‐2 cells ,and increased the levels of Rho kinase ,COLⅠ and Ⅲprotein ,while the Rho kinase inhibitor Y27632 and ALD receptor inhibitor eplerenone could antagonize these effects .Conclusion ALD could activate Rho kinase signal transduction pathway in HK‐2 cells and accelerate the progression of tubular interstitial fibro‐sis via Rho kinase induced expression of COL Ⅰ and Ⅲ .

ABSTRACT

OBJECTIVETo explore an early stage repair method for soft tissue defect of limbs of modern firearm wound, and to improve treating result.

METHODSDefects of the hind limbs of dogs were repaired with skin, muscle and myocutaneous flaps.

RESULTSWounds healed within 2 weeks in the experimental group except one that healed in 3 weeks because of infection. Limb function was close to normal. The treatment result was better in the experimental group than the control.

CONCLUSIONSSkin, muscle and myocutaneous flaps can cover soft tissue defect at an early stage, prevent and reduce infection, promote the healing and recovery of combined injury, reduce the time of treatment and disability rate.

ABSTRACT

Objective To optimize the processing technology of Fructus Ligustri Lucidi. Methods A RP-HPLC method was carried out to determine the contents of oleanolic acid and ursolic acid in the fruits of Ligustrum lucidum and an orthogonal design to optimize the processing conditions with the indexes of oleanolic acid content and ursolic acid content. Results Linear ranges of oleanolic acid and ursolic acid were in 0.019 78~0.197 8 mg/mL(r=0.999 8)and in 0.010 26-0.102 6 mg/mL(r=0.999 8)respectively,and average recoveries of oleanolic acid and ursolic acid were 99.53 %(RSD=2.08 %)and 99.88 %(RSD=1.76 %)respectively.Orthogonal design showed the optimum conditions were as follows:soaking for 8 hours with 2 gram wine(5 gram of Fructus Ligustri Lucidi),then steaming for 8 hours. Conclusion HPLC is an accurate and quick method to determinate the contents of oleanolic acid and ursolic acid, and can be used as a content determination method for Fructus Ligustri Lucidi.Soaking time, steaming time and wine content have no significant influences on the contents of oleanolic acid and ursolic acid.
