ABSTRACT

Objective@#To investigate the association between APOB gene R532W polymorphism and the risk of coronary heart disease (CHD) in patients without lipid-lowering treatment and to analyze the interactions between the variation of R532W and different risk factors of CHD.@*Methods@#CHD and non-CHD were diagnosed according to coronary artery angiography (CAG) and/or coronary computed tomography angiogram (CTA) results, as well as clinical features. Blood samples from 771 CHD patients and 772 age- and sex-matched non-CHD controls, who never accepted any lipid-lowering treatments, were collected. R532W was genotyped by HumanExome BeadChip at BGI and strict quality control was made. Firstly, the association between R532W polymorphism and the risk of CHD in 3 genetic models (GA+ AA vs.GG, AA vs. GG+ GA, AA vs. GA vs. GG) after adjusting confounding factors was explored. Then, the interactions between the variation of this loci and risk factors related to CHD were investigated.@*Results@#(1) Total cholesterol (TC) levels were significantly lower in AA genotype than in GA genotype in the total cohort and non-CHD controls, but was similar among the 3 genotypes in CHD patients. (2) R532W GG, GA and AA distribution was 80.7%, 18.2% and 1.2% in CHD patients, and 74.6%, 23.8% and 1.6% in non-CHD controls (P<0.05). (3) R532 polymorphism was related to the incidence of CHD in the dominant model, and A-allele carriers were related to about 35% reduced risk of CHD (OR=0.653, 95% CI 0.502-0.849, P=0.001) after adjusting for confounding factors. (4) R532W polymorphism had positive interactions with hypertension (1.452) and smoke (1.077), while negative interaction with diabetes (0.553) in the occurrence of CHD.@*Conclusions@#APOB gene R532W polymorphism is related to TC levels in Chinese north Han population. A-allele carries of R532W loci is linked with reduced risk of CHD in the absence of lipid-lowering treatment. R532W polymorphism has a positive additive interaction with hypertension and smoke, while a negative additive interaction with diabetes mellitus in the occurrence of CHD.

ABSTRACT

Objective To assess the difference in left atrial properties between elderly and younger control subjects and the role of left atrium remodeling in patients of different ages with atrial fibrillation.Methods A total of 194 patients with non-valvular atrial fibrillation were enrolled from September 2014 to June 2016.Based on age,patients were divided into an elderly group (≥60 years,n=129) and a younger group (<60 years,n=65).We evaluated remodeling parameters for the left atrium using an Ultrasound Cardiography (UCG) system in 125 elder subjects,together with 64 control subjects.All remodeling parameters were recorded,including left atrial diameter (LAD),left atrial square (LAS),left atrial pressure (LAP) and left ventricular end-diastolic dimension (LVEDD).Results The elderly group had more female patients and more patients with persistent atrial fibrillation.Meanwhile,scores of CHA2DS2Vsc and levels of N-terminal pro-brain natriuretic peptide (NT-ProBNP) were significantly increased in elderly patients (both P<0.05).Moreover,the elderly group was associated with increased values of LAD and LAS[(39.1±4.4)mm vs.(37.1±5.3)mm,P<0.01;and(23.3±4.5)cm2 vs.(21.4±4.8)cm2,P<0.01;respectively],compared with those in the control group.Spearman's correlation analysis showed that LAD,LAS and LAP were all markedly related to age (r=0.213,P<0.05;r=-0.175,P<0.05;r=0.170,P<0.05;respectively),persistent onset of atrial fibrillation (r=0.401,P<0.05;r=0.446,P<0.05;r=0.160,P<0.05;respectively),and impaired heart function,measured by left ventricular ejection fraction (r=-0.4371,P<0.05;r=-0.403,P<0.05;r=-0.364,P<0.05;respectively) and NT-ProBNP (r=0.485,P<0.01;r=0.483,P< 0.01;r =0.293,P< 0.01;respectively).Conclusions Left atrial remodeling properties measured by the UCG system in the elderly with non-valvular atrial fibrillation are more serious than those in mid-aged and young subjects.As a convenient and accurate assessment of remodeling parameters,the UCG system is an excellent option for measuring left atrial remodeling in the elderly population.

ABSTRACT

<p><b>BACKGROUND</b>Recently, studies have disclosed soluble CD40 ligand (sCD40L) during atherosclerosis development and plaque destabilization. The objective of the present study was to test the hypothesis that sCD40L levels are higher in acute coronary syndrome (ACS) patients with a greater extent of angiographic coronary involvement.</p><p><b>METHODS</b>This cross-sectional study examined ACS patients who underwent coronary angiography by measuring their sCD40L levels. In order to estimate the serum levels of sCD40L, 10 ml of peripheral venous blood was drawn within 24 hours of admission. sCD40L levels were measured using an enzyme-linked immunosorbent assay (ELISA, RapidBio, West Hills, CA, USA). Demographic data, presence of concomitant diseases, ACS characteristics, and angiographic findings were evaluated. A review of medical records and patient interviews were conducted to assess coronary risk factors. And the severity of coronary artery disease was evaluated using the Gensini score index.</p><p><b>RESULTS</b>Two hundred and eighty-nine patients were included in the study, of whom 186 were male, with an average age of 64.1 ± 10.0 years. Median sCD40L levels were 1.7 ng/ml (0.3-7.3 ng/ml) and Gensini scores were 50 (0-228). After adjusting for demographic variables and cardiovascular risk factors, the Gensini score was associated with the natural logarithm of the sCD40L level (Coefficient b = 0.002, 95% CI 0.000-0.003, P = 0.029).</p><p><b>CONCLUSION</b>sCD40L levels were independently associated with angiographic severity of coronary artery disease in patients with ACS.</p>

ABSTRACT

Objective To compare two separation medium of isolation of adult rat cardiomyocytes , and to observe the characteristics of excitation-contraction coupling of cardiomyocytes .Methods The isolated adult rat heart was hanged on to the Langendorff apparatus for aortic counter-current perfusion and collagenase digestion using two different separation medium.The single cardiomyocytes were cultured and infected with adenovirus . The morphological features of cardiomyocytes were observed with microscope and fluorescent microscope . The shortening-re-lengthening features of sarcomere and the intake-discharge features of calcium were simultaneously recorded by IonOptix equipment .Results 70%rod-shaped with clear-striation adult rat cardiomyocytes could be obtained with the stated two separation medium and cultured in serum-free medium for more than 7 days.GFP could express more than 7 days when the cardiomyocytes were infected with adenovirus .Cardiomyocytes obtained by the first separation medium could not contract with the electrical stimulation, while cardiomyoctyes obtained by the second separation medium could be used for the detection of excitation -contraction coupling .The shortening fraction of sarcomere was 11.61%±2.15% and the relaxing time was ( 0.177 ± 0.031) s.The amplitude of calcium transient was 30.79% ±9.74 % and the decaying time of calcium transient was (0.300 ±0.074) s.Conclusion With the stated two separation medium , adult rat cardiomyocytes can be well isolated , cultured and infected with adenovirus .The second separation medium can be used for the detection of excitation-contraction coupling characteristics .

ABSTRACT

Objective To assess the differences in carotid artery stiffness properties and endothelium-independent dilation (EID)between elderly and young patients,and evaluate the echotracking (ET)system for vascular stiffness at different ages.Methods A total of 79 outpatients with multiple cardiovascular risks were recruited.Clinical data including medical history,height,weight,blood pressure,fasting blood glucose and blood lipid were collected.We evaluated the arterial stiffness parameters of carotid artery and EID using an ultrasonic ET system in 46 elderly subjects,compared with 33 sex-matched non-elderly subjects.The impaired EID function was defined as brachial artery nitroglycerin mediated dilation (NMD)below 4%.Results All stiffness parameters including pressure-strain elasticity modulus stiffness index β (Ep),pulse wave velocity β (PWVβ)and augmentation index (AI)were significantly increased in elderly group compared with the non-elderly group [(138.9±64.7)kPa vs.(100.6±30.8)kPa,(10.9±4.7)vs.(8.2±2.3),and (6.9±1.4)m/s vs.(6.1±0.9)m/s,P<0.05 respectively],while the exception of arterial compliance (AC)was reduced (0.9±0.3)mm2/kPa vs.(1.0±0.5)mm2/kPa(P<0.05).The incidence of impaired EID in elderly group was higher than in non-elderly group [56.5% (26 cases) vs.33.3% (11 cases),P<0.05].ET parameters including Ep,stiffness index β,PWVβ,AC and AI were related to age (r=-0.44,-0.45,-0.40,-0.40,0.34,all P<0.01); Ep,stiffness index β,PWVβ and AC were also related to impaired EDI (r=-0.38,-0.40,-0.34,-0.29,all P<0.01).Conclusions Arterial stiffness properties and EID measured by ET system was more serious in elderly with multiple cardiovascular risks than in non-elderly subjects.As a convenient and accurate assessment of stiffness parameters,ET system is optimal option for measuring arterial stiffness and EID in elderly people.

ABSTRACT

Objective To discuss the clinical effect and postoperative complications of endoscope with plas-ma eletrosection and holmium laser in the treatment of urethral stricture. Methods 37 patients, suffering urethral stricture were treated by endoscopic plasma eletrosection and holmium laser. Tthe therapeutic effect was evaluated. Results All patients were succeeded in operation, and with almost no bleeding. Then indwelling catheter for 3-5 weeks. After the removal of the urinary catheter, 33 patients were voided well, and no complications such as urinary incontinence ; Within 3 days 4 cases were urine thinning, avoiding difficulty, given regular urethral dilation, 3 cases ineffective, and one case of the resumption. All patients were followed up for 6-24 months. There are 8 cases of plasma in patients with resection in voiding difficulty after 1-8 months, given regular urethral dilatation, then: 2 ca-ses of recovery ; 5 cases of poor results were given plasma vaporization of the trip again, haven good results ; 1 case was given routine Bladder ostomy. Conclusion The right intracavity, endoscopic endovascular treatment of urethral stricture for other lower urinary tract obstruction disease is a safe, minimally invasive and effective treatment meth-od.

ABSTRACT

Objective: To study the effect of hyperplasia suppressor gene (HSG) in inducing vascular smooth muscle cell apoptosis and the underlying mechanisms. Methods: The cultured VSMCs were transfected with an adenoviral vector containing rat HSG gene. Effects of HSG on VSMC apoptosis were investigated by fluorescent dye staining to detect the tact of nuclei, and by flow cytometry to define the content of DNA and to detect the levels of caspase-3. The expressions of Bcl-2 and Bax were also performed by Western blot analysis. Results: The increased expression of HSG in VSMCs infected with AdHSG induced apoptotic cell death detected by flow cytometry assay and nucleic staining. Compared with control groups, HSG induced vascular smooth muscle cell apoptosis 72 h after infected with adenoviral vector (39.6%?3.2% vs. 2.6%?0.9%,P
