ABSTRACT
Objective: To compare the content of total saponins and total polysaccharides between formula granule and traditional decoction of Sijunzi decoction.Methods: UV spectrophotometry was used to determine the content of total saponins and total polysaccharides in formula granule and traditional decoction of Sijunzi decoction respectively at the wavelength of 540 nm and 488 nm.Results: The absorbance of ginsenoside Re had a good linear correlation with the concentration within the range of 10.909-65.454 μg · ml-1 (r=0.999 7), and the average recovery was 98.49%(RSD=0.85%, n=6);the absorbance of D-anhydrous glucose had a good linear correlation with the concentration within the range of 2.160-12.960 μg · ml-1 (r=0.999 7), and the average recovery was 99.46%(RSD=0.73%, n=6).The contents of total saponins from 3 batches of formula granule were slightly higher than those from traditional decoction,and that of total polysaccharides in formula granule was slightly lower than that in traditional decoction,and there was no significant difference between the two groups (P>0.05)Conclusion: The difference of material basis between formula granule and traditional decoction of Sijunzi decoction is small, and formula granule is more feasible for clinical application.

Objective To evaluate the diagnostic value of multi-slice spiral CT (MSCT) in classifying coronary arteries of complete transposition of great arteries (D-TGA). Methods The clinical and imaging data of 367 patients with D-TGA who had undergone MSCT examination from March 2005 to June 2015 were retrospectively analyzed. The origin and course of the coronary arteries of the patients were classified according to the Marie Lannelongue classification. There were four patterns of courses: normal, looping, intramural and miscellaneous. And the four patterns were subdivided into eleven subgroups. The anatomic classification of coronary arteries in D?TGA were recorded, and the ratio of descriptive statistics was used according to categorical variable data. Results All the origin and course of the coronary arteries could be clearly displayed on MSCT. Of 367 patients with D-TGA, 209 cases (56.95%) were normal course (typeⅠ), 138 cases (37.60%) were looping course (typeⅡ), 16 cases (4.36%) were intramural course (typeⅢ), and 4 cases (1.09%) were miscellaneous course (typeⅣ). In looping course, the posterior looping (typeⅡA), anterior looping (typeⅡB) and double looping (typeⅡC) were found in 63 cases (17.17%), 30 cases (8.17%) and 45 cases (12.26%), respectively. The ratios of the anatomic classification of looping courses wereⅡA-1 44(11.99%),ⅡA-2 19(5.18%),ⅡB-1 12(3.27%),ⅡB-2 8(2.18%),ⅡB-3 10(2.72%),ⅡC-1 25 (6.81%),ⅡC-2 17(4.63%),ⅡC-3 3(0.82%). Conclusions MSCT is an effective technique to visualize and classify the coronary arteries in patients with D-TGA. And it is helpful for successful transfer of the coronary arteries and reducing the rate of coronary events after operation.

Objective To make a comparison of transthoracic echocardiography (TTE) and cardiac multi-slice spiral computed tomography (MSCT) in the diagnosis of partial anomalous pulmonary venous connection (PAPVC) in children.Methods One hundred and one patients admitted from February 2008 to April 2014 in Guangdong General Hospital primarily diagnosed with PAPVC with or without other congenital heart diseases were analyzed retrospectively,all the cases underwent routine TTE,which 74 cases underwent cardiac MSCT examination,and the TEE and cardiac MSCT diagnosis results were compared with final surgical findings,then the TTE and cardiac MSCT diagnosis accuracy was calculated.SPSS 13.0 software was used to analyze the data.Results All the patients underwent TTE examination,meanwhile,74 cases received cardiac MSCT examination.Ninety-three cases were confirmed by surgical findings,while 8 cases were misdiagnosed with PAPVC.Sixty-eight cases diagnosed by TTE coincided with surgical findings,and the diagnosis accuracy was 73.12％ (68/93 cases);65 cases diagnosed by cardiac MSCT coincided with surgical findings,and the diagnosis accuracy was 94.20％ (65/69 cases).Among the patients who received both TTE and cardiac MSCT examinations,66 cases were confirmed through final surgical operation,2 cases were mis-diagnosis,so the diagnosis accuracy was 97.06％ (66/68 cases).Conclusions In terms of the diagnosis accuracy of PAPVC,cardial MSCT examination is superior to TTE,as almost all of the cases could be diagnosed by TTE combined with cardial MSCT.In consideration of the acoustics window of children,apart from the conventional parasternal approach of detecting pulmonary veins,the importance of suprasternal and subcostal views should also be attached.

Objective:To investigate the inhibitory effects of Sedum Sarmentosum total flavonoids ( SSTF) on hepatic fibrosis in-duced by CCl4 ,and examine the effects on the expression of TGF-β1 and Smad7. Methods:Sixty male SD rats were randomly divided into the normal control group, the model group, 100, 200 and 400 mg·kg-1 SSTF groups and colchicine positive control group. The experimental model of hepatic fibrosis in rats was established by the injection of CCL4 . The liver histopathology was examined by Mas-son stain, and the protein expression and mRNA of TGF-β 1 and Smad7 were assessed by RT-PCR and Western blotting. Results:Compared with the model group, every SSTF group could significantly reduce the degree of liver fibrosis induced by CCL4(P<0. 05). The middle and high dose SSTF gouprs could significantly reduce the protein expression and mRNA of TGF-β1 (P<0. 05), and sig-nificantly increase the protein expression and mRNA of Smad 7 (P<0. 05). Conclusion:SSTF exhibits anti-hepatic fibrosis effects in rats through down-regulating the expression of TGF-β1 and up-regulating the expression of Smad7 in fibrotic liver tissue.

A method based on stable isotope dilution gas chromatography-mass spectrometry ( GC-MS ) was established to determine five kinds of organophosphate esters ( OPEs ) in different kinds of cosmetics. For cream and wax-based cosmetics, samples were extracted by appropriate solvent, and then purified by ENVI-Carb solid phase extracting column. For powder and aqueous samples, GC-MS test could be operated directly after solvent extraction and concentration. All of the samples were separated by DB-5MS (30 m×0. 25 mm× 0. 25 μm) chromatographic column, and detected by selected ion monitoring ( SIM ) mode. The target analytes were qualitatively confirmed by retention time and abundance ratio of characteristic ions, and quantified by stable isotope diluted internal standard method. Under the optimized conditions, the linear coefficients of the OPEs were larger than 0. 9995, the detection limits were 1. 0-30 μg/kg, the average recoveries ( n=6 ) ranged from 89 . 5% to 105%, and the RSDs were in the range of 2 . 9%-9 . 1%. The developed method, which was characteristic by satisfactory purification effect, favorable matrix effect elimination capacity, as well as high recovery, was suitable for the determination of OPEs in different kinds of cosmetic matrices.

Thymic epithelial cells (TECs) are one of the most important components in thymic microenvironment supporting thymocyte development and maturation. TECs, composed of cortical and medullary TECs, are derived from a common bipotent progenitor, mediating thymocyte positive and negative selections. Multiple levels of signals including intracellular signaling networks and cell-cell interaction are required for TEC development and differentiation. Transcription factors Foxn1 and autoimmune regulator (Aire) are powerful regulators promoting TEC development and differentiation. Crosstalks with thymocytes and other stromal cells for extrinsic signals like RANKL, CD40L, lymphotoxin, fibroblast growth factor (FGF) and Wnt are also definitely required to establish a functional thymic microenvironment. In this review, we will summarize our current understanding about TEC development and differentiation, and its underlying multiple signal pathways.

Objective To investigate pathogens distribution and their drug resistance of hospital acquired pneumonia (HAP) in local elderly paraplegic patients,and to help to gain experience in early using antibiotics.Methods One hundred and thirty six elderly patients diagnosed as HAP from January 2007 to December 2010 in our hospital were selected.Pathogens distribution and their drug resistance were detected.Results One hundred and fifty two pathogens are isolated from the 136 patients,and most of them are gram negative bacteria which accounts for 70.4％.The first three pathogens are Klebsiella pneumonia(24.3％),Escherichia coli (20.4％) and Pseudomonas Aeruginosa (18.4％).Gram-positive cocci accounts for 25.0％ in total pathogens,among them,staphylococcus aureus and streptococcus pneumoniae account for the most,and the number of Fungi is the fewest.Drug resistance rate of gram-negative bacteria is higher than that of Gram-positive bacteria.Gram-negative bacteria has higher resistance to ampicillin,cefoperazone,ciprofloxacin,levofloxacin and eotrimoxazole.Gram-positive bacteria has higher resistance to penicillin,cefazolin and gentamicin.Conclusion To elderly paraplegic HAP patients,the main pathogenic flora is gram-negative bacterium which shows multiple resistances.Being familiar with the features of pathogens and their drug resistance will provide better guidance on early treatment and improve prognosis of elderly paraplegic HAP patients.

Objective To investigate the value of the boundary sign and cortex sign in differential diagnosis of solitary brain metastasis and astrocytoma. Methods Thirty-one patients with high grade astrocytoma and 42 patients with solitary brain metastasis diagnosed either pathologically or clinically were included in this study. The differential diagnostic value of the boundary and the cortex sign were analyzed retrospectively. Results The boundary sigh was found in 39 of 73 patients, which included 32 patients with solitary metastasis and 7 patients with astrocytoma. The sensitivity and specificity of the boundary sign was 76.20% and 77.42%, respectively, and the positive predictive value for solitary metastasis was 82.05%. The cortex sign was found in 34 of 70 patients, including 25 with astrocytoma and 9 with solitary metastasis. The sensitivity and specificity of the cortex sign was 80.65% and 78.57%, respectively, and the positive predictive value for astrocytoma was 73.53%. The boundary sigh was singlely foundin 24 patients, including 21 with astrocytoma and 3 with solitary metastasis. The area under the curve for both two signs was 0.85. Conclusion The boundary sign and the cortex sign are specific signs in differentiation between solitary brain metastasis and astrocytoma on routine MR series. Combination of this two signs is very useful in distinguishing solitary brain metastasis from astrocytoma.

Objective To investigate the association SLC25A12 and SCN2A2 gene single nucleotide polymorphisms(SNPs) and susceptibility to autism among 105 Japanese family trios consisting of fathers,mothers,and affected offsprings with autism.Methods Genomic DNA was isolated from the whole blood samples.The PCR-single stranded conformational polymorphism(SSCP) technique was used to test genotype of SNPs(rs3770448,rs3769955) at SLC25A12 and SCN2A2 genes.Results The distributions of genotypic and allelic frequencies of rs3770448 and rs3769955 were not deviated from the Hardy-Weinberg equilibrium.The results of transmission disequilibrium test(TDT) indicated that the allelic frequency transmitted from the heterozygote parents didn′t deviate 50%.Conclusion The polymorphism of rs3770448 in the SLC25A12 and rs3769955 in the SCN2A2 locus may not be associated with autism.But the association of the other SNPs at the SLC25A12 and SCN2A2 locus with the illness can not be ruled out.

To investigate the therapeutic effect of ascitic fluid ultrafiltration refusion and perfusion of ?-elemene milk into abdominal cavity on carcinomatous ascites. Seventy-two cases of malignant tumor complicated with carcinomatous ascites were randomized to two groups: group A treated with perfusion of ?-elemene milk into abdominal cavity after ascitic fluid ultrafiltration refusion and group B with perfusion of ?-elemene milk into abdominal cavity after letting-out of ascitic fluid by regular and repeated abdominal paracentesis. Objective To construct adenovirus vector harboring CTLA4Ig-IRES-CTLA4 gene, and to express the gene in bone marrow mesenchymal stem cells (BMMSCs). Methods The cDNA fragments of CTLA4, and the extracellular domains of CTLA4 and IgGFc were cloned to pT-Easy vectors by RT-PCR from the activated splenocytes of Wistar rats. Recombinant adenovirus vector harboring CTLA4Ig-IRES-CTLA4 was produced from AdeasyTM Adenoviral Vector System by homologous recombination between the Adtrack vector (pAdtrack-GFP-CTLA4Ig-IRES-CTLA4) and the plasmid pAdEasy-1 containing most of the human adenovirus serotype 5 (Ad5) genome in E. coli BJ5183 strain, and then packaged and propagated in 293 cells. BMMSCs were infected with the recombinant adenovirus, and the co-expression of both CTLA4Ig and CTLA4 was then detected by RT-PCR and Western blotting. The immunosuppression function of the transfected BMMSCs was investigated by mixed lymphocyte response (MLR). Results Fusion gene CTLA4Ig was constructed successfully, and the recombinant CTLA4Ig-IRES-CTLA4 adenovirus was then generated by homologous recombination and packaged in 293 cells. The transduction efficiency of recombinant adenovirus in BMMSCs was elevated up to 98.67% determined by flow cytometry using a GFP as marker. CTLA4Ig and CTLA4 were expressed at the same time in BMMSCs detected by RT-PCR and Western blot, and the transfected stem cells showed stronger immunosuppression function than that of BMMSCs or BMMSCs transduced by Ad-CTLA4Ig. Conclusion The BMMSCs transfected by the recombinant adenovirus can co-express CTLA4Ig and CTLA4, which improved the immunosuppression function of BMMSCs in the way of costimulatory pathway blockade.
