ABSTRACT

Objective:To investigate the effects of mindfulness relaxation training on stress response and postoperative rehabilitation of patients undergoing general surgery.Methods:A total of 110 patients who underwent general surgery were randomly divided into study group and control group, with 55 cases in each group. The control group received routine peri-operational period nursing, and the study group carried out mindfulness relaxation training on the basis of control group for 4 weeks. The levels changes of serum insulin, cortisol were detected between two groups. The psychological stress of two groups were evaluated with Symptom Checklist 90 (SCL-90). The off-bed activity time, anus exhaust time, bowel sounds recovery time and hospital stay time were recorded to evaluate the quality of patients ′ postoperative rehabilitation. Results:The levels of insulin, cortisol were at minimum on admission in two groups, and then increased, the highest level was at 2 hours after operation. There was no significant difference in insulin, cortisol level on admission in two groups ( P>0.05), however, the insulin [(120.15±22.63) μg/L, (122.34±15.75) μg/L], cortisol [(10.41±3.07) mmol/L, (12.69±4.50) mmol/L] levels were significantly decreased in the study group compared to the control group [(129.85±15.43) μg/L, (131.34±22.79) μg/L and (11.69±2.99) mmol/L, (14.84±4.04) mmol/L] before anesthesia and at 2 hours after operation ( t value was 2.206-2.638, P<0.05). After intervention, the total scores of SCL-90 (120.91±11.84), somatization (1.26±0.13), compulsion (1.43±0.08), depression (1.18±0.12), anxiety (1.14±0.10) and fear (1.34±0.08) were all lower in the study group than those in the control group (130.47±14.13, 1.83±0.53, 1.65±0.58, 1.71±0.12, 1.76±0.13, 1.41±0.12), and the differences were statistically significant ( t value was 2.828-28.277, all P<0.05). Besides, off-bed activity time [(1.55±0.18) d] and hospital stay time[ (6.51±2.20) d] in the study group were significantly less than in the control group [(1.73±0.25) d and (7.65±2.19 d)] ( t value was 2.737,4.592, P<0.05). Conclusion:The mindfulness relaxation training in nursing general surgery patients during peri-operational period is helpful to ameliorate the patients ′ degree of physiological and psychological stress reactions, and this mode has important significance in promoting the quality of patients' postoperative rehabilitation.

ABSTRACT

Objective To assess the value of functional magnetic resonance urography (fMRU)for the unilateral renal function in children with hydronephrosis.Methods Fourteen children with congenital hydronephrosis (unilateral hydronephrosis in 1 2 cases,bilateral hydronephrosis in 2 cases)examined by fMRU in Beijing Children′s Hospital,Capital Medical University,were enrolled.In 7 patients of them,diuretic renal scintigraphy (DRS)was per-formed within 1 0 days before fMRU examination.The following parameters in fMRU,as renal parenchymal volume,volu-metric differential renal function (vDRF),Patlak,Patlak differential renal function (pDRF),index of glomerular filtra-tion rate (GFR)and differential renal function based on index of GFR (gDRF),were calculated and analyzed.Statisti-cal analysis was performed by using SPSS 1 3.0.Results In 7 cases whose fMRU and DRS were examined,the indexes of GFR obtained from fMRU and GFR from DRS were well correlated (r =0.892,P <0.001 )in 1 4 kidneys.The gDRF determined by 2 methods on the left kidneys[the average was(46.80 ±1 9.20)% and(45.1 8 ±20.29)%,respective-ly]had no significant difference (t =0.051 6,P =0.624),which was also highly correlated (r =0.91 2,P =0.004). In 1 2 cases with unilateral hydronephrosis,vDRF,pDRF,index of GFR and gDRF in hydronephrotic side[(43.54 ± 9.61 )%,(42.80 ±1 0.83)%,(38.56 ±29.23)mL/min,(38.37 ±1 3.61 )%]were all less than those in the con-tralateral side[(56.46 ±9.61 )%,(57.1 9 ±1 0.83)%,(57.02 ±26.22)mL/min,(61 .63 ±1 3.61 )%](t =2.326, 2.300,2.422,2.960;P =0.040,0.042,0.034,0.01 3).However,there was no statistical difference in both renal pa-renchymal volume and Patlak between the hydronephrotic and the contralateral side kidneys(t =1 .765,1 .450;P =0.1 05,0.1 75).Conclusions fMRU is a very valuable examination method in evaluating single kidney function in children with congenital hydronephrosis,and able to demonstrate that gDRF,indexes of GFR,vDRF and pDRF decrease in the hydronephrotic kidney.

ABSTRACT

Objective To investigate the effect of mycophenolic acid (MPA) on the proliferation of rat pulmonary microvascular endothelial cells (PMVECs) and the effects of MPA on the secretion of endothelin-1 (ET-1) and interleukin-6 (IL-6).Methods PMVECs were cultured by tissue-sticking method.Cells were divided into 5 groups,named as the control group,low-dose MPA group,moderate-dose MPA group,highdose MPA group and super high-dose MPA group,according to the final concentration of MPA (0,0.1,1,10 and 100 μmol/L,respectively).The proliferation of rat PMVECs was detected by cell counting kit-8 (CCK-8).The ET-1 and IL-6 secretion were assessed by radio-immunoassay.One-way analysis of variance was used to detect the differences between the group means.Results The cultured PMVECs of rats in vitro showed fusiform or polygonal in shape,and the expressed factor Ⅷ associated antigen stain was positive.At 24 h,the A values of the 5 groups (from control group to super high-dose MPA group) were 1.15±0.20,1.20±0.13,0.88±0.20,0.62±0.05 and 0.56±0.10,respectively,there was no significant difference in the A values on 450 nm between the control group and the low-dose MPA group (P=0.388),as well as the high-dose MPA group and super high-dose MPA group (P=0.292),but there were significant differences between any other two groups (P<0.01).At 48 h,the A values of the 5 groups (from control group to super high-dose MPA group)were 2.61±0.21,2.52±0.23,1.99±0.17,0.84±0.16 and 0.65±0.12,respectively,there was no significant difference in the A values on 450 nm between the control group and low-dose MPA group (P=0.094),but there were significant differences in the A values between any other two groups (P<0.01).At 72 h,the A values of the 5 groups (from control group to super high-dose MPA group) were 2.62±0.24,2.62±0.19,2.37± 0.38,0.79±0.17 and 0.61±0.17,respectively,there was no significant difference in the A values on 450 nm between the control group and low-dose MPA group (P=0.931),but there were significant differences in the A values between any other two groups (P<0.05).There were no significant differences in the ET-1 concentrations between the control group and low-dose MPA group (P=0.156),and high-dose MPA group and super high-dose MPA group (P=0.262),but there were significant differences in the concentrations between any other two groups (P<0.01).There were no significant differences in the IL-6 concentrations of the five groups between any of the two groups (P>0.05).Conclusion MPA can inhibit fetal bovine serum induced rat PMVECs proliferation and the secretion of ET-1 in a dose-dependent manner.

ABSTRACT

Objective To investigate the effect of Sangi Tonglero decoction on dynamic electrocardiogram and clinical results, in order to improve the level of clinical treatment. Method 110 Patients with variant angina pectoris in February 2010 to February 2013 were selected and divided randomly into two groups. 55 patients in control group were given amlodipine treatment , the other in observation group were added Sangi Tonglero decoction beside amlodipine treatment. The differences in electrocardiogram and clinical effects between two groups after treatment were compared. Results The clinical results of dynamic electrocardiogram and angina were showed that the efficiency rate and total efficiency rate in observation group were significantly higher than that in control group(P<0.05). The difference of lipid levels, blood rheology indicators between two groups after treatment were statistically significant (P<0.05). Conclusion Dynamic electrocardiogram is a preferred method of checking variant angina pectoris , and Sangi Tonglero decoction can significantly improve the therapeutic effect .

ABSTRACT

ObjectiveTo find the proper way of assessing the image noise characteristic through the analysis of noise characteristic in the digital mammographic images.MethodsThe CDMAM phantom was firstly exposed with different filter materials and tube voltages in the range of 23 to 35 kVp at a fixed tube current 90 mAs,then exposed with different filter materials and tube current in the range of 20 to 200 mAs at a fixed tube voltage 29 kVp.The parameters such as background standard deviation,joint standard deviation,subtracted standard deviation and relative standard deviation were calculated and measured from both 60 pixels of signal disc and background beside of the disk for all the images.The IQFinvs were calculated for the achieved images.Non-parametric rank sum tests were used to compare the difference of the background standard deviation,joint standard deviation and subtracted standard deviation,t test was used to compare the IQFinvs between Mo and Rh filers in each group.ResultsWith the same exposure parameters in the test above,the maximum value was the background standard deviation,while the subtracted standard deviation was the minimum,and the value of joint standard deviation was in the middle. With other parameters unchanged,the values of three kind of standard deviation increased with the increasing of tube voltage or tube current,while the differences between them getting more significant.Under the condition of 90 mAs and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 10.91,10.34 and 9.87,the results were 11.24,10.57 and 9.96 while Rh filter material was used,significant difference was found ( x2 =26.0,P < 0.01 ). Under the condition of 29 kVp and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 12.30,11.61 and 11.05,the results were 12.29,11.58 and 10.87 while Rh filter material was used,significant difference was found( x2 =20.0,P < 0.01 ).The IQFinvs were 149.14 ± 23.87 and 139.16 ± 23.14 under the condition of 90 mAs and Mo filter material ( t =9.129,P < 0.01 ).The IQFinvs were 149.75 ±34.85 and 145.2 ±34.64 while the parameters were 29 kVp and Rh filter material(t =1.304,P >0.05 ).The value of relative standard deviation decreased with the increasing of tube voltage or tube current,and the image quality got better with the decreasing of relative standard deviation.ConclusionRelative standard deviation is suitable not only for the expression of the digital mammographic images,but also for the evaluation of the image quality.

ABSTRACT

Objective To determine the clinical and serological characteristics of autoimmune hemolytic anemia (AIHA) in systemic lupus erythematosus (SLE) patients.Methods Seventeen patients were studied who had clinical features of AIHA among 222 hospitalized SLE patients in the Department of Rheumatology of Beijing Chaoyang Hospital were studied.Among the SLE patients without AIHA,34 patients were chosen by random sampling as controls.The patients' demographic data,clinical manifestations,laboratory test results and other examinations were analyzed retrospectively.Student's t test was used for the comparison of continuous variables,and chi-square test was used for the comparison of proportions.Results The prevalence of AIHA was 7.6% in cohort of lupus patients.The positive rate of IgG anticardiolipin antibody in the AIHA group was significantly higher than that in the control group (71% vs 15%,x2=15.366,P<0.01 ).The occurrence of antiphospholipid antibody syndrome between the two groups had no statistical significant difference (x2=0.000,P=1.000).The frequencies of arthritis (12%,x2=4.554,P=0.033),malar rash (6%,x2=4.443,P=0.033),leukocytopenia (12%,x2=8.061,P=0.005) and lymphocytopenia (41%,x2=5.075,P=0.024) were lower in the AIHA group than those in the control group.Three patients in the AIHA group had alveolar hemorrhage in the course of SLE,while there was none in the control group (x2=3.586,P=0.058).Conclusion When lupus patients with AIHA are compared with those without AIHA,the positive rate of anticardiolipin antibody is higher,but the occurrence of antiphospholipid antibody syndrome does not increase in SLE patients with AIHA.And the frequencies of arthritis,malar rash,leukocytopenia and lymphocytopenia are lower in those patients with AIHA,however,patients with AIHA are prone to develop alveolar hemorrhage.

ABSTRACT

ObjectiveThis study is aimed to investigate the association of human leukocyte antigen (HLA)-DRB1 with rheumatoid arthritis (RA) in Chinese Han population.MethodsA total of 281 Chinese Han patients with RA and 202 healthy controls were recruited.DNA was extracted from PBMC and HLA typing was performed by sequence based typing and PCR-Sequence Specific Primer.The frequency of HLADRB1 was compared between patients and controls using x2 test with continuity correction.ResultsThe susceptible HLA-DRB1 alleles were * 0101,* 0102,*0404,* 0405,and * 0410 which belonged to QRRAA.DRRAA and DERAA were protective alleles.At genotypic level,The association of S3P and S3D was detected.However,the protective effect of S3D was shown to be in a recessive mode.ConclusionOur results have shown that there are racial differences in RA susceptibility between Chinese Han population and Caucasians.

ABSTRACT

OBJECTIVE@#To evaluate the therapeutic effect of supracricoid partial laryngectomy(SCPL).@*METHOD@#A retrospective analysis of the clinical data of 24 patients undergoing laryngeal carcinoma (including 5 senile patients). In those patients, 14 were glottic cancer, 9 were supraglottic cancer,and 1 were transglottic cancer. Fourteen cases underwent cricohyoidoepiglottopexy, and 10 cases treated with cricohyoidopexy.@*RESULT@#In this study, 3 years and 5 years survival rates were 91.7% and 78.6%. The decannulation rate was 91.7%. All the patients resumed physiologic swallowing.@*CONCLUSION@#Supracricoid partial laryngectomy for selected laryngeal cancer is feasible, and also for senile patients. The patients can gain satisfied survival rate and physiologic function.

ABSTRACT

Objective To investigate the manifestations of esophageal motility disorders and evaluate the association between them and dysphagia, laboratory tests and other accessory examinations in patients with Sj(o)gren's syndrome (SS). Methods Esophageal manometry was performed in 31 patients with SS and 18 healthy volunteers by the step pull-through method. Results Decreased upper esophageal sphincter pressure was detected in 19 of the 31 patients (61%) with SS, while 4 of 18 (22%) in controls. The frequency was significantly higher in patients than in healthy controls (P=0.008). Fifteen of 31 patients (48%) showed various patterns of esophageal dysfunction including ineffective esophageal motility in 6 patients, nutcracker esophagus in 3 patients and nonspecific dysmotility in 6 patients. No major differences were found in esophageal parameters (peak amplitude, wave duration and velocity) when comparing primary SS with secondary SS. These esophageal abnormalities were not correlated with clinical manifestations, laboratory examinations and other auxiliary examinations. Conclusion Patients with SS may have esophageal motility disorders, which can presents with different patterns.

BACKGROUND: Researches on vitodynamics present that percutaneous vertebroplasty (PVP) can strengthen hardness and intension of single osteoporosis vertebral body after injection of bone cement; however, the infused volume and site of bone cement for maintaining mechanical stability of vertebral body should be further studied.OBJECTIVE: A numerical calculation method on finite element models (FEM) for biomechanical analysis has been developed, while a boundary condition describing the relative L1 -L2 displacement is imposed on the FEM to account for three-dimensional physiological states.DESIGN: Non-randomized control study.SEITING: Beijing Neurosurgical Institute.MATERIALS: One female patient aged 54 years with compressibility fracture at lumbar vertebrae L1-L2 induced by osteoporosis was diagnosed with CT examination, and the patient was told the fact. Based on CT scanning before and after PVP, 90 serial sections with the thickness of 1 mm were obtained and the size of each pixei was 0.33 mm.METHODS: The experiment was carried out Beijing Neurosurgery Institute from October 2005 to June 2006. ①Establishment of three-dimensional FEM: Integrating the anatomical structure from the spine CT and MRI image of a patient, a novel three-dimensional geometric model of lumbar functional spinal units (FSUs) has been built. Meanwhile, 90 serial sections were obtained to exchange data of CT sections and divide imagings. Based on the geometric model, two kinds of three-dimensional FEM of L1-L2 segments for preoperative and postoperative vertebrae were created. ② Model evaluation: The lcad of (500 N, 1 000 N, 1 500 N, 2 000 N, 2 500 N) axial compression were applied to the superior surface of the model in the form of a uniformly concentrated lcad over all L1 superior surface nodes respectively. We could observe the stress distribution of L1-L2 segment by applying the load and clue on the high stress concentration region as the most likely areas fracture.MAIN OUTCOME MEASURES: ① Changes of displacement, stress and strain distributions of FEM at L1-L2 segment of lumbar vertebrae under various loads; ② Effect of increase of bone cement volume on displacement, stress and strain distribution.RESULTS: ① The increase in displacement, stress and strain of FE model with the increase of loading in the postoperarive cases. The tendency was approximately linear which also illustrates the spine have flexible biomechanical characteristics. The region was a common place for injures due to loading. The magnitude of stress in the intervertebral disc depended on the proportion of load applied to the superior surface of the motion segment. The heavier the lcad was, the stronger the stress was. ② With the increase of stress on intervertebral disc, the increasing volume of bone cement could induce transfusion of load of nearby vertebral body.CONCLUSION: Building three-dimensional FEM of L1-L2 segments for preoperative and postoperative PVP can explain three-dimensional physiological status of vertebral body based on calculating relative shift of L1-L2 segment of lumbar vertebrae. Meanwhile, simulative calculation can clearly express distribution of stain and stress and preoperative and postoperative deformity of vertebral body.

ABSTRACT

This paper provides a new medical image segmentation algorithm using a deformable contour model, which integrates Fuzzy C-Means(FCM) Clustering technique and deformable contour model. An external fuzzy constrain is defined from the membership function value of FCM, which joins the external constrain of the deformable model and drives the deformable model towards the contour ideal edge of the object. Examples are presented to demonstrate the efficiency and feasibility of the approach on spinal MRI images and the results are encouraging.

ABSTRACT

Feature extraction of event related potential (ERP) plays an important part in both fundamental and clinical researches for cerebral neurophysiology.Based on the prior knowledge of feature distribution of ERP,this paper introduces an approach to feature extraction of ERP from composite EEG,which is combined with wavelet multiresolution analysis (MRA) and radial basis function neural network (RBFNN).The components related to low-frequency response can be extracted from the wavelet decomposition coefficients by RBFNN.Then signal reconstruction is implemented to obtain the feature of ERP.Experimental result demonstrates that the approach is reliable.

ABSTRACT

Magnetic resonance diffusion tensor imaging (DTI) has been established as the primary imaging modality for non-invasive characterization of the direction-dependent motion of water molecules in the living tissue. DTI provides each image voxel with a 3x3 tensor matrix that can be processed to probe tissue microstructure and microarchitecture. In this paper, the imaging principle and diffusion tensor calculation are introduced. Examples are given to describe image-processing techniques for DTI visualization, fiber tracking, and applications to central nervous system.

ABSTRACT

Objective To investigate a wavelet-transform-based approach that reduces the noise of medical images, and to compare the difference of the effects by different wavelet types. Methods A soft threshold approach based on the modification of local coefficient of wavelets was proposed. Firstly, a local modulus extrema distribution of the image, M j,m,n is obtained using wavelet transform. Then the modulus maximum was calculated and a threshold Tm was defined according to the statistical properties of the local modulus extrema distribution. If the extremum of the wavelet transform was greater than or equal to the threshold Tm, the corresponding wavelet coefficient was kept unchanged; while if the extremum of the wavelet transform was less than the threshold Tm, its corresponding wavelet coefficient was calculated using the soft threshold approach. Lastly, an inverse wavelet transform was performed according to the wavelet coefficients of these two parts so that the image could be reconstructed. Results The proposed approach could filter out the noise in medical images effectively, and the effects of noise reduction by different wavelets were different. Conclusion A useful wavelet threshold noise reduction algorithm can be obtained by wavelet multi-dimensional decomposition of image with proper selection of wavelet base function, and comparatively ideal effect of noise reduction can be achieved using this algorithm.

Objective To search for a candidate DNA vaccine of Fasciola hepatica. Methods Using RT-PCR and digestion with Hind III and BamHI, Fasciola hepatica secreted cathepsin L1(FheCL1) cDNA was cloned into the expression vector pcDNA3.1. Results The cloning was successful, the cDNA sequence and its deduced amino acid sequence were analyzed. There was much difference between the cloned FheCL1 and the published one. But the first 20 residues of their amino acid sequences were the same. Conclusion The recombinant plasmid pcDNA3.1-FheCL1 may be a new type of candidate DNA vaccine candidate for Fasciola hepatica. It is possible that Fasciola hepatica presents different sub-species but their amino acid residues (1 to 20) encoded by FheCL1 might build up membrane spanning helix.

ABSTRACT

Objective To compare the morphological features and the development of Raillietina echinobothrida and R.tetragona from chicken.Methods The parasites were collected from chicken digestive tract, and 3-5 fresh scolex fragments were separated.After being washed, fixed, dried, glided by ion sputter, the specimens were observed and photographed under scanning electron microscope.The cestodes were immersed in tap water for natural stretch and proglottids of different development stages were used for serial paraffin sections by conventional method, stained with HE and mounted in gum.Morphological observation was carried out by optical microscopy.The naturally stretched worms were pressed between two glass, stained, dehydrated, cleared, mounted, and observed under optical microscope.Results The body of the two species consists of scolex, neck and strobilae.Each mature proglottid showed a set of male and female reproductive system and genital openings on one side.Testes located on both sides of the ovary and behind vitellarium.A complete worm of R.echinobothrida was shorter than R.tetragona, with a round scolex and suckers and short neck.The ovary looked like leaf and vitellarium was in kidney-shape.There were many acid particles and calcareous corpuscles in gravid proglottids.Egg capsule showed no clear boundary and contained only one egg.However, the scolex and suckers of R.tetragona were oval in shape, and the neck was long and thin.The ovary was flower-like.Each egg capsule contained 4-12 eggs and many calcareous corpuscles, each of which was surrounded by a membrane.The male reproductive system matures first in both species.As the two reproductive systems matured, the proglottids became gravid after fertilization.The formation of egg capsule in the two species was similar.Conclusion There are no significant morphological and development differences between the two species of Raillietina.
