ABSTRACT

Objective To explore the effect on immunologic function of gastric carcinoma patients after operation by transposition of a transverse colon segment as a gastric reservoir following total gastrectomy. Methods A prospective,randomized,double-blind clinical trial was performed. One hundred and sixty-seven gastric carcinoma patients were divided into control group and research group in terms of balanced random groups. We reconstructed digestive tract with conventional Schlatter esophagojejunostomy and Roux-enY esophagojejunostomy in control group, while we reconstructed digestive tract with transposition of a transverse colon segment as a gastric reservoir in rcsearch group.the changes of T cell subset,IL-2,immunologic function of red blood cell, acute inflammatory mediator were analyzed in the patients respectively at the first day before operation,at the first day after operation,at the ninth day after operation.Postoperative fatality rate was also detected in two groups. Results Compared with control group,The changes of T cell subset,IL-2 had no significant differences in research group at the first day after operation (P > 0.05).chaplet Meanwhile, the changes of C3b receptor chaplet rate and immunocomplex receptor rate had no significant differences in rcscarch group compared with control group at the first day after operation (P > 0.05).There were no obvious differences for acute inflammatory mediators between two groups at the first day after operation (P>0.05).At the ninth day after operation, the levels of CD4+ (44.68 ±5.92)% in control group were obviously lower than those (48.75 ± 6.24)% in research group (P < 0.05).However,the percentage of CD8 + T cells (27.21 ± 3.07) % in control group was significantly higher than that (24.26 ±2.39) % in research group (P < 0.05).The levels of IL-2 in control group and research group were (1.49 ± 0.32) μg/L and (1.67 ± 0.35) μg/L,with obvious differences between two groups (P < 0.05).At the ninth day after operation,C3b receptor chaplet rate (39.27 ± 6.88) % in research group was markedly higher than that (30.25 ± 6.65) % in control group (P < 0.05).The levels of IL-6 in control group were (125.51 ± 18.24) pg/mL,while the levels of IL-6 in research group were(87.34 ± 12.88) pg/mL,with obvious differences (P < 0.05).There was no significant difference for postoperative fatality rate between two groups (P > 0.05).Conclusions The transposition of a transverse colon segment as a gastric reservoir is a safely new mode in the reconstruction of alimentary. It is positive to improve immunologic function for gastric carcinoma patients after operation.

ABSTRACT

Objective To evaluate the possibility and reliability of laparoscopic management of cholelithiasis merging extrahepatic and acalculous and benign obstructive jaundice. Methods Eleven patients with cholelithiasis merging extrahepatic and acalculous and benign obstructive jaundice who underwent laparoscopic management were retrospectively studied. Results Ten cases were successful with laparoscopic management, among the total,4 patients were underwent laparoscopic cholecystectomy (LC),6 cases were experienced LC and laparoscopic common bile duct exploration (LCBDE).One case was converted to open operation. No serious complications were observed. All the patients were cured after the operative treatment.None of the complications could be traced after follow-up for 6 months. Conclusions Cholelithiasis merging extrahepatic and acalculous and benign obstructive jaundice is a challenge to laparoscopic surgeons.Intraoperative diagnosis is very difficultly, but very important. It is better prognosis by careful and skilled operators and reasonable operative method to be chose.

ABSTRACT

ObjectiveTo evaluate the expression of heat shock protein in child asthma patient and investigate the risk factors of child's asthma. MethodPatients with clinical data of pediatric asthma history from outpatients department in Wuhan fifth hospital and follow up data were selected. The analysis of interrelated element was used with conditional logistic regression. ResultThe result of logistic analysis demonstrated that asthma was related with these risk factors, including history of upper respiratory infection, changes of weather, individual history of sensitive disease, family history of sensitive disease and heat shock protein. Fierce sports, factors of diet, passive smoking, and etc. Were not related to the degree of danger of asthma in this study. ConclusionHistory of upper respiratory infection, changeable weather, individual history of sensitive disease, family history of sensitive disease and heat shock protein were related with the degree of asthma. It should be helpful to treat asthma with continued propagation and education to advocate standardized treatment.

ABSTRACT

Objective To investingate the effect of survivin shRNA on chemotherapy resistance in GBC-SD cells. Methods They were divided into three groups of GBC-SD, GBC-SD/EGFP and GBC-SD/survivin. MTT assay was used to detect cell viability in the 3groups. mRNA and protein of survivin were tested by RT-PCR and Western blot. Then the cells were treated with proper construction DDP. The cell survival rate and IC_(50) were determined with MTT, cell apoptosis detected by FACS and the alteration of nucleus observed by TUNEL. Meanwhile, caspase-3 activity was determined using the colorimetric method. Results Cell viability was reduced remarkably in GBC-SD/survivin and survivin expression was decreased obviously. After being treated with DDP, cell survival rate and IC_(50) were decreased obviously in GBC-SD/survivin, apoptotic rate elevated remarkably compared with other groups. There were brownly nucleuses in three groups. Caspase-3 activity increased first and then decreased, but it exceeded in GBC-SD/survivin than that in other two groups. Conclusion The survivin shRNA can down-regulate the expression of survivin in GBC-SD cells remarkably and improve the sensibility to chemotherapy.

ABSTRACT

Objective To study the inhibit effect of survivin short hairpin RNA on the survivin mRNA and protein expression of gallbladder carcinoma cells. Methods Human gallbladder carcinoma cells(GBC-SD) were transfected with recombinant plasmid.RT-PCR and Western blot were used to analyze the changes in(expression) levels of survivinmRNA and protein. Results The size of the PCR product was 350bp.DNA(sequencing) showed that the sequence of recombinant vector pshRNA-survivin was successfully constructed and suppressed the expression of(GBC-SD) survivinmRNA.Conclusions The recombinant plasmid constructed can inhibit the expression of survivinmRNA in transfected cells. This provides a new method and material for the biological therapy of cancer.

ABSTRACT

Objective To study the effect of CO2 pneumoperitoneum on the anti-apoptotic capability of gallbladder carcinoma cell line (GBC-SD).Methods The CO2 pneumoperitoneum was established in vitro. After GBC-SD cells were exposed to CO2 pneumoperitoneum at 12 mmHg for 1,2,3,4 h,the expressions of survivin mRNA was examined by RT-PCR.The untreated GBC-SD cells served as control. Results Under a pressure of 12 mmHg,the expressions of survivn had an increasing tendency with the exposure time. Conclusions Under certain pressure and within certain exposure time,CO2 pneumoperitoneum could improve anti-apoptotic capability of GBC-SD cells, which may be one of the mechanisms of the promoting effect of carbon dioxide pneumoperitoneum on malignant cell growth.
