ABSTRACT

Two hundred and eighty Health care; physicians, nurses and technicians were included in the study, 133 [47.5] from 6[th] discrete HCF, 54 [19.3%] from Duwaiqa and 93 [33.2%] from Al Haggana. Ages ranged between 22 and 59 years [35.14 +/- 10.13], years of experience in the study group ranged between 1 and 35 years [8.72 +/- 8.18] and years of work in group ranged between 1 and 40 years [10.43 +/- 8.33]. Doctors and nurses were males [72.55%]and females [86%] respectively while technicians were mostly males [60%]. The teamwork climate score was 3.98 +/- 0.87 +/- 64.0% answered high or very high, 16.4% answered inadequate and 18% had answered few or very few. Safety climate score was 3.61 +/- 0.63, 49.28% had high or very high score, 17.14% answered inadequate and 32.15% had answered few or very few. Job satisfaction score was 3.91 +/- 0.80, 32.15% had answered few or very few, 17.14% answered inadequate and 49.28% answered high or very high. Stress recognition score was 3.61 +/- 0.79, 25% had answered few or very few, 28.6% answered inadequate and 45.7% answered high or very high. Perception of management score was 3.48 +/- 0.80, 23.2% had answered few or very few, 17.8% answered inadequate and 57.6% answered high or very high. Working condition score was 3.51 +/- 0.84, 46.8% had answered few or very few, 17.1% answered inadequate and 35.7% answered high or very high. A significant difference regarding team work score, safety climate score, perception of management score, working condition score with highest value in doctors and lowest in technicians. On the other hand no significant difference was detected regarding job satisfaction score and stress recognition score.A significant difference regarding team work score, safety climate score and perception of management score with high values among older groups. No significant difference was detected regarding job satisfaction score, stress recognition score and working condition score. Also, a significant difference regarding team work score and safety climate score with higher value in males. Significant difference regarding job satisfaction score and working condition score was detected with higher value in females. No significant difference was detected regarding stress recognition score and perception of management scores

[haematological, immunological, radiological, nerphysiological, etc...] -To provide there patients with various types of treatment [Medical, physical, and surgical] and devices. 30640 patients attending rheumatology and rehabilitation outpatient clinic [for adult] in one year [2007] above age of 12 years old. -Disease history -Clinical examination - Investigation: haematological, immunological, radiological, neurophysiological, etc.... -Diagnosis -To know number of cases of each disease and the commonest disease. Disease: Rheumatological Disease, L.M.N.L, Post operative, Post traumatic, Post stroke, Other, Sum. No. of Cases: 28211, 423, 304, 229, 224, 1249, 30640. Percentage%: 92.07%, 1.38%, 0.99%, 0.74%, 0.73%, 4.07%, 100%. The commonest disease seen in rheumatology and rehabilitation outpatient clinic [for adult] in national institute of neuromotor rehabilitation in one year [2007]. Rheumatological diseases followed by L.M.N.L The institute must be provided with investigation for these diseases [haematological, immunological, radiological, neurophysiolgical, etc..] and different types of treatment [medical, physical and surgical] and with equipment and devices

ABSTRACT

Up till now there is no guidelines for blood transfusion in healthy preterm infants. In this study we try to show if blood lactate measurement may be used as in index for transfusion in those preterms. The present study included 33 healthy preterm infants, received no medications except for theophyline and prophylactic antibiotics, required no intravenous fluids or oxygen therapy. They were divided into two groups, [group I] included 23 babies, given once transfusion according to the decision of physician in charge. The main indications for transfusion were Hb below 8 gm/dl, Hct value below 30% and poor weight gain. The remaining 10 babies were not transfused [group II] and used as a control to measure normal blood lactate. The infants in the two groups were subjected to thorough history, clinical examination and selective laboratory investigations including CBC and blood lactate measurement. The results of group I were compared with each other before and after transfusion and with that obtained from group II. Comparing data of group I with that of group II, significant increase in blood lactate and decrease in Hb, Hct and body weight were detected in group I before blood transfusion. Pre and post transfusion data were compared in group I, there were significant decrease in blood lactate levels and significant increase in Hb, Hct and body weight, after transfusion. Blood lactate had a high specificity 100% and sensitivity 91.3%. We can conclude that measurement of blood lactate level as an indicator of early tissue hypoxia could identify otherwise healthy preterm infants who might benefit from blood transfusion. Thus, it may prevent unnecessary transfusion

ABSTRACT

This study is a retrospective analysis of 86 evaluable patients with pathologically documented cerebellar medulloblastoma presented to the King Faisal Specialist Hospital and Research Center between 1975-1985. The overall 5-year survival rate of the group was 56 +/- 7% and the progression free survival [PFS] was 47 +/- 7%. Patients with total surgical excision of their tumors had a significantly higher 5-year survival rate of 91 +/- 9% compared to 43 +/- 8% for those with less radical procedures [p = 0.002]. Patients below 5 years of age had a significantly lower 5-year survival of 35 +/- 14% compared to 62 +/- 8% for older patients [p = 0.01]. The dose to the posterior fossa [PF] and the spinal cord significantly influenced the survival of the patients. Patients who received a dose < 50 Gy had a 5-year survival of 45 +/- 12% compared to 63 +/- 8% for patients treated with higher doses to the PF [p = 0.05]. Similarly, patients who received > 30 Gy to the spinal cord had a 5-year survival rate 65 +/- 8% compared to 37 +/- 12% for patients treated with lower doses [p = 0.01]. Multivariate analysis of the effect of age, dose to the posterior fossa, dose to the spinal cord and extent of surgical resection on survival revealed that the extent of surgical resection was the only significantly independent factor
