ABSTRACT

Diaphragmatic injuries are not uncommon injury in the blunt abdominal trauma, having almost 0.8-8.0% of those blunt injuries. Multiple imaging tools are needed lo evaluate the presence of those diaphragmatic tears. Some diaphragmatic injuries might be undiagnosed until late, especially those patients with early conventional films revealing still apparent intact copula, or those with basal hemi-thorax opacification, where the pleural fluid cannot be delineated from the diaphragmatic outlines. Each imaging modality from conventional chest radiographs, or the ordinary CT, can miss the early appearance of those tears. With the advances in CT technology, and introduction of multi-detector CT [MDCT], dramatic reduction of motion and beam-hardening have been implemented. The marked improvement in the spatial resolution especially with the improvement in the volume Z-Axis direction offered by the MDCT with better isotropic recognition is also achieved. Also, there is the added value in detection of other multiple injuries offered by the extended field of view of the MDCT

ABSTRACT

Male infertility represents a percentage of the many other causes of infertility among couples. Obstructed male reproductive pathway till the prostatic urethra, represents one of the male causes which can be treated in confidence. Introduction of Trans-Rectal Ultrasonography [TRUS] with localization and cannulation of the seminal vesicles added much to the diagnosis of that problem, especially in obstructed cases. Fourteen patients were the material of our study. TRUS with needle insertion followed by fluoroscopic injection of the contrast to the seminal vesicles was performed. The patients attended with a mean age of 30 years, having different complains from painful ejaculation [8], perineal pain [8], infertility [7], and heamospermia [6]. After vesiculography, there were 12 patients having complete opacification of the distal ejaculatory ducts and partial opacification in 2 patients. Delayed complications in the form of epididymitis were seen in one patient

ABSTRACT

Bladder cancer represents a major health problem not only in Egypt [where bilharziasis is endemic] but also allover the world. Preoperative tumour staging is still of prime importance for both treatment planning and prognostic assessment. Evaluation of the role of computed tomography in the diagnosis and staging of bladder cancer. 58 patients with bladder neoplasm had been subjected to the current study where they all were thoroughly examined for pelvic examination, cystocopic evaluation and biopsy, radiological examination and CT examination for the pelvis. CT could detect accurately cancer bladder in 48/58 of the cases with cancer bladder with high degree of agreement in cases with stage C and D between the CT and pathological staging [29/35, i.e. 82, 9% agreement]. While the stage B showed higher false negative in CT assessment 13/23 [i.e. 56.5% agreement]. Concerning lymph node involvement 14/58 of cases [24.1%] were found to have lymph node metastasis. However, CT could detect only 6/14 [i.e. 42.9%] of these lymph node metastasis which was attributed by the presentation of lymph streaks in the false negative cases. Pelvic CT is a valuable diagnostic tool that can help in diagnosis and management of cases of cancer bladder
