ABSTRACT

Objective To investigate the depressive disorders in patients with respiratory diseases and analyze the countermeasures against it to provide reference for improving clinical treatment compliance. Methods 80 cases was selected according to the screening and exclusion criteria from 262 patients with respiratory diseases from January, 2014 to December, 2015. Then divided them randomly into treatment group and control group. The SDS and SAS show no statistical signiifcance before treatment (P>0.05). The control group with clinical disease symptomatic and supportive treatment, the treatment group combined with anti-anxiety treatment on the basis of clinical symptomatic and supportive group. Then compare the SDS, SAS and dyspnea improvement between the two groups before and after treatment. Results The SDS and SAS scores showed no significant difference before treatment (P>0.05). While after treatment, the SDS and SAS scores in treatment group were signiifcantly lower, and lower than the control group, the difference was statistically signiifcant (P0.05). After treatment, the condition of dyspnea improved greater in treatment group than the control group, the difference was statistically signiifcant (P<0.05). Conclusions Because the respiratory disease of patients suffered leads to psychological burden, negative emotional depression anxiety appears not well with the clinical work, by using Deanxit improve patient confidence and compliance, there is a certain positive to improve the clinical symptoms with good clinical value.

ABSTRACT

Objective To study the effect of chest drainage and injection of pleural adhesion agent in the treatment of the malignant pleural effusion (MPE),and to discuss the influences about it.Methods The data of the in-hospital patients with MPE who received thoracostomic drainage and pleural adhesions were retrospectively analyzed.11 factors were selected as research subjects,such as sex,age,KPS score,tissue origin of primary tumor,the position of hydrothorax,the cell number,the level of protein,the level of LDH in the effusion,species of sclerosant,the fluid volume of hydrothorax in 24 hours before using sclerosant,the duration between pleurodesis and removing the tude.Then,the factors that affected the effect of pleurodesis were analyzed.Results In total 196 cases,61 patients acquired significant efficacy,71 men achieved good result,and 64 people had no effect.The total effective rate was 67.3%.Univariate analysis showed that KPS score,tissue origin of primary tumor,the position of hydrothorax,the level of protein and LDH in the effusion,species of sclerosant,the fluid volumeof hydrothorax in 24 hours before using sclerosant were related to the effect.Multivariate Logistic regression analysis indicated that KPS score,tissue origin of primary tumor,the position of hydrothorax,the level of protein and LDH in the effusion,species of sclerosant,the fluid volume of hydrothorax in 24 hours before using sclerosant were indepent factors for efficacy of the pleurodesis.Conclusion It seems that good adhesion effect achieved in patients with MPE that KPS score ≥70,primary breast carcinoma,using sapylin as a sclerosant,the fluid volume of hydrothorax ≤ 200nmL in 24 hours before using sclerosant.

ABSTRACT

Objective To evaluate the value of transrectal contrast enhanced ultrasound(TR-CEUS) in diagnosis of benign and malignant prostatic neoplasms.Methods Sixty patients with elevated level of serum prostate specific antigen and suspected prostate diseases were examined with transrectal uhrasound(TRUS), and TR-CEUS.The pattern and intensity of CEUS in these patients were observed;and the patients with nodules were examined with CEUS guided biopsy and sextant system biopsy after ultrasound imaging.Time-intensity curves (TIC) were drawn to calculate the parameters, and the difference between benign and malignant nodes was compared.Results All sixty prostate patients were confirmed by pathological examination.Thirty-seven patients belong to benign lesions, among them 15 patients with nodule lesion had total 20 nodules, while 22 cases had benign prostatic hyperplasia.In 23 cases of malignant lesions, 18 cases had centralized nodules and 5 cases showed diffuse pathologic changes.Benign nodes of inner gland showed a main pattern of homogenous enhancement and a clear node zone, whereas, malignant nodes displayed significant enhancement in peripheral tissue.The time to peak and accelerating time (ACT) of malignant nodes were shorter than those of benign nodes (P <0.05).There was no significance in arrival time (AT) between the two groups (P>0.05).The accordance rate of TR-CEUS in the differential diagnosis of benign and malignant prostatic lesions was higher than that of TRUS (P<0.05).In addition, the sensitivity, specificity and accurate rate of TR-CEUS were higher than those of TRUS, whereas, both misdiagnosis and missed diagnosis rate of TR-CEUS were lower than those of TRUS.Conclusions TR-CEUS has clinical value for early discovery of prostatic cancer and has higher application value to differentiate malignant from benign diseases.

ABSTRACT

Objective To evaluate diagnostic value of transrectal gray scale contrast enhanced ultrasound (CEUS) in distinguishing benign from malignant nodes of prostate. Methods Thirty-three patients with 38 prostate nodes underwent real-time transrectal gray scale CEUS with SonoVue and contrast pulsed sequence (CPS). Contrast enhancement pattern of the prostate nodes were recorded and time-intensity curves (TIC) were drawn to calculate the parameters and difference between benign and malignant nodes. Results Twenty benign nodes (17 in inner gland) and 18 malignant ones (14 in external gland) were confirmed by pathology. Compared with normal peripheral zone, malignant lesions showed significantly earlier time to enhancement. The time to peak (TTP), accelerating time (ACT) and peak intensity (PI) of malignant lesions were lower than those of benign lesions (P0.05). Conclusion Of all parameters in CEUS, TTP, ACT and PI are different between prostate benign and malignant lesions, and thus contribute to discriminate prostate cancer from benign diseases.

ABSTRACT

AIM:Mesenchymal cells migrating towards fracture site and differentiating into chondrocytes and osteoblasts is the key process during fracture healing. This article is aimed to observe the changes of the expression of bone morphogenetic protein(BMP)-2 in the early stage of tibia fracture in osteoporotic rats so as to evaluate the reason of delayed fracture healing in osteoporotic rats. METHODS:The experiment was conducted at the Key Laboratory for Oral Biomedical Engineering of Ministry of Education from August 2005 to October 2006. ①Sixty-four 6-month-old female SD rats were randomly divided into ovariectomized(OVX) group and control group with 32 in each group. ②The rats in the OVX group underwent ovariectomy to establish type Ⅰ osteoporotic models. A small quantity of fat tissues was removed in the control group. Three months later,standardized tibia fracture was introduced. ③Eight rats were selected at days 7,14,21 and 28 after operation in the two groups. Some soft tissues around tibia and broken ends of fractured bone,bony callus,cortical bone and medullary canal were collected and treated with histological analysis,immunohistochemical staining,semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR),respectively. RESULTS:①Histological analysis showed membrane bone formation and endochondral bone. A mass of chondrocytes appeared in bony callus of the OVX group at days 21 and 28 after operation. ②The results of immunohistochemical staining showed that mean absorbance(A) of BMP-2 was higher in the control group than the OVX group at days 7 and 14,but lower at day 21. ③The findings of RT-PCR revealed that BMP 2 level was higher in the control group than the OVX group at day 7,but lower at day 14. CONCLUSION:BMP-2 plays a key role in the early phase of bone callus formation during fracture healing. The declined and delayed expression of BMP-2 in osteoporotic rats may be an important reason for delayed fracture healing in osteoporotic rats.
