ABSTRACT

RESUMEN El síndrome de Miescher-Melkerson-Rosenthal es una condición clínica poco frecuente caracterizada por un edema orofacial episódico, paresia facial periférica recurrente, lengua geográfica (lingua plicata) y granulomas en biopsia de piel o labial. Se ha reportado más frecuentemente en mujeres, y su incidencia es probablemente subestimada por el desconocimiento del síndrome, su presentación clínica incompleta y la plétora de diagnósticos diferenciales. Se debe considerar en pacientes que presenten al menos dos de las cuatro características mencionadas. Su tratamiento, con esteroides vía oral, está dirigido a los episodios de edema orofacial y/o paresia facial. A continuación se describen tres casos del síndrome valorados en el servicio de neurología de la Fundación Cardioinfantil y se presenta una breve revisión de la literatura.


SUMMARY Miescher-Melkerson-Rosenthal syndrome is an uncommon clinical condition characterized by episodic orofacial oedema, recurrent peripheral facial paralysis, geographic tongue (lingua plicata) and granulomas in skin or labial biopsy. It is reported more frequently in women, and its incidence is probably underestimated due to lack of knowledge of the syndrome, incomplete clinical presentation and the plethora of differential diagnosis. It should be considered in patients with at least two of the four mentioned characteristics. Its treatment, with oral steroids, is directed at the episodes of orofacial oedema and/or the facial paralysis. We described three cases of the syndrome seen at the Fundacion Cardioinfantil's neurology service and present a short review of the literature.

ABSTRACT

RESUMEN PROPÓSITO: La pandemia mundial por covid-19 ha impuesto nuevos retos para la atención de pacientes con infarto cerebral con oclusión de gran vaso, el tratamiento endovascular con trombectomía mecánica es la elección para estos casos. Las recomendaciones que se presentan buscan mantener la eficacia, la celeridad y la seguridad de la intervención siguiendo las nuevas medidas de bioseguridad. METODOLOGÍA: Revisión no sistemática de la literatura de manejo endovascular para el infarto cerebral en época pre covid-19 y durante la pandemia por covid-19. Los aspectos de la intervención, que a juicio de los autores se modificaron, se presentan en forma de recomendaciones narrativas. RESULTADOS: Se identificaron tres fases en el manejo endovascular del infarto cerebral: preintervención, intervención y postintervención. En la primera se incluyen los temas de selección del candidato, la tamización para covid-19, el consentimiento informado, la notificación-preparación del equipo y la modalidad anestésica. En las otras dos fases se hacen recomendaciones generales. Por último, se identifican puntos importantes para la toma de decisiones en el manejo endovascular del infarto cerebral. CONCLUSIÓN: Las recomendaciones pretenden facilitar la adaptación de los protocolos, evitar retrasos y mantener la seguridad de la intervención endovascular en pacientes con infarto cerebral con oclusión de gran vaso candidatos a trombectomía mecánica en época de covid-19.


SUMMARY PURPOSE: The global pandemic of COVID-19 has imposed new challenges for the attention of patients with ischemic stroke and large vessel occlusion. Endovascular treatment with mechanical thrombectomy is the therapy of choice for these patients. The recommendations intend to maintain the efficacy, times and safety of the intervention following the new biosafety measures. METHODOLOGY: A non-systematic review of the literature on endovascular management for ischemic stroke in the pre-COVID-19 period and during the COVID-19 pandemic. The aspects of the intervention, modified in the author's opinion, are presented in the form of narrative recommendations. RESULTS: Three phases were identified in the endovascular management for cerebral infarction: Pre-intervention, intervention, and post-intervention. The first one includes candidate selection, screening for COVID-19, informed consent, notification-preparation of the team/suite and anesthetic modality. For the other two phases general recommendations were developed. Finally, we identified key points for decision-making in the endovascular management of ischemic stroke. CONCLUSION: These recommendations pretend to facilitate adaptations of protocols, avoid delays and maintain the safety of endovascular intervention in patients with ischemic stroke with large vessel occlusion candidates for mechanical thrombectomy during COVID-19 pandemic.

Introducción: La Infertilidad ha mostrado un aumento progresivo de su prevalencia. Se estima que alcanza un 15 porciento de las parejas, con un 35 porciento a 40 porciento de componente masculino y un 20 a 30 porciento afectando a ambos miembros. El objetivo de este estudio es la descripción de las características de los hombres que consultan por infertilidad a nivel local así como los resultados de sus estudios iniciales.Material y métodos. Estudio transversal descriptivo en donde se revisaron fichas de 500 pacientes de sexo masculino que consultaron por infertilidad entre enero de 2006 y junio de 2016. De cada paciente se registró edad, paridad previa, frecuencia sexual, factores de riesgo como antecedentes de criptorquidea, varicocele, cáncer, entre otros. Además se registraron los resultados de estudios realizados. Los datos fueron analizados con el sistema estadístico STATA 13.0.Resultados. La mediana de edad fue de 34 años (15-68 años) con un 70,29 porciento de infertilidad primaria. Respecto al tiempo de exposición la mediana fue de 7 meses (3-31) y el promedio de frecuencia sexual fue de 2,5 veces por semana (0-7). En un 48,69 porciento se encontró un examen genital alterado, un 9,32 porciento tuvo un desarrollo puberal alterado, y el antecedente de criptorquidea y varicocele estuvo presente en un 15 porciento y 23,2 porciento respectivamente. El 18,4 porciento presentó alguna disfunción sexual, el 46 porciento era tabáquico y un 8,77 porciento consumía marihuana. Respecto al estudio se observaron 2 casos de microdeleciones del cromosoma Y, y 2 casos de cariogramas alterados. Se rescataron 254 espermiogramas con un 67,32 porciento de azospermicos, de los cuales un 14,62 porciento fueron obstructivos.Conclusión. El componente masculino en la infertilidad requiere de una evaluación anamnésica prolija debido a la variedad de factores de riesgo involucrados, siendo importante la pesquisa de condiciones que puedan intervenirse precozmente para así lograr mayores tasas de éxito en la recuperación de espermatozoides y posterior técnica de fertilidad. Existen escasos datos epidemiológicos de infertilidad en Chile, por lo que este trabajo nos permite conocer el perfil de pacientes que consultan por esta patología a nivel local.(AU)

Regarding the exposure time the median was 7 months (3-31) and the average sexual frequency was 2.5 times per week (0-7). In 48.69 pertcent an altered genital examination was found, 9.32 pertcent had an altered pubertal development, and the antecedent of cryptoquidea and varicocele was present in 15 pertcent and 23.2 pertcent respectively. 18.4 pertcent had sexual dysfunction, 46 pertcent were smokers and 8.77 pertcent used marijuana. Regarding the exams, 2 cases of microdeletions of the Y chromosome were observed, and 2 cases of Klinefelter Syndrome. 254 spermiograms were rescued with 67.32 pertcent of azoospermia, of which 14.62 pertcent were obstructive.Conclusion. The male component in infertility requires a complete anamnestic evaluation due to the variety of risk factors involved, being important the research of conditions that can intervene early to achieve higher rates of success in sperm retrieval and subsequent fertility technique. There are few epidemiological data on infertility in Chile, so this work allows us to know the profile of patients who consult for this pathology at the local population.(AU)

ABSTRACT

Objetivo: revisar las manifestaciones de la neurofibromatosis de presentación en la región genital, el diagnóstico diferencial y el manejo de las lesiones.Materiales y métodos: paciente de 63 años remitida por masa en región genital a la Unidad de Colposcopia y Patología del Tracto Genital Inferior del Hospital de Suba, institución pública de segundo nivel de complejidad ubicada en Bogotá, Colombia, donde se atiende a población del régimen subsidiado. Al examen físico se encontraron manchas café-au-lait y lesiones compatibles con neurofibromas que orientaron el diagnóstico de neurofibromatosis tipo 1. Se realizó una búsqueda sistemática de la literatura en Medline-Pubmed usando los términos MeSH neurofibromatosis, enfermedad de von Recklinghausen, vagina, tracto genital, tumor pélvico; se presenta el caso clínico y la revisión del tema.Resultados: se encontraron 7991 publicaciones relacionadas con neurofibromatosis, y de estas solo 30 relacionadas con lesiones pélvicas y de tracto genitourinario; solo 6 reportes de casos y una serie de casos presentaban lesiones localizadas en tracto genital inferior. Para la descripción de las generalidades y los criterios diagnósticos se seleccionaron 12 artículos de revisión del tema. El diagnóstico diferencial incluye schwannoma, neuroma, mixoma, leiomioma y pólipo fibroepitelial.Conclusiones: el hallazgo de neurofibromas en el tracto genitourinario es un evento de baja ocurrencia y es un tema de interés para ginecólogos, pediatras y urólogos pues constituye un reto diagnóstico.


Objective: Reviewing manifestations of neurofibromatosis presenting in the genital region, their differential diagnosis and managing lesions. Materials and methods: The case of a 63-year-old patient admitted to the Suba hospital’s colposcopy and pathology of the lower genital tract service due to having a mass in the genital region is presented; the hospital is a second level complexity public institution in Bogotá, Colombia, dealing with a subsidised regime population. Physical examination revealed café-au-lait spots and lesions compatible with neurofibroma, thereby orientating diagnosis towards neurofibromatosis type 1. A systematic search was made of the literature in Medline-Pubmed, using the MeSH terms: neurofibromatosis, von Recklinghausen’s disease, vagina, genital tract, pelvic tumour. A clinical case and review of the topic are presented. Results: 7,991 publications related to neurofibromatosis were found. Just 30 of them were related to pelvic lesions and the genitourinary tract and only 6 case reports and one case series dealt with lesions located in the lower genital tract; 12 articles reviewing the topic were selected for a description of the diagnostic generalities and criteria. Differential diagnosis included schwannoma, neuroma, myxoma, leiomyoma and fibroepithelial polyp. Conclusions: A neurofibroma being identified in the genitourinary tract is an infrequently occurring event, constituting a topic of interest for gynaecologists, paediatricians and urologists as it represents a diagnostic challenge.

ABSTRACT

It has been accepted that neuroinflammation, oxidative stress and glial activation are involved in the central sensitization underlying neuropathic and inflammatory pain. Vimang® is the brand name of an aqueous extract of Mangifera indica L., Anacardiaceae, traditionally used in Cuba for its antioxidant, antiinflammatory, analgesic, and immunomodulatory properties. In the present study, we determined the possible effects of Vimang formulations in acute herpes zoster (n=12) patients, that received a daily dose of 1800 mg of extract (two coated Vimang tablets, 300 mg each, three times daily before meals) associated to low doses of amitriptyline (10-25 mg/d). In addition to the tablets, they utilized compresses containing Vimang dissolution at 2 percent on skin lesions for thirty days. The average daily pain score using a Likert scale and variations in concomitant drug daily dosage were determined. The analgesic effect was observed from week 1 (p<0.001) with respect to baseline data and none showed post-herpetic neuralgia. Significant reduction of antidepressant medication (p<0.01) and analgesic rescue dosages (p=0.0035) with respect to the initial daily dosage were showed. No adverse events were reported. The results obtained in this report of cases suggest that Vimang supplementation might be beneficial to prevent and treat neuropathic pain.

ABSTRACT

Horse radish peroxidase (HRP) from Armoracia rusticana catalyses the oxidation of (R)-limonene into the oxidized derivatives carveol and carvone. This study compares the direct addition (DA) of hydrogen peroxide with its continuous electrogeneration (EG) during the enzymatic oxidation of (R)-limonene. Reaction mixtures containing HRP, (R)-limonene as substrate, and hydrogen peroxide, added directly or electrogenerated, in 100 mM sodium-potassium phosphate buffer pH 7.0, at 25ºC were studied. Two electrochemical systems for the hydrogen peroxide electrogeneration were evaluated, both containing as auxiliary electrode (AE) a platinum wire and saturated calomel electrode (SCE) as reference. Reticulated vitreous carbon foam (RVCF) and an electrolytic copper web (CW) were evaluated as working electrodes (WE). Results were compared in terms of hydrogen peroxide electrogeneration, (R)-limonene residual concentration or conversion and product selectivity. Best results in terms of maximum H2O2 concentration (1.2 mM) were obtained using the CW electrode at -620 mV SCE, and continuous aeration. Use of the EG system under optimized conditions, which included the use of acetone (30% v/v) as a cosolvent in a 3 hrs enzymatic reaction, lead to a 45% conversion of (R)-limonene into carveol and carvone (2:1). In comparison to the results obtained with DA, the use of EG also improved the half-life of the enzyme.

Se presenta un caso de síndrome de Zollinger Ellison, cuya manifestación principal fue diarrea crónica, sin datos de enfermedad ácido péptica. El objetivo del presente trabajo es informar una expresión clínica poco usual en una paciente con gastrinoma; así como los métodos empleados (determinación de gastrina, estimulación secretina y angiotomografía) para la cofirmación del diagnóstico y su manejo terapéutico quirúrgico con resección del tumor.
