ABSTRACT

Moderate to severe unilateral ptosis has previously been treated with levator resection or frontalis sling and cosmetic double fold in the opposite eyelid. This method often resulted in undesirable asymmetrical lid lag phenomenon in down gaze. From December 1991 to December 1994, we treated 65 cases of unilateral ptosis with frontalis sling or levator resection, and simultaneously cosmetic frontalis sling have been performed in the unaffected normal eyes of 41 patients and cosmetic double fold have been performed in the unaffected normal eyes of 11 patients. After 6 months of operation, authors survey the patients content. The number of cases of content were 4(36.36%) in 11 cosmetic double fold group and 31(75.6%) in 41 cosmetic frontalis sling group. The content after cosmetic frontalis sling was higher than cosmetic double fold. Asymmetrical lid lag phenomenon on downward gaze which is inevitable complication postoperatively was reduced by means of cosmetic frontalis sling of unafeected normal eye in unilatral ptosis. The authors expect that the patient will be more satisfied with the result of symmetrical lid lag in down gaze after cosmetic frontalis sling of unaffected normal eye in aesthetic aspect.

ABSTRACT

The surgical treatments of blepharoptosis by resection of the levator palpebrae superioris(LPS) muscle and levator aponeurotic repair, Fasanella-Servat procedure have occasionally complication like as overcorrection, especially in acquired cases. The recession of levator palpebrae superioris(LPS) muscle (anterior recession and posterior tenotomy) and/or marginal myotomy had been performed on 6 overcorrected blepharoptotic patients from November, 1987 to July, 1990 in Keon Yang medical foundation and the results were satisfactory.

ABSTRACT

To evaluate the clinical usefulness of new handheld air impulse tonometer(Pulsair, Keeler), the intraocular pressures of 147 eyes of 80 men and women at the age range of 9 to 82 year without corneal opacity or corneal edema were measured with the Goldmann applanation tonometer as well as noncontact air impulse tonometer. The IOPs measured with both tonometer were compared to evaluate the correlation coefficiency. The correlation coefficiency between two instruments is 0.89. Therefore, the new type of noncontact tonometer could be useful for clinical applications due to its accuracy, simplicity, painlessness, portability, and repeatability-especially at glaucoma screening programs.

ABSTRACT

Thirty cases of lacerated canaliculi were repaired by bicanalicular silicone intubation(Pilling(R) Intubation Set)with use of a silicone sponge for nasal fixation. The results were as follows: Canalicular laceration occurred mainly in males(25 cases), and in young age group(10-39yrs). An isolated inferior canalicular laceration was the most common and occurred in 23 cases. There was no significant difference in laterality between right and left. The average follow up period was 13.8 months and final results were better in patients who were repaired earlier and had a longer intubation time. Postoperative complications occurred in 7 cases and all of them were canalicular slits. Canalicular slits were not related to the duration of intubation and did not affect the final results significantly. The advantages of this operation were good cosmetic results, minimal irritation by tube, and easy removal of tube.

ABSTRACT

The author evaluated 63 patients(95 eyes) of congenital blepharoptosis whowere operated by frontalis sling and external levator resection at the Keon Yang Medical Foundation Kim's Eye Clinic. To obtain the best results in blepharoptosis, it is necessary to evaluate degree of levator function, amount of ptosis and its etiology. The results are as follows: 1. The majority(78%) were operated at the age between the first and the second decade. 2. Levator function of congenital ptosis was poor(under 4mm) in 84 eyes and fair(5~7mm) in 11 eyes. 3. The author obtained good surgical result in 20 eyes(80%) by frontalis sling and in 35 eyes(92%) by external levator resection. 4. Most common complication was undercorrection(8 cases). 5. External levator resection is the primary choice of operation for ptosis with poor levator function(3~4mm).

ABSTRACT

There has been many ophthalmologists warning sudden loss of central vision and small residual visual fields in far advanced glaucomatous damaged eyes after filtration surgery. Although this warning against glaucoma surgery in patients with limited visual fields has been a concern for over a century, little data exist to convincingly support this belief. The purpose of this paper is to share our experience in facing this dilemma. Authors performed filtration surgery on 31 eyes with advanced glaucomatous field defects and evaluated postoperative visual field and visual acuity at 8 weeks postoperatively in order to ensure if there was any change. The eyes were classified into three categories according to the severity of the visual field loss evaluated with Humphrey computerized visual field analyser. No patients suffered a sudden loss of central visual acuity or a rapidly progressive loss of small residual visual fields after filtration surgery during 8 weeks follow-up period. In the light of our clinical experiences, authers concluded that there was nothing to worry about in performing filtration surgery on far advanced glaucomatous damaged eyes just only because they have small residual visual fields.

Argon laser iridotomy is at present the procedure of choice in Korea for the treatment of pupillarx block and angle closure glaucoma. However, despite modified argon laser techniques, difficulties in penetration of irides and closure of hole have been widely reported. Recently reported Q-switched Nd-YAG laser iridotomy have a many complications such as bleeding, lenticular opacity and small hole. And so argon laser and Nd-YAG laser combined iridotomy was performed on 80 eyes, 57 patients with angle closure glaucoma. The follow up period ranged from 1 to 14 months. 71(88.7%) of 80 eyes treated required a single lasing session for patency, and six eyes required two sessions and three eyes required three sessions. Closure of the iridotomy site occurred in 6.2% of eyes, an incidence remarkably lower than that of argon laser iridotomy alone. Localized lenticular opacity and bleeding were not observed, but immediate postoperative iritis and elevation of intraocular pressure were observed.

ABSTRACT

Pheochromocytoma-arises in the adrenal proper or in paraganglions, Occasionally produce Hypertension, Hyperglycemia and Hypermetabolism, A case of typical functioning pheochromocytoma associated with Hypertensive Retinopathy is reported because of its rarity. A 17 year old student was admitted to the Ewha Womans University Hospital with Hypertension (200/140). Headache, palpitation and blurring' vision. On admission. the fundoscopica-l findings were arteriolar narrowing, Exudates, Hemorrhages and macular star in both fund i and visual acuity were 20/200 (N. C.) in each eye. The patient's general and Ocular changes returned to nearly normal level after surgical removal of the tumor from the kidney.

ABSTRACT

The normal intraocular pressure which are known to us are the standards of ocular pressure obtained by testing foreigners. According to the reports, the scope of normal I.O.P. is 15-25 mmHg. However, in the case of our neighboring Japanese the scope of normal I.O.P. is 10-15 mmHg and its average is 18.11 mmHg by Schiotz's tonometer. In order to study the I.O.P. of Korean, the writers of this report have tested 405 normal persons(810 eyes) and herewith report the results obtained from the tests. Conclusion: 1. The I.O.P. of 405 normal Koreans is as follows. Reliability range: 12-19 mmHg, Average Value: 15.5 mmHg. 2. As for the diurnal variation, the minimum mean of 10.8 mmHg was found at 1.00 P.M. and the maximum mean of 16.5 mmHg was found at 7.00 A.M. the standard error in each was 0.28 and 0.50 respectively and its difference was 4.5. 3. No difference was found by age or sex.
