ABSTRACT

Objective:To investigate the risk factors of acute gastrointestinal failure in patients with traumatic brain injury(TBI).Methods:A retrospective analysis of 297 patients admitted to the Rugao Hospital Affiliated to Nantong University for traumatic brain injury from March 2018 to March 2020 was performed. Multivariate Logistic regression analysis was used to test the risk factors of acute gastrointestinal failure in patients with traumatic brain injury.Results:Of the 297 patients with TBI, 92 (30.98%) had acute gastrointestinal failure, of which 56 (18.86%) were acute gastrointestinal injury(AGI) Ⅲ and 36 (12.12%) were AGI Ⅳ. Multivariate Logistic regression analysis showed that female, low Glasgow Coma Scale (GCS) scores, frontal lobe injury, abnormal serum sodium, lung infection and intracranial infection were independent risk factors for acute gastrointestinal failure in TBI patients ( P<0.05). The length of ICU stay in the acute gastrointestinal failure group was significantly higher than that of the non-acute gastrointestinal failure group: (18.5 ± 2.6) d vs. (6.3 ± 1.2) d, and the incidence of good prognosis was significantly lower than that of the non-acute gastrointestinal failure group: 60.87%(56/92) vs. 80.49(165/205), the differences were statistically significant ( P<0.05). Conclusions:That female, low GCS scores, frontal lobe injury, abnormal serum sodium, lung infection and intracranial infection are independent risk factors for acute gastrointestinal failure in TBI patients

ABSTRACT

Objective:To investigate the efficacy of pixel 2 940 nm erbium laser combined with vitamin B12 in the treatment of acne sunken scar.Methods:From July 2017 to June 2019, 96 patients with acne sunken scar were treated in the Department of Burn and Plastic Surgery of the Second People's Hospital of Jiaozuo City, Henan Province. They were randomly divided into two groups with 48 cases in each group. The control group was treated with routine therapy and the observation group was treated with pixel 2 940 nm erbium laser combined with vitamin B12. The therapeutic effects, acne scar weight score (ECCA), skin condition and adverse reactions were compared between the two groups.Results:The total effective rate of the control group was 77.08% (37/48), which was lower than that of the observation group 91.67% (44/48) ( P<0.05); The incidence of adverse reactions, pore and amethyst in ECCA score of the control group were (18.14±9.33), 18.75% (9/48), (32.25±8.41) and (18.24±5.12), which were higher than those of the observation group (26.45±12.60), 4.17% (2/48), (28.24±7.52) and (15.63±4.34) ( P<0.05), respectively. Conclusions:Pixel 2 940 nm erbium laser combined with vitamin B12 is effective in the treatment of acne sunken scar, with less adverse reactions and high safety. It is worthy of clinical application.

ABSTRACT

Objective To explore the influence of dexmedetomidine on blood gas and inflammation cytokines in patients with one-lung ventilation (OLV).Methods Sixty-two patients undergoing radical resection of pulmonary carcinoma were divided into observation group and control group in sequence,31 cases in each group.All patients treated with intravenous sufentanil 0.3-0.5 μg/kg,propofol 1.5-2.5 mg/kg,midazolam 0.5 mg/kg and rocuronium 0.9 mg/kg for induction of anesthesia;intravenous infusion of propofol 6-8 mg·kg-1 ·h-1 and remifentanil 0.1-0.5 μg·kg-1 ·min-1,intermittent intravenous injection of cisatracurium for anesthesia maintenance.The patients in the observation group were given 1μg/kg loading dose dexmedetomidine 10 min before anesthesia induction (infusion in 10 min),maintaining at 0.5 μg·kg-1 ·h-1 until 30 min before the end of surgery.The patients in the control group were given the same amount of saline as a control.The blood gas analysis index,pH,partial oxygen pressure (PaO2),CO2 partial pressure (PaCO2) and such inflammation cytokines as tumor necrosis factor alpha (TNF-α) and interleukin 6 (IL-6) at OLV (T0),after OLVlh (T1),the end of OLV (T2) were respectively tested,the dosage of propofol in the two groups were recorded.Results The dosage of propofol in observation group was significantly lower than that in the control group (P<0.05);pH declined in the two groups (P<0.05),but the difference had no statistical significance;PaO2 was rising then declined in the two groups (P<0.05),but it was significantly higher in the observation group than in the control group(P<0.05);PaCO2 in the two groups increased gradually(P<0.05),it was significantly lower in the observation group than in the control group (P<0.05);The serum TNF-α,IL-6 in the two groups showed a trend of increasing (P<0.05),it was significantly lower in the observation group than in the control group (P<0.05).Conclusion Dexmedetomidine can maintain blood gas stability and reduce inflammation caused by single lung ventilation.

ABSTRACT

Objective To observe the effect of sling exercise therapy combined with acupuncture at Jiaji acupoints on balance function of stroke patients with hemiplegia. Methods From October, 2013 to October, 2015, 40 hemiplegic stroke patients with balance dysfunction were randomized to control group and treatment group equally. Both groups accepted routine rehabilitation, while the treatment group com-bined with sling exercise therapy and acupuncture at Jiaji acupoints. They were assessed with Berg Balance Scale (BBS), 10-metre maxi-mum walking speed (10MWS), Fugl-Meyer Assessment of lower limbs (FMA-L) and modified Barthel Index (MBI) before and four weeks after treatment. Results The scores of BBS, FMA-L and MBI, and 10MWS improved in both groups (t>2.249, P2.954, P<0.01). Conclusion Sling exercise therapy combined with acupuncture at Jiaji acupoints can further improve the function of balance, walking and activities of daily living in stroke patients with hemiplegia.

ABSTRACT

Objective To investigate the effects of angiotensin Ⅱ (Ang Ⅱ) or high glucose on the toll-like receptor 4 (TLR4) expression,inflammatory cytokines and fibrotic factors in human tubular epithelial cells (HK-2),revealing the innate immune-related pathogenesis of diabetic nephropathy (DN) which may have clinical implications.Methods Three TLR4 siRNA sequences were designed and synthetized.After transfection,the most effective siRNA was selected to use for further expriments.The experiment consisted of 2 parts.Part 1:Cells were divided into three groups:normal-glucose group (NG,5.5mmol/L glucose),mannose group (M,5.5 mmol/L glucose + 19.5 mmol/L mannose),High-glucose group (HG,25 mmol/L glucose),preliminary validated the effects of high glucose and high osmotic pressure.Part 2:Cells were divided into seven groups:NG group,HG group,Ang Ⅱ group,Ang Ⅱ + negative group,HG+ negative group,Ang Ⅱ + siRNA group and HG+ siRNA group.Real time PCR was used to analyze the mRNA expression of TLR4,myeloid differentiation factor 88 (MyD88),heat shock protein 47 (HSP47).Western blotting was used to observe the protein expression of TLR4,MyD88,HSP47,NF-κB,type Ⅳ collagen (ColⅣ).ELISA was used to detect the expression of monocyte chemotactic protein-1 (MCP-1) and interleukin-6 (IL-6).Results Compared with NG group,TLR4,MyD88,HSP47 mRNA and TLR4,MyD88,NF-κB,ColⅣ,HSP47 protein were highly expressed under high glucose or Ang Ⅱconditions (P < 0.01),and the expression levels of MCP-1 and IL-6 also increased significantly (P < 0.01).Compared with HG or Ang Ⅱ group,the above indicators were obviously inhibited in the TLR4 siRNA groups (P<0.01).Comparison between blank vector transfected groups and HG group as well as Ang Ⅱ group indicated no statistic significance (P > 0.05).Conclusions Both Ang Ⅱ and high glucose stimulate TLR4 expression,which result in the up-regulation of inflammatory and fibrotic factors in HK-2.Specific target of TLR4 gene silencing can block the TLR4 pathway that is activated by high glucose and Ang Ⅱ,and thus reduce the inflammatory and fibtogenic factors' release.TLR4 signal is the common innate immune response pathway which induces the release of inflammatory and fibrotic factors in HK-2 under high glucose or high angiotension conditions.

ABSTRACT

Objective To preliminary screen serum microRNA which was related to class Ⅳ and class Ⅳ+Ⅴ lupus nephritis (LN)by using microRNA array technology,and explore the role of serum microRNA in renal pathological classification of lupus nephritis.Methods Eight serum samples of lupus nephritis patients from biological specimen library in our hospital were enrolled. Experimental group was composed of 4 class Ⅳ cases and 4 class Ⅳ+Ⅴ cases,control group was composed of 4 healthy persons. MicroRNA array was used to screen the differentially expressing microRNA.Results There were part of same miRNA in class Ⅳand classⅣ+Ⅴ LN,among which expression of 22 miRNA were significantly different,29 miRNA of classⅣ was unique,3 miRNA of class Ⅳ+Ⅴ was unique.Conclusion Serum miRNA has a potential to serve as a biomarker to identify class Ⅳ and classⅣ+Ⅴlupus nephritis.

ObjectiveTostudytheroleof hotshockprotein (HSP)47in tubulointerstitial fibrosis induced by transforming growth factor β1(TGF-β1),and to explore its possible mechanism.Methods Human proximal tubular epithelial cells(HK-2) were divided into threegroups:control,TGF-β1andHSP47siRNA. Theexpressionsof HSP47, collagenⅣ,fibronectin(FN),plasminogen activator inhibitor 1(PAI-1) mRNA and HSP47,collagen Ⅳ,FN protein were detected by RT-PCR and Western blotting respectively.PAl-1 protein was detected by ELISA. ResultsHK-2expressedHSP47innormalmedium. ThemRNAandprotein expressions of HSP47 up-regulated in concentration- and time-dependent manner in HK-2 cells induced with increasing concentrations of TGF-β1(0,2.5,5,10 μg/L) and with prolong times (12,24,48 h),and peaked at 10 μg/L TGF-β1 for 48 h.Similar phenomena was observed in the mRNA andproteinexpressionsof collagenⅣ, FN, PAI-1inHK-2 cellsinducedbyincreasing concentrations of TGF-β1 (0,2.5,5,10 μg/L) at different time points (12,24,48 h),and peaked at 10 μg/L TGF-β1 for 48 h.HSP47 siRNA could significantly reduce the up-regulation of mRNA and protein expressions of HSP47,collagen Ⅳ,FN,PAI-1 in HK-2 cells induced by TGF-β1.Conclusion HSP47 can promote renal tubulointerstitial fibrosis maybe through the regulation of the expressions of collagen Ⅳ,FN,PAI-1.

ABSTRACT

Objective To test the effect of Undaria pinnatifida soluble dietary fiber on endothelial function in hyperlipidemic rats. Method Forty rats were divided into 4 groups(n=10) :control group,hyperlipidemic model group,low dose dietary fiber-treated group(5%) ,high dose dietary fiber-treated group(10%) . After treatment for 8 w. Endothelium-dependent vasorelaxation in isolated aortic rings,the content of plasma malondialdohyde(MDA) ,nitric oxide(NO) and endothelin-1(ET-1) were determined. The protein expression of endothelial NO synthase(eNOS) was measured by Western blotting. Results Undaria pinnatifida soluble dietary fiber treatment significantly decreased MDA and ET-1 level. It also significantly improved endothelial function and plasma NO level concomitantly with unregulation of the expression of eNOS protein. Conclusion In hyperlipidemic rats Undaria pinnatifida soluble dietary fiber could improve vascular endotheliual function,which might be explained by its action to decreasing plasma ET-1 level and increased NO production.
