ABSTRACT

Objective To evaluate the result of laparoscopic pancreaticoduodenectomy (LPD).Methods We retrospectively analyzed the perioperative clinical data of 111 consecutive LPD procedures performed from Jan 2015 to Aug 2018 at Department of General Surgery,the Affiliated Hospital of Xuzhou Medical University.Results Of the 111 patients undergoing laparoscopic procedure,24 patients (21.6%) had abdominal surgery history.The mean operative time was (360.9 ± 108.9)minutes.The mean blood loss was (270.9 ± 184.9)m1.The mean operative time of pancreatojejunostomy and choledochojejunostomy was (46.2 ± 11.6) minutes and (19.0±7.2) minutes,respectively.The mean postoperative hospital stay was (14.9 ± 4.7) days.The rate of pancreatic fistula grade B was 6.3% and pancreatic fistula grade A was 20.7%.The rate of bile leakage was 1.8%.Second surgical operation was necessary for 5 cases (4.5%),delayed postoperative hemorrhage occurred in 2 cases (1.8%).The mean number of lymph nodes harvested was(14.2 ± 7.9) and the positive rate number was (0.9 ± 1.8).Conclusions LPD improves the operation quality,reduces intraoperative hemorrhage and accelerates the postoperative recovery.

ABSTRACT

Objective To observe the effects of Zhennao-Ning capsule on cerebral microcirculation.Methods Cerebral hypoperfusion model of mices was induced by right common carotid arteries ligation method,The cerebral blood flow for 5min was recorded under normal circumstances comand for 30min when modeled,and then calculate the rate of blood flow changes.Microcirculatory impairment model of rats was induced by intravenous injection of high molecular dextran(HMD).The time of the emergence of microcirculatory impairment was recorded.Vascular flow,blood flow pattern and the number of outlets in the same range were recorded.Results The blood flow was significant decrease in the cerebral hypoperfusion model mice,when compared with the control mice.Zhennao-Ning capsules at the dosage of 1500 mg/kg、750 mg/kg treatment could improve the cerebral blood flow of rats at 10,20,and 30 min after modeled (56%,51%,50% and 42%,49%,51%,respectively,P<0.01).Zhennao-Ning capsules at the dosage of 600 mg/kg,300 mg/kg could significantly reduce the change of blood flow pattern(P<0.01),improve vascular flow rate (26%,36%,P<0.05 or P<0.01),extend the time of the emergence in the microcirculatory impairment model rat (53%,34%,P<0.01).Conclusion Zhcnnao-Ning capsules could improve microcirculatory impairment and partly cure the clinical migraine disease.

ABSTRACT

OBJECTIVE:To study the protective effects of Shenqi fuzheng injection on ischemia/reperfusion injury of the lungs of dogs following cardiopulmonary bypass(CPB).METHODS:The CPB dog model was induced,then the model dogs we-re randomly divided into three groups:control group,Shenqi fuzheng injection high dosage group,and Shenqi fuzheng injection low dosage group. The lung tissue samples of 1.0 cm3 from the same lobes of lung were obtained at 10 min before CPB and 60 min and 120 min after reperfusion,respectively for determination of the contents of superoxide dismutase(SOD),malondialdehyde (MDA),wet / dry ratio of lung (W/D) etc. The femoral arterial blood gas analysis was performed in two groups at 30 min and 120 min after reperfusion for calculation of oxygenation index(OI) and respiratory index(RI).The lung tissue samples(1.0 cm3) from the same lobes of lung were obtained at 120 min after reperfusion for HE staining and observation of pathological changes under light microscope. RESULTS:In the control group,OI were significantly decreased but RI were sig-nificantly increased at 120 min after reperfusion as compared with at 30 min after reperfusion; in Shenqi fuzheng injection high dosage group at 30 min and 120 min,OI was significantly higher but RI was significantly lower than in control group. In the control group at 60,120 min after reperfusion,the SOD content were decreased significantly,MDA content and W/D ratio in lung tissues were increased significantly compared with at 10 min before CPB; in Shenqi fuzheng injection high dosage group at 60 min and 120 min,MDA content and W/D ratio were significantly lower but SOD content was significantly higher than in the control group.The pathological changes at 120 min after reperfusion showed that there were severe leukocyte infiltration and pulmonary edema in the control group,but the pathological changes in the Shenqi fuzheng injection high dosage group were milder than in the control group. CONCLUSION:Administration of Shenqi fuzheng injection in dog during CPB has beneficial protective effects against I/R lung injury,and the effect may be associated to its function in alleviating the damage induced by oxygen free radicals in lung tissues.

ABSTRACT

Objective:To evaluate the recovery effect of early enteral nutrition (EEN) in elderly patients with severe chest trauma.Methods:83 elderly patients with severe chest trauma were divided randomly into two groups,research group(46 patients) and control group(37 patients).In both groups the nutrition indicator,the complication and the recovery time were detected on admission (T1),the 1st week (T2) and 2ed week (T3) after admission.Results:After 2 weeks,the level of nutrition indicator in research group was significantly higher than in control group,and the incidence of complication was significantly lower than in control group.Conclusion:Early enteral nutritional support is helpful for the nutritional support and reduce the support of complication in elderly patients with severe chest trauma.
