ABSTRACT

Objective To investigate the clinical effect of perioperative electroacupuncture intervention on postoperative delirium (POD) in hip replacement surgery patients.Method One hundred and twenty-eight patients who would undergo hip replacement surgery were randomly allocated to treatment and control groups, 64 cases each. The treatment group received perioperative electroacupuncture and the control group, routine preoperative preparation and postoperative treatment. Serum S100β protein concentration was measured in the two groups of patients at one day before, the end of and one day after surgery. The incidences of POD were compared between the two groups at one, two and three days after surgery.Result There was a statistically significant difference in the incidence of POD between at one day after surgery and recovery from anesthesia in the two groups (P<0.01). In the treatment group, the incidence of POD was significantly lower at two and three days than at one day after surgery (bothP<0.05) and at three days than at two days after surgery (bothP<0.05). There was a statistically significant difference in the incidence of POD between the treatment and control groups at different time points (recovery from anesthesia, and one, two and three days after surgery) (P<0.01). In the treatment group, there was a statistically significant difference in S100β protein concentration between at one day after and the end of surgery (P<0.05). There was a statistically significant difference in S100β protein concentration between the treatment and control groups at the end of and one day after surgery (P<0.05).Conclusions Perioperative electroacupuncture treatment can effectively decrease postoperative incidence of delirium and its duration in hip replacement surgery patients.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical efficacy of laparoscopic ventriculoperitoneal shunt with temporary external drainage in the treatment of hydrocephalus.</p><p><b>METHODS</b>Fifty-two cases of hydrocephalus randomized into two groups to receive laparoscopic assisted ventriculoperitoneal shunt with temporary external drainage (19 male and 7 female patients) and conventional ventriculoperitoneal shunt (20 male and 6 female patients). The catheterization time in the abdominal cavity, release time of intracranial hypertension, average hospital stay, postoperative pains, and postoperative complications were compared between the two groups.</p><p><b>RESULTS</b>Laparoscopic ventriculoperitoneal shunt with temporary external drainage was performed successfully in all the cases without intraoperative conversion to open surgery. Compared with the conventional ventriculoperitoneal shunt, laparoscopic ventriculoperitoneal shunt with temporary external drainage was associated with significantly shortened catheterization time in the abdominal cavity, release time of intracranial hypertension, and average hospital stay (P<0.01) as well as lowered postoperative pain score at 4, 8, 16, and 24 h after the operation. The pain scores at 48 and 72 h postoperatively were comparable between the two groups. During the follow-up 3 months, the patients receiving laparoscopic ventriculoperitoneal shunt were found to have significantly lower rates of peritoneal end obstruction and abdominal cavity infection than those having conventional shunt (3.8% vs 19.2%, P<0.01; 1.0% vs 23.1%, P<0.01).</p><p><b>CONCLUSION</b>Laparoscopic ventriculoperitoneal shunt with temporary external drainage is feasible and produces better clinical therapeutic effect for management of hydrocephalus.</p>

ABSTRACT

Objective To induce and identify the differentiation of rat bone marrow mesenchymal stem cells(MSCs)into cardiomyocytes in vitro, and observe the expression of Nesprin protein during the differentiation. Methods Rat MSCs were isolated and purified by Ficoll density gradient centrifugation, and adhered for serial subcultivation.Surface-associated antigens of MSCs of the second passage were dedected by flow cytometry.MSCs of the second passage were induced by 10μmol/L 5-azacytidine(5-Aza)to differentiate into cardiomyocytes, and the morphological changes were observed. The expression of Desmin,α-sarcomeric actin and cardiac Troponin I(cTnI) mRNA and protein was detected by RT-PCR,immunocytochemistIv and immunofluorescence staining, and the expression of Nesprin protein was detected by Western blotting. Results The morphology of MSCs induced by 5-Aza was bigger and longer, and the nuclei became bigger, exhibiting more consistent patterns. The expression of Desmin,α-sarcomeric actin and cTnI mRNA and protein of MSCs was positive.Immunofluorescence staining revealed that Nesprin protein positioned in the nuclear membrane, and Western blotting detection demonstrated that the expression of Nesprin protein significantly increased after differentiation(P<0.05).Conclusion MSCs may be successfully induced to differentiate into cardiomyocytes. The expression of Nesprin protein in the differentiated MSCs may significantly increase, indicating Nesprin may play a role in the differentiation from MSCs to cardiomyocytes.

ABSTRACT

BACKGROUND:Crainocerebral missile wound (CMW) is the common severe trauma of war. Environmental factor is the important factor that aggravates the development of sickness and threatens the life of the sick and wounded at early stage. The study on basic theory and treatment of CMW under hyperthermia and high-humidity is the major task in military medicine.OBJECTIVE: To observe the characters of bacterial growth in wound cavity and peripheral tissue after CMW in cats under hyperthermia and high-humidity so as to provide the reference data to the tissue repair and its functional recovery in CMW.DESIGN: Randomized controlled experiment.SETTING: Surgery Department of Neurology in Zhujiang Hospital affiliated to Southern Medical University(First Military Medical University).MATERIALS: The experiment was performed in Department of Tropical Medicine and Hygiene in Southern Medical University from April to May 2003. In the experiment, 24 hybrid cats were employed, of clean grade, of either sex, body-weighted (2.5±0.2) kg, provided from Experimental Animal Center of Southern Medical University. The animals were randomized into 4 groups, named the control at common temperature (5 cats), common temperature group (8 cats), hyperthermia and high-humidity group (8 cats) and blank control (3 cats).METHODS: After weighted, the cat was injected abdominally with 30 g/L pentobarbitol sodium solution at 30 mg/kg. After successful establishment of craniocerebral blindgut wound model, in common temperature group, the cats were placed in simulated climatic cabin at 25 ℃ and 50% relevant humidity for 6 hours and in hyperthermia and high-humidity group, the cats were placed in the hyperthermia and high-humidity environment for 6 hours. With biopsy, the fragmented brain tissue in wound tract and the brain tissue on the wall of wound tract far from 0-5 mm and 5-10 mm were collected and preparedinto successively natural solution and homogenate, and counting was done in 24 hours of bacterial culture to calculate bacterial content in wound cavity and peripheral tissue. In blank control, the cats were sacrificed with anesthesia and the material was collected directly, the rest steps were same as the control at common temperature.MAIN OUTCOME MEASURES: Bacterial content in wound cavity and peripheral tissue in each group.RESULTS: Totally 23 animals were employed in outcome analysis. Bacterial contents in same regions were different remarkably in different groups (F=171.423, P=0.000). It was indicated in paired comparison that significant difference happens between the control at common temperature, common temperature group and hyperthermia and high-humidity group and blank control successively (P < 0.01). The difference was not significant in the comparison among the control at common temperature, common temperature group and hyperthermia and high-humidity group (P > 0.05). Bacterial contents at different distances from the wound cavity were different remarkably in each group (F=14.865, P = 0.000).CONCLUSION: Hyperthermia and high-humidity does not impact significantly the bacterial growth of CMW in 6 hours and bacterial reproduction has not entered high-rate increasing stage yet, due to which, it is still safe for debridement repair.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effects of skeletal muscle satellite cells implanted into infarcted myocardium on the volume of remnant myocytes.</p><p><b>METHODS</b>Thirty-six adult mongrel canines were divided randomly into implantation group and control group. In the implantation group, skeletal muscle satellite cells taken from the gluteus maximus muscles of the dogs were cultured, proliferated and labeled with 4',6-diamidino-2-phenylindone (DAPI) in vitro. In both groups, a model of acute myocardial infarction was established in every dog. In the implantation group, each dog was injected with M199 solution containing autologous skeletal muscle satellite cells. The dogs in the control group received M199 solution without skeletal muscle satellite cells. The dogs of both groups were killed 2, 4 and 8 weeks after implantation (six dogs in a separate group each time). Both infarcted myocardium and normal myocytes distal from the infracted regions isolated were observed under optical and fluorescent microscope. Their volumes were determined using a confocal microscopy image analysis system and analyzed using SAS. A P < 0.05 was considered significant.</p><p><b>RESULTS</b>A portion of the implanted cells differentiated into muscle fiber with striations and were connected with intercalated discs. Cross-sectional area and cell volume were increased in normal myocardium. Hypertrophy of remnant myocytes in the infarcted site after skeletal muscle cell implantation was much more evident than in the control group. Cross-sectional area, cell area and cell volume differed significantly from those of the control group (P < 0.05). Hypertrophy of the cells occurred predominantly in terms of width and thickness, whereas cell length remained unchanged.</p><p><b>CONCLUSION</b>Skeletal muscle satellite cells implanted into infarct myocardium, could induce the hypertrophy of remnant myocyte cells in the infarcted site and could also aid in the recovery of the contractile force of the infarcted myocardium.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of different access routes on autologous satellite cell implantation to stimulate myocardial regeneration.</p><p><b>METHODS</b>Satellite cells were procured from skeletal muscle (gluteus max) of adult mongrel canine, cultured, proliferated and labeled with 4', 6-diamidino-2-phenylindone (DAPI) in vitro. The cells were autologously implanted into the site of acute myocardial infarction by local injection or perfusion through the ligated distal left anterior descending coronary artery. Specimens were harvested 2, 4 and 8 weeks later for histological study.</p><p><b>RESULTS</b>The labeling efficiency of satellite cells with DAPI was close to 100%. Fluorescent cells were found at the infarcted zone, papillary muscle and local injection site. Some of these cells had progressively differentiated into striated muscle fibers connected to intercalated discs. The infant cells appeared different from the mature myocardium under an electron microscope. Satellite cells implanted by perfusion through the coronary artery were arranged in order of consistency with host myocardial fibers. The satellite cells, implanted by local injection, were found growing in a disordered way.</p><p><b>CONCLUSION</b>Satellite cells, implanted by coronary artery perfusion, can progressively differentiate into striated muscle fibers, arranging in order and disseminating over the infarcted zone. This approach seems more favorable for the recovery of myocardial contractile function than that of local injection.</p>

ABSTRACT

@#Objective To investigate the effect of adenosine on pulmonary injury after extracorporeal circulation. Methods Twelve mongrel dogs were randomly divided into two groups, the test group and control group. The model of extracorporeal circulation was established. In the test group and control group, adenosine(50μg/kg*min) and normal saline were infused before extracorporeal circulation respectively. Hemodynamics, right cardiac function and arterial blood gas were assessed. Lung water and malondialdehyde content were assessed. Histological evaluation was also made. Results There were no differences between the two groups with respect to heart rate, mean systemic artery pressure, left atrial pressure, and central venous pressure. In the test group, pulmonary vascular resistance was lower and right cardiac function improved remarkably; arterial oxygen pressure (PaO2) was also higher, less lung water was observed and the malondialdehyde content in the pulmonary tissue was lower (P<0.05,P<0.01). Histological evaluation of the test group revealed normal pulmonary architecture without abundant intra-alveolar neutrophils. Conclusion Adenosine can attenuate ischemia-reperfusion lung injury and right cardiac insufficiency caused by extracorporeal circulation. At a proper dose, there was no significant effect on systemic hemodynamics.

ABSTRACT

This study was designed to determine the protective effect of scopolamine and catalase on the arterial endothelial cells during reoxygenation. The culture of endothelial cells from bovine aorta subjected to ischemia for 2 hours and reoxygenation for 30 minutes was used as a model for inves-tigaion. In the experimental group, scopolamine was added to the arterial endothelial cells culture medium, Hanks solution containing no sugar or oxygen; or in the control group no medicine was added to the ordinary Hanks solution. The alteration of glutathione ( GSH ) and malondialdehyde (MDA) of endothelial cells, and nitric oxide (NO) and lac-tate dehydrogenase (LDH) in the supernatant fluid were determined. The results demonstrated the following changes: (1) GSH in the control group significant reduction from the preischemia value 44. 5 + 2.45 to postischemia and reoxy genation value 35.08 + 5.72ug/mg protein, vs the post-ischemia and reoxygenatior value of 46. 67+3.20ug/ mg protein (scopolamine) and 43.38+2.98ug/mg protein (catalase). The differences between the GSH changes of control group and medicine treated group are statistically significant (P

ABSTRACT

OBJECTIVE: To study the influence of auto messenchymal stem cells(MSCs)transplant into the infracted area on the ventricle reconstruction in mini pig model of acute myocardial infarction. METHODS: The bone marrow of mini pig was collected and centrifuged in vitro for MSCs. Then after cultivation, transformation and labeling MSCs were injected into the ligated left anterior descending coronary artery and myocardium of mini pig, while the control group was administered with DMEM of the same dose. Doppler ultrasonography was performed separately at 3 weeks and 6 weeks. RESULTS: The left ventricular ejection fraction, shortening fraction, lateral thickening , and interventricular thickening in the experimental group were significantly improved, as compared with the control group, while the ventricular chamber in the experimental group was smaller than that in the control group.
