ABSTRACT

<p><b>OBJECTIVE</b>To explore the correlation between methylation of insulin-like growth factor 1 (IGF-1) gene promoter and its placenta-specific expression and fetal macrosoma.</p><p><b>METHODS</b>One hundred twenty nine healthy pregnant women were recruited between April 2011 and March 2012. Baseline data were collected with self-report questionnaires. Real-time quantitative PCR was used to determine the expression of IGF-1 mRNA in the placenta. Methylation level of the IGF 1 gene was determined with matrix-assisted laser desorption/ionization-time of flight mass spectrometry.</p><p><b>RESULTS</b>The expression of IGF-1 in placenta and its methylation level showed no significant difference between macrosomic fetuses and controls. No linear correlation was found between IGF-1 mRNA expression and methylation level of IGF-1 promoter (r=0.128, P=0.295). IGF-1 promoter region in placenta showed a hypomethylation status. However, a positive correlation was found between IGF-1 expression and birth weight below 4260 g (r=0.264, P=0.022). The expression of IGF-1 mRNA was significantly higher in those with a birth weight below 4260 g, which suggested that placental IGF-1 expression may contribute to increased birth weight. In regard to fetal overgrowth, however, there seemed to be a negative correlation in which placental IGF-1 expression was downregulated to limit fetal overgrowth.</p><p><b>CONCLUSION</b>No linear correlation was found between placental IGF-1 expression and methylation level of IGF-1 promoter with a hypomethylation status. The contribution of placental IGF-1 expression to birth weight is bidirectional. Increased expression seems to promote fetal growth, while decreased expressions may curb overgrowth, therefore control fetal growth in a relatively normal range.</p>

ABSTRACT

Objective To evaluate factors associated with mechanical ventilation in infants and young children undergoing cardiac surgery with pulmonary hypertension.Methods 161 Children who were ≤36 months of age, had congenital heart disease with pulmonary hypertension, and had been treated at our hospital between October 2011 and April 2013 were included in this retrospective study.Stepwise linear regression analysis was used to analyse the independent risk factors associated with mechanical ventilation (MV).Results The average of MV time was 40.9 h, the independent risk factors for MV were cardiopulmonary bypass(CPB) time (P =0.020), risk adjustment for surgery for congenital heart disease (RACHS-1) (P =0.014) ,low cardiac output syndrome(LCOS) (P <0.001), ventilator-associated pneumonia(VAP) (P =0.001) and postoperative pulmonary artery systolic pressure (sPAP) (P < 0.001).Conclusion The present results suggest that CPB time,RACHS-1 score, LCOS, VAP and postoperative sPAP can significantly affect the MV time in infants and young children with pulmonary hypertension after cardiac surgery.

ABSTRACT

Objective To investigate the effect of neurolytic celiac plexus block (NCPB) on the inflammatory reaction of the remaining liver tissue and liver function after partial hepatectomy (PH) in rats.Methods Thirty male Sprague-Dawley rats of SPF were constructed as a PH model with deligation and ablation operated on their left and middle lobes,respectively.Then,they were randomly divided into two groups:NCPB group and control group.Twelve hours after the surgery,0.5% lidocaine was given in the NCPB group once a day,while 0.9% saline was given in the control group.Determination of liver function,generation of the remaining liver,and deposition of IL-β,TNF-α of the pathological section was respectively made on Day 1,3 and 7 after the surgery.Results On Day 1,3 and 7 after surgery,both the aspartate aminotransferase (AST) and the alanine aminotransferase (ALT) levels in NCPB group were,to different degrees,lower than those in the control group (P < 0.05).Strikingly,total bilirubin in NCPB group was lower than that in control group (P < 0.01) on Day 7,while the level of semm albumin was higher than that in control group (P < 0.01).There was no statistically significant difference on the generation of the remaining livers between NCPB and control groups.On Day 3 and 7,the deposition of IL-β,TNF-α in the pathological sections of NCPB group were lower than those in control group.Conclusion NCPB can not only effectively reduce the damage of liver function caused by PH surgery,but also improve the inflammatory reaction of the residual liver.

ABSTRACT

Hemolymphangioma is a kind of deformity of the lymphatic vessel and venule,which is rarely seen in clinical practice.A 66-year-old female patient who had a chief complaint of distention in left upper quadrant of the abdomen for 6 months was admitted to the General Hospital of Chengdu Military Region of PLA.Examination of computed tomography and B ultrasound confirmed that the patient had a retroperitoneal cystic mass.During the exploratory laparotomy,the soft mass was found at the left upper quadrant of the abdomen and had invaded to the pancreas.Postoperative pathological examination confirmed that the mass was a pancreatic hemolymphangioma.

ABSTRACT

Objective To investigate the short term effect of gastric bypass surgery for the treatment of nonobese type 2 diabetes mellitus and possible mechanisms. Methods The clinical data of 58 patients with nonobese type 2 diabetes mellitus who received gastric bypass surgery from March to August, 2009 were retrospectively analyzed. The levels of fasting plasms glucose (FPG), 2-hour postprandial plasma glucose (2h PG) and glycosylated hemoglobin (HbA1c) were dynamically monitored, and the insulin resistance index (HOMA-IR) and body mass index ( BMI) were calculated. All data were analyzed using variance of analysis and LSD test. Results Of the 58 patients, 48 (83% ) met the requirement of complete response criteria and stopped administration of hypoglycemic agents; 7( 12% ) had to use hypoglycemic agents, but the dose of the agents was lowered by 50% compared to that before surgery. The surgery was ineffective in 3 patients (5% ). The levels of FPG, 2h PG, HbAlc and HOMA-IR of the 58 patients showed a significant decreasing trend after surgery when compared to those before surgery (F = 67. 867, 50. 885, 78. 278, 572. 757, P 0. 05 ). Conclusions Gastric bypass surgery has a good effect on nonobese patients with type 2 diabetes mellitus whose BMI was less than 25 kg/m2. The improvement of insulin resistance after the surgery might be the main reason.

ABSTRACT

Objective To investigate the relationship between portal vein pressure and liver regeneration after 90% portal branch ligation in rats.Methods Forty-five male SD rats underwent 90% portal branch ligation (including 5 rats underwent sham operation),and then the changes of portal vein pressure and weight of unligated hepatic lobes were detected.The morphological changes of hepatocytes of the unligated hepatic lobes were observed under a light microscope.Proliferative cell nuclear antigen(PCNA)index was detected by immunohistochemistry,and hepatocyte apoptosis of the unligated hepatic lobes by TUNEL method.All data were analyzed by Pearson rank correlation analysis and t test.Results Thirty-eight out of 40 rats survived(95%).The ligated hepatic lobes diminished progressively,whereas the unligated hepatic lobes regenerated.Preoperative portal vein pressure was(9.1±1.8)cm H_2O(1 cm H_2O=0.098 kPa),and it was increased significantly shortly after the ligation and reached (15.8±2.7)cm H_2O 12 hours later(t=6.847,P<0.05).The portal vein pressure decreased from(13.6±2.3)cm H_2O at day 1 to(9.3±2.0)cm H_2O at day 28.Preoperative positive PCNA index was 7%±3%,which was significantly lower than 14%±5%at postoperative 12 hours,21%±6%at day 3 and 26%±7%at day 5(t=9.129,P<0.05),and it began to return to normal at day 5.Few apoptotic hepatoeytes were observed in preoperative liver tissue and unligated hepatic lobes.The expression of PCNA in unligated hepatic lobes and portal vein pressure had apositive correlation at postoperative day 1,3,5(r=0.913,0.896,0.908,P<0.05)and a negative correlation at postoperative day 14(r=-0.926,P<0.05).Conclusions The regeneration of hepatocytes in unligated hepatic lobes is activated after 90% portal branch ligation,and the regenerated liver compensates the weight loss of the ligated hepatic lobes.Liver is regenerated mainly by speeding hepatocyte proliferation rather than reducing hepatocyte apoptosis.Changes of portal vein pressure may play an important role in liver regeneration.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy of Prostat in the treatment of NIH category IIIA prostatitis.</p><p><b>METHODS</b>An open clinical trial was conducted among 106 NIH category IIIA prostatitis patients. For each patient 1 tablet of Prostat (70 mg P5 + 4 mg EA10) was orally administered twice a day.</p><p><b>RESULTS</b>Subjective symptoms of the patients were significantly improved, particularly those of frequent micturition, urgent urination and peripheral region pains. Objective parameters also showed significant improvement. No adverse reaction associated with the medication was observed.</p><p><b>CONCLUSION</b>Prostat was shown to be an appropriate medicine in the treatment of NIH category IIIA prostatitis, with good curative effect and high safety.</p>

Objective To study the expression of p53 and vascular endothelial growth factor(VEGF) in bladder transitional cell carcinoma(BTCC) tissue and its relationship with the clinicopathologic parameters,and to understand the role of p53 and VEGF in the progression of tumors.Methods The expression of p53 and VEGF in 86 cases of BTCC were determined by immunohistochemical LDP methods.Results The positive expression rates of p53 and VEGF in 86 cases of BTCC were 46.51% and 66.28%,respectively.The positive correlation was noted between p53 expression and VEGF expression(P
