ABSTRACT

Chronic constipation (CC) is one of the most common functional gastrointestinal diseases. At present, the overall therapeutic effect of CC is still not satisfactory worldwide, which seriously affects the quality of life and social function of patients. The etiology and pathophysiological mechanism of constipation are still unclear. It involves comprehensive factors such as heredity, social psychology, diet, intestinal flora imbalance, intestinal motility disorder, visceral sensitivity change, pelvic floor muscle group dysfunction and enteric nervous system (ENS) disorder. Among them, the abnormal factors of the brain-gut-microbiome axis are particularly significant. The brain-gut-microbiome axis is a complex network of interactions between the intestine and the brain, integrating and coordinating the physiological functions and pathological processes of the gastrointestinal tract. Microorganisms in the intestinal lumen play an important role in it, and can communicate with the intestinal tract and the central nervous system through nerve, endocrine and immune pathways. As a key brain-gut peptide in the regulation pathway of the brain-gut-microbiome axis, 5-hydroxytryptamine (5-HT) is involved in the regulation of gastrointestinal motility, sensation and secretion. The abnormal conduction of the 5-HT signaling pathway is closely related to the occurrence and development of constipation. Traditional Chinese medicine(TCM) is a unique precious resource in China, which has good curative effects and safety. In recent years, it has received extensive attention in the treatment of constipation. TCM and active ingredients, acupuncture and massage specifically regulate 5-HT signal transmission, so that the expressions of related molecules tend to be suitable for individual disease state levels to effectively improve constipation symptoms, with unique advantages. Therefore, this study used ''constipation'', ''intestinal flora'', ''5-HT'', and ''traditional Chinese medicine'' as the keywords to search PubMed, China National Knowledge Infrastructure (CNKI) and other literature databases. The correlation between 5-HT and constipation as well as brain-gut-microbiome axis and the research progress of TCM intervention in the 5-HT signaling pathway in the treatment of constipation were reviewed in order to explore the potential therapeutic value of 5-HT system in this disease and provide references for subsequent research.

ABSTRACT

OBJECTIVE:To study the effects of Yangrong runchangshu mixture on tyrosine kinase receptor protein CD117 (c-Kit),substance P(SP)and myenteric nerve plexus vasoactive intestinal peptide(VIP)in the colon interstitial cell of Cajal of rat with slow transit constipation(STC). METHODS:Morphine hydrochloride(2.5 mg/kg)was given sc once daily for 45 consecu-tive days to establish STC rat models. 120 rats were randomized into a normal control (isometric normal saline) group,a model (isometric normal saline)group,Maren pill(positive control,14.7 g/kg)group and Yangrong runchangshu mixture high-dose and low-dose [15 and 7.5 g(medicinal materials)/kg] groups. After successful establishment of the models,drugs were given ig once dai-ly for 10 consecutive days. Immunohistochemical method was employed for staining. The changes in the structure and c-Kit,SP and VIP expressions in rats’colonic tissues were observed under the light microscope. RESULTS:Compared to the normal control group,those in the model group demonstrated weaker expressions of c-Kit and SP and less positive cells in colon,stronger expres-sion of VIP and more positive cells,with statistical significances (P<0.01);cellular morphology became abnormal obviously. Compared to the model group,those in Maren pill group and Yangrong runchangshu mixture high-dose and low-dose groups showed stronger expressions of c-Kit,SP and more positive cells in colon,weaker expressions of VIP and less positive cells with statistical significances (P<0.05);cellular morphology was improved to some extent. CONCLUSIONS:Yangrong runchangshu mixture can improve the contents of SP and VIP and the expression of c-Kit in rats with STC to some extent.

ABSTRACT

Aim To investigate the influence of Shen-mai injection ( SMI ) on the expression of cytochrome P450(CYP450) system in rat′s hearts. Methods Rat hearts were prepared after a fourteen-day continuous administration of SMI. The expression of several CYP genes, ANP, BNP and EPHX2 were measured by qPCR. Results SMI induced the increase in the ex-pression of other CYP genes except CYP2 B1、CYP4 A3 and CYP4 F6;HSI caused an induction of CYP2 E1 , CYP4A3,CYP4F1 and EHPX2 as compared with the control. In addition, there was a significant induction of ANP, BNP and EHPX2 and a significant inhibition of CYP2B1 and CYP2C11 after treated with MDI. Conclusion Although there is no significant change in the gene expression of CYP2 B1 after the treatment with SMI, but there is a general trend of induction, and MDI shows a significant inhibition of CYP2 B1 , therefore HSI has greater effect on CYP 2 B 1 than MDI . SMI causes a significant induction of CYP2 E1 , CYP4F1 and EHPX2 , similarly there is an induction of CYP2E1,CYP4F1 and EHPX2 by HSI and MDI, indi-cating that Hongshen and Maidong are both involved in the induction. MDI has a greater inductive effect than HSI on ANP and BNP. SMI is widely used for the treatment of cardiovascular diseases due to its regula-tion of CYP2J3、ANP and BNP mRNA expression.

ABSTRACT

[Objective]To study the changes of biomechanical efficiency of absorbable balloon vertebraplasty associated with pedicle screw system for treatment of thoracolumbar fracture.[Method]Six fresh adult thoracolumbar specimens were harvested and 10 segmental specimens(T_(11)~L_1、 L_(2~4)、 L_(1~3)) were processed.After burst fracture created by free-drop test,the specimens were braced and reduced by APF pedicle screw,then absorbable balloon were placed through the traumatic vertebral pedicle and were infused with autosolidification calcium phosphate cement to process vertebroplasty.The stiffness in middle vertebrae were measured under flexion,extension,lateral flexion and torsion stress before fracture and after absorbable balloon vertebroplasty respectively.[Result]The crushing strength in traumatic vertebrae after vertebroplasty with absorbable saeculi increased showed 8% under flexion,15% under extension and 9.8% under lateral flexion,but with significant increasing under extension(P0.05).[Conclusion](1)absorbable balloon vertebraplasty associated with pedicle screw system conduces to the reconstruction of traumatic vertebrae,and making postoperative spinal biomechanical characteristics approach the level before fracture;(2)Vertebroplasty with absorbable sacculus for treatment of thoracolumbar burst fracture based on the premise that avoid the potential danger induced by leakage of bone cement,and can reestablish intact corporeal structure of traumatic vertebrae and restore their height essentially,so that spinal physiologic curve can be gained to restore and maintain,tardive lumbodorsal pain and spinal cord injury and nerve injury caused by secondary kyphosis can be prevented,this can fundamentally solve complications left over by thoracolumbar burst fracture treated using pedicle screw system simply.The new method prorides a rational method for clinical treatment of thoracolumbar burst fractures.
