ABSTRACT

OBJECTIVE To study the inhibitory effects and possible mechanism of naringenin on the activation of hepatic stellate cells. METHODS Using human hepatocytes LO2 as reference, based on drug intervention concentration screened by MTT assay, the effects of naringenin (Western blot assay and trypan blue staining test in 10, 20, 40 μmol/L, immunofluorescence assay in 40 μmol/L) on the expressions of liver fibrosis markers protein (collagen Ⅰ, α-SMA) and mRNA (α1-pro collagen Ⅰ, α-SMA) in human hepatic stellate cells LX2, and the expressions of cell apoptosis and apoptosis-related proteins (Bcl-2, Bax, cleaved caspase-3) were investigated. The apoptosis agents (Z-VAD-FMK, FMK), ferroptosis pathway inhibitor ferrostatin-1, and programmed death pathway inhibitor necrostatin-1 were used to verify the mechanism of the above effects. RESULTS The naringenin could significantly down-regulate protein expressions of collagen Ⅰ (except for naringenin 10 μmol/L) and α-SMA, mRNA expressions of α1-pro collagen Ⅰ (except for naringenin 10 μmol/L) and α-SMA (P<0.05); it also induced LX2 cell apoptosis and increased its apoptotic ratio, down-regulated the protein expression of Bcl-2 while up-regulated the protein expressions of Bax (except for naringenin 10 μmol/L) and cleaved caspase-3 (except for naringenin 10 μmol/L). FMK could reverse above effects of naringenin on LX2 cells (P<0.05). CONCLUSIONS Naringenin can inhibit the activation of hepatic stellate cells LX2 through activating the cell apoptosis signal, which plays ameliorative role in liver fibrosis.

ABSTRACT

Objective:To explore the expression of programmed cell death-ligand 1 (PD-L1) in invasive breast cancer and its relationship with clinicopathological characteristics. Methods:A total of 651 paraffin-embedded specimens of newly diagnosed invasive breast cancer patients including 98 cases of triple-negative breast cancer (TNBC) in the Affiliated Nanjing Drum Tower Hospital of Medical School of Nanjing University from January 2018 to August 2019 were collected. The immunohistochemical EnVision method was used to detect the expression of PD-L1 protein in tumor cells of breast invasive cancer and infiltrating lymphocytes. The expression rate of PD-L1 in tumor cells of cancer section tissues and tumor interstitial lymphocytes ≥1% was positive and < 1% was negative. Results:The positive expression rate of PD-L1 expression in breast invasive carcinoma was 47.0% (306/651), among which the positive expression rate of TNBC was 69.3% (68/98). PD-L1 had a high positive expression rate in patients with high World Health Organization (WHO) grade, large tumor size, vascular invasion, advanced pathological stage, negative estrogen receptor (ER) expression, negative progesterone receptor (PR) expression, positive p53, positive CK5/6, positive epidermal growth factor receptor (EGFR), and high Ki-67 value (all P < 0.05). In TNBC, PD-L1 had a high positive expression rate in patients with high WHO grade, invaded nerves, high Ki-67 value, positive CK5/6, and positive EGFR (all P < 0.05). Multivariate analysis showed that WHO grade ( HR = 1.511, 95% CI 1.139-2.003, P = 0.004), Ki-67 value ( HR = 1.847, 95% CI 1.259-2.709, P = 0.002), p53 expression ( HR = 2.083, 95% CI 1.487-2.916, P < 0.01), and EGFR expression ( HR = 3.490, 95% CI 2.002-6.086, P < 0.01) were independent influencing factors of PD-L1 expression. Conclusions:PD-L1 is highly expressed in invasive breast cancer, especially in TNBC. It could be used as a potential treatment target for patients with invasive breast cancer.

ABSTRACT

OBJECTIVE:To know about the research status of health utility value obtained by mapping method in pharmacoeconomic evaluation ,and to provide reference for bibliometric study in pharmacoeconomic evaluation . METHODS : Using“Mapping method ”“Health utility value ”“Cost-utility”“Utility point system ”as Chinese and English keywords ,retrieved from CNKI ,Wanfang database ,PubMed,Medline,Ebsco,Ovid and Wiley database ,empirical journal documents published from the inception to Dec. 31st,2018 about using mapping method to obtain health utility value were collected. The bibliometrics was used to statistically analyze basic information of included literature ,the construction and test of the model ,the type of the best model and so on. RESULTS :The 124 included documents were all published in English journal. In the construction and testing of the mapping model ,the most frequently used econometric methods ,performance evaluation indicators and model testing methods were ordinary least squares (OLS),mean absolute error (MAE)and residual normality test ,application frequency of which were 97 times(31.60%),89 times(24.93%)and 62 times(21.09%). There are 117 articles that define the best mapping model ,of which 101 articles(86.32%)have the best direct mapping effect. Most of the non-utility measurement scales adopted specific scales (92 articles,77.97%),and a few literatures adopted the universal scale (26 articles,22.03%). The most utility measurement scales were 3-level European 5-dimensional health scale (79 articles,66.95%). CONCLUSIONS :The domestic empirical researches that use the mapping method to obtain health utility values need to be developed yet. A series of mapping models successfully developed by foreign scholars not only provide the feasibility of using non-utility measurement scales for cost-utility analysis ,but also provide more ideas for China to choose the corresponding econometric methods ,evaluation indicators and mapping methods in the empirical research of the mapping method in the future.

ABSTRACT

OBJECTIVE@#To detect variant of APOE gene in a Chinese Tibetan patient with lipoprotein glomerulopathy (LPG) confirmed by renal biopsy and to explore its pathogenesis.@*METHODS@#Clinical and pathological data was collected. DNA was extracted from peripheral blood sample of the patient and subjected to PCR and Sanger sequencing. Pathogenicity of the variant was analyzed by bioinformatics software.@*RESULTS@#Renal biopsy of the patient has confirmed the diagnosis of LPG. DNA sequencing suggested that the patient has carried a heterozygous c.527G>C (p.R176P) variant of the APOE gene (APOE Osaka/Kurashiki). Four cases of LPG have been found to carry the same variant, and the encoded amino acid (p.176R) is highly conserved during evolution. Bioinformatic analysis using SIFT, PolyPhen2 and PANTHER software all predicted the variant to be pathogenic.@*CONCLUSION@#The discovery of author's patient provided further evidence for the pathogenicity of APOE Osaka/Kurashiki and, more importantly, provide new evidence for the multiracial origin of LPG-related APOE variants.

Methods A total of 141 paraffin-embedded specimens of patients with newly diagnosed lung adenocarcinoma by pathology from January to August 2018 in the Affiliated Nanjing Drum Tower Hospital of Nanjing University Medical School were collected. The expression of PD-L1 in the tumor cells (TC) and tumor-infiltrating immune cells (IC) were detected by immunohistochemistry EnVision staining. Based on the percentage of expression, PD-L1 in TC and IC were divided into TC3 (≥50%), TC2 (≥5% and <50%), TC1 (≥1% and <5%), and TC0 (<1%); IC3 (≥10%), IC2 (≥5% and <10%), IC1 (≥1% and <5%), and IC0 (<1%). Results PD-L1 expression was positive in 90 cases, and the positive rate was 63.8% (90/141) at a TC1 or IC1 threshold. When a TC3 or IC3 threshold was applied, PD-L1 expression was positive in 17 cases, and the positive rate was 12.1% (17/141). In the TC1 or IC1 threshold group, PD-L1 expressions were different in patients with different sex, nerve invasion and pleural invasion, and the differences were statistically significant (all P < 0.05). While in the TC3 or IC3 threshold group, PD-L1 expressions were different in patients with different cell differentiation, clinical stage, lymphocyte infiltration, vascular invasion and nerve invasion, and the differences were statistically significant (all P < 0.05). In both TC1 or IC1 threshold group and TC3 or IC3 threshold group, PD-L1 expression was associated with nerve invasion (both P < 0.05). Conclusions PD-L1 expression is associated with nerve invasion. PD-L1 inhibitors expect to be used as a potential treatment strategy for recurrent and progressive lung adenocarcinoma patients with high level expression of PD-L1.

ABSTRACT

Objective@#To investigate the clinical and histological features, diagnosis and differential diagnosis of myofibroma/myofibromatosis.@*Methods@#The clinical data and pathology features of nine cases of myofibroma/myofibromatosis were collected from August 2011 to November 2016 in Affiliated Drum Tower Hospital, Nanjing University Medical School and Children′s Hospital of Nanjing Medical University. Immunohistochemistry(IHC), PDGFRB molecular analysis and ETV6-NTRK3 gene fusion were performed and relevant literature reviewed.@*Results@#There were 7 males and 2 females, with age ranging from 3 days to 18 years (mean 5 years). The tumors were located in head and neck (eight cases) and trunk (one case). Clinically, the tumors presented as freely movable nodules. Microscopically, they appeared biphasic with alternating light- and dark-staining areas. The light-staining area consisted mainly of plump myoid spindle cells with eosinophilic cytoplasm arranged in nodules, short fascicles, or whorls. The dark-staining area was composed of round or polygonal cells with slightly hyperchromatic nuclei or small spindle cells arranged around a distinct hemangiopericytoma-like vascular pattern. IHC showed the tumor cells in the light-staining area were strongly positive for vimentin and SMA, while cells in dark-staining area were strongly positive for vimentin, and weakly for SMA. Tumor cells were negative for desmin, S-100 protein, h-Caldesmon, CD34 and STAT6. Analysis of PDGFRB mutations was performed in seven cases. Two cases showed 12 exon point mutation c. 1681 c>T(p.R561C), one case showed 14 exon point mutation c. 1998C>G (p.N666K). ETV6-NTRK3 gene fusion was not detected by fluorescence in situ hybridization in four patients under three years old. All cases were followed for 6 to 68 months, with two recurrences.@*Conclusions@#Myofibroma/myofibromatosis is an uncommon benign myofibroblastic tumor of infancy and childhood. The tumor can appear biphasic, and may show PDGFRB point mutation which is of potential diagnostic value.

ABSTRACT

Objective To identify the half-sibling relationship by comprehensive using three different methods. Methods STR genotype was performed on A, A's mother, B and B's mother by using PowerPlex21 kit, AGCU Expressmarker 21+1 kit, Microreader 23sp-B kit and AGCU X-19 STR kit respectively. Based on the results of STR genotype and X-STR, we determined half-sibling relationship by ITO, discriminant function analysis and IBS. Results HIS was between 1.36×102and 2.09×105in ITO which indicated that A and B had the same father. The IBS and discriminant function analysis also had the same conclusion. Conclusion Comprehensive using multiple methods can obtain reliable result to identify the half-sibling relationship from the same father.

ABSTRACT

Objective To study the clinical, endoscopic and pathological features of gastrointestinal and mesenteric reactive nodular fibrous pseudotumor (RNFPT). Methods A retrospective analysis was conducted on data of 24 RNFPT patients in Nanjing Drum Tower Hospital admitted from October 2008 to June 2016. The clinical, endoscopic, pathological and immunohistochemical characteristics were analyzed. Results Among the 24 patients, 16 complained about discomfort in the upper abdomen and 10 had a history of surgery or trauma. Twenty-one had isolated masses and 3 had multiple masses, with diameter of 0. 5-4. 0 cm. Endoscopically, the tumors were mainly hard submucosal masses with broad base, and smooth surface with no mucosal bridge. Seventeen patients underwent endoscopic ultrasonography, which showed low echoes in lesions and nonuniform echoes partly. Among them, 13 lesions derived from muscularis, 4 others from submucosa. Microscopically, the tumors had clear boundary with no envelope, and most areas showed disorderly arranged spindle cells and extensively collagenous degenerated mesenchyma. The spindle cells had shuttle fibroblast-like morphology and elongated nucleus with no visible necrosis or mitosis. Inflammatory cells scattered between the tumor cells, and lymphoid follicles and calcium deposition could be seen in local areas. Immunohistochemically, SMA was focally positive in 7 cases and only 4 cases expressed CD117 scattered. Desmin, Dog-1, CD34, ALK-1 and S-100 were all negative, and Ki-67 proliferation index was lower than 1%. Conclusion RNFPT has diverse clinical manifestations, with a good prognosis and unlikely recurrences, and should be distinguished from spindle cell tumors.
