ABSTRACT

Objective:To investigate the effect of type ⅡNKT cells activated by sulfatide on airway inflammation in a murine model of asthma.Methods:Thirty-two BALB/c mice were randomly divided into four groups:normal control group ( n=8 ) , asthma group (n=8),sulfatide treatment group (n=8) and adoptive transfer group (n=8).The murine model of asthma was established by sensitization with intraperitoneal injection of ovalbumin ( OVA) and intranasal challenge in all animals except for the normal control group where PBS was used instead.Intraperitoneal injection of sulfatide in a sulfatide treatment group, adoptive transfer of sulfatide-activated typeⅡNKT cells in adoptive transfer group and PBS in asthma group were carried out 1 hour before the first challenge.PBS was used for intraperitoneal administration in the normal control group.Lung histology and goblet cell hyperplasia were analyzed by HE or PAS staining.Differential cell count in bronchial alveolar lavage ( BALF) was measured by May-Gruenwald Giemsa;levels of OVA-specific IgE in serum and L-4,IL-5 in BALF were measured by ELISA.The percentages of lung type Ⅱ NKT cells,IL-4+and IFN-γ+typeⅡNKT cells were detected by flow cytometry.Results:Inflammatory cell infiltration in lung tissue and goblet cell hyperplasia in the airway were decreased in sulfatide treatment group and adoptive transfer group.Percentages of eosinophil in BALF,level of OVA-specific IgE in serum,and levels of IL-4,IL-5 in BALF in sulfatide treatment group and adoptive transfer group were significantly lower than those in asthma group (all P<0.05).The percentages of lung IL-4+and IFN-γ+typeⅡNKT cells in sulfatide treatment group was significantly higher than those in asthma group ( P<0.01 ).Conclusion: Type Ⅱ NKT cells activated by sulfatide may inhibit airway inflammation in a murine model of asthma.

ABSTRACT

Objective To evaluate the impact of vagal denervation (VD) that is derived from circumferential ablation of pulmonary vein ostium for paroxysmal atrial fibrillation (AF) on the therapeutic results. Methods A total of 50 patients with paroxysmal atrial fibrillation were enrolled in this study. Circumferential ablation of pulmonary vein ostium was carried out in all the patients. The end point of ablation was pulmonary vein electricity isolation. The patients in whom VD occurred during the performance of ablation were regarded as VD- positive group (n = 19), and the remaining patients were used as VD- negative group (n = 31). The recurrence rate of AF six months after the treatment was recorded, and the results were compared between the two groups. Results The end point of ablation was successfully achieved in all the fifty cases. Six months after the ablation, the therapeutic effect of VD- positive group was significantly better than that of VD- negative group (84.21% vs 64.51%, P ≤ 0.05). Conclusion The vagal denervation effect that is derived from circumferential ablation of pulmonary vein ostium in treating AF can significantly increase the success rate of radiofrequency ablation for AF.

ABSTRACT

Objective To investigate the clinical significance of high mobility group box-1 protein (HMGB1) in chronic,severe hepatitis B patients and liver cirrhosis.Methods The contents of HMGB1 was measured by enzyme-linked immunoassay (ELISA) in the patients (include 40 patients with chronic hepatitis B and 18 severe hepatitis B patients and 18 liver cirrhosis patients) and 20 health controls.The levels of relative biochemical indicators,prothrombin time activity percentage (PTA) and the hepatic fibrosis were determined by biochemical methods.Results The contents of HMGB1 was significantly increased in severe hepatitis patients in comparison with that in chronic hepatitis B patients(P <0.01).The contents of HMGB1 in the chronic hepatitis were significantly increased in health controls(P <0.01).The contents of HMGB1 had significant difference among the liver cirrhosis patients,the severe hepatitis patients and the chronic hepatitis(P <0.01).There was positive correlation among the contents of HMGB1 and total bilirubin (TBIL),the proportionality of aspartate aminotransferase and alanine aminotransferase (AST/ALT),hyaluronic acid (HA) and procollagen Ⅲ N-terminal peptide (P Ⅲ NP) (r = 0.865,0.646,0.783,0.662,P < 0.01).There was negative correlation among the contents of HMGB1 and prothrombin time activity percentage (PTA) and albumin (ALB) (r =-0.915,-0.852,P <0.01).Conclusion The contents of serum HMGB1 were closely associated with disease severity in chronic hepatitis B patients.HMGB1 was an index to auxiliary diagnosis hepatic fibrosis.

ABSTRACT

Objective To explore the value of combining pulmonary rehabilitation with heat and magnetic vibration (HMV) therapy and tiotropium for patients with stable but severe chronic obstructive pulmonary disease (COPD). Methods This was a paralleled, controlled, randomized study. Thirty-seven patients with stable severe COPD were enrolled and divided into two groups at random. One was the tiotropium group (T group) , while the other combined tiotropium therapy with HMV (the T + HMV group). The time span was 4 weeks. The examinations were performed at week 0, week 2 and week 4. The examinations included pulmonary function tests, arterial blood gas analysis, the 6 minute walking test (6MWT) , Borg's score and St George's Respiratory Questionaire (SGRQ). Results Inspiratory capacity (IC) increased in both groups. Forced expiratory volume in one second ( FEV1.0) , percent predicted FEV1.0 and FEV1.0/forced vital capacity ( FVC) increased significantly only in the T + HMV group. The average parameters of the pulmonary function test in the T + HMV group were significantly higher than in the T group. In both groups, alveolar PO2 ( PaO2) improved but alveolar PCO2 ( PaCO2 ) did not change and in this there was no significant difference between the groups. The 6 minute walking distance increased and the average Borg score decreased in both groups, and there was no difference between the groups. SCRQ dropped more than 4 scores in both groups, but the decrease in the T + HMV group was significantly greater. Conclusions Tiotropium can play an important role in the rehabilitation of patients with stable severe COPD. The combination of tiotropium with HMV therapy is superior to tiotropium alone in pulmonary rehabilitation for stable but severe COPD patients.

ABSTRACT

Objective To evaluate the safety and efficacy of domestic biodegradable drug-eluting stent, EXCEL, in clinical treatment of coronary heart disease.Methods During the period of Jan.2008-Oct.2008, 100 patients with coronary heart disease who consented to PCI and postoperative coronary angiography were selected and enrolled in the study.The patients were divided into EXCEL group(n = 58) and Cypher group(n = 42).The immediate successful rate of interventional therapy and the complications occurred during hospitalization were observed.Postoperative follow-up and checkup coronary angiography were conducted.Results No significant difference in the relevant clinical data, in the number and type of diseased coronary branch and in the mean length of implanted stents existed between two groups.Also, no statistically significant difference in the occurrence of major adverse cardiac event(MACE), in-stent restenosis and thrombosis was present between two groups.Conclusion Compared with Cypher stenting, EXCEL stenting carries similar, i.e.rather low, occurrence of MACE and in-stent restenosis.

OBJECTIVE To discuss the reasonable application of multi-enzyme abluent in the process of endoscopic cleaning and disinfecting.METHODS The multi-enzyme abluent was reasonably used to clean the endoscope in the pre-cleaning and disinfection process at thebedside,to enable it to achieve the best cleaning effect.RESULTS The reasonable application of multi-enzyme abluent could enhance the effect of endoscope cleaning to ensure the success of endoscopic disinfection.CONCLUSIONS Multi-enzyme abluent can protect the successful endoscopic disinfection and prevent the iatrogenic infection.

ABSTRACT

BACKGROUND: The morbidity of diabetes mellitus increased recently.There are few reports about the pulmonary function status of the patients with diabetes mellitus.OBJECTIVE: To comparably observe the difference of the pulmonary function status between patients with diabetes mellitus and normal persons,and to probe into the relationship between the course of disease of diabetes mellitus and the functional change of pulmonary function.DESIGN: Case controlled observation SETTING: Renmin Hospital, Wuhan University PARTICIPANTS: Totally 90 patients with diabetes mellitus hospitalized in the Renmin Hospital, Wuhan University between January 2000 and De cember 2003 were recruited. All the patients participated in the experiment voluntarily. Totally 20 cases were of insulin dependent diabetes mellitns (IDDM) and 70 cases were of non-insulin dependent diabetes mellitus (NIDDM). Another 90 cases were divided into 2 groups with the boundary line of 10 years: group with disease course <10 years (n=40) and group with disease course >10 years (n=50). And 30 normal persons with healthy pulmonary function was set as healthy control group, all the testees were voluntary participants in the experiment.METHODS: Pulmonary function index of the participants of each group was measured: vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1%), peak expiratory flow rate(PEFR)maximal voluntary ventilation (MVV) and comparison was performed between them.Comparison of the pulmonary function status of the patients between different levels of diabetes mellitus.pulmonary function between different groups : Compared with health control group, VC , FVC and MVV of the patients with diabetes mellitus was significantly decreased (t=1.999-2.301, P < 0.05).There was no significant difference in the VC, FVC and MVV between patients with monary function between patients with different diabetes mellitus: the pulmonary function status in the group with disease course >10 years was improved less than that in the group with disease course <10 years [ VC (L):2.62±0.65 vs 3.25±1.25;FVC(L): 2.40±0.52 vs 3.21±0.98;FEV1%:75.31±2.31 vs 80.63±5.56;PEFR (L/s):4.33±0.68 vs 5.98±1.02;MVV (L/s): 76.22±6.98 vs 89.21±5.69 (t=2.012-2.350, P < 0.05)].CONCLUSION: Pulmonary function of the patients with diabetes mellitus has reduced tendency compared with the normal persons, and the pulmonary function of the patients with long course of disease decreases significantly. More attention should be paid to diabetes patients' pulmonary function changes.

ABSTRACT

Objective To explore the association between the lung function and the related electroneurophysiological parameters in patients with chronic obstructive pulmonary disease (COPD). Methods Fifty COPD patients were studied. The lung function and the phrenic motor nerve conduction (PNC) were examined, respectively. Thirty cases of non respiratory disorders served as control. The lung function tests included maximal voluntary ventilation (MVV%), forced vital capacity (FVC), force expiratory volume (FEV 1) and RV/TLC% (residual volume, RV; total lung capacity, TLC). The phrenic nerve was stimulated electrically at the midpoint of the posterior border of the sternomastoid muscle, and the diaphragmatic compound muscle action potential (dCMAP) was recorded between the 7th and 8th intercostal space and xiphoid process. Results The relative values of MVV%, FVC 1/FVC 1% and RV/TLC% were 49.6?18.8, 62.5?16.4 and 54.1?8.0, respectively, which was significantly lower than that expected. There was no significant difference with regard to the PNC latency between the patients and the controls. The dCMAP amplitude ratio in the COPD patients was significantly lower than that of the controls, i.e., the dCMAP amplitude decreased in the COPD patients. Conclusion The decreased dCMAP might be associated with abnormal lung function. The PNC examination would provide valuable information for the evaluation of diaphragmatic dysfunction in those with abnormal lung function.

ABSTRACT

Object To compare the hupzine A (Hup A) in Huperzia serrata (Thunb.) Trev. obtained by different extracting methods and investigate the amount of alkaloids and the content of Hup A from different parts of the plants and from different places. Methods Using HPLC for the determination of Hup A. Results The content of Hup A in the stem and leaf is richer than that in the root. The content of Hup A from Guizhou, Guangdong and Anhui Provinces is 0.018%, 0.021% and 0.020% repectively; The difference of extract method of Hup A is no prominence. Conclusion The content of Hup A in the ground is richer than that of underground, and there are some difference in the content of Hup A obtained from different places.
