ABSTRACT

Objective:To develop the nursing staff's perception scale of patients′ suicide risk and test its reliability and validity, so as to provide basis for managers to formulate training strategies for nurses' suicide gatekeepers.Methods:This was a cross-sectional study. Based on cognitive-experiential self-theory, and on the basis of literature review and semi-structured interviews at home and abroad, a scale item pool was formed, and an initial scale was formed through expert inquiry and pre-investigation. In September, 2022, a cross-sectional survey was conducted among 459 nurses from three tertiary general hospitals and oncology hospitals by convenience sampling method in Jinan and Dezhou, Shandong Province. SPSS25.0 and AMOS23.0 software were used to analyze the items and test the reliability and validity of the scale.Results:The formal scale included three dimensions, namely, cognitive judgment, emotional evaluation, and concern, with 15 items in total. Exploratory factor analysis extracted three common factors, and the cumulative variance contribution rate was 67.845%. The results of confirmatory factor analysis showed that each test quantity of the fitness of the formal scale model is within the acceptable range and loads of 15 items in the corresponding factors were all >0.45; the content validity index S-CVI of the scale was 0.972, Cronbach′s α coefficient of the total scale was 0.907, and the half reliability was 0.814. Conclusion:The scale of nurses′ perception of patients′ suicide risk has good reliability and validity, and it can be used to evaluate nurses′ perceptions of patients′ suicide risk.

ABSTRACT

Objective:To describe the status and influencing factors of the empowerment of caregivers after gastrointestinal tumor.Methods:From March 2019 to October 2019, using convenience sampling method, 124 pairs of gastrointestinal tumor patients and caregivers who were hospitalized in the general surgery and anorectal surgery of the department of Shandong Tai′an City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University were selected as the subjects. A general information questionnaire, Main Caregivers′Empowerment Measurement were used as research tools. To investigate the empowerment ability of caregivers of patients with gastrointestinal tumors after operation.Results:The total caregiver′s empowerment score was 168.81 ± 19.49. The scores of each dimension ranged from high to low: relationship with caregivers (3.83 ± 0.43), expectation of care outcomes (3.80 ± 0.43), awareness of caregiving roles (3.70 ± 0.51), caregivers′ subjectivity (3.37 ± 0.60), caregiving beliefs (3.34 ± 0.67), caregiving knowledge and skills (3.27 ± 0.73), personal resources (2.94 ± 0.65), goodwill caregiving (2.85 ± 0.65) , the scruples around (2.75 ± 0.88). Multiple linear regression results showed that, patient gender, whether the disease is first attack, whether the patient is aware of the disease, the caregiver marriage, and the caregiver working status were the influencing factors of the caregiver′s empowerment ability after gastrointestinal tumor operation ( t values were -8.15-8.72, R2=0.62, all P<0.05). Conclusions:There is room for further improvement in the empowerment of caregivers after gastrointestinal tumor surgery, especially in personal resources, goodwill care and scruples around. The results suggest that medical staff should guide caregivers to emancipate their minds, enhance the breadth of empowerment, and promote self care and fast rehabilitation of patients.

ABSTRACT

Objective:To analyze the status and influencing factors of long-term care demand of the community elderly in Taian and in order to provide a theoretical basis for the government to formulate policies to meet the needs of the elderly.Methods:A self-designed questionnaire was used to investigate 294 elderly aged 60 years in three areas in Tai'an using Convenient sampling.Results:The total average score of long-term care demand was (2.67±0.46). The top three demand are economic, chronic disease rehabilitation, and dignity demand. Multiple linear regression showed that age, living conditions, place of residence, and chronic disease were the influencing factors of long-term care needs ( P<0.05). Conclusions:The long-term care needs of the elderly in Tai'an community were at a medium level, Age, living conditions, place of residence and chronic diseases have different effects on long-term care demand. It is suggested that the government and medical staff should pay more attention to the economic, rehabilitation and dignity needs of the elderly in the community, and formulate and implement relevant policies to meet the needs of the elderly in order to improve the quality of life and happiness of the elderly.

ABSTRACT

Objective:To investigate the present situation of delayed medical care for patients with senile cataract and analyze the influencing factors.Methods:Totally 636 senile cataract patients in Baoji Eye Hospital from January to September, 2017 were recruited. They were investigated using the current status of medical delay questionnaire.Results:The detection rate of delayed medical care for elderly patients with cataract was 36.95%(235/650). Logistic analysis demonstrated that significant influencing factors for patient delay included age, place of residence, educational level, hypertension, diabetes, medical insurance, distance and time for place of residence to the nearest medical unit for elderly cataract patients ( P<0.05) . Conclusion:There are widespread medical delays in elderly cataract patients in Baoji City. We should strengthen health education for elderly cataract patients, provide social support, and develop targeted measures to promote timely medical treatment, reduce blindness and increase quality of life.

Objective@#To explain the true experience of death anxiety in elderly inpatients with chronic diseases, and provide a theoretical basis for hospice care.@*Methods@#The phenomenological study method was used to deeply interview the true feelings of death anxiety in 13 elderly inpatients with chronic diseases.@*Results@#Through analysis, collation and refinement, the five themes of death anxiety, life regret, death attitude, death reminder and final decision power.@*Conclusions@#As clinical medical workers, we must always pay attention to the death anxiety of elderly inpatients with chronic diseases and strengthen the assessment of death anxiety. In particular, patients with cancer and patients at the end of life are provided with personalized care to remind them of the meaning of life and reduce the anxiety level of death.

ABSTRACT

Objective@#The purpose of the research was to draw up the questionnaire of the integrated services demand for medicine, pension, rehabilitation and care of community elderly and to provide elderly people with a effective tool for evaluating the needs of medicine, pension, rehabilitation and care.@*Methods@#The theoretical framework of the questionnaire was based on the qualitative interview results of 12 community elderly and community health care workers. A preliminary draft was formed through two rounds of expert consultation and a preliminary survey of 102 elderly people in 3 communities in Taian. Construct validity was used to evaluate the validity of the questionnaire, and Cronbach's alpha coefficient and split half reliability were used to evaluate the reliability of the questionnaire.@*Results@#Sixteen experts were invited to conduct two rounds of consultation on 61 items of the questionnaire. The authoritative coefficient of experts was 0.881, and the positive coefficient of experts was 100%. In the first round, items I-CVI, S-CVI/UA and S-CVI/Ave were 0.68-1.00, 0.72 and 0.83, respectively. Seven items were modified, six items were deleted and three items were added. In the second round, one item was deleted and one item was added, and a questionnaire consisting of seven dimensions and 58 items was formed.7 common factors were extracted from exploratory factor analysis, and the cumulative contribution rate was 63.078%. Finally, 7 dimensions and 51 item questionnaires were formed. Each dimension is the community public health service and general services, health education, pension services, rehabilitation services, family beds, service charges The total Cronbach's alpha coefficient was 0.919, the Cronbach' s alpha coefficient of each dimension was 0.615-0.808, the retest reliability coefficient of the total questionnaire was 0.862.and the Guttman half coefficient is 0.865. The total questionnaire CVI was 0.85 and the item CVI was 0.80-1.00. Item analysis showed that the correlation coefficients of the 7 items were lower than 0.3, and the rest were all between 0.3~0.7, and were significantly correlated at P<0.05 level.@*Conclusion@#The questionnaire of the integrated services demand for medicine, pension, rehabilitation and care of community elderly had good reliability and validity, and it could be used to evaluate the needs of medicine, pension, rehabilitation and care in the elderly.

ABSTRACT

Objective@#To investigate the present situation of spiritual health and empathy of undergraduate nursing students, and explore the correlation between them.@*Methods@#Convenient sampling was used and 498 senior nursing students were recruited from 2 universities in Shandong Province. They were investigated using the general questionnaire, the Spiritual Health Scale Short Form and Chinese version of the Interpersonal Reactivity Index.@*Results@#The total score of spiritual health was (92.87±12.28), the 5 dimensions of the item score from high to low were connection to others (4.46±0.62), meaning derived from living (4.15±0.69), self-understanding (4.03±0.72), transcendence (3.95±0.74) and religious attachment (2.60±1.08). The total score of Interpersonal Reactivity Index was (57.50±8.35), the 5 dimensions of the item score from high to low were perspective taking (2.78±0.61), empathy concern (2.72±0.56), fantasy (2.61±0.57), personal distress (2.32±0.78). Nursing students’ spiritual health score and empathy was positively correlated (r=0.334, P<0.01). Regression analysis, single-child (B=0.202, P<0.05), exercise habit (B=0.363, P<0.01), religious belief (B=0.381, P<0.01), academic performance (B=0.205, P<0.05), nursing career in the future (B=0.285, P<0.01) were the influencing factors of nursing students′ spiritual health.@*Conclusions@#Spiritual health is closely related to empathy; the higher the level of spiritual health, the higher the empathy ability. It suggests that nursing educators can improve nursing students′ empathy by improving the spiritual health of nursing students.

ABSTRACT

Objective To explore the status and influencing factors of the integrated service needs of medicine, pension, rehabilitation and care of the community elderly in order to provide a theoretical basis for the construction of an old-age service model in line with the national conditions. Methods A total of 330 elderly people from three communities in Tai'an were selected and investigated by self-made questionnaire of the integrated services needs for medicine, pension, rehabilitation and care of community elderly. Results Nearly 83.54%(259/310) of the elderly had moderate and above pension needs, and the first three were rehabilitation services, family beds and community public health services. Age, educational level, self-rated health and number of chronic diseases were the influencing factors of the integrated service needs of medicine, pension, rehabilitation and care(t =2.374-4.844, all P<0.05). Conclusions The majority of elderly people have the moderate and above health care integration service demand, the elderly have the highest demand for rehabilitation services. Age, education, self-rated health status and the number of chronic diseases have different influence on the needs of the elderly;government, society, education and medical treatment institutions interdisciplinary multi linkage, and strengthen the professionals training of health, rehabilitation, health care, in order to explore the best old-age service model in line with national conditions.

ABSTRACT

Objective This paper reviews the research context of the fear of cancer recurrence abroad, and presents the research hotspots and frontiers in this field, which will further provide references for the research of the fear of cancer recurrence in China. Methods Web of Science database was retrieved,the time of papers published, the number of publications and periodicals were analyzed.The Map drawing was accomplished with the help of Histcite software. Results Total 5322 effective literatures were investigated,most of the papers are published by the United States, Butow P was the high yield authors. and the core journal is "PSYCHO-ONCOLOGY". High frequency key words mainly concentrated on cancer, breast cancer, prostate cancer, quality of life, prognosis, chemotherapy (symptom distress and management). Conclusion In the aspect of fear of cancer recurrence, Europe and America are in a leading position, compared with the international level,there is still a gap in the field of FCR in China , the depth and breadth of research needs to be extended further, We should keep path with the international FCR research hotspot, to broaden our research scope and improve the quality of related research.

ABSTRACT

Objective To validate the effectiveness of nursing intervention on alleviating negative emotions and improving quality of cancer patients′ life based on story theory. Methods This study adopted the method of quasi-experimental research,100 patients with cancer chemotherapy were divided into the experimental group(50 cases),and the control group(50 cases).Based on conventional care,the experimental group received 3 weeks of nursing intervention based on story theory.The scores of mood and quality of life were collected in the 1st day and 23rd day after chemotherapy by using Profile of Mood States-Short Form(POMS-SF)and Quality of Life Questionnaire-core(QLQ-C30),thus evaluated the effect of intervention. Results The scores of POMS-SF in the 1st day and 23rd day after chemotherapy were (17.28±5.41),(35.84±12.35)points in the experimental group,and the difference was significant(t=7.54, P<0.05).The scores of QLQ-C30 in the 1st day and 23rd day after chemotherapy were(7.72±1.53),(8.17± 0.94) points in the control group, and the difference was significant(t =-4.07, P<0.05). There was no statistical difference in the scores of POMS-SF and QLQ-C30 between the 1st day and 23rd day after chemotherapy in the control group(P>0.05). Conclusions The nursing intervention based on story theory demonstrated significant improvement of negative emotions and quality of life, which is a quite practical psychological care for cancer chemotherapy patients.

ABSTRACT

Objective To explore the effects of three kinds of respiratory training methods on pulmonary function and quality of life in patients with thoracic cancer radiotherapy. Methods A total of 76 patients with thoracic cancer were divided into the control group (20 cases), the retracting-lip breathing group (19 cases), the blowing balloons group (18 cases), and the resisting respiratory exercise group (19 cases) by the draw method. Based on conventional health services, the retracting-lip breathing group, the blowing balloons group, and the resisting respiratory exercise group were given respiratory training (one month), whereas the control group only followed routine nursing care. The pulmonary function and quality of life were measured at the first and the thirtieth day of radiotherapy by electronic pulmometer and quality of life instruments for cancer patients-general module respectively. Results The indicators of pulmonary function at thirtieth day of patients in each group were lower than those at the first day, the intragroup differences were significantly (t=2.17-6.32, P0.05). Scores of quality of life of the retracting-lip breathing group were (78.74±5.51) points, the blowing balloons group were (78.43 ± 7.41) points, the resisting respiratory exercise group were (79.11 ± 8.84) points, which were significantly increased than that of control group (73.01 ± 7.46) points(F=3.00, P < 0.05). Conclusions The pulmonary function of patients with thoracic cancer radiotherapy significantly reduces after radiotherapy. Three kinds of respiratory training show no effects on pulmonary function, but the quality of life of patients with thoracic cancer radiotherapy could be improved.

ABSTRACT

Objective To describe the impact of administrative penalties for medical disputes on the medical staff, including their perception on the penalty and its impact on their mind and body, professional behavior, career development, etc, and to provide a theoretical basis for health policy making and hospital management. Methods The phenomenological research method was adopted to conduct in-depth interview to 10 hospital staff in Taian who were involved in medical disputes and got punished, and the Giorgi qualitative data analysis was used in the research. Results The interviewees′perception and impacts of the punishment were concluded into five aspects:physiologically, psychologically, cognitively, behaviorally and professional development. Conclusions It is concluded that the current punishment to individuals has caused quite negative effects, and a new mechanism should be established to systematically reconstruct the procedure and reduce the medical disputes to the minimum.

ABSTRACT

Objective To describe the level of self-perceived burden (SPB) and analyze its influencing factors among the patients with advanced lung cancer, in order to provide a theoretical basis for individualized nursing care. Methods A total of 102 hospitalized patients with lung cancer from 3 hospitals in Shandong province were investigated by the Self-Perceived Burden Scale for Cancer Patient (SPBS-CP). Results The total score of the SPBS-CP was (60.31±17.06) points, which indicated at a moderate level. Those patients rated the highest score on the sub-scale of economic/family burden (22.04±6.72) points, the lowest score on the sub scale of care burden (10.28±3.62) points. Single factor analysis and regression results showed that different work conditions, different stages were related with SPB. Conclusions Patients with moderate and advanced lung cancer had a moderate or higher level of SPB. Staff nurses should focus on the psychological status of the patients and take positive interventions to release the negative emotional experiences.

Objective To investigate the level and influencing factors of stigma among patients with lung cancer, in order to provide evidence for staff nurses to take pertinent interventions for assisting patients to deal with stigma. Methods Totally 234 hospitalized patients with lung cancer were cross-sectional survey with General Demographic Questionnaire, Disease Information Questionnaire, Chinese version of Cataldo Lung Cancer Stigma Scale (CLCSS-CV). Results The total score of stigma was (72.35±10.85) points. The total scale score and each subscale score was divided by the item number. The mean score of CLCSS-CV was (2.68±0.40) points, the rate of the patients with lung cancer with moderate above stigma was 51.20% (120/234). The patients rated higher scores on the subscales of smoking (2.90±0.58) points, while lower scores on the subscales of discrimination (2.47 ±0.55) points. Multiple stepwise regression analysis showed that the number of children and the clinical stages were influencing factors of lung cancer stigma, where regression coefficients were -0.065, 0.136 respectively, coefficients of determination was 0.178. Conclusions Patients with lung cancer have a moderate or higher level of stigma. The medical staff should communicate with patients, evaluate the psychological status, impose public health education, mobilize the social support for assisting them to deal with stigma.

ABSTRACT

Objective To develop pain assessment scale for cognifively impaired elderly (PACIE) and to test its reliabihty and validity. Methods Based on the foreign related pain assessment tools, the pain as-sessment scale for cognitively impaired elderly was developed. Five experts specialized in pain, anaesthesia and nursing were invited to evaluate the content validity of the pain assessment instrument and made modification. Reliability and validity of the pain assessment instruments was evaluated. Results PACIE had 8 items, index of content validity (CVI) of PACIE was 0.978, factor analysis demonstrated that two factors soluted in PACIE, that cumulative explained 69.008% of the total variability, the two factors were named pain objective factor and pain subjective factor. PACIE and NRS were significantly correlated, it could identify pain and painlessness patients overtly that demonstrate the discriminate validity was fine. Adequate levels of inter-rater reliability of PACIE was achieved between two raters with the intra-class correlation coefficient as 0.954 (95%CI=0.812~ 0.989). Satisfactory test-retest reliability was achieved with the Cronbach'α ot as 0.851. Conclusions The pain assessment scale for cognitively impaired elderly (PAC1E) has satisfactory validity and reliability, is suit-able for assessing the pain indentity of cognitively impaired elderly.

Results The project had appropriate indicators and objective and reasonable weight.By applying this system,the general state of intemship students asessod by the clinical teachers and the head nurses was evaluated fuzzily,which was in accordance with the actual performance of students.Conclusions The index system with the characteristics of modern practice teaching concepts combines comprehensiveness with innovation,importance with completeness,accords with the teaching goals of nursing students,and the fuzzy evaluation of that will be better to evaluate the internship quality of nursing students comprehensively and objectively.
