ABSTRACT

@#Objective To develop a framework and indicators system for disability-related service data in China. Methods Using International Classification of Functioning, Disability and Health (ICF) framework and approach, the framework and indicators system were developed using content analysis, logical reasoning and expert consultation. Results A system with nine aspects, 35 dimensions and 115 indicators was established. Eleven experts with disability-related background all accepted the system, and satisfied in the importance and operability.Conclusion A data framework and indicators system with nine aspects, 35 dimensions and 115 indicators has been established for disability-related service, which can be used in further data collection.

ABSTRACT

@#Objective To explore the rehabilitation needs and its influential factors of adults with physical disability in Shanghai, and to provide evidence for decision-making of the precise rehabilitation service. Methods From November to December, 2016, the rehabilitation demands, utilization rate and its influential factors of 390 registered people with physical disability were investigated with a questionnaire. Results The service of occupational rehabilitation met 62.34%and educational rehabilitation met 52.00%of the needs of adults with physi-cal disability at the age of 18 to 40 years. The service of occupational rehabilitation met 38.71%and educational rehabilitation met 35.29%of the needs of adults with physical disability of grade two. The main obstacle to the service utilization wasDo not know(67.34%). As a part ofotherreason, the cumulative frequency oftemporarily not neededaccounted for 16.41%in all reasons. Conclusion The rehabili-tation service should focus unmet needs to complete the community rehabilitation and health governance, and optimize the service system.

ABSTRACT

Objective To analyze the current situation of institutional care for people with disabilities in Shanghai, China. Methods Sec-ondary data were collected from the synthetical information platform of Shanghai Disabled Persons' Federation, including data about the res-idential institutions and people with disabilities living in the institutions from 2004 to 2015. Results The number of residential institutions and people with disabilities showed an increasing tendency from 2004 to 2015. At the end of 2015, there was 368 residential institutions in Shanghai, providing institutional care for 5821 people with disabilities. People in the institutions were mainly with intelligence disability and mental disability, and mainly in 35 to 59 years old. Conclusion The institutional care for people with disabilities was generally in a good condition in Shanghai and met people's needs.

ABSTRACT

@#Objective To analyze the effect of assistive device adaption on quality of life of people with disabilities in 2 districts in Shanghai. Methods A survey on disabled people's quality of life was conducted in 2 districts in Shanghai in September, 2011. A total of 326 people with disabilities completed the SF-12 Questionnaire. Results Assistive device adaption significantly improved their quality of life (P< 0.001). The shorter the years of being disabled was, and the lower the income was, the better the improvement of physical health and psychological health was. Conclusion An early-intervention and constant-revisit service system should be established. More attention should be paid to the low-income family which has disabled members. It is needed to improve the tools for assessment of quality of life of people with disabilities.

@#Objective To study the effects of assistive devices adaption on activities of disabled persons in 2 districts in Shanghai. Methods 326 disabled persons who accepted the assistive devices adaption in 2 districts in Shanghai in September, 2011, were investigated with the self-designed questionnaire. Results Limb disability was the most of these disabled people, and 2/3 of them stayed in disabled condition more than 10 years. More than 70% of them agreed that assistive devices adaption improved their activities and more than 80% of them were satisfied with the effect of barrier-free remodification. Limb disabled persons benefited from the significant improvement of activities. Conclusion Assistive devices adaption is important to improve the activities of disabled persons.

@#ObjectiveTo analyze burden and tolerance of caregivers of elders with dementia in Shanghai.MethodsOn the basis of surveyed before, total 1271 caregivers of elders with dementia were investigated with the questionnaire that concerned the demographic characteristics, influence on body, psychology, life and tolerance.ResultsNearly 60% of the caregivers felt stress, nearly 40% of the caregivers thought that mental pressure had a negative impact on their health as a result of caring patients. Most of the caregivers responded with tolerance to non-adaptive behavior.ConclusionDementia caregivers are old, the burden is heavy. It is necessary to establish a social support system for elderly with dementia as soon as possible.

@# ObjectiveTo evaluate probability and severity of behavioral and psychological symptoms in dementia (BPSD) in elderly with dementia in Shanghai.MethodsOn the basis surveyed before, total 1271 elderly with dementia were investigated with the international neuropsychiatric inventory (NPI) to evaluate BPSD.ResultsThe proportion of elderly with dementia that had BPSD was 50.95% , it was 32.17% for clinical significant disturbance (CSD). The symptoms had greatest impact on the caregivers were paranoia (2.32±1.48), followed by abnormal motor behavior (2.30±1.31) and hallucinations (2.16±1.25). The severity rating included mild 620 cases, moderate 238 cases, and severe 406 cases.ConclusionThe incidence rate of BPSD in elderly with dementia was high. The standard of severity grading should be selected with purposes.

ABSTRACT

@#ObjectiveTo investigate the epidemiology of senile dementia in Shanghai.Methods15158 residents aged 65 or over were selected by stratified random cluster sampling from 3 districts in Shanghai. Two stages was carried to ascertain dementia: in the first stage, Mini Mental State Examination (MMSE) and Activity of Daily Living Scale (ADL) were used as screening tools; in the second stage, DSM-Ⅳ criteria for dementia was applied to diagnosis.ResultsScreening positive rate of MMSE and ADL are both 9.9%. 1271 cases of elderly with dementia were identified. The crude and age-standardized prevalence in residents aged 65 or over was 8.1% and 6.1%. The female, older age, lower education and worse marriage seemed to be more susceptible to senile dementia(P<0.01).ConclusionSenile dementia has become one of major diseases harmful to elderly's health.

@#ObjectiveTo explore the risk factors related to senile dementia to develop intervention method.MethodsA combination of qualitative and quantitative methods was used to analyze the factors and their degree related to the incidence of dementia for the elderly.ResultsAgeing was the deterministic factor to senile dementia, and vascular factor was common risk for Alzheimer's disease (AD) and vascular dementia (VD).ConclusionActive intervention in early life, concern about women's groups especially in the mental health, and detemination of value range of the risk factors are needed.

@#ObjectiveTo analyze application of screening and assessment scales for senile dementia in China.MethodsVarieties of screening and assessment scales were found in the studies on screening and assessment of senile dementia in China, as well as their common features and epidemiological results. A combination of qualitative and quantitative methods was used in the analysis.ResultsMini Mental State Examination (MMSE) is preferred tool in screening and Activity of Daily Living Scale (ADL) is the secondary.ConclusionThe future research direction should include: studying the appropriate boundary value in MMSE for China; optimizing the combination of screening scale and developing more appropriate screening tool.

@#ObjectiveTo analyze past study on senile dementia in China, and shape the foundation for constructing social support system for elderly with dementia.MethodsA combination of qualitative and quantitative method was used to analyze 406 literatures related to the elderly with dementia published in China since 2000.ResultsDiagnosis method for the senile dementia has matured; but the causes of the disease is still not clear and there is no thorough remedy; study on early intervention, nursing care and social support are inadequate.ConclusionDiagnostic criteria should be standardized, community screening assessment should be done on a regular basis; etiology based on risk factors should be strengthened; systematic study on intervention should be carried out; training on home care techniques should be enhanced. Resolving the care of the elderly with dementia depends on the social support system construction eventually.

ABSTRACT

@#Objective To create the target value of third-degree index and the weight coefficient of evaluation index system of rehabilitation service for all disabled in Shanghai. Methods Compositive quantitative and qualitative research methods, such as document analysis, Delphi method, questionnaire survey, specialist consultation were carried in the study. Results The enthusiasm coefficient of specialist in three times were 100%, 100%, 92.3% respectively, the degree of specialist authority was 0.84, the coordination coefficient of specialist for the first-and second-index were 0.79 and 0.72 respectively. The weight of the more important first-degree index namely community rehabilitation, outlay support, organization network and policy environment were 0.1836, 0.1781, 0.1360 and 0.1274.Conclusion The enthusiasm, authority and coordination of specialist of this evaluation index system are high. The index weight can offer evaluation basement.

@#Objective To investigate the status of rehabilitation demands and services of persons with disability in rural area in Shanghai.MethodsXinhe town of Chongmin county was selected as the sample by stratified-cluster random sampling method, and the rehabilitation demands and services of 1128 persons with disability were surveyed.ResultsThe realization rate of rehabilitation demands in Shanghai suburb was 81.3%. The realization rate of different rehabilitation demands was different. Only 4.0% persons with disability had obtained assistive devices.ConclusionIn order to improve the realization rate of rehabilitation demand, more rehabilitation relief, balanceable development of rehabilitation service and assistive devices service should be enhanced.

@#This paper introduced the project of "the Study on the Evaluation Index System of Rehabilitation Service for All Disabled in Shanghai" in brief, including research background, objectives, researching objects and sampling. The project aimed at constructing the evaluation index system of rehabilitation service for all disabled in Shanghai. According to the research purpose, the persons with disability in 19 regions (or counties) and 2 towns were investigated with 12 consultation conference and 3 Delphi's surveys to find out the rehabilitation demand and service. The Evaluation Index System of Rehabilitation Service for All Disabled in Shanghai that finally comes into being includes 8 first-degree index, 32 second-degree index and 100 third-degree index.

@#Objective To investigate and assess the current situations of propagandizing and inculcating rehabilitation information in Shanghai. Methods The propagandizing and inculcating rehabilitation information in 6 districts in Shanghai were assessed in field, the fulfilling of the first- and second-degree index and the standard-reaching rate of the third-degree index were calculated separately according to the first-degree index- propagandizing and inculcating rehabilitation information of the Evaluation Index System of Rehabilitation Service for All Disabled in Shanghai.Results The average fulfilling degree of the first-degree index was 82.3%. There was 1 district reached the standard of Propagandizing Rehabilitation Information by Broadcast, Television, Newspaper, Magazine and Internet; the average fulfilling degrees of the Community Rehabilitation Function for the Disabled and the Community School Propagandizes and Inculcates the Rehabilitation Information for the Disabled are 66.7% and 33.3%, respectively. Conclusion The support from the government could make propagandizing and inculcating rehabilitation information carry out more successfully; utilizing the media adequately is important; propagandizing and inculcating rehabilitation information must be strengthened in community.

@#Objective To investigate and assess the current situations on information management of rehabilitation service in Shanghai. Methods The information management in 6 districts in Shanghai were assessed in field, the fulfilling of the first- and second-degree index and the standard-reaching rate of the third-degree index were calculated separately according to the first-degree index- information management of the Evaluation Index System of Rehabilitation Service for All Disabled in Shanghai. Results The average fulfilling degree of the first-degree index was 73.1%. There were only 2 districts reached the standard of Registration of Rehabilitation. The average fulfilling degrees of the Development and Dynamic Maintenance of Rehabilitation Service Information Database were 50.0% and 66.7%, respectively. The standard-reaching rate of the Disabled Children First-diagnose Report System, Effectual Providing Referral Rehabilitation Service Across-the-board, Suspicious Disabled Persons Report System in Community were 16.7%, 16.7% and 50%, respectively. Conclusion The development and supervision of rehabilitation service information should be reinforced and the referral rehabilitation service should be improved.
