ABSTRACT

OBJECTIVE@#To compare the hemostatic effects of local packing of Nasopore combined with hemocoagulase injection and local packing of Nasopore combined with saline injection for postoperative management of functional endoscopic sinus surgery by a double-blind, randomized control clinical trial.@*METHOD@#Sixty-eight cases of chronic sinusitis needed functional endoscopic sinus surgery were randomly divided into the experimental group of 40 cases and control group of 28 cases, respectively. For the experimental group, 1 U of hemocoagulase dissolved in 0.5 ml saline was injected into Nasopore which was packed into the nasal cavity after operation. For the control group, 0.5 ml of saline was injected. The postoperative bleeding of the two groups were scored by visual analogue scale.@*RESULT@#There was statistically significant difference between the bleeding VAS scores assessed 6 hours and the ones assessed 1, 2 and 3 days after the operation in the control group (P < 0.05). There was the statistically significant difference between the bleeding VAS scores of experimental group and control group assessed 6 h after the operation (P < 0.05).@*CONCLUSION@#The hemocoagulase may improve the hemostatic effect of Nasopore 6 hours after the operation by combined injection with Nasopore as nasal cavity packing.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the association of rs9272346 polymorphism of HLA-DQA1 gene with clinical outcome of hepatitis B virus (HBV) infection in ethnic Han population from Hubei, China.</p><p><b>METHODS</b>A case-control study was conducted, which have involved 1028 unrelated subjects including 238 asymptomatic HBV carriers (AHC), 173 acute liver failure (ALF), 292 liver cirrhosis (LC) and 325 hepatocellular carcinoma (HCC). Genotypes of rs9272346 were determined by real-time polymerase chain reaction with a TaqMan MGB probe. Statistical results were analyzed using Chi square test, student's t test and unconditional logistic regression.</p><p><b>RESULTS</b>No significant differences were detected in the frequencies of G allele between ALF, LC, HCC and AHC groups (P= 0.312, 0.314, 0.264). Compared with the AA genotype, the GG and GA genotypes were not associated with the patients groups under the dominant model: ALF group vs. AHC group (adjusted OR= 1.08, 95%CI: 0.7-1.68), LC group vs. AHC group (adjusted OR= 1.11, 95%CI: 0.87-1.26), HCC group vs. AHC group (adjusted OR= 0.93, 95%CI: 0.65-1.33). For women, the GG and GA genotypes have conferred a protective effect for the outcome of ALF (OR= 0.30, 95%CI: 0.1-1.87).</p><p><b>CONCLUSION</b>Our results suggested that rs9272346 polymorphism of HLA-DQA1 may not independently influence the outcome of HBV infection in ethnic Han Chinese in Hubei, while the GG and GA genotypes may confer a protective effect against ALF in women.</p>

ABSTRACT

Objective To investigate the influence of VEGF expression on the apoptosis of pancreas subjected to ischemia/reperfu-sion injury. Methods Thirty male SD rats were randomly divided into three groups (rt = 10). Group A was served as sham-operation group. Groups B were subjected to 30 min of ischemia by clamping of celiac artery and superior mesenterie artery then releasing for 6 hours to produce ischemia/reperfusion injury model. Groups C were treated with VEGF antisense oligodeexynueleotide after isehemia. Rats were sacrificed, the pancreas was obtained to detect the VEGF expression by immunohistochemical method, and apoptosis was determined by TUNEL staining. Results Apoptosis appeared and VEGF expression up regulated in pancreas after Isehemia/reperfusion injury. Comparison between Groups C and Groups B showed a significant down regulation of VEGF expression (P <0. 05) and a notable increase of apoptotic index (P < 0. 05). Conclusion VEGF expression suppresses apeptosis of pancreas during the course of ischemia/reperfusion injury and may play an important role in protection of pancreas against the ischemia/repeffusion injury.

ABSTRACT

OBJECTIVE@#To investigate the advantages and disadvantages between two surgical approaches of trans scalp coronary incision (TCI) and functional endoscopic sinus surgery (FESS) by which front-ethmoid-orbital masses being resected.@*METHOD@#Thirty-four cases of front-ethmoid-orbital mass from 1995-2006 were retrospectively studied, among which lesions of 18 cases resected by FESS, lesions of 16 cases via TCI. The follow-up time varied from 2 to 4 years.@*RESULT@#Five of 18 cases by FESS had residual dizziness or nasal pain, one case recurred 2 years after the operation. None of TCI cases experienced recurrence, whereas dizziness presented in 3 cases, regional dull sensation of the operative scalp in 12 cases.@*CONCLUSION@#FESS and TCI approach has its own benefits and shortcomings thus has its indications respectively.

ABSTRACT

OBJECTIVE@#To provide transsphenoidal endoscopic surgery with the anatomic reference through anatomic study on the lateral wall of sphenoid sinus under transnasal endoscope.@*METHOD@#The dissection of the lateral wall of sphenoid sinus were performed on 10(20 sides) damp cadaveric heads of Chinese adults via transsphenoidal approach. The anatomic relationship of the lateral wall of sphenoid sinus and its adjacent structures were observed. The distance and angle from the bony prominence of optic canal and internal carotid artery to the columella nasi and midline of sellar floor were measured by ruler and protractor respectively.@*RESULT@#From the study of endoscopic anatomy, the specific structure of optic canal and internal carotid artery were found, and the lateral wall of the cavernous sinus could be exposed by tracing the internal carotid artery. The mean distance from the midpoint of the medial wall of optic canal orbital aperture and the anterior bony prominence of internal carotid artery to the columella nasi were (75. 33 +/- 5. 59)mm and (81. 02 +/- 5. 29) mm respectively, to the midline of sellar floor were (5. 81+/- 1. 52)mm and (5. 53 +/- l. 47)mm respectively. The angle from the midpoint of the medial wall of optic canal orbital aperture to columella nasi was (53.4 +/- 4. 1) degrees.@*CONCLUSION@#Taken the columella nasi and midline of sellar floor as the reference points, the anatomic observation and measurement under endoscope via transsphenoidal approach can provide a guide for surgeons.
