ABSTRACT

Objective ToexploreimagingfeaturesanddiagnosticvalueofCTonpulmonarycomplicationsafterhematopoietic stemcelltransplantation (HSCT).Methods ThoracicCTimagesoffiftyGfourpatientswithpulmonarycomplicationafter HSCT werecollectedandtheimagingfindingswereretrospectivelyanalyzed.Results Accordingtoclinicaldiagnosis,fiftyGfourpatientswere classifiedasfungalinfection (30cases),cytomegalovirus (CMV)pneumonia(6cases),bacterialpneumonia(5cases),diffusealveolar hemorrhage(DAH)(4cases),pulmonaryedema(2cases),idiopathicpneumoniasyndrome(IPS)(1cases),bronchiolitisobliterans (BO)(3cases)andbronchiolitisobliteransorganizingpneumonia (BOOP)(3cases).OnCT,thefungalinfectionappearedasscattered nodule,consolidationorgroundglassopacity,andaircrescentsignwasseeninsixcases.Thebacterialpneumoniaappearedaspatchy, lineandgroundglassopacityorlargeconsolidation.TheDAHappearedasdiffuseexudate,alveolarnodularorconsolidation.Thepulmonary edemaappearedastypical"batwing"pattern.TheCMVpneumoniaandIPSappearedasdiffusegroundglassopacity,nodular,consolidation oratelectasis.TheBOshowedlargegroundglassopacitywithdilatedbronchioles,whiletheBOOPappearedaspatchyconsolidation andgroundglassopacity.Conclusion Patientsafter HSCT mayhavepulmonaryinfectiousandnonGinfectiouslesions.CTfeatures combinedwithclinicalsymptomsandpostGtransplantationtimecanhelptoevaluatecomplicationsaccuratelyandreducemortality.

ABSTRACT

Objective:To explore the diagnostic value of color soppler flow imaging(CDFI) be applied to pseudoaneurysms in different parts of the body.Methods: Retrospectively analyzed clinical data of 29 pseudoaneurysms patients who were diagnosed by Color Doppler Flow Imaging during january 2012-october 2013, to discuss the features of two-dimensional echocardiography and CDFI signal,all the diagnostic results were compared to the operation results.Results: The biggest lump among 29 patients is 6.3 cm×11.8 cm×12.5 cm, located in femoral artery, the smallest one is 2.3 cm×2.8 cm×0.5 cm, located in carotid artery,all the patients were confirmed by operations, The coincident diagnostic rate was 100%, 39 Pseudoaneurysms had been found in total, among these, the number of patients have single Pseudoaneurysms were 22, about 75.86%, 4 patients have one more pseudoaneurysms, about 13.79%, 3 patients have two more pseudoaneurysms,about 10.34%. 23 breachs have been found by two-dimensional echocardiography, and 6 breachs haven’t been found, but all the breachs have been found by CDFI. Back and forth movement flow pattern was the main diagnostic sign.Conclusion: Color doppler flow Imaging is safe and noninvasive,and have high diagnostic value in diagnosing pseudoaneurysms, it is the preferred method in diagnosis.

ABSTRACT

Purpose Protein is the main influencing factors for diffusion weighted imaging (DWI) signals and apparent diffusion coefficient (ADC), it results in hyperintensity on DWI and low ADC, but not fully matched in clinic. This paper aims to investigate the effect of protein type and concentration on the signal intensity (SI) and ADC of DWI. Materials and Methods Different concentrations of albumin, globulin solution and the mixed solution were created in vitro. DWI was performed on GE 1.5T superconducting nuclear MRI system. Results ① There was a linear negative correlation between the ADC value and the concentrations of protein solution (at 37℃, ra= - 0.849, Pa0.05; at 40℃ , r= - 0.270, P>0.05). ② SI of the albumin solution was significantly higher than the globulin solution at the same concentration and temperature (t=3.96, P0.05). Conclusion The nature of the cystic fluid can be understood preliminarily through quantitative analysis of the cystic fluid DWI and ADC values, so as to provide theoretical basis for the qualitative diagnosis of cystic lesions in vivo.

ABSTRACT

OBJECTIVE@#To propose the concept of rhinogenic otitis media and explore its pathomechanism through analyzing the diagnosis and treatment on secretory otitis media caused by unhealthy nasal cavity structure.@*METHOD@#Conservative treatment and correlative operation under nasoscope were undertaken in 176 otitis media patients with unhealthy nasal cavity structure.@*RESULT@#Of 176 cases, 156 cases recovered completely (88.64%), 18 cases got effective treatment (10.23%), and 2 cases got ineffective treatment (1.14%).@*CONCLUSION@#One important cause of the secretory otitis media is unhealthy nasal cavity structure, so correcting the unhealthy nasal cavity structure is the main ways to treat rhinogenic otitis media.

ABSTRACT

Objective To study the activated status of insula during the performance of word generation task, and to explore the function of different part of insula with functional MRI (fMRI). Methods Twenty-eight subjects were recruited in this study (male 15, female 13), all of them underwent block-designed fMRI with word generation tasks and resting-state scan. SPM 5 and REST 1.3 were used to process the data. Bilateral anterior insula and posterior insula were selected as seeds to calculate the connectivity coefficiency with other voxels, and differences between the anterior and the posterior insula were compared. Results Bilateral anterior insula was significantly activated, while bilateral posterior insula was significantly deactivated. The functional connectivity areas with left anterior insula included right anterior insula, right posterior insula, supplementary motor area (SMA), left superior temporal gyrus, left middle frontal gyrus, left superior frontal gyrus, left inferior parietal lobe, middle cingulate gyrus, right striatum and right inferior frontal gyrus. The functional connectivity areas with left posterior insula included right posterior insula, left anterior insula, right superior temporal gyrus, middle cingulate gyrus, right precentral gyrus and right striatum. The functional connectivity areas with right anterior insula included SMA, left inferior frontal gyrus, right inferior parietal lobe, left inferior parietal lobe, left superior temporal gyrus, right precentral gyrus, right striatum, middle cingulate gyrus, left middle frontal gyrus, left striatum, right middle frontal gyrus, right inferior frontal gyrus and left transverse temporal gyrus. The functional connectivity areas with right posterior insula included right precentral gyrus, left superior temporal gyrus, left anterior insula, left posterior insula, right supramarginal gyrus and middle cingulate gyrus. Conclusion Anterior insula and posterior insula are functionally connected with different areas, and concerned with the language function in different manners. Left lateral anterior insula may play an important role in the language function.

ABSTRACT

Objective To evaluate the MRI features of neuropsychiatric abnormalities in systemic lupus erythematosus(NPSLE).Methods Brain MRI images of 21 cases with NPSLE confirmed by clinic were analyzed retrospectively.ADC values of the lesions and normal brain tissue were measured.Results Brain abnormal MRI findings in 20/21 cases were found(95%)including:(1)High signal intensity on DWI in 14 cases,11 were diffuse lesions,3 were focal lesions,4 of 14 combined with brain atrophy.The lesions mostly localized in white matter.The ADC values of the lesions in gray matter were decreased(t=2.513,P=0.019),while the ADC values of the lesions in white matter were increased(t=2.877,P=0.007).(2)2 cases only showed brain atrophy.(3)Leukoaraiosis presented in 2 cases.(4)Encephalomalacie presented in 2 cases,1 accompanied with brain atrophy.(5)No enhancement(3/5)and little patchy enhancement(2/5)were observed at contrast MRI study.Conclusion MRI plays an important role in displaying cerebral lesions and the location of it,progression and succession of NPSLE.

ABSTRACT

Objective To evaluate the activation of premotor area (PMA) in verb generation task, and to discuss the possible function of PMA in language expression. Methods Block-designed fMRI with verb generation task was performed on 23 subjects with GE 1.5T MR Scanner. During the test, the subjects were asked to generate a verb based on a given noun word. The white + appeared on the center of the black screen was used as control. The fMRI data were processed with SPM 2. Group analysis was performed with single sample t-test. Average mapping was obtained and overlapped onto standard MNI template. Activation of the PMA was analyzed. Results The fMRI data of eleven subjects were selected for group analysis after head motion effect was ruled out. Average mapping showed activation in the Broca's area, posterior part of the right inferior frontal gyrus, bilateral PMA and supplemental areas (SMA), left posterior parietal cortex, right thalamus, left basal ganglions, right cerebellum, and posterior part of the right temporal lobe. The area with the greatest activated intensity in the brain was the left PMA. Conclusion PMC is important in verb generation and may be responsible for voice processing, motor imagery, word extracting as well as advanced regulation of information.

ABSTRACT

OBJECTIVE To improve the knowledge on the early diagnosis and treatment of the isolated fungus ball sphenoid sinusitis.METHODS A retrospe ctive study was made on10cases of isolated fungus ball sphenoid sinusitis to discuss the clinical manifestation,radiographic features and treatment with endoscopic sinus surgery.RESULTS The common symptoms in10cases were headache and/or bloody nasal discharge.Radiographic features of the disease were a soft tissue image occupying the sphenoid sinus with bones wall thickening.A soft tissue image with calcification in the sphenoid sinus was the feature of CT image.All the patients were treated by nasal endoscopic sinus surgery and cured after followed up for3to18.CONCLUSION Isolated fungus ball sphenoid sinusitis was difficult to diagnose in early stage because of its non-specif ic symptoms.The nasal endoscopy and CT scan make the early diagnosis possible.The diagnosis can be def initely confi rmed by pathological studies.Treatment with endoscopic sinus surgery is an effective method for fungus ball sphenoid sinusitis.

ABSTRACT

Objective To research and construct dynamic electrocardiograph (ECG) simulation model/algorithm to simulate different kinds of clinical ECG,where the heart rate can be infinitely variable,and to develop one platform for fast-demo and analysis of theoretical wave based on this algorithm which can be implemented in ECG training and teaching. Methods According to different characteristics decoupled one complete ECG wave to 4 parts,including P-wave,PR-section,QT -wave and TP -section. P -wave and QT -wave database were set up accordingly to find the relationship algorithm between PR-section and heart rate,TP-section and heart rate. With the synthesis work of the algorithm and clinical diagnosis theory,ECG simulation mathematical model /algorithm,dynamic link library.(dll) and software interface were developed by C++. Results Through comparing the ECG simulation results with clinical samples,the veracity and validity of the mathematical model/algorithm was verified. Conclusion The software module based on this model/algorithm is very valuable for ECG training and teaching in clinic,and can be implemented in the development of more advanced ECG relative software and devices.

ABSTRACT

Objective To evaluate the prevalence of NI in the SICU at our hospital. Methods 181 NI patients in the SICU were retrospectively analysed during Jan 1996~Dec 2000.Results The average NI rate was 9.81%. The major sites of NI were respiratory tract(36.96 %),thoracic/abdominal cavity(25.47 %)and bloodstream infections(9.32 %).The difference in major pathogens of infections in different sites reached statistical significance. For respiratory tract, thoracic/abdominal cavity and bloodstream infections,bacteria were the most common pathogens. Fungi were the moat frequent isolate from urine and stool. Mixed infection proportion was 52.25 %. The most common pathogens were Enterococci, Methicillin resistant Staphylococci、 Pseudomonas Aecruginosa、Escherichia Coli、Candida Albicans and Candida Tropicalis. Conclusions The most common pathogens of NI in SICU are different in different infection sites. The pathogens were complicated and most strains are antibiotics resistant. So it is important to establish NI control and to understand the changes of pathogens so as to prevent the infection.
