ABSTRACT

Objective:To make a self-made fall/fall protection cover and explore its application effect.Methods:Five high-risk fall wards in Songjiang District Central Hospital were selected as the pilot wards. According to the length of stay, 80 elderly patients treated from June to December in 2020 were selected as the control group and 80 patients treated from January to June in 2021 were selected as the experimental group. The control group was treated with routine fall prevention / bed falling intervention measures, and the experimental group was treated with self-made fall prevention / bed falling protective cover on the basis of routine intervention measures. The incidence of falling/falling bed, physical restraint rate, comfort and satisfaction of hospitalized patients were compared between the two groups.Results:During the study period, there was no fall in the experimental group and one patient in the control group. The incidence of fall was 0.125 ‰ (1/8 000) ( t=-0.06, P>0.05). The rate of physical restraint was 8.75%(875/10 000) in the experimental group, and 26.25%(2 100/8 000) in the control group, the difference was statistically significant( t=-5.51, P<0.05). The scores of comfort and satisfaction were (93.43 ± 13.01), (134.50 ± 15.56) points in the experimental group and (81.68 ± 13.41), (111.88 ± 16.22) points in the control group, the differences were statistically significant ( t=8.03, 12.92, both P<0.05). Conclusions:The self-made fall / fall protection cover can reduce the utilization rate of physical constraints, improve the overall comfort of patients, improve patients′ satisfaction.It has clinical promotion value.

ABSTRACT

Objective To explore the clinical efficacy of erythromycin combined with azithromycin in the treatment of mycoplasma pneumonia in children.Methods 98 children with mycoplasma pneumonia were selected.According to the digital chart,the patients were randomly divided into the observation group and control group,with 49 cases in each group.On the basis of routine treatment,the control group was treated with azithromycin,and the observation group received erythromycin combined with azithromycin.After treatment,the disappearance time of the disease (fever,cough,pulmonary rale),hospitalization time and adverse reactions were observed in the two groups,and the clinical efficacy of the two groups was compared.Results The total effective rate in the observation group was 95.91%,which was significantly higher than that in the control group (79.5%),and the difference was statistically significant (x2 =4.721,P < 0.05).Compared with the control group,the disappearance time (fever,cough,pulmonary rales) and hospitalization time of the observation group were significantly decreased (t =1.965,1.943,1.987,1.952,all P < 0.01).The incidence rate of adverse reactions in the control group was 8.16%,which in the observation group was 12.24%,there was no statistically significant difference between the two groups (x2 =0.445,P > 0.05).Conclusion Compared with azithromycin alone,erythromycin combined with azithromycin is effective and safe in the treatment of children with mycoplasma pneumonia.It is beneficial to shorten the course of treatment and improve the efficacy,and is worthy of further popularization and application.

ABSTRACT

Objective To explore the value of cytological and histological noninvasive methods in the screening of endometrial cancer(EC).Methods Seventy-six cases of suspected EC high risk patients diagnosed in the gynecological outpatient department of the People Hospital of Longhua from January 2015 to May 2017 were selected as the research object,the endometrial cells and tissues were collected for cytological and histological examination,the diagnosis of curettage or hysteroscopy performed as a "gold standard" to determine the effect of cytology,histology and combined examination on the ensitivity,specificity,positive coincidence rate of EC diagnosis.Results 76 patients with suspected EC high risk factors were diagnosed as EC 48 cases(63.16%)via "gold standard".The sensitivity,specificity and positive coincidence rate of cytology were(75.00%,89.29%,80.26%,χ2=1.0105,P=0.6033),histology were(79.17%,92.86%,84.21%,χ2=0.3117,P=0.8557)and combined examination in EC were(83.33%,92.86%,86.84%,χ2 =1.2260,P=0.5417).Conclusion Cytological and histological noninvasive screening has high sensitivity,specificity and positive coincidence rate in EC and has a high diagnostic value for individual or combined exams and can be used as a routine screening for EC in clinical practice.

ABSTRACT

<p><b>OBJECTIVE</b>To study the relationship between compliance with anti-hypertension therapy and assessment of quality of health care in hypertensive patients in community.</p><p><b>METHODS</b>According to multi-stage cluster random sampling, we selected two communities from 36 streets in certain areas of Shanghai respectively using random number method on May, 2013, 72 communities in total, then we randomly selected 25 patients who were archived in the community health center from each community of the street.We conducted 1 800 questionnaires in total in which 1 172 patients were under medication study. The Morisky Medication Adherence Scale was used to evaluate medication compliance while the Patient Assessment of Chronic Illness Care Scale was applied to measuring the quality of patient's self-reported health service. The Spearman rank correlation analysis was used to evaluate the relationship between assessment of quality of health care for chronic illness and compliance with anti-hypertension therapy. Binary logistic analysis was applied to evaluate the factors which influenced patient's medicine compliance.</p><p><b>RESULTS</b>There were 567(48.4%) respondents with good compliance. Age less than or equal to 65 accounted for 43.3% (245 people) while age greater than 65 accounted for 56.7% (321 people). The average scores of total and difference dimensions remained at a low level, ranging from 1.97 to 2.67 out of 5. The Assessment of quality of health care score was (2.67 ± 1.33)scores, in which the highest score of the dimensions was patient activation which was (2.56 ± 1.41)scores and the lowest was follow-up / collaboration which was (1.97 ± 1.49)scores. The Spearman rank correlation analysis showed that negative association between accumulate points of patient medicine compliance and patient activation was found (rs = -0.09, P < 0.01). The result of binary logistic analysis indicated that medication adherence of age greater than 65 was 1.49 times (95%CI: 1.11-2.00) higher than age less than or equal to 65. The patient activation was a protective factors of good medicine adherence (OR = 1.35, 95%CI:1.14-1.59).</p><p><b>CONCLUSIONS</b>The medicine adherence of hypertension patients in community medication compliance is not very good at the moment. The self-rated quality of health care is still relative poor. Positive activation from interaction of physicians and patients can help enhancing patient's medicine compliance.</p>

ABSTRACT

Objective To investigate the quality of life among residents in Pudong New Area and ananlyze the main influencing factors for quality of life among residents. Methods 3 642 residents over 18 years old in Pudong New Area were investigated by SF-16. Results Their scores of PF, RP, BP, GH, VT, SF, RE and MH were 86.94, 78.03, 73.74, 65.86, 72.93, 86.44, 80.01, and 79.19, respectively, and the total score was 77.17. The sex, age, family income and health payout were the influencing factors for quality of life among residents in Pudong New Area by Logistic Regression. Conclusion The condition of general life quality among residents in Pudong New Area was well and that of male residents was better than that of female ones. The psychosomatic factor was the important one influencing life quality. The women should be the target population in community health works and it should pay more attention on mental health education.
