ABSTRACT

El objetivo de este artículo es presentar un caso de raro de tumor pulmonar benigno, hemangioma esclerosante (neumocitoma), en una mujer de 24 años, asintomática, con hallazgo radiológico incidental. Se describen las características clínicas, imagenológicos y anátomo-patológicos de la entidad, con una breve revisión bibliográfica.


We present a rare case of benign lung neoplasia, sclerosing hemangioma (pneumocytoma), in a woman 24 years old, asymptomatic, with radiological incidental find. The clinical, imaging and pathological features of the entity are described, with brief bibliographic review.

ABSTRACT

Introduction: The definitive diagnosis of parathyroid cancer is extremely difficult, from the clinical approach to the molecular diagnosis. A gene mutation was detected recently in patients with parathyroid cancer. It is a suppressor tumor gene called HRPT2, which codifies for a protein that participates in PAF1 complex, the parafibromin. It has been observed that the expression of this protein it's altered in parathyroid cancer, what would serve like method of diagnosis by immunohystochemistry, with a sensitivity and specificity of 73-96 percent and 99-100 percent respectively. Material and Method: The anti-parafibromin immunohysto-chemistry staining was made in 23 parathyroids tissue samples (5 adenomas, 6 hyperplasia, 7 normal and 5 carcinomas). Results: A positive pattern is observed in almost 100 percent of benign pathology and 100 percent in normal tissue. In the cases of carcinoma only 2 of 5 had a strong positivity. Conclusions: The pathological clinical correlation does not allow the association of the loss of parafibromin immunoreactivity in some unequivocal cases of parathyroid cancer. The parafibromin immunostaining does not allow to discriminate between benign or malign pathologies.


Introducción: El diagnóstico definitivo de cáncer de paratiroides es extremadamente difícil, desde el acercamiento clínico hasta el diagnóstico molecular. Se detectó recientemente en pacientes con cáncer de paratirodes un gen supresor de tumor mutado (HRPT2), que codifica para una proteína que participa en el complejo PAF1, la parafibromina. Se ha observado que la expresión de esta proteína está alterada en los casos de cáncer de paratiroides, lo que serviría como método de diagnóstico por inmunohistoquímica, con una sensibilidad y especificidad de 73-96 por ciento y 99-100 por ciento, respectivamente. Material y Método: Se realizó tinción inmunohistoquímica anti parafibromina en 23 muestras de tejido paratiroideo (5 adenomas, 6 hiper-plasias, 7 normales y 5 carcinomas). Resultados: Se observa un patrón positivo fuerte en casi 100 por ciento de la patología benigna y 100 por ciento en tejido normal. En los casos de carcinoma sólo 2 de 5 tenían positividad fuerte. Conclusiones: La correlación clínico patológica no permite asociar la pérdida de tinción de parafibromina en algunos casos de cáncer inequívocos. La tinción de parafibromina no permite discriminar entre patología benigna y maligna.

ABSTRACT

Reportamos caso clínico de una mujer de 22 años quien presentó un fibroadenoma mixoide de localización mamaria. Previamente se había diagnosticado un microadenoma pituitario y tiroiditis con bocio difuso leve. Se realizó un estudio multidisciplinario para descartar otras localizaciones de tumores mixoides en el contexto del infrecuente síndrome de Carney.


We report a 22-year-old woman, who presented a myxoid fibroadenoma affecting the breast. Previously a pituitary microadenoma and thyroiditis with mild diffuse goiter was diagnosed. A multidisciplinary study was performed to rule out the location of other myxoid tumors in the context of infrequent Carney syndrome.

ABSTRACT

El estudio de lesiones tiroídeas con citología aspirativa con aguja fina (PAAF) es el método más eficiente en el diagnóstico de éstas. Presentamos 640 casos consecutivos, estudiados entre 2006-2010, con obtención de muestra por radiólogo bajo ultrasonido y asistencia “in situ” del citopatólogo, quién efectuó frotis concentrados, realizando el diagnóstico inmediato de calidad de muestra. Este método, exclusivo de nuestra clínica a nivel nacional, proporcionó material suficiente en 99,53 por ciento de los casos. Se usó clasificación de Bethesda para categorizar diagnósticos. 77,39 por ciento de los diagnósticos fueron benignos, 9,26 por ciento neoplasia folicular, 9,42 por ciento malignos, y en conjunto 2,93 por ciento correspondieron a lesiones foliculares de significado incierto, atípicas y no diagnósticas. En 75 casos con diagnóstico de carcinoma papilar o neoplasia folicular hubo correlación histológica, con un 87 por ciento de exactitud para neoplasia (100 por ciento en carcinoma papilar).


The study of thyroid lesions with fine needle aspiration cytology (FNAC) is the most efficient diagnostic in this topic. We report 640 consecutive cases studied between 2006-2010, with samples that were collected by radiologist using ultrasound and on-site assistance of the cytopathologist; who made concentrated smears and immediate diagnosis of the sample quality. This method, unique in our hospital in Chile, gave sufficient material in 99.53 percent of cases. Bethesda classification was used to categorize diagnoses. 77.39 percent of diagnoses were benign, 9.26 percent were follicular neoplasm, 9.42 percent were malignant, and 2.93 percent all together belonged to follicular lesions of uncertain significance, atypical and non diagnostic. 75 cases diagnosed as papillary carcinoma or follicular neoplasm, had histological correlation, with 87 percent accuracy for neoplasia (100 percent in papillary carcinoma).

ABSTRACT

Se presenta el caso de una paciente de 41 años portadora de mamas axilares bilaterales que consulta por dolor más masa palpable en una de ellas. La paciente es sometida a resección quirúrgica que mostró que el tumor era un fibroadenoma.


This is a case report, of a 41 years old patient, with bilateral axillar fibroadenoma. The aim of the query was pain and a tangibly mass in one of it. A surgical resection was made, and it showed a fibroadenoma neoplasm.

ABSTRACT

Los tumores de cabeza y cuello corresponden a un heterogéneo grupo de enfermedades, destacando entre ellas el carcinoma epidermoide de laringe, faringe y cavidad oral, así como las neoplasias glandulares (tiroides y glándulas salivales). Constituyen una patología altamente desafiante, considerando la complejidad anatómica de la región cérvico-facial y las repercusiones funcionales y estéticas que pueden producir tanto la enfermedad como su tratamiento. Estos tumores se manifiestan clínicamente como masas palpables o por la producción de síntomas persistentes en la vía aéreo-digestiva superior. La imagenología y los estudios endoscópicos complementan la evaluación del paciente; la histopatología es esencial para determinar el diagnóstico definitivo. El manejo de los tumores de cabeza y cuello debe estar a cargo de equipos multidisciplinarios, siendo la cirugía y la radioterapia los pilares del tratamiento. El pronóstico de esta enfermedad está determinado, principalmente, por el tipo histológico y el estadio tumoral.

ABSTRACT

Background: Swine hepatitis E virus (HEV) has a cross-reaction with human anti-HEV antibodies. Therefore, pigs could be an animal reservoir, rendering hepatitis E as a zoonosis. The spread of this infection among infected pigs across countries would be possible through trading. Previously, using an anti-human conjugate, we detected anti-HEV antobodies in adult pigs in Chile. Aim: To detect anti-HEV (ELISA) in a cohort of swine at different ages. Material and Methods: Two hundred pigs aged 42 to 360 days, divided in 20 groups of 10 animals were tested. Anti-HEV was detected by ELISA using anti-pig IgG horseradish peroxidase instead of anti-human conjugates. Results: Anti-HEV were detected in one animal aged 90 days, two animals aged 120 days, one animal aged 260 days and 2 animals aged 360 days, five pregnant sows and two old hogs. This represents a total of 14 animals or 7 percent of the sample. Conclusions: There is a significant prevalence of anti-HEV in pigs from 90 days of birth, suggesting that these swine are aprobable reservoir.

ABSTRACT

Introducción: Este trabajo consiste en un modelo experimental para la aplicación de técnica mínimamente invasiva a la cirugía de cabeza y cuello. En la literatura se han reportado dos vías de abordaje para la extirpación de glándula salival, una cervical y la otra vía piso de boca. Como hipótesis nos cuestionamos si acaso era posible hacer una extirpación de la glándula salival por vía cervical, con técnica endoscópica, en conejos. Objetivo: Desarrollar una técnica de abordaje, disección y extirpación de la glándula submandibular utilizando una técnica endoscópica en conejos. Material y Método: Este proyecto se realizó durante el período de Mayo hasta Julio de 2005, en el CEICIL. Diez conejos de laboratorio fueron operados con técnica endoscópica. Cervicotomía transversa de 2 cm de longitud. Insuflación de CO2 con aguja de Veress, creando un enfisema subcutáneo. Colocación de trocar para óptica de 10 mm, colocación de dos agujas de Veress triangulado con la cámara. Abordaje de compartimento glandular por vía subcutánea, con disección roma, identificando y aislando la glándula. Resultados: Se completó la cirugía en los 10 conejos. Fallecieron dos antes de terminado el procedimiento. Tiempo promedio de 40 minutos. Hubo complicaciones con el sangrado en 2 de los conejos. Conclusiones: Es factible la extirpación glandular por vía cervical con técnica endoscópica, en conejos. La complicación hemorrágica es un hecho a evaluar en estudios futuros, en lo posible contando con técnicas hemostáticas más avanzadas, como el Ligasure de minilaparoscopía.


Background: Salivary gland excision can be performed using a cervical approach using an endoscopic technique. Aim: To develop an endoscopic technique to excise the submandibular gland using a cervical approach in rabbits. Material and methods: Ten rabbits were operated using an endoscopic approach. A 2 cm transverse cervical incision was done and CO2 was insufflated with a Veress needle, creating a subcutaneous emphysema. A 10 cm optical trocar was placed and two Veress needles were triangulated with the camera. The glandular compartment was approached using a subcutaneous pathway, a blunt dissection was carried out identifying and isolating the gland. Results: Two rabbits died before completing the procedure. Mean surgical time was 40 minutes. Two rabbits had a bleeding as complication. Conclusions: Endoscopic submandibular gland excision through a cervical approach is feasible in rabbits. Bleeding can be a problem that must be solved using more advanced hemostatic techniques.

ABSTRACT

The social and sanitary changes that Chile is experiencing will change the epidemiologic profile of viral hepatitis. Virus A hepatitis will displace to older ages, and immunization plans with specific vaccines should be considered. The real prevalence of hepatitis B may be higher, due to an underreporting of the disease. The education and vaccination of high risk groups should be reinforced. E virus hepatitis requires more research in risk groups and in certain animal species consumed by humans. C virus hepatitis is the greatest challenge as it causes chronic liver disease and is the main cause for liver transplantation.

Background: The seroprevalence of antibodies against hepatitis A virus (HAV) is decreasing in many Latin American countries, along with improvements in sanitary standards. However, there is no information available about low socioeconomic status (LSE) populations. Aim: To assess the evolution of hepatitis A and E virus antibodies in a cohort of LSE Chilean children. Material and methods: One hundred sixty eight children aged four years, 97 males, coming from public primary care clinics, were studied. Two blood samples were obtained with an interval of one year. Anti-HAV and anti-hepatitis E virus (HEV) antibodies, were detected by ELISA using Abbott kits. Results: Anti-HAV was positive in 19 children (11.3%). After one year of follow-up, only 10 children had sustained reactivity (52.6%). Fourteen children, initially negative, became positive during the follow up (9.4%). Antibody titers to HAV were significantly higher in samples that remained positive, compared with those that lost reactivity. Anti-HEV was found positive in two children (1.2%). One remained positive and the other became negative. Conclusions: In this cohort of LSE Chilean children, the prevalence to antibodies against HAV and HEV is low. Follow-up detected loss of reactivity to HAV in nearly one half of the children, probably related to lower antibody levels.

ABSTRACT

Los parangangliomas son tumores altamente vascularizados que se originan de células provenientes de la cresta neural. El tratamiento de elección es la resección quirúrgica completa, lo cual se ve dificultado por su abundante irrigación y por su estrecha relación anatómica con importantes estructuras vásculo-nerviosas. En el presente estudio se revisa retrospectivamenete la experiencia del Departamento de Otorrinolaringología de Clínica Las Condes en el manejo quirúrgico de estas neoplasias. Entre los años 1998 y 2003 se trataron 5 pacientes portadores de paragangliomas de cabeza y cuello: un glomus yugular (GY), 2 glomus carotídeo (GC) y 2 glomus timpánicos (GT). El estudio imagenológico consistió en tomografía computarizada (TC) para todos los casos, complementada con resonancia nuclear magnética (RNM) y/o angiografía en algunos pacientes. El abordaje quirúrgico fue el indicado para cada lesión: timpanotomía retroauricular (GT), cervicotomía (GC) y abordaje infratemporal tipo A de Fisch (GY). En todos se logró una resección tumoral completa, sin presentarse complicaciones perioperatorias de consideración ni recurrencias hasta la fecha. Contando con un adecuado estudio imagenológico y una cuidadosa planificación quirúrgica, la resección de los paragangliomas de cabeza y cuello puede realizarse con un bajo índice de complicaciones, reservando la radioterapia como alternativa terapéutica únicamente en aquellos casos inoperables.

ABSTRACT

Glucagonomas are alpha pancreatic islet cell tumors that, when they are active, produce a syndrome characterized by necrolytic migratory erythema, diabetes mellitus, weight loss, anemia, glossitis, thromboembolism, neuropsychiatric disturbances and hyperglucagonemia. We report a 43 years old male presenting with a five years history of dermatological lesions, associated with weight loss, glossitis and onicodystrophy. Serum glucagon was 2200 pg/ml and a CAT scan showed a tumor in the tail of the pancreas. The tumor was surgically excised but one year later, hepatic metastases were found. These were excised surgically, treated with long acting octeotride and finally treated with radiotherapy using Y-DOTATOC. In the last control in November, 2001, the patient is asymptomatic

Background: Sanitary and socioeconomic changes and the identification of new causative virus, have changed the epidemiology of hepatitis in Chile. Aim: To study the natural history of acute hepatitis caused by virus A, E and non A-E in Chilean adults. Patients and methods: A special study protocol was followed for patients with a clinical picture of acute hepatitis. Anti HAV IgM, anti HBc IgM, anti HEV IgG and IgM and Anti HCV antibodies were determined by ELISA. Results: Fifty nine patients (30 male), aged 15 to 58 years old were studied. Eighty nine percent had jaundice and 50 to 70 percent had malaise and abdominal pain. Virus A was positive in 80 percent, virus E in 7 percent. In 14 percent of patients, all viral markers were negative. The evolution was typical in 78 percent, biphasic in 14 percent and cholestatic in 5 percent. One patient had a prolonged and one a fulminant course. Mean ALT was 1148 U/l and mean total bilirubin was 5.5 mg/dl. Seventy three percent of cases occurred during early winter and spring and 27 percent during summer and early autumm. Conclusions: The main etiology of acute viral hepatitis in Chile is virus A and most cases occur during the rainy season. Clinical features of hepatitis non A-E are similar to enteral transmission forms
