ABSTRACT

Since Mycobacterium Leprae was founded by Dr. Armauer Hansen in 1873, leprosy was proven to infectious disease by a germ not from hereditaty, from a cause, or from sin. For it has no definite method of treatment, made a conclusion at the 1st international leprosy association meeting at Berlin in 1897, isolation is the only way to prevent the disease. So all country started to built a leprosarium and isolated the leprosy patients. Various methods and drugs were used for leprosy treatment including potassium iodide, arsenic, antimony, copper, sera, vaccines and aniline dyes and then X-ray, radium, electric current till 1925. Chaulmoogra oil was introduced to western world in 1854 by Dr. FJ Mouat and used for the leprosy treatment drug. Dr RM Wilson in Kwangju Leprosy Hospital started to use Chaulmoogra oil since 1909 and reported the results of it at JAMA in 1923. But it was replaced to sulfones in 1940'. Mordern treatment started in 1937 when Parke-Davis co. synthesized promin But promin is expensive and have to injection. Then Dapsone delivered from promin and it could be used per oral. Dr. RM Wilson In Aeyang Hospsoital (former Kwangju leposy hospial) started to use Dapson in 1946 with his son Dr. J Wilson. And it was the first episode to use DDS in Korea. When Dr. Cochraine came and visited all the leprosy centers in Korea in 1955 he noticed that some hospital like Aeyangwon and St. Nazarus used DDS but not other hospital. DDS was adopted as main drug of choice in Carville, Loisiana but noticed dapsone resistant bacilli and then WHO recommended the MDT from 1981.

ABSTRACT

Claw hand deformity is caused by the lost function of intrinsic muscles and it is due to low median nerve and ulnar nerve palsy. Traumatic nerve injury and leprosy are the common cause of that deformity. Sensory loss is accompanied with claw hand deformity and the joint get stiffened by frequent trauma and it is followed by mutilated fingers. Surgical treatment of the claw hand restored the lost function of hand as well as recovered the inferiority complex of the deformed patient. The purpose of surgery is stabilized the hyperextended MP joint and Stiles-Bunnel and Brand operation are useful method.

ABSTRACT

OBJECTIVE: A sleep apnea is the most frequent symptom among sleep disorders. As the number of aged people are increasing, research activities are also increasing to monitor sleeping disorders of the elderly who lives alone. We proposed a new processing algorithm to measure the quantity of breaths accurately. METHODS: We improved the conventional center-of-mass method and further apply the projection profile method. RESULTS: In this study a system that can monitors respiration non-invasively and automatically, by identifying the movement of the chest and abdomen using image processing technique during sleep, was implemented. We can see breathing status in real time based on the acquired breathing waves. To verify the designed system, the values from the polysomnography were compared and analyzed to validate the system accuracy. As a result, the mean accuracy is 96%. CONCLUSION: We can simply monitor the sleep apnea with no sleep interferences.

ABSTRACT

As wireless network technology and various mobile communication equipments have widely used and rapidly developed, many research workers are studying about a measurable system for several medical data or bio-signals, such as electrocardiogram, pulse-oximetry and so forth, by wireless. And, even generic people who are in a good state of health as well as patients in the hospital, whose concern and care about healthcare has increased greatly, so many research workers are studying about Ubiquitous Healthcare of new technology. This technology will offer natural medical services freely in anywhere and in a quiet way, even if people can not go to hospital. Our research made a possibility to measure environment data in each infant incubator by using temperature and humidity sensor, prior to measure directly bio-signals of patients. Each sensor was composed of wireless network based on TinyOS, and that wireless network connected to Personal Digital Assistants(PDA). So we were studied about telemetry system by using wireless sensor network. It was researches about telemetry system to measure environment data in infant incubator before, by web-based RF modules. But they has problems that incubator's movement restriction by wire, wire net's limitation. We solved these problems by organizing wireless sensor network that connected together sensors and measurement devices. This wireless sensor network organization, based on TinyOS, is possible to receive sensor's data of long distance by a relay station and to save power. In future, this network will be basic of Ubiquitous Healthcare as well as medical information system in hospital.

ABSTRACT

The prevalence of supernumerary teeth is between 1~3%. Of these, 90~98% occur in the maxilla with particular predilection for the premaxilla, preceded by mandibular third premolars, mandibular forth molars and maxillary paramolars. The most common ones occur in the maxillary anterior region, they may occur singly between the central incisors(mesiodens), or they may be double(mesiodentes). Maxillary anterior supernumeraries may erupt into the oral cavity or remain unerupted. It is found that approximately 25% are erupted, while the rest are unerupted. They are usually a small tooth with a cone-shaped crown and a short root. They may cause delayed eruption, median diastema, bodily displacement or rotation of the adjacent permanent teeth. Occasionally they may lead to the developement of dentigerous cyst or primordial cyst, or eruption into nasal cavity. In this study, 109 supernumeraries surgically extracted from premaxilla in 84 patients are analysed. The results are as follows : 1. Sex distribution of male and female is 2.2 : 1 2. Almost cases are in pediatric age from 5 to 12 years old (87%). 3. Problems with supernumeraries are tooth malpositon, diastema, delayed eruption, eruption to nasal cavity and cyst formation. 4. Of 109 supernumeraries, 16(15%) are erupted into oral cavity, 92(84%) are impacted, and 1(1%) is erupted into nasal cavity. 5. Of 84 patients, 59(70%) have one supernumeray while 25(30%) have two supernumeraries. 6. Of 109 supernumeraries, 96(88%) are found within the region of the central incisors. 7. Of 109 supernumeraries, 94(86%) are vertically impacted , 11(10%) are horizontally impacted , 3(3%) are labiopalatally impacted and 1(1%) is impacted in nasal cavity. 8. Of 84 patients, we used palatal flap in 67(80%), labial flap in 6(7%), both flaps in 4(5%) and no flap in 7(8%). And incisive nerve was cut in 33(49%) of 67 palatal flaps. 9. Extration with ostectomy was done in 72 supernumeraries(66%), without ostectomy in 37 teeth(34%). 10. Extraction with tooth sectioning was done in 21 supernumeraries(19%), without tooth sectioning was done in 88 teeth(81%). 11. We used local anesthesia in 70 patients(83%) and general anesthesia in 14 ones(17%).

ABSTRACT

We measured the bone ingrowth rate on 22 retrieved human cementless porous coated acetabular cups by the help of computer graphics. All the acetabular cups had functioned well, and had had evidence of radiologic bony ingrowth before revising the acetabular cup. The reason for retrieval of the acetabular cups were: one with habitual dislocation, who with stem loosening, and 19 with polyethylene cup wearing. The period from initial surgery to retrieval surgery was 28 months to 108 months(average 56.1 months). All the retrieved porous coated acetabular cups showed macroscophic bony ingrowth, and the ratio of the whole porous surface to bony ingrowth area was showed 1.14% to 92.27% (average 36%).

ABSTRACT

The hip joint subluxation and dislocation due to poliomyelitis worsen the limping and caused the painful hip. Shelf procedure has been described as a safe, conservative and reliable approach to the of a dysplastic acetabulum. We performed shelf procedure for the dysplastic hip in poliomyelitis in thirteen patients since May, 1990. The age at operation was 21 to 38 years old and three of them were male and ten were female. All the patients were followed up for average 2.5 years(1-4. 5yrs). The hip joints showed better stability after shel'f procedure and less limping. There was no complication except one redislocation.

ABSTRACT

Limb lengthening with any kind of lengthening apparatus is accepted as a standard method to correct leg length discrepancy. And furthermore, the deformity accompanying shortening is corrected by multifocal lengthener. twenty two patients have undergone lower limb reconstruction by the technique of unilateral bone transport for diaphyseal bone defect, nonunion or deformity in the presence of shortening from May 1990 to August 1993 in Wilson Leprosy Center & Rehabilitation Hospital. All Patients had bifocal procedure using bifocal monofixator(Orthofix) and bone defects were graduaily filled by bone transport. Average healing index(days/cm)was 61. Average bone defect was 7,4cm, and average transported length was 5.4cm. In conclusion, the unilateral bone transport system using bifocal monofixator has satisfactory outcome, and salvaged limbs where amputation has been previously the only option.

ABSTRACT

Deep infection following total hip replacement arthroplasties remains one of the most serious complications in orthopaedic surgery. Between Jan. 1986 to Dec. 1991, 1130 cementless total hip arthroplasties were performed at Wilson rehabilitation hospital. Among them, fourteen patients (incidence :1.2%) developed deep wound infection, and they were retrospectively reviewed including clinical features, laboratory datas and their managements. The infection was noted in ten patients within three months, in one patient between three to twelve months and in three patients after twelve months from cementless total hip replacement arthroplasties. All of them were suffered from hip pain, six patients were manifested with generalized fever, and twelve patients with draining fistulas. Thirteen patients showed elevated ESR. Major infecting organism was Staphylococcus in ten patients. They were initially treated with meticulous debridement, ingress and eress tube irrigation, and antibiotics, but four patients had to be operated Girdlestone arthroplasties due to recurrence of infection and loosening of the prosthesis. Eight patients got quiescency from infection for at least five months from the last drainage operation. But two patients still have draining fistulas inspite of bony ingrowth achieved to the prosthesis.

ABSTRACT

Combined procedue of open reduction and shortening of femur for treatment of congenital hip dislocation in the older child is known to minmize the incidence of avascular necrosis, redislocation and stiffness. Thirty six hips in thirty three children were operated upon in the past six years with combined procedue of open reduction and shortening of femur. And twenty eight hips in twenty five children who were followed up more than one year were analysed. By the results, we obtained excellent in twenty nine percent, good in fifty three percent, fair in fourteen percent and poor in four percent.

ABSTRACT

Two hundred and thirty-seven patients who had paresis or paralysis of the quadriceps muscles underwent a transfer of the biceps femoris by Caldwell's method or medial hamstring to the patella. Seventy-six legs of seventy-two patients who had followed-up more than one year were reviewed and analysed. The results showed there was an overall improvement measured by decreased extension lag in all age groups and relatively uniform good results in young age group. Again we reviewed the forty-six patients chronologically to eliminate the outer factors who were transfered normal biceps femoris or medial hamstring power to absent quadriceps. By that we found the extension power of the knee joint after muscle transfer had been decreased by age.

ABSTRACT

The “whistling face” syndrome is a dominantly inherited disorder involving the face and musculoskeletal system. Sjnce its first description in 1938 by Freeman and Sheldon, sporadic case reports have appeared in the literature under various syndromes, cranio-carpo-tarsal dystrophy, cranio-facio-corporal syndrome, cranio-carpo-tarsal dysplasia, whistling face-Windmill Vane Hand syndrome and the chirocheilo-podalic syndrome. This report reviews the clinical manifestation of 5 cases of whistling face syndrome in 3 generation in one family.

ABSTRACT

Great strides has been made in the past 20 years in the treatment of scoliosis. Treatment before scoliosis becomes severe eliminates the need for extensive surgery. Early detection of scoliosis by school screening has proven to be valuable for early diagnosis and prevention of severe deformities. Authors screened 5,256 middle and high school students in Seoul, who were chosen arbitrarily, to determine the incidence rates of scoliosis, and following results were obtained. 1. Scoliosis was diagnosed on a combination of at least one positive physical sign and a lateral curvature more than 5 degrees on an antero-posterior standing X-ray. 2. The overall incidence of scoliosis was 4.68% (Male 0.74%, Female 6.43%). 3. Idiopathic scoliosis was found in 238 students, congenital scoliosis in 5, and paralytic scoliosis in 3. 4. The most common pattern of curvature was thoracic curve. 5. In the single curve right to left ratio was 2.1, but 8.4 in the upper curve of double curve. 6. Close observations were required for all the 246 students and treatment was required in 14 students who had more than 20 degrees, 9 students for Milwaukee brace, and 5 students for surgical correction.

ABSTRACT

Giant cell tumor of the spine is regarded as a cllnically mallgnant tumor because of its difficulty in diagnosis, cord compression and its frequent recurrence. The consensus of current report on giant cell tumor is that local resection is the treatment of choice where it is feasible. In the spine, however, because of its inoperable location, irradiation has become an accepted mode of treatment, although the results of which are at best uncertain. Total spondylectomy Is a formidable undertaking and its report is scarce. In 1966, Lievre reported total spondylectomy of the fourth lumber vertebra. In 1968, Stener and Johnsen performed total spondylectomy of three vertebral bodies, the eleventh thoracic through the first lumbar. Recently, in the summer of 1979, Edwards reported prosthetic replacement after total spondylectomy. This report presents our experience with giant cell tumor of the thlrd lumbar vertebra, which was successively treated by an excislonal biopsy through anterior retroperitoneal approach, complete removal of the vertebral body and interbody fusion of the adjoining vertebrae through an anterior medlan transperitoneal approach, and finally by excislon of the remaining posterior elemsnt and a posterior H-graft supplimented by loop wire flxation. At six month follow-up, fusion was solid and there was no exidence of recurrence of the tumor.

ABSTRACT

Limbic bone or persistant epiphysis seen roentgenographically on the anterosuperior aspects of the vertebral bodies represents another form of discal extrusion, seperating a small fragment of bone from the body of the involved vertebra. Lindblom, in 1951, demonstrated the pathophysiology with discography. Anterior herniation of the nucleus pulposus was also revealed by discography by Cloword and Buzaid in 1952. The authors reviewed 3 cases of limbic bone of the vertebrae.

ABSTRACT

Tuberculosis of the spine had been treated with posterior fusion of the spine mostly in 1950s and anterior fusion in 1960s in Korea. Posterior fusion is more simple procedure but cannot decompress in cord involvement nor confirm the diagnosis. Anterior fusion alone induce ofen progressive kyphosis in children who has more than two joint spaces involvement or who has already moderate kyphosis, and is hard to get solid anterior fusion in moderate or severe kyphosis in adult tuberculosis, Combined anterior and posterior fusion had been carried out in one or two stage since early 1970s and satisfactory results were obtained in 27 cases with follow up period of 6 moriths to 6 years. It is found that there are many advantages of one stage spine fusion and the patient tolerated the procedure well. It is concluded that the treatment of choice in most of the spine tuberculosis in children who has moderate kyphosis or more than two level involvement, and moderate kyphosis in adult are anterior and posterior fusion in one stage.
