ABSTRACT

In order to evaluate the effect of Q-switched YAG laser iridotomy to the intraocular pressure, we performed the Q-switched Nd-YAG laser iridotomy in 39 eyes of 28 patients with angle closure glaucoma. Postoperative difference of the intraocular pressure was 27.5 +/- 27.0mmHg and the intraocular pressure was reduced. After 2 months, 12 eyes had needed one kind of anti-glaucomatous medication, 7 eyes had needed no medication, 6 eyes had needed two medications and 1 eye had needed three medications. Postoperative complications were iris bleeding(10 eyes), transient pressure rising(9 eyes), focal corneal opacities(2 eyes) and closure of iridotomy(2 eyes). These result shows that laser iridotomy should be considered in the management of angle closure glaucoma.

ABSTRACT

Screening ocular examinations were performed in 509 leprosy patients(84%) among 605 lepers, who resided in 8 resettlement villages of Kyungpook Province f rom Oct. till Dec., 1988. The results were as follows: 1. Of the 496 patients studied, excluding the 13 patients who did not check visual acuity, 241 were males(48.6%), and 255 were females(51.4%). The patients' ages were varied from 26 to 80 years old and the peak age group was from 61 to 70 years old, which comprised of 160 patients(32.3%) among 496 patients. 2. The types of leprosy were L type in 379(76%), T type in 103(21%), B type in 5(1%), and I type in 9 patients(2%). 3. Among 992 eyes, 59 eyes were blind states(visual acuity, below 0.02) and 12 patients(2.4%) were binocular blind states. The numbers of blind eyes were the most from 71 to 80 years old as 25 eyes. 4. The ocular complications in leprosy, in order of frequency, were chronic anterior uveitis(23.8%), lagophthalmos(21.8%), corneal disease(20.8%), aphakia(5.2%), trichiasis(4.6%), complicated cataract(4.6%), phthisis bulbi(1.3%), and anophthalmos(0.5%). 5. Surgical treatment was necessary in 48 patients(9.7%) with lagophthalmos, and in 16 patients(3.2%) with cataract.

ABSTRACT

We had performed eight incision radial keratotomy employing the American technique on 247 eyes of 166 patients from July, 1986 to December, 1987. The clinical results including postoperative uncorrected visual acuity, refractive power, and keratometric power were followed up at 6 months on 92(37.2%) of 247 cases. The results were as follows; 1. The mean postoperative uncorrected visual acuity was 0.595 and 60.9% of all cases had 20/40 or better uncorrected visual acuity. 2. The mean reduction in postoperative refractive power was 3.26 diopters. 3. The mean reduction in postoperative keratometric power was 3.08 diopters. 4. Operative and postoperative complications were visual fluctuation(142 eyes), photophobia(52 eyes), microperforation(7 eyes), transient increase of IOP(5 eyes), changes of peripheral cornea(6 eyes), and one case of postoperative cataract with persistent hypotony and uveitis.

ABSTRACT

Double elevator paralysis is a syndrome in which the superior rectus and the inferior oblique muscle of the same eye are paralyzed. The authors have experienced a case of double elevator paralysis with 35 prism diopters hypotropia and 20 prism diopters exotropia on the right eye. The patient was 19 years old female who visited our hospital because of poor cosmetic appearance. Knapp procedure was performed primarily to correct hypotropia, but 20 prism diopters hypotropia remained. 5 months later, a second operation was performed consisted of 7.5 mm recession of the left lateral rectus and 5mm recession of the right inferior rectus in order to correct the exotropia and residual hypotropia. After the second operation, ocular position in primary position was nearly orthophoric and upward movement of the right eye was considerably improved.
